A Case of Angioimmunoblastic T-Cell Lymphoma Misdiagnosed as Eczema-Like Dermatitis in an Elderly Patient.

Angioimmunoblastic T-cell lymphoma (AITL) is a clinically heterogeneous subtype of peripheral T-cell lymphoma that frequently presents with nonspecific cutaneous manifestations. Approximately 40%-50% of patients initially develop diffuse erythema or papular eruptions that may mimic benign inflammatory or allergic dermatoses, posing a diagnostic challenge in early disease stages. We report the case of an 81-year-old man who presented with generalized pruritic papules and was initially diagnosed with an eczema-like inflammatory dermatitis. Although the skin lesions showed transient improvement w

B-cell and plasma cell proliferations associated with T follicular helper cell lymphomas: clinical-pathological and genetic features.

Nodal T-follicular helper cell lymphomas (nTFHL) are aggressive peripheral T-cell lymphomas with unique clinical-biological features. B-cell and/or plasma cell proliferations (B/PCP) are frequently reported in nTFHL, yet their histological spectrum remains incompletely defined. This study aims to delineate the clinical, pathological and molecular features of a retrospective series of nTFHL-associated B/PCP, with particular emphasis on recurrent disease patterns. To this aim, we retrospectively analyzed 39 nTFHL with 45 synchronous and/or metachronous B/PCP, collected from five Italian referral

Spatially decoding genotype-associated epigenetic landscapes in human lymphoma FFPE tissues via epi-Patho-DBiT.

While formalin-fixed paraffin-embedded (FFPE) samples are invaluable for human non-Hodgkin B-cell lymphoma translational research, effective methods for spatial profiling of chromatin accessibility and histone modifications in these tissues remain limited. Here, we introduce epi-Patho-DBiT, a platform that combines reverse crosslinking of FFPE tissues with spatially resolved assays for transposase-accessible chromatin using sequencing (spatial-FFPE-ATAC) or cleavage under targets and tagmentation (spatial-FFPE-CUT&Tag). Using spatial-FFPE-ATAC, we map epigenetic landscapes in mucosa-associated

Extracellular Vesicles in B-Cell Non-Hodgkin Lymphomas: Pathogenesis, Therapeutic Implications, and Biomarker Potential.

Extracellular vesicles (EVs), as key mediators of intercellular communication, play multifaceted roles in the pathogenesis, treatment, drug resistance, and monitoring of B-cell non-Hodgkin lymphomas (B-NHLs), including diffuse large B-cell lymphoma (DLBCL), Burkitt lymphoma (BL), follicular lymphoma (FL), and mantle cell lymphoma (MCL). EVs derived from lymphoma cells or tumor microenvironment cells carry diverse cargoes such as proteins, microRNAs (miRNAs), and viral oncoproteins, which regulate tumor progression by modulating signaling pathways related to cell proliferation, invasion, apopto

New Diagnosis of B-cell Follicular Lymphoma Temporally Associated With Routine Vaccination: A Report of Two Cases.

Follicular lymphoma (FL) is an indolent B-cell non-Hodgkin lymphoma with potential for transformation into diffuse large B-cell lymphoma (DLBCL). While transient lymphadenopathy is a known response to vaccination, particularly with mRNA COVID-19 vaccines, recent case reports suggest that immune activation may potentially unmask previously undocumented subclinical lymphoproliferative disorders. We report two cases of newly diagnosed FL following routine adult vaccinations. Case 1 involves a 69-year-old male with no prior history of malignancy or immunosuppression, who developed bilateral axilla

Molecular Mechanisms of Resistance to Bispecific Antibodies in Diffuse Large B-Cell Lymphoma.

CD20 × CD3 bispecific antibodies (BsAbs) have emerged as a meaningful therapeutic option for relapsed or refractory diffuse large B-cell lymphoma (DLBCL), redirecting endogenous T cells against malignant B cells independently of major histocompatibility complex-mediated antigen presentation, and have received regulatory approval after at least two prior lines of therapy. However, a substantial proportion of patients experience primary resistance or early relapse, underscoring the need to characterize the underlying biological mechanisms, which are the focus of this review. Several tumor-intrin

Patient-reported time toxicity with bispecific antibody therapies in relapsed/refractory follicular lymphoma and diffuse large B-cell lymphoma.

