Burkitt Lymphoma-A Guide to Biological Features, Diagnosis and Differential Diagnosis.

Burkitt lymphoma (BL) is an aggressive mature B-cell lymphoma that represents one of the most studied human malignancies. Initially described in equatorial Africa by the Irish surgeon Denis P. Burkitt, African (endemic) Burkitt lymphoma was the first human neoplasm shown to be associated with a virus, the Epstein-Barr virus (EBV), and also the first human neoplasm shown to harbor a recurrent chromosomal aberration, the t(8;14) (q24;q32) translocation that led to the identification of the central role of the MYC gene in tumorigenesis. In this review, we provide a brief historical introduction,

Decoding NOTCH1: From T-Cell Development Guardian to Driver of Pediatric T-Cell Lymphoblastic Lymphoma.

T-cell lymphoblastic lymphoma (T-LBL) is an aggressive malignancy of immature T-cells accounting for a substantial proportion of pediatric non-Hodgkin lymphoma cases. Current chemotherapeutic regimens achieve five-year event-free survival rates of 80-90%, yet relapse occurs in approximately 20% of patients and remains a major therapeutic challenge. This underscores the need for improved, molecularly informed treatment strategies. Recent genomic profiling has highlighted the central role of NOTCH1 signaling in T-LBL pathogenesis. NOTCH1, a transmembrane receptor critical for T-cell differentiat

CAR-T cell therapy for pediatric solid tumors: armored CAR-T cells and beyond.

CAR-T cell therapy, which uses endogenous T cells engineered to target specific cancer antigens, is one of the most promising recent developments in the treatment of hematologic malignancies in both children and adults. CAR-T cells have shown tremendous success in treating B-cell lymphoma, acute lymphoblastic leukemia, and multiple myeloma, and they are currently FDA-approved for the treatment of six hematologic malignancies. Its success in solid tumors has been more modest, which has been attributed to several factors including the hostile tumor microenvironment (TME), poor persistence of CAR

Enhanced antitumoral activity of the academic CAR-T ARI0002h against normal and low BCMA-expressing myeloma cells after incorporating a transmembrane CD28 domain.

BACKGROUND: B-cell maturation antigen (BCMA) is the main target for chimeric antigen receptor (CAR)-T cells in multiple myeloma (MM), demonstrating promising outcomes. However, unlike what happens with CART19 in lymphoblastic leukemia and non-Hodgkin's lymphoma, a high proportion of patients will relapse after CAR-T BCMA therapy due to insufficient antigen expression, low CAR-T cell persistence and/or T-cell exhaustion. In other B cell malignancies, second-generation anti-CD19 4-1BB CARs with CD28-transmembrane domain (TMD) have shown high efficacy and a favorable toxicity profile. We have dev

T-cell Acute Lymphoblastic Leukemia Presenting With Bilateral Breast Masses.

T-cell lymphoma involvement of the breast is rare and represents a diagnostic challenge. Here, we present a case of T-cell acute lymphoblastic leukemia (ALL) in a 20-year-old woman who presented in September 2023 with a history of bilateral breast enlargement associated with general weakness, weight loss, and a feeling of heat, which had lasted for about two months. She was examined clinically and found to have huge bilateral breast enlargement. A breast ultrasound (US) was performed and showed a diffuse abnormal heterogeneous parenchymal appearance of both breasts with multiple enlarged bilat

"Per aspera ad astra": the transformative contribution of glucarpidase to the improved management of delayed methotrexate elimination after high-dose therapy.

INTRODUCTION: High-dose methotrexate (HDMTX) is an important therapeutic tool for various malignancies; its use can be associated with severe treatment-emergent toxicities, particularly nephrotoxicity. These can disrupt anticancer treatment and increase morbidity and mortality. Glucarpidase (carboxypeptidase G2), a recombinant bacterial enzyme, rapidly converts toxic levels of circulating methotrexate (MTX) into nontoxic metabolites in patients with delayed MTX elimination and/or at risk of MTX toxicity. The reduction in MTX-associated toxicity and mortality can improve patients' outcomes. MTX

Outcomes of point-of-care manufactured CAR T cell therapy for B cell acute lymphoblastic leukemia and non-Hodgkin lymphoma in Vietnam.

Chimeric antigen receptor (CAR) T cell therapy has transformed the treatment of relapsed/refractory (R/R) B cell acute lymphoblastic leukemia (ALL) and non-Hodgkin lymphoma (NHL), but access remains limited in resource-constrained settings. This phase I study evaluated the safety and feasibility of point-of-care (PoC) manufactured CD19-targeted CAR T cell therapy in Vietnamese patients. Between August 2023 and June 2025, 16 patients, eight with R/R ALL and eight with R/R NHL, were enrolled. All received fresh CAR T cells produced on-site using the CliniMACS Prodigy system, with a median dose o

Relapse of adult B-cell acute lymphoblastic leukemia in the bilateral lacrimal glands: A case report and literature review.

RATIONALE: Extramedullary relapse of acute lymphoblastic leukemia (ALL) involving the orbit is uncommon, and lacrimal gland infiltration is particularly rare in adults. Bilateral lacrimal gland enlargement can be clinically nonspecific and may be misinterpreted as inflammatory or autoimmune disease, potentially delaying diagnosis. This case report describes a rare adult case of extramedullary relapse of B-cell ALL presenting as bilateral lacrimal gland enlargement following allogeneic hematopoietic stem cell transplantation (HSCT). PATIENT CONCERNS: A 55-year-old woman presented with a 4-day

Incidence, clinical features and risk factors of interstitial pneumonia in children with acute lymphoblastic leukemia.

