Rapid CAR-T manufacturing with low-intensity priming enhances CAR-T cell fitness and limits inflammatory programming in a phase 1 study.

Conventional chimeric antigen receptor (CAR)-T manufacturing requires prolonged ex vivo activation and expansion, which may compromise T cell fitness and promote inflammatory differentiation. Here, we developed PrimeCAR, a 2-day manufacturing strategy combining low-intensity anti-CD3 priming with IL-7 and IL-21. Compared with conventionally manufactured CAR-T cells, PrimeCAR products exhibited a distinct transcriptional state characterized by preserved stemness-associated programs and a PrimeCAR-enriched primed naive T cell (Tn)-like state. Following antigen stimulation, PrimeCAR cells prefere

Disease-specific heterogeneity of C-reactive protein across 21 hematologic disorders reflects divergent inflammatory and hematopoietic phenotypes.

BACKGROUND: C-reactive protein (CRP) is routinely used to quantify systemic inflammation, yet its interpretation in hematologic disorders is complicated by marked biological heterogeneity. We characterized pretreatment CRP distributions across a broad hematologic disease spectrum and examined whether CRP-laboratory associations differed among clinically relevant subtypes. METHODS: This retrospective study included 4,823 patients aged ≥14 years with 21 hematologic disorders evaluated at Nanjing Drum Tower Hospital between 2009 and 2023. Baseline CRP and routine laboratory variables obtained be

Flow Cytometry-Guided Diagnosis of Cortical T-Lymphoblastic Lymphoma Presenting With a Massive Mediastinal Mass and Borderline Bone Marrow Involvement.

BACKGROUND: T-cell lymphoblastic lymphoma (T-LBL) and T-cell acute lymphoblastic leukemia (T-ALL) are considered a single biological entity, distinguished primarily by the extent of bone marrow infiltration. This distinction becomes diagnostically challenging in patients with borderline marrow blast counts and predominantly extramedullary disease, risking overclassification as leukemia. METHODS: We describe a 57-year-old man who presented with dyspnea and a large mediastinal mass causing superior vena cava syndrome, with pleural and pericardial effusions. Diagnostic workup included complete b

Clinical, economic, and quality-of-life outcomes of brexucabtagene autoleucel for the treatment of relapsed/refractory mantle cell lymphoma and B-cell acute lymphoblastic leukemia: A systematic literature review and meta-analysis.

BACKGROUND: Brexucabtagene autoleucel (brexu-cel), a CD19 chimeric antigen receptor T-cell therapy, has shown efficacy in clinical trials for relapsed/refractory (R/R) mantle cell lymphoma (MCL) and B-cell acute lymphoblastic leukemia (B-ALL). However, although effectiveness and safety evidence is well established, real-world evidence (RWE) on health care resource use (HCRU), costs, and health-related quality of life (HRQoL) remains limited. OBJECTIVE: To systematically review and synthesize RWE on clinical, HCRU, and cost outcomes, and both RWE and trial evidence on HRQoL, for brexu-cel in a

Certification or cash prize: the heterogeneous effect of venture competitions.

Cancer cells expressing constitutively active phosphatidylinositol-3 kinase (PI3K) are proliferative regardless of the absence of insulin, and they form dietary restriction (DR)-resistant tumors in vivo. Because the binding of insulin to its receptors activates the PI3K/AKT/mammalian target of rapamycin (mTOR) signaling cascade, activating mutations in the PIK3CA oncogene may determine tumor response to DR-like pharmacological strategies targeting the insulin and mTOR pathways. The anti-diabetic drug metformin is a stereotypical DR mimetic that exerts its anti-cancer activity through a dual me

CD47 blockade-driven necroptosis complements BCL-2 inhibition-driven apoptosis in lymphoid malignancies.

BACKGROUND: Immune checkpoint blockade of CD47 has shown promising results in lymphoid malignancies, with its effects attributed to enabling tumor-cell phagocytosis. However, alternate cytotoxic cell death mechanisms have been reported, potentially contributing to the overall anti-tumor activity. Although previous studies have highlighted a mechanism of caspase-independent cell death, this mechanism has yet to be well-characterized, thereby warranting further investigation to comprehensively understand the anti-tumor mechanism of CD47 blockade to facilitate optimal drug partner selection for c

Reliable leukemia detection via transfer-enhanced Bayesian CNNs.

