Pitfalls in the diagnosis of diffuse large B-cell lymphoma with flow cytometry.

INTRODUCTION: Although flow cytometry (FCM) is not required for the diagnosis of diffuse large B-cell lymphoma (DLBCL) according to World Health Organization criteria, FCM plays an auxiliary role in the work-up of non-Hodgkin lymphomas. The sensitivity of FCM for DLBCL remains variable. Literature reports a detection rate of 63% to 97%. This study aims to identify factors that may influence the sensitivity. METHODS: A retrospective data-analysis was performed on 53 patient samples with a diagnosis of DLBCL. A lymphoma screening panel was performed on the same sample as the histopathological d

Immunohistochemistry for c-MET, HLA class II, and CD68 does not show prognostic value in early-stage classic Hodgkin lymphoma patients in the EORTC-GELA H9 intergroup randomised trial.

BACKGROUND: In classic Hodgkin lymphoma (cHL), identifying high-risk patients is crucial for tailored treatment. Clinical risk factors are useful, but there is room for improvement. Although many diagnostic tissue-based biomarkers have been described to correlate with prognosis, none have achieved the accuracy to be included into daily clinical practice. METHODS: The most promising tissue biomarkers, c-MET, HLA class II, and CD68, were tested in early-stage cHL for prognostic validation, individually and in combination with each other and established clinical risk factors. All biomarkers were

Nivolumab with doxorubicin, vinblastine, and dacarbazine in older adults with classic Hodgkin lymphoma: do S1826 results hold up in the real world?

Older adults (OA, ≥60 years) with classic Hodgkin lymphoma (cHL) have historically experienced inferior outcomes compared to younger patients. Nivolumab in combination with doxorubicin, vinblastine, and dacarbazine (NAVD) has demonstrated high efficacy in clinical trials, but real-world data in OA are limited. We conducted a multicenter retrospective study across 20 centers in patients with newly diagnosed advanced stage cHL treated with NAVD and analyzed outcomes in OA across the aging spectrum. Among 327 total patients, 81 (25%) were ≥60 years, with most OA presenting with stage IV disease (

Recent trends in childhood, teenage and young adult cancer incidence in England (2001-2020).

INTRODUCTION: Cancer remains a leading cause of death among children, teenagers and young adults (CTYAs) in England with a distinct incidence profile compared to older adults. We examined trends in cancer incidence among individuals aged < 25 years in England from 2001 to 2020 using openly available National Disease Registration Service (NDRS) data. MATERIALS AND METHODS: All malignancies (ICD-10 C00-C97) diagnosed between 2001 and 2020 were extracted from published NDRS official registration statistics. Analyses were stratified into children (0-14 years) and TYAs (15-24 years), with subanaly

A Matching-Adjusted Indirect Comparison of PET-Guided BrECADD Versus PET-Guided ABVD, BEACOPP, and eBEACOPP in Advanced Hodgkin Lymphoma.

INTRODUCTION: In the absence of head-to-head trial data, a matching-adjusted indirect comparison (MAIC) was conducted to compare the effectiveness, with respect to overall survival (OS) and progression-free survival (PFS), of PET-guided BrECADD versus PET-guided ABVD, BEACOPP, and eBEACOPP in advanced Hodgkin lymphoma. METHODS: Individual patient data (IPD) from patients in HD21 and aggregate data from the SWOG S0816, RATHL, HD9, and Mondello 2020 studies were utilized. Potential effect modifiers and prognostic variables for advanced Hodgkin lymphoma were identified via literature review, cli

Matching-adjusted indirect comparison of zanubrutinib and real-world chemotherapy, rituximab and chemoimmunotherapy in relapsed or refractory marginal zone lymphoma.