Interstitial pneumonia (IP) is an important pulmonary complication in children with acute lymphoblastic leukemia (ALL), yet its incidence, clinical characteristics, and predictors remain insufficiently defined. We conducted a single-center retrospective study including 484 children diagnosed with ALL and treated at the First Affiliated Hospital of Sun Yat-sen University between February 2002 and January 2022. Among 484 patients, 33 (6.82%) developed IP. The incidence was higher in boys than girls (8.92% vs. 2.94%) and significantly elevated in high-risk versus non–high-risk ALL (16.67% vs. 3.7

UMG1 Defines a Targetable Subset of T-Cell Lymphomas and Enables Precision Immunotherapy With a First-in-Class CD3ε Bispecific Engager.

T-cell lymphomas (TCLs) account for a relatively small fraction of lymphoid malignancies and are characterized by highly aggressive course often refractory to current available therapies. We previously reported potent in vitro and in vivo antitumor activity of a Bispecific T-Cell Engager (UMG1/CD3ε-BTCE) directed against UMG1, a unique CD43 epitope that is abundantly expressed on T-cell acute lymphoblastic leukemia (T-ALL) and diffuse large B-cell lymphoma (DLBCL) cells, while absent in most normal tissues, except thymocytes and a small fraction of peripheral blood T lymphocytes (< 5%). A global multidimensional analysis of the chimeric antigen receptor T-cell therapy clinical trial landscape and development trends.

Chimeric antigen receptor T-cell (CAR-T) therapy has advanced significantly in oncology. To reveal the main trends affecting this field, we collected trial information of 1,908 CAR-T clinical trials using the INFORMA database. Since 2010, the number of CAR-T trials has increased sharply. China (1,006 trials) and the United States (549 trials) accounted for over 80% of all studies. Only 4.2% were sex-restricted studies and only 22% involved children. Although most studies were early phase, the number of phase III/IV trials has been steadily increasing. Research has predominantly focused on hema

Impaired hematopoiesis affects apheresis and CAR T-cell product composition and treatment response.

BACKGROUND: Chimeric antigen receptor (CAR) T-cell therapy has transformed the treatment of hematologic malignancies, but its success depends on obtaining sufficient CD3+ T-cell yields during leukapheresis. This can be difficult in heavily pretreated patients, who often show leukopenia and reduced T-cell fitness. METHODS: We analyzed 166 leukapheresis products from 154 patients undergoing manufacturing of CD19-directed CAR T-cells, including 146 from non-Hodgkin's lymphoma (NHL) and 20 from acute lymphoblastic leukemia (ALL). Collections were performed for commercial CAR T-cell products (axi-

[Expert consensus on the diagnosis and treatment of non-Hodgkin lymphoma in children and adolescents (2026 edition)].

Non-Hodgkin lymphoma (NHL) is a common malignant tumor in children, accounting for approximately 7% of all childhood malignancies, with a higher incidence rate among adolescents. After standardized treatment, the overall survival rate exceeds 80%. Lymphoblastic lymphoma (LBL), mature B-cell non-Hodgkin lymphoma (mB-NHL), and anaplastic large cell lymphoma (ALCL) account for 90% of pediatric and adolescent NHL cases. The Lymphoma Group of the Oncology Branch of the Chinese Medical Association has organized multidisciplinary experts from pediatric oncology, pediatrics, internal medicine, patholo

Two Faces of NOTCH1 in Childhood Lymphoblastic T-Cell Neoplasia: Prognostic Divergence of Mutational and Structural Aberrations.

In pediatric patients, T-cell lymphoblastic lymphoma (T-LBL) survival exceeds 80%. Relapse remains associated with limited curative options. Frontline treatment is largely extrapolated from T-cell acute lymphoblastic leukemia (T-ALL) treatment, reflecting the ongoing debate, whether both entities represent distinct diseases or variants within one spectrum. This review focuses on T-LBL, summarizing shared and distinct clinical, pathological, and biological features in comparison to T-ALL. We address diagnostic and staging challenges, and current limitations of clinical risk stratification. Biom

Frailty, Sarcopenia, and Functional Decline After CAR-T Therapy: The Case for Structured Rehabilitation.

Chimeric antigen receptor T-cell (CAR-T) therapy has revolutionized the treatment landscape of relapsed or refractory hematologic malignancies, offering unprecedented response rates and durable remissions in diseases such as B-cell acute lymphoblastic leukemia, diffuse large B-cell lymphoma, and multiple myeloma. By genetically engineering autologous T cells to recognize tumor-associated antigens, CAR-T therapy enables targeted immune-mediated cytotoxicity against malignant cells. Although early clinical experience has largely focused on acute toxicities including cytokine release syndrome and

Emerging Applications of CAR-T Cell Therapy in Overcoming Resistance and Expanding Targets in Hematologic Malignancies: Insights from Recent Research.

Chimeric antigen receptor T-cell (CAR-T) therapy has revolutionized the treatment outcomes of patients with hematologic malignancies, including B-cell acute lymphoblastic leukemia (B-ALL), diffuse large B-cell lymphoma (DLBCL), and multiple myeloma (MM). However, issues such as antigen escape, T-cell exhaustion, and the limited efficacy of CAR-T cells in acute myeloid leukemia (AML) and T-cell leukemias still persist. This review aims to summarize human trials and achievements from 2015 with a focus on the innovations within the last two years. We discuss strategies to overcome resistance, inc
