BACKGROUND: In classic Hodgkin lymphoma (cHL), identifying high-risk patients is crucial for tailored treatment. Clinical risk factors are useful, but there is room for improvement. Although many diagnostic tissue-based biomarkers have been described to correlate with prognosis, none have achieved the accuracy to be included into daily clinical practice.
METHODS: The most promising tissue biomarkers, c-MET, HLA class II, and CD68, were tested in early-stage cHL for prognostic validation, individually and in combination with each other and established clinical risk factors. All biomarkers were

Lymphadenopathy is a common presentation in urgent care and primary care settings and is most often due to benign or infectious causes. However, persistent or unexplained lymphadenopathy may represent an underlying malignancy, including Hodgkin lymphoma. We present the case of a 20-year-old female patient who presented to urgent care with a two-week history of cervical and axillary lymphadenopathy without B symptoms. Initial laboratory evaluation was notable for a positive mumps IgM, suggesting an infectious etiology. However, imaging revealed multistation cervical, axillary, and mediastinal l

The immune microenvironment of classical Hodgkin lymphoma (cHL) is characterized by rare Hodgkin-Reed-Sternberg (HRS) cells surrounded by dysfunctional lymphocytes. HRS cells exploit the PD-L1/PD-1 axis to suppress antitumor immunity. Although PD1 blockade is effective in some relapsed cHL patients, the mechanisms of resistance remain unclear. We investigated T-cell dynamics in cHL using whole-exome sequencing (WES) of plasma circulating tumor DNA and longitudinal single-cell RNA and T-cell receptor sequencing of peripheral blood lymphocytes from a relapsed patient before and after anti-PD1 th

Neurolymphomatosis (NL) is a rare neurological complication of lymphoma, characterised by direct infiltration of peripheral nerves by neoplastic cells. It most commonly occurs in association with non-Hodgkin's lymphoma (NHL) and poses significant diagnostic challenges due to its non-specific clinical presentation. Classically, NL presents as a painful, rapidly progressive, and asymmetric neuropathy, frequently mimicking inflammatory or vasculitic neuropathies. Here, we present the case of a 69-year-old woman with a history of NHL, in remission for five years, who presented with a progressive,

A targeted salvage therapy with Brentuximab vedotin in heavily treated refractory or relapsed pediatric Hodgkin lymphoma patients before and after stem cell transplantation. Turk J Pediatr 2019; 61: 671-676. Hodgkin`s lymphoma (HL) is highly curable disease in its early stages, but in advanced stages, it presents a dilemma when it becomes refractory or relapses after several rounds of chemotherapy. Brentuximab vedotin (BV) is an antibody-drug conjugate that targets the tumor necrosis receptor fa

AIMS: Status epilepticus is common in children with severe falciparum malaria and is associated with poor outcome. Phenytoin is often used to control status epilepticus, but its water-soluble prodrug, fosphenytoin, may be more useful as it is easier to administer. We studied the pharmacokinetics and clinical effects of phenytoin and fosphenytoin sodium in children with severe falciparum malaria and status epilepticus.
METHODS: Children received intravenous (i.v.) phenytoin as a 18 mg kg-1 loading dose infused over 20 min followed by a 2.5 mg x kg(-1) 12 hourly maintenance dose infused over 5

BACKGROUND: Paediatric classical Hodgkin lymphoma (cHL) is the most common malignancy in adolescents, and despite excellent survival, a subset of patients experiences treatment failure or severe long-term toxicity, underscoring the need for improved risk stratification. Early response assessment is particularly important, as it guides decisions on radiotherapy, where overtreatment can lead to substantial late effects.
METHODS: In the context of a large-scale systems-level immunomonitoring initiative, we specifically examined paediatric cHL and profiled their systemic immunology alongside chil

Vanishing bile duct syndrome (VBDS) is an acquired condition of progressive intrahepatic bile duct loss, resulting in severe cholestasis. Here, we present a case of VBDS in a patient with newly diagnosed classic Hodgkin's lymphoma (cHL). Our patient presented with cholestatic liver test elevations, including bilirubin and alkaline phosphatase. Liver biopsy confirmed the paucity of bile ducts and did not show involvement by lymphoma. Though the exact pathophysiology is unclear, it is postulated to be driven by cytokine release and T-lymphocyte cell dysregulation. Our patient has made a remarkab

Immune checkpoint proteins including PD-1, CTLA-4, LAG-3, TIM-3, and TIGIT regulate T-cell functions, which are essential for anti-tumor immunity. Over-expression of these immune checkpoint proteins leads to T-cell exhaustion and a significant impairment of anti-tumor immunity. Rejuvenation of effector T-cell function with immune checkpoint inhibitors (ICI) restores anti-tumor immunity, which translates into clinical efficacy in the frontline and salvage treatment of various hematological malignancies. Efficacy of ICIs is highest in classical Hodgkin lymphoma, primary mediastinal large B-cell

Older patients with classic Hodgkin lymphoma experience poorer outcomes than younger individuals, due to comorbidities and functional limitations affecting treatment tolerance. In this retrospective single-centre study of 140 patients, we evaluated treatment patterns, outcomes and the prognostic value of two geriatric scores: the Age, Comorbidities and Albumin (ACA) Index and the Frailty Score. Most patients (87.9%) received standard ABVD (doxorubicin, bleomycin, vinblastine and dacarbazine)/AVD (doxorubicin, vinblastine and dacarbazine) regimens. At median follow-up (65 months), 5-year progre

