Optimizing the commercial manufacturing of tisagenlecleucel for patients in Japan: A 4-year experiential journey.

BACKGROUND: Tisagenlecleucel, an autologous CD19-directed chimeric antigen receptor T-cell therapy, was approved in Japan, in March 2019 for patients with relapsed/refractory (r/r) B-cell acute lymphoblastic leukamia and r/r diffuse large B-cell lymphoma, and in August 2022 for patients with r/r follicular lymphoma. Post-approval, a key goal has been to upscale and continuously improve manufacturing and shipment success rates (MSR and SSR, respectively) in the commercial setting to meet the needs of patients worldwide, including in Japan. Herein, we report accrued experience from a 4-year jour

The Role of AI in Lymphoma: An Update.

Malignant lymphomas encompass a range of malignancies with incidence rising globally, particularly with age. In younger populations, Hodgkin and Burkitt lymphomas predominate, while older populations more commonly experience subtypes such as diffuse large B-cell, follicular, marginal zone, and mantle cell lymphomas. Positron emission tomography/computed tomography (PET/CT) using [18F] fluorodeoxyglucose (FDG) is the gold standard for staging, treatment response assessment, and prognostication in lymphoma. However, interpretation of PET/CT is complex, time-consuming, and reliant on expert imagi

Circulating tumor DNA in lymphoma: technologies and applications.

Lymphoma, a malignant tumor derived from lymphocytes and lymphoid tissues, presents with complex and heterogeneous clinical manifestations, requiring accurate patient classification for appropriate treatment. While invasive pathological examination of lymph nodes or lymphoid tissue remains the gold standard for lymphoma diagnosis, its utility is limited in cases of deep-seated tumors such as intraperitoneal and central nervous system lymphomas. In addition, biopsy procedures carry an inherent risk of complications. Computed tomography (CT) and positron emission tomography/computed tomography (

Thromboinflammatory Biomarkers in Lymphomas: Linking Inflammation to Thrombosis Risk.

Thrombosis is a critical complication in lymphomas, driven by chronic inflammation. To observe this systemic mechanism, we evaluated inflammatory cytokines, neutrophil and monocyte activation, and platelet function in diffuse large B-cell lymphoma (DLBCL), follicular lymphoma (FL), and Hodgkin lymphoma (HL), with and without thrombosis using ELISA and flow cytometry according to laboratory and clinical data. Interleukin-1β was elevated across lymphomas and inversely correlated with the Khorana score for venous thromboembolism, while increased tumor necrosis factor-alpha (TNF-α) was inversely a

Biology as vulnerability in follicular lymphoma: genetics, epigenetics, and immunogenetics.

Follicular lymphoma (FL) represents a heterogeneous group of B-cell neoplasms with distinct genetic, epigenetic, microenvironmental, and clinical features. It is the most prevalent indolent non-Hodgkin lymphoma, characterized by a relapsing course and risk of transformation to aggressive diffuse large B-cell lymphoma. Recent advances in high-throughput sequencing, spatial transcriptomics, and imaging technologies uncovered genetic, epigenetic, and immunogenetic features underpinning FL, offering insights into its biology and potential therapeutic vulnerabilities. Although FL is primarily drive

Blunted CD40-responsive enhancer activation in CREBBP-mutant lymphomas can be restored by enforced CD4 T-cell engagement.

The CREBBP lysine acetyltransferase (KAT) is frequently mutated in follicular lymphoma and diffuse large B-cell lymphoma and has been studied using gene knockout in murine and human cells. However, most CREBBP mutations encode amino acid substitutions within the catalytic KAT domain (CREBBP KAT-PM) that retain an inactive protein and have not been extensively characterized. Using CRISPR gene editing and extensive epigenomic characterization of lymphoma cell lines, we found that CREBBP KAT-PM lead to unloading of CREBBP from chromatin, loss of enhancer acetylation, and prevention of EP300 compe

An evaluation of odronextamab for the treatment of multiple subtypes of relapsed/refractory B-cell non-Hodgkin lymphoma.

INTRODUCTION: Patients with relapsed/refractory B-cell non-Hodgkin lymphoma (B-NHL) have a poor median survival rate when treated with traditional salvage therapies. Bispecific antibodies (BsAbs) are an emerging class of 'off-the-shelf' immunotherapies that show promising efficacy in this population. Odronextamab is a CD20×CD3 targeting bispecific antibody that is being investigated in multiple subtypes of relapsed/refractory B-NHL. AREAS COVERED: This article describes the development of odronextamab from pre-clinical work through to ongoing clinical trials in relapsed/refractory B-NHL. The

Prognostic impact of metabolic tumor volume using the SUV4.0 segmentation threshold in 1,960 lymphoma patients from prospective LYSA trials.

PURPOSE: This study compared the prognostic value of total metabolic tumor volume (TMTV) in lymphoma measured with the recently proposed SUV4.0 segmentation threshold versus the 41% SUVmax across LYSA trials and its impact on intensity and dissemination PET features. METHODS: A total of 1960 baseline PET/CT scans of Diffuse Large B cell lymphoma (DLBCL), follicular lymphoma (FL) and Hodgkin lymphoma (HL) patients were collected. After a semi-automatic preselection of region of interest, two different segmentation threshold were applied: 41% SUVmax (TMTV41%) and SUV > 4.0 (TMTV4.0). RESULTS:

Phosphorylation status and prognostic impacts of RSK2, PDPK1, and AKT in malignant lymphoma.