Aim: Traditional management of relapsed or refractory (R/R) marginal zone lymphoma (MZL) comprises chemotherapy, rituximab or rituximab-based chemoimmunotherapy (CIT). Zanubrutinib, a Bruton tyrosine kinase inhibitor, has shown high overall response rates and durable responses across MZL subtypes in two Phase II, single-arm trials. As no head-to-head randomized study exists, an unanchored matching-adjusted indirect comparison was performed to estimate the relative efficacy of zanubrutinib versus traditional agents. Materials & methods: Pooled patient-level data for 86 evaluable patients from z

Genetic variation in IL-6 rs1800796 and its association with non-Hodgkin lymphoma susceptibility in the Egyptian population.

Non-Hodgkin lymphomas (NHLs) are associated malignancies with different morphological, genetic, immunophenotypic, and clinical characteristics. Research on genetic susceptibility to NHL is needed to identify at-risk groups and define important disease pathways. Interleukins (ILs) are cytokines that play important immunomodulatory roles. IL-6 stimulates IgG production, exerts antiviral effects, and has been linked to various diseases. Research on IL-6 mutations and NHL is important due to the role of IL-6 in immune regulation and cancer pathogenesis; however, direct causative links remain uncle

Immunotherapy-Based Therapies in Pediatric Hodgkin Lymphoma: Potential to Narrow the Gap in Long-Term Survival.

Pediatric advanced-stage Hodgkin lymphoma (HL) is highly curable, yet survivors treated with historical regimens face substantial late morbidity and excess mortality. Immunotherapy-based frontline therapies improve disease control, but late toxicities remain unknown. We developed a simulation model to project lifetime outcomes after contemporary brentuximab vedotin- or nivolumab-containing regimens using trial data (COG AHOD1331 and SWOG S1826), Childhood Cancer Survivor Study data, and national databases. We assessed how frontline and relapse therapy changes could influence long-term survival

Advances in diagnosis and treatment of angioimmunoblastic T-cell lymphoma.

Angioimmunoblastic T-cell lymphoma (AITL) is a peripheral T-cell lymphoma (PTCL) that originates from follicular helper T cells (Tfh) and represents a rare subtype of non-Hodgkin lymphoma (NHL). The disease is characterized by unique morphological, molecular, and clinical features, and exhibits aggressive behavior. Currently, there is no standard treatment protocol, and patients treated with first-line CHOP-like chemotherapy show low remission rates, high recurrence rates, and poor overall prognosis. In recent years, with the in-depth study of the molecular genetic mechanism of AITL, various n

Complex venous access in superior vena cava syndrome secondary to gray zone lymphoma, case report.

INTRODUCTION: Mediastinal gray zone lymphoma (MGZL) is a rare and biologically complex lymphoma with overlapping features of primary mediastinal B-cell lymphoma and classical Hodgkin lymphoma. Bulky mediastinal disease frequently complicates central venous access, particularly in the presence of superior vena cava (SVC) syndrome. Reports addressing optimal vascular access strategies in this setting remain limited. CASE PRESENTATION: A 41-year-old man presented with newly diagnosed MGZL and a large mediastinal mass causing clinically significant SVC obstruction. The patient required urgent ini

[The Janus face of breast implant diseases II.].

In modern plastic surgery, breast implants are an indispensable surgical tool in both reconstructive and cosmetic procedures. The clear positive effect of implantation is that it improves body image and thus self-esteem, but over the past twenty years, alongside its successful use, a downside to implants has emerged, known in medical literature as implant diseases. These are autoimmune breast implant illness (BII), breast implant-associated anaplastic large cell lymphoma (BIA-ALCL) and breast implant-associated squamous cell carcinoma (BIA-SCC). BIA-ALCL is a rare but well-defined syndrome, an

Successful treatment of refractory hemophagocytic lymphohistiocytosis complicating Epstein-Barr virus-positive classic Hodgkin lymphoma with rituximab.

Hemophagocytic lymphohistiocytosis (HLH) complicating classic Hodgkin lymphoma (HL-HLH) is a rare but frequently life-threatening condition that is strongly associated with Epstein-Barr virus (EBV) infection, particularly in the mixed-cellularity and lymphocyte-depleted subtypes. No consensus has been reached regarding treatment for HL-HLH, and the myelotoxicity induced by etoposide-based HLH-directed therapy precludes its safe addition to intensive lymphoma-directed chemotherapy, particularly in older patients. We here report the case of a male in his early 70s with advanced EBV-positive mixe

Tumor microenvironment differences between diagnostic and relapsed classic Hodgkin lymphoma revealed by scRNA-seq.

