Beyond Thresholds: CMV Replication Kinetics as Early Predictors of Clinical Risk in Transplant Recipients.

Cytomegalovirus (CMV) is a pervasive beta herpes virus which establishes latency after primary infection. CMV infection after solid organ transplant is common and associated with negative outcomes for both the allograft and patient. The medications that are available can be associated with significant toxicity and do not provide a cure. Reliable protection from recurrence and freedom from antiviral therapy requires development of CMV-specific cell-mediated immunity (CMI). This can be difficult to achieve after transplant in the setting of immunosuppressive medications due to the complexity of

Recent advances and future perspectives in frontline treatment of peripheral T-cell lymphoma.

Peripheral T-cell lymphoma (PTCL) represents a heterogeneous group of lymphomas with a generally poor prognosis. With recent advances in our understanding of the cellular origins and genomic profiles of PTCL, lymphomas of follicular helper T-cell (TFH) origins with distinct markers and gene mutation profiles are categorized as a single entity, nodal TFH lymphomas-angioimmunoblastic type, follicular type, and not otherwise specified, in the fifth edition of the WHO classification. The standard treatment for PTCL has been CHOP or CHOP-like regimens for decades. The ECHELON-2 trial marked a signi

4D Imaging of Brown Algal Cells.

In vivo imaging of brown algal cells in 3D is extremely challenging because of the presence of pigments, such as fucoxanthin and chlorophyll, that diffract light. Moreover, brown algae live in seawater, a high ionic environment that can change the fluorochrome behavior or cause aggregates. Despite the importance of in vivo monitoring the developmental process of brown algal tissues, 4D imaging (x, y, z, t) on a conventional fluorescence microscope is limited. Here, we propose a detailed protocol using a new orange-emitting fluorochrome, styryl benzoindoleninium sulfonate (SBIS), suitable for l

IL4/IL13 Inhibition via Dupilumab Reduces Malignant T-cell Proliferation and Promotes Antitumor Immunity in Sézary Syndrome.

Patients with Sézary syndrome, the aggressive leukemic variant of cutaneous T-cell lymphoma, have few therapeutic options and a poor prognosis. We previously showed that the IL4/IL13 signaling pathway affects Sézary syndrome tumorigenesis. In this study, we investigated the potential therapeutic effect of REGN668 (dupilumab), an mAb that blocks the IL4/IL13 pathway by targeting the receptors' common IL4Rα subunit. We used single-cell RNA sequencing coupled with T-cell immune repertoire analysis to define the transcriptional changes and molecular mechanisms associated with REGN668 treatment in

A phase 1 study of blinatumomab/lenalidomide in relapsed/refractory B-cell lymphoma: toxicity, efficacy, and correlative analysis.

Despite a recent increase in therapeutic options, patients with relapsed/refractory B-cell non-Hodgkin lymphoma (R/R B-NHL) eventually require novel therapies. We conducted a phase 1 trial of blinatumomab and lenalidomide in R/R B-NHL. Three dose levels representing 2 schedules were explored. The primary end points were adverse events (AEs) and determining the maximum tolerated dose (MTD)/recommended phase 2 dose (RP2D). Thirty-five patients were enrolled, and 34 patients initiated treatment with a median number of prior regimens of 3 (range, 2-8). There were no dose-limiting toxicities (DLTs)

Long-term follow-up of gastrointestinal CAR T-cell lymphoma: homing, clonal expansion, and response to cyclosporine.

Chimeric antigen receptor (CAR) T-cell therapy has emerged as a transformative treatment for hematological malignancies, yet its potential to drive lymphomagenesis poses significant clinical concerns. In this study, we investigated the mechanisms underlying CAR T-cell-associated lymphomagenesis in the gastrointestinal (GI) tract on a single case, focusing specifically on the role of integrin α4β7 expression and a predisposing somatic SH2B3 mutation. We observed oligoclonal CAR T cells homing to, and clonally expanding in, the GI tract, with the dominant expanded clone harboring both a pathogen

Peripheral Blasts at Apheresis as a Risk Factor for Survival following Tisagenlecleucel in Children and Young Adults.