Accurate and early detection of Acute Lymphoblastic Leukemia (ALL) is critical for timely intervention and improved patient outcomes. However, the development of reliable deep learning models for hematological image analysis is challenged by limited data availability, dataset bias, and the need for trustworthy predictions in clinical settings. In this study, we propose a Bayesian deep learning framework that integrates transfer learning, data augmentation, and uncertainty quantification for robust classification of leukemic and healthy lymphocytes from peripheral blood smear images. Three wide

Evaluation of iron accumulation during childhood cancer treatment.

BACKGROUND: Iron overload is a major concern in pediatric oncology, especially among patients receiving frequent blood transfusions. Although serum ferritin (SF) is widely used as a surrogate marker, T2* magnetic resonance imaging (MRI) of the liver and heart remains the gold standard for quantifying tissue iron deposition. This study evaluated the relationship between serum ferritin levels and T2* MRI findings in pediatric cancer patients, focusing on those with ferritin levels >1,000 µg/L. METHODS: This retrospective study included patients aged 10-25 years diagnosed with malignancies and f

Digitally derived Ki-67 proliferation index for GastroEnteroPancreatic neuroendocrine neoplasms.

Ki-67 proliferation indices (PIs) define the grading of GastroEnteroPancreatic NeuroEndocrine Neoplasms (GEPNENs) and are crucial for therapeutic decisions. The precise Ki-67 assessment relies on manual counting, which is time-consuming, hardly accessible during routine pathological signout and thus usually replaced by the easier eye-estimation/balling method prone to interobserver variability and differences originating from the hot-spot size, localisation and tumor heterogeneity. These discrepancies can significantly affect the final PI resulting in misgrading of GEPNENs with potential adver

T-cell lymphoblastic lymphoma presenting with type B lactic acidosis and acute kidney injury in an adolescent.

Acute kidney injury (AKI) in children is a major cause of morbidity and mortality and is most commonly associated with dehydration, sepsis, or nephrotoxic exposures. In rare instances, however, hematologic malignancies may lead to AKI through direct renal infiltration. We report on a 13-year-old boy who presented with AKI, refractory metabolic acidosis, and severe hyperlactatemia in the absence of sepsis or hypoperfusion. Imaging studies revealed bilateral nephromegaly with heterogeneous renal parenchyma. Kidney biopsy demonstrated diffuse infiltration of the kidneys by T-cell lymphoblastic ly

Time to access matters: patient gains from faster CAR T-cell reimbursement in Europe.

BACKGROUND: Despite clinical efficacy and cost-effectiveness, time to reimbursement for chimeric antigen receptor (CAR) T-cell therapies varies greatly across Europe. We examined these differences, and quantified potential patient benefits with faster access. METHODS: A targeted literature search collated reimbursement statuses for all approved CAR T-cell therapy indications by 1 October 2024. Time to reimbursement decision (TTRD), defined as time between marketing authorization and published reimbursement decision, was assessed. By benchmarking against the TTRD of the three fastest countries

Burkitt Lymphoma-A Guide to Biological Features, Diagnosis and Differential Diagnosis.

Burkitt lymphoma (BL) is an aggressive mature B-cell lymphoma that represents one of the most studied human malignancies. Initially described in equatorial Africa by the Irish surgeon Denis P. Burkitt, African (endemic) Burkitt lymphoma was the first human neoplasm shown to be associated with a virus, the Epstein-Barr virus (EBV), and also the first human neoplasm shown to harbor a recurrent chromosomal aberration, the t(8;14) (q24;q32) translocation that led to the identification of the central role of the MYC gene in tumorigenesis. In this review, we provide a brief historical introduction,

Decoding NOTCH1: From T-Cell Development Guardian to Driver of Pediatric T-Cell Lymphoblastic Lymphoma.

T-cell lymphoblastic lymphoma (T-LBL) is an aggressive malignancy of immature T-cells accounting for a substantial proportion of pediatric non-Hodgkin lymphoma cases. Current chemotherapeutic regimens achieve five-year event-free survival rates of 80-90%, yet relapse occurs in approximately 20% of patients and remains a major therapeutic challenge. This underscores the need for improved, molecularly informed treatment strategies. Recent genomic profiling has highlighted the central role of NOTCH1 signaling in T-LBL pathogenesis. NOTCH1, a transmembrane receptor critical for T-cell differentiat