BACKGROUND: Several bispecific antibodies (BsAbs), including mosunetuzumab, glofitamab, and epcoritamab, are approved for non-Hodgkin lymphoma (NHL). However, real-world patient-centered data evaluating treatment-related time burden and preferences are lacking. METHODS: We conducted an online survey of 120 patients with relapsed/refractory follicular lymphoma (FL; N = 60) and diffuse large B-cell lymphoma (DLBCL; N = 60) receiving BsAbs. Participants reported all-cause healthcare visits and time spent on cancer-related healthcare resource utilization over the past 30 days, including travel, t

Identification of Novel Risk Loci for Common B-Cell Lymphoma Subtypes Through Cross-Trait Analysis with Idiopathic Inflammatory Myopathies.

Objectives: The genetic architecture of B-cell lymphomas is not fully characterized. We aimed to identify novel candidate variants associated with four common B-cell lymphoma subtypes-diffuse large B-cell lymphoma (DLBCL), follicular lymphoma (FL), chronic lymphocytic leukemia (CLL), and marginal zone lymphoma (MZL)-through cross-trait analyses with correlated idiopathic inflammatory myopathies subtypes, dermatomyositis (DM) and polymyositis (PM). Methods: Leveraging shared genetic susceptibility with DM and PM, we applied a pleiotropy-informed conditional false discovery rate (condFDR) method

Single-cell analyses redefine cell heterogeneity signatures in diffuse large B-cell lymphoma and follicular lymphoma: Molecular insights and clinical impacts.

B-cell non-Hodgkin's lymphoma (B-NHL) involves biologically and clinically heterogeneous lymphoid malignancies that arise from B cells in the lymphatic system. Understanding cell heterogeneity in B-NHL is of paramount importance for clinical diagnosis and treatment. In recent years, the rapid development of single-cell technologies has significantly improved B-NHL research in dissecting cell heterogeneity signatures at an unprecedented level. In this review, we aimed to characterize previously unrecognized cell subtypes, genetic and molecular diversity, and the dynamic interplay within the tum

The role of EZH2 dysregulation in the pathogenesis of B-cell lymphomas and its implications as a target therapy.

B-cell lymphomas are a group of heterogeneic diseases. During B-cell development, the germinal center (GC) reaction is a complex process, in which transcripcional programs have an epigenetic regulation. Enhancer of zeste homologue-2 (EZH2) is a histone methyltransferase, which catalyzes trimethylation of histone H3 at lysine 27 (H3K27me3) to control gene transcription critical for cell proliferation, function, differentiation and expansion. It controls the normal biology of germinal B cells. Epigenetic dysregulation is a hallmark of Diffuse large B- cell lymphoma (DLBCL) and follicular lymphom

Clinicopathologic spectrum and diagnostic strategies of intrahepatic neoplasms with prominent inflammatory background.

Intrahepatic neoplastic lesions with prominent inflammatory background (HNLwPIBs) constitute a heterogeneous group of entities that frequently pose substantial diagnostic challenges because of overlapping morphological features. To characterize their clinicopathological spectrum, diagnostic pitfalls, and key distinguishing clues, we retrospectively analyzed 60 cases of HNLwPIBs diagnosed at our institution over a 11-year period (2015-2025). The cohort comprised 8 distinct entities, including lymphoepithelioma-like cholangiocarcinoma (LEL-ICC), lymphocyte-rich hepatocellular carcinoma (LR-HCC),

Clinicopathologic spectrum and outcomes of primary ovarian lymphoma: a retrospective case series.