Precursor B-cell acute lymphoblastic leukemia (B-ALL) is the most common childhood malignancy. Relapsed or refractory (r/r) B-ALL carries a dismal prognosis.1-3 CD19-specific CAR T cell therapy has emerged as a promising treatment, yet relapse rates among responders remain high, with 40-50% relapsing post-therapy.4-12 This underscores the need for improved strategies to predict and prevent relapses following CAR T cell therapy. This study reports the impact of peripheral blasts at the time of apheresis on outcomes in pediatric and young adult patients with r/r B-ALL treated with CD19-specific

Early time to relapse as a survival prognosticator in nodal mature T-cell lymphomas: results from the PETAL consortium.

We assessed the overall survival (OS) impact of time to relapse (TTR) in multinational cohorts with independent observational and randomized validation. Patients with nMTCL with frontline complete response were assigned to TTR12 (≤12 months) or without TTR12 based on time to progression or next therapy. OS analyses included modified landmark (m-LM), standard landmark (s-LM), and time-dependent Cox (td-Cox), adjusting for age, histology, and prognostic index for T-cell lymphoma (PIT) score. Across 452 patients, 165 (36.5%) had TTR12, 181 (40%) relapsed at ≥12 months, and 106 (23.5%) remained re

Cross-resistance of Belinostat and Romidepsin in Non-T Follicular Helper Peripheral T-cell Lymphoma Models Suggests Subtype-Specific Implications for Belinostat-CHOP.

The phase III study of romidepsin plus CHOP for peripheral T-cell lymphoma (PTCL) yielded negative results in the overall analysis and showed that outcomes were only improved in the T follicular helper (TFH) subgroup of PTCL, with no benefit in non-TFH PTCL. Belinostat, another histone deacetylase (HDAC) inhibitor, is now the primary candidate to test whether HDAC inhibition can improve treatment outcomes in first-line PTCL. Whether belinostat shares romidepsin's TFH-specific clinical activity remains uncertain, and therefore whether it is better to include or exclude non-TFH subtypes may be a

Clinical and Cytokine Features of Immune Effector Cell-Associated Hemophagocytic Lymphohistiocytosis-Like Syndrome.

UNLABELLED: Immune effector cell-associated hemophagocytic lymphohistiocytosis-like syndrome (IEC-HS) is a poorly characterized inflammatory toxicity of chimeric antigen receptor (CAR) T cells with high risk of mortality. In this study, we describe IEC-HS manifestations in patients with large B-cell lymphoma and B-cell acute lymphoblastic leukemia after CD22-directed CAR T cells. IEC-HS occurred in 19 of 54 patients (35%), including 11 grade 1 and 8 grade 2 or higher. IEC-HS was associated with higher nonrelapse mortality (NRM) yet lower relapse rates. CAR T-cell expansion in peripheral blood

Patterns, risk factors and management of CD19-directed chimeric antigen receptor T-cell therapy failure in CNS lymphoma.

BACKGROUND: CD19-directed chimeric antigen receptor T-cell therapy (CD19-CAR) has yielded encouraging efficacy in CNS lymphomas (CNSL), but most patients ultimately experience progressive disease (PD). Risk factors, progression patterns as well as optimal salvage therapies remain unclear. METHODS: Clinical and radiological characteristics of CD19-CAR failure were therefore retrospectively defined in CNSL treated at Massachusetts General Hospital from 2018 to 2024. PD patterns were defined as local or distant. CNS-progression-free survival from CD19-CAR infusion (CNS-PFS1) and first subsequent

ORY1001 enhances anti-tumor immunity via CXCL10-mediated macrophage modulation in EBV-positive DLBCL.

Epstein-Barr virus-positive diffuse large B-cell lymphoma (EBV+ DLBCL) is an aggressive malignancy characterized by an immunosuppressive tumor microenvironment (TME), where tumor-associated macrophages (TAMs) often exhibit an M2-like phenotype that facilitates immune evasion. Here, we show that the LSD1 inhibitor ORY1001 reprograms the TME of EBV+ DLBCL through CXCL10-mediated modulation of macrophage polarization. Using a reductionist model in which EBV-negative DLBCL cell lines were transfected with LMP1, we demonstrate that ORY1001 induces robust expression of CXCL10, TNF-α and IL-6 in LMP1

Dysregulation of caspase-8 and caspase-9 in T-lymphocyte apoptosis: implications for pathogenesis, diagnosis, and therapeutic targeting in psoriatic arthritis.