Classical Hodgkin lymphoma (CHL) is characterized by a complex tumor microenvironment (TME) that supports disease progression. Although immune cell recruitment by Hodgkin and Reed-Sternberg (HRS) cells is well documented, the role of nonmalignant B cells in relapse remains unclear. Using single-cell RNA sequencing (scRNA-seq) on paired diagnostic and relapsed CHL samples, we identified distinct shifts in B-cell populations, particularly an enrichment of naïve B cells and a reduction of memory B cells in early-relapse CHL compared to late-relapse and newly diagnosed CHL. The enrichment of naïve

PD-1-based combinations before autologous transplant are associated with improved outcomes in classical Hodgkin lymphoma.

Combination therapy incorporating programmed cell death protein 1 (PD-1) blockade results in unprecedented response rates in both frontline and relapsed/refractory (R/R) classical Hodgkin lymphoma (cHL). Previous retrospective studies have suggested benefit for PD-1 blockade before autologous stem cell transplant (ASCT) but included few patients receiving PD-1 blockade with cytotoxic chemotherapy. To explore the impact of anti-PD-1 based salvage on outcomes for patients with R/R cHL, we retrospectively reviewed 1280 patients with R/R cHL who underwent ASCT from 2010 to 2022 at 6 transplant cen

Brentuximab vedotin and nivolumab in combination with chemotherapy for nonbulky, early-stage classical Hodgkin lymphoma.

Most patients with early-stage classical Hodgkin lymphoma (cHL) are treated with doxorubicin, bleomycin, vinblastine, and dacarbazine with or without radiation therapy, although studies are now evaluating the incorporation of novel agents paired with abbreviated chemotherapy. We present the efficacy and safety of AN+AD (brentuximab vedotin [BV] and nivolumab in combination with doxorubicin and dacarbazine) in patients with early-stage cHL. In this phase 2 study, patients with nonbulky (<10 cm) Ann Arbor stage I or II cHL received 4 cycles of AN+AD. The primary end point was complete response (

Review of OEPA/COPDAC regimen in pediatric Hodgkin lymphoma; institutional experience from a low middle-income country.

Pediatric Hodgkin lymphoma (HL) treatment has increasingly shifted toward response-adapted protocols, aiming to minimize radiotherapy and employ intensive chemotherapy such as OEPA/COPDAC. We retrospectively reviewed treatment outcomes and toxicities in pediatric HL patients treated with OEPA/COPDAC between 2015 and 2019 at a tertiary care center in Pakistan, a resource limited country, following the Euronet PHL-C1 protocol with radiotherapy reserved for inadequate interim responses. Clinical features, treatment-related toxicities, and hospital admissions were documented. The cohort included 2

Selection of a Salvage Regimen for Relapsed or Refractory Classic Hodgkin Lymphoma in the Era of Novel Therapies.

The treatment landscape of classical Hodgkin lymphoma continues to evolve with earlier use of novel agents. Most recently, the addition of PD-1 inhibitors into frontline therapy has improved cure rates but introduced new challenges at relapse. How prior exposure to these agents impacts disease biology and response to salvage regimens remains unclear, thereby complicating second-line treatment decisions. This review discusses current considerations for selecting a salvage therapy in the era of frontline novel agents.

Management of relapsed/refractory DLBCL in the era of novel agents and new approaches.

The therapeutic landscape of relapsed/refractory (R/R) diffuse large B-cell lymphoma (DLBCL) is rapidly evolving. Novel immunotherapies and targeted therapies have expanded treatment options and improved outcomes for patients with historically limited effective options. These advances raise important questions regarding sequencing, patient selection, and mechanisms of resistance. At the same time, frontline therapy is shifting toward biomarker-informed and risk-adapted approaches, with interim imaging and circulating tumor DNA increasingly incorporated into trial design. As advanced therapies