Primary ovarian lymphoma (POL) is rare and often presents as an adnexal mass, creating diagnostic and management challenges. We performed a retrospective cohort study of 13 women diagnosed with POL between January 2018 and December 2023. Median age was 44 years (range 13-69), with 69.2% being premenopausal. LDH was elevated in 9/13 (69.2%) and CA-125 in 8/10 (80.0%) while CEA and CA19-9 were negative. Histology was heterogeneous: diffuse large B-cell lymphoma (DLBCL, 46.2%), extranodal NK/T-cell lymphoma (23.1%), Burkitt lymphoma (15.4%), marginal zone lymphoma (7.7%), and follicular lymphoma

Genomic heterogeneity in primary cutaneous follicular center lymphomas reveals different clinicopathological subgroups.

Primary cutaneous follicle center lymphomas (PCFCLs) are indolent B-cell neoplasms limited to the skin and effectively managed with local therapies. Distinguishing PCFCL from systemic follicular lymphoma (sFL) with cutaneous involvement (FL_CI) is challenging due to overlapping features. We performed an integrated pathological and genetic analysis of skin samples of 24 PCFCL and 10 FL_CI, which showed subtle pathological changes (decreased BCL2 and CD10, increased CD23 expression in PCFCL), but markedly distinct mutational landscapes. FL_CI exhibited recurrent mutations in chromatin-modifying

A "Double-Hit" Lymphoma With Plasmablastic Features and Aberrant Keratin Expression Likely Transforming From Follicular Lymphoma.

Large B-cell lymphomas may exhibit morphology typical of a diffuse large B-cell lymphoma (DLBCL) or a Burkitt lymphoma. Rarely, they may exhibit either a blastoid morphology or features that are intermediate between a DLBCL and Burkitt lymphoma (the so-called intermediate/blastoid morphology); the term high-grade B-cell lymphoma (HGBL) is used to describe the latter. The presence of MYC and BCL2 rearrangements can be seen either in HGBL or in lymphomas with DLBCL morphology (so-called double-hit lymphoma) and rarely in plasmablastic lymphomas. Herein, we report a rare occurrence of a double-hi

Molecular Insights and Novel Therapies for Lymphoproliferative Disorders.

Hematological malignancies encompass a broad spectrum of relatively rare cancers with diverse biological and clinical characteristics that are capable of affecting individuals across all age groups, though certain subtypes show a predilection for specific age ranges. Advances in next-generation sequencing have greatly enhanced our understanding of the molecular and genetic basis of these diseases, while epigenetic, transcriptional, and proteomic analyses have further clarified their pathogenesis. These developments have shaped the classification and treatment of lymphoma. Updated classificatio

Outcomes of CAR T-Cell Therapy in transformed indolent Non-Hodgkin Lymphomas and de novo DLBCL: A comparative analysis from the Italian CAR T-SIE study.

BACKGROUND: Transformed indolent non-Hodgkin lymphoma (TiNHL) generally has poorer outcomes than de novo diffuse large B-cell lymphoma (dnDLBCL). Although anti-CD19 CAR T-cell therapy is standard for relapsed/refractory (R/R) large B-cell lymphoma (LBCL), its prognostic role across histologies remains unclear. METHODS: We conducted a multicenter, retrospective/prospective observational study comparing R/R TiNHL and R/R dnDLBCL patients treated with commercial CAR T-cell therapy and enrolled in the Italian CART-SIE study. FINDINGS: Among 438 patients, 107 had TiNHL (96 transformed follicular

Rule-Based Algorithm to Identify Recurrent Non-Hodgkin Lymphoma in Electronic Health Data.

PURPOSE: Recurrent cancers are not captured in a standardized way by US tumor registries, making it difficult to conduct research on risk factors for cancer recurrence. We developed rule-based algorithms to be used with electronic health data to identify recurrent cases of diffuse large B-cell lymphoma (DLBCL) and follicular lymphoma (FL). METHODS: Incident DLBCL and FL cases (2000-2018) were identified in tumor registry data at two health plan study sites. We captured pharmacy and procedure codes to indicate first-line treatment initiation. Recurrent cases were defined as those who completed