Despite the oncogenic roles of the serine/threonine kinase PDPK1 and its key effectors, RSK2 and AKT, their activation status and prognostic significance remain unexamined in B cell lymphomas (BCLs). This study evaluated the phosphorylation states of PDPK1, the RSK2-N-terminal kinase domain (NTKD), and AKT through immunohistochemical analyses of 468 biopsied samples from patients with nine subtypes of MLs, including diffuse large B cell lymphoma (DLBCL) (n = 277) and follicular lymphoma (FL) (n = 121). PDPK1 was frequently phosphorylated in most subtypes, showing 98% in DLBCL and 76% in FL. RS

Extramedullary Hematopoiesis in the Appendix Associated With Myelodysplastic Syndrome: A Case Report.

Extramedullary hematopoiesis (EMH) is characterized by the presence of hematopoietic tissue outside the bone marrow, more commonly in the liver and the spleen. However, there are very few reports of nonhepatosplenic extramedullary hematopoiesis (NHS-EMH). Among the NHS-EMHs, NHS-EMH in the appendix is extremely uncommon. We hereby present a case of appendiceal NHS-EMH and discuss it with a literature review. The patient was a gentleman in his 70s who had been treated for follicular lymphoma seven years ago. He gradually developed leukopenia and six months ago was diagnosed with myelodysplastic

Mendelian randomization of immune cell phenotypes to discover potential drug targets for B-cell malignancy.

Although treatment options for B-cell malignancies have expanded, many patients continue to face limited response rates, highlighting an urgent need for new therapeutic targets. To prioritize candidate drug targets for B-cell malignancies, we employed Mendelian Randomization to estimate potentially causal relationships between 445 immune cell traits and six B-cell cancers: follicular lymphoma (FL), diffuse large B-cell lymphoma (DLBCL), Hodgkin lymphoma (HL), marginal zone lymphoma (MZL), chronic lymphocytic leukemia (CLL), and multiple myeloma (MM), totaling 22,922 cases and 394,204 controls.

Follicular lymphoma grade 3A: aggressiveness and curability in an Asian cohort of 781 patients.

Follicular lymphoma grade 3A (FL3A) is an uncommon subtype of FL with histology falling between FL grade 1/2 (FL1/2) and FL grade 3B (FL3B). The behavior of FL3A is currently debated, with some studies suggesting it may be potentially curable while others deem it incurable. However, the lack of large-scale studies with prolonged follow-up and changes in prognosis due to the use of rituximab (R) call for further investigation into the clinical course, prognosis, and treatment of FL3A in the R era. We conducted a follow-up for a median of 7.3 years (30% exceeded 10 years) on 104 FL3A cases who r

The Current State of Bispecific Antibodies and T-Cell Directed Therapy in NHL.

Relapsed/refractory non-Hodgkin lymphoma (r/r/NHL) is an aggressive disease with overall poor response rates to chemo-immunotherapy and autologous stem-cell transplant, especially in patients with diffuse large B-cell lymphoma. Major improvements in this disease space have come through the incorporation of novel immune therapies, including CD19/CD20 directed CAR-T cells and bispecific antibodies. These exciting new therapies continue to change the landscape of treatment for r/r NHL and have been incorporated in earlier lines of therapy with demonstrated efficacy and patient safety. In this rev

Unveiling Primary Cutaneous B-Cell Lymphomas: New Insights into Diagnosis and Treatment Strategies.

BACKGROUND/OBJECTIVES: Primary cutaneous B-cell lymphomas (PCBCL) are a rare and heterogeneous group of non-Hodgkin lymphomas that are confined to the skin at diagnosis and exhibit a tendency for cutaneous recurrence. The 5th edition of the World Health Organization and the 2022 International Consensus Classification recognize three main subtypes: primary cutaneous follicle center lymphoma (PCFCL), primary cutaneous marginal zone lymphoma/lymphoproliferative disorder (PCMZL/LPD), and primary cutaneous diffuse large B-cell lymphoma, leg type (PCDLBCL,LT). These subtypes differ in clinical behav

Successful treatment of primary refractory DLBCL/HGBL - MYC/BCL2 transformed from FL using glofitamab: a case report.

Diffuse large B-cell lymphoma/high-grade B-cell lymphoma with MYC and BCL2 rearrangements (DLBCL/HGBL-MYC/BCL2) represents a distinct entity of mature aggressive B-cell lymphoma, constituting a substantial gap in the clinical management of DLBCL. Conventional R-CHOP-like chemoimmunotherapy regimens have demonstrated limited efficacy in DLBCL/HGBL-MYC/BCL2, and the clinical outcome remains poor, with a median overall survival of less than 2 years, and even shorter in cases transformed from indolent lymphoma. We reported a 66-year-old female was firstly diagnosed with follicular lymphoma, but pr

Prognostic relevance of bone marrow immune cell fractions in newly diagnosed B-cell non-Hodgkin lymphoma patients.

INTRODUCTION: Non-Hodgkin lymphomas (NHLs) are the most common hematological malignancies worldwide. Among these, B-cell lymphomas (B-NHLs) are the second leading cause of death in hematologic neoplasms. MATERIAL AND METHODS: In this study, a detailed immunophenotypic analysis of lymphocytes in the bone marrow aspirate (BMA) of 75 patients with four different subtypes of B-NHLs was performed at diagnosis. The samples were analyzed by flow cytometry (FC) using a stain-lyse-no wash technique and a comprehensive six-color antibody panel. RESULTS: Our data showed a different trend in the percent