Recent advances in molecular pathology have profoundly reshaped the understanding and classification of mediastinal lymphomas, particularly primary mediastinal large B-cell lymphoma (PMBL) and mediastinal gray zone lymphoma (MGZL). These entities, long defined by morphological and immunophenotypic criteria, are now characterized by distinct genetic and transcriptional signatures. PMBL is defined by recurrent alterations involving CIITA, 9p24 (PD-L1/PD-L2), SOCS1, STAT6, and B2M, reflecting constitutive activation of the JAK/STAT and NF-κB pathways. Transcriptomic studies confirm its distinctio

Epstein-Barr virus (EBV)-associated lymphoproliferative disorders have been reported in patients receiving immunosuppressive therapy for inflammatory bowel disease; however, the onset of classic Hodgkin lymphoma (cHL) during vedolizumab therapy is extremely rare. A 73 year-old woman with ulcerative colitis (UC) was steroid-dependent, showing disease exacerbation when prednisolone was tapered or discontinued. Azathioprine was administered eight years prior but was discontinued after a short period due to liver dysfunction. Vedolizumab was initiated 31 months before presentation. Despite initial

The occurrence of both non-Hodgkin lymphoma (NHL) and classic Hodgkin lymphoma (cHL) in an individual patient (hereafter referred to as composite lymphoma) is a relatively rare and poorly characterized phenomenon. We hypothesized that analysis of the shared and divergent mutations harbored by composite lymphomas might shed light on genetic drivers of composite lymphomagenesis. We performed exome sequencing of two cases of composite NHL and cHL, and validated somatic variants using the AmpliSeq platform. Additionally, we utilized B-cell receptor sequencing of the immunoglobulin heavy chain (IGH

Classic Hodgkin Lymphoma (CHL) remains difficult to treat in patients with relapsed and refractory disease. The utility of immunotherapies, many of which target B-cell markers, is still under investigation. Although the neoplastic Hodgkin and Reed-Sternberg (HRS) cells are thought to lose most B-cell markers, studies have shown retention of these markers, mainly CD20, in some cases. However, the CD19 staining profile of HRS cells has not been thoroughly assessed and warrants exploration in the era of CD19-directed immunotherapies. We assessed CD19 immunohistochemical staining in 41 cases of CH

Mediastinal grey zone lymphoma (MGZL) is a rare disease with overlapping features between classical Hodgkin lymphoma (CHL) and primary mediastinal large B-cell lymphoma (PMBL). The recent classifications limit the disease to mediastinal cases. Epstein-Barr virus (EBV) is negative in most instances. Predominant resemblance to CHL (MGZL-HL) or primary mediastinal B-cell lymphoma (MGZL-PMBL) can commonly be defined, although mixed features do occur. The morpho-phenotypic heterogeneity of MGZL has significantly affected diagnostic reproducibility also among experts and the proposed diagnostic crit

Richter's transformation (RT) is a feared and not completely understood complication of chronic lymphocytic leukemia or small lymphocytic lymphoma (CLL/SLL). While most cases involve transformation to diffuse large B-cell lymphoma (DLBCL), CLL may, albeit rarely, progress to Hodgkin lymphoma (HL). We report the case of a 74-year-old woman initially diagnosed with CLL/SLL who progressed to a rare form of HL subtype affecting the spinal cord. After receiving six cycles of brentuximab+doxorubicin, vinblastine, and dacarbazine (A+AVD) therapy at our Department of Hematology (University of Debrece

BACKGROUND: Pediatric Hodgkin lymphoma (PHL) is the most common lymphoreticular cancer in adolescents aged between 15-19 years. The survival of PHL is over 95% in developed countries with or without radiation therapy. Proper staging and risk stratification of PHL before initiation of treatment is fundamental for cure. Although outcomes for PHL in developed countries are excellent, survival in low-income countries remains lower. PHL is one of the WHO six indexed cancers with a target survival of >60% by 2030 globally. The aim of the study was to evaluate clinicopathologic characteristics and tr

OBJECTIVE: Allogeneic hematopoietic cell transplantation (allo-HCT) remains a curative option for relapsed or refractory (R/R) Hodgkin lymphoma despite advances with novel therapeutic agents such as brentuximab vedotin (BV) and immune checkpoint inhibitors (CPIs). This study aimed to assess the benefit of prior exposure to novel therapeutic agents prior to allo-HCT in patients with R/R Hodgkin lymphoma.
MATERIALS AND METHODS: This study’s retrospective multicentric analysis involved 70 patients with R/R classical Hodgkin lymphoma who underwent allo-HCT.
RESULTS: The analyzed patients were sp

No universal gold standard chemotherapy exists for the treatment of older (≥60 years) patients with classical Hodgkin lymphoma (cHL). To evaluate the current curative treatment strategies used in the Nordic countries, we collected data from patients diagnosed in Sweden, Denmark and Norway during the years 2000 to 2021. We included 1569 patients: 704 (45%) Swedish, 671 (43%) Danish, and 194 (12%) Norwegian. Of these, 671 (43%) received doxorubicin, bleomycin, vinblastine, dacarbazine (ABVD), 123 (8%) received doxorubicin, vinblastine, and dacarbazine (AVD), 465 (31%) received cyclophosphamide,

Pediatric Hodgkin lymphoma (HL) is highly curable, and reducing the treatment intensity in patients who respond well to induction therapy is a key strategy for minimizing long-term adverse effects. Biomarkers that identify good responders at diagnosis would enable further de-escalation of the treatment. Circulating microRNAs (miRNAs) have shown promise as noninvasive indicators of therapeutic response in hematological cancers, yet their association with early metabolic response on quantitative 18F-Fluorodeoxyglucose-Positron Emission Tomography (18F-FDG-PET) in pediatric HL has not been define