BACKGROUND: Apoptotic changes in keratinocytes and alterations in T-lymphocytes were shown to have a role in the progression of psoriatic arthritis (PsA). OBJECTIVES: To determine the possible role of the apoptotic changes in peripheral T-lymphocytes in association with the activity of both caspase-8 and caspase-9, and the expression of B-cell lymphoma 2 (Bcl-2) and Annexin V in the pathogenesis of PsA. In addition, the relationships between these apoptotic-related markers, the clinical severity of PsA, and the expression of inflammatory cytokines tumor necrosis factor (TNF-α) and interleukin-

Serum free fatty acid promotes tumor progression and predicts golidocitinib sensitivity in peripheral T-cell lymphoma.

Peripheral T-cell lymphoma (PTCL) is an aggressive and heterogeneous lymphoma subtype with high chemoresistance and poor prognosis. Common peripheral blood disease biomarkers with therapeutic potential are lacking. In this study, we analyzed the serum metabolic profiles of 557 patients with newly diagnosed PTCL, including 278 extranodal natural killer/T-cell lymphoma (NKTCL), 117 patients with nodal T-follicular helper cell lymphoma (nTFHL), 92 with PTCL, not otherwise specified (PTCL-NOS), 36 with anaplastic lymphoma kinase (ALK)-positive anaplastic large-cell lymphoma (ALCL), and 34 with ALK

Bendamustine in combination with gemcitabine and vinorelbine as salvage regimen for relapsed or refractory peripheral T-cell lymphomas: A retrospective single-centre experience.

The prognosis of peripheral T-cell non-Hodgkin lymphomas (PTCL) is dismal, particularly in the relapsed/refractory (r/r) setting, where 3-year overall survival (OS) is 20%-30%. No superior second-line therapy for PTCL has been universally established, and durable remissions rely on consolidation with allogeneic haematopoietic stem cell transplantation (alloHSCT). Enhancing response rates to salvage therapy is, therefore, crucial to increase transplant eligibility. We retrospectively evaluated the efficacy of bendamustine, gemcitabine and vinorelbine combination (BeGeV) in 24 consecutive patien

Molecular features of response and resistance to glofitamab, a T-cell engager for treatment of large B-cell lymphoma.

T-cell engagers (TCEs) have recently transformed the therapeutic landscape of hematological malignancies, including relapsed or refractory (R/R) B-cell non-Hodgkin lymphoma (B-NHL). However, the variability in patient responses underscores the need for a deeper mechanistic understanding of the factors driving efficacy. Immune cell composition and T-cell functional states are emerging as critical determinants of immunotherapy outcomes. Recent advances in single-cell RNA sequencing (scRNA-seq) technologies have enabled high-resolution characterization of T-cell states, revealing a spectrum from

Mucosa-associated lymphoid tissue lymphoma with monoclonal T-cell expansion.

Prior studies of marginal zone lymphoma of mucosa-associated lymphoid tissue (MALT lymphoma) have mainly emphasized B-cell-intrinsic oncogenic mechanisms and the supportive role of T-cells. However, whether neoplastic B-cells' microenvironment can reciprocally drive T-cell clonality or even promote T-cell neoplastic evolution remains poorly understood. Prompted by an index case where MALT lymphoma evolved into composite lymphoma with peripheral T-cell lymphoma, not otherwise specified (PTCL-NOS), we systematically investigated T-cell receptor (TCR) gene rearrangements in 147 consecutive MALT l

Can baseline red blood cell distribution width predict outcomes in peripheral T-cell lymphoma patients?

Peripheral T-cell lymphoma (PTCL) is a heterogeneous subtype of non-Hodgkin lymphoma with a poor prognosis. Red blood cell distribution width (RDW) is a hematological parameter reflecting variability in erythrocyte volume and has been associated with prognosis in various diseases. Therefore, we conducted a retrospective study on 629 newly diagnosed patients with PTCL to explore the relationship between baseline RDW, prognosis, and response to first-line treatment. The cutoff value for baseline RDW was 14.45%. Patients with baseline RDW ≥ 14.45% had a lower overall response rate than those with

CAR-T cells with the CD38-CD73-Tim-3-HLA-DR+ phenotype predict the efficacy of tisagenlecleucel as a treatment for B cell precursor ALL.

Anti-CD19 chimeric antigen receptor T cell (CAR-T) therapy is highly effective for B cell precursor acute lymphoblastic leukemia (BCP-ALL); however, approximately half of the patients relapse. Thus, there is an urgent need to identify factors that improve efficacy. This study enrolls 19 patients with BCP-ALL (16 children and 3 young adults) who receive tisagenlecleucel. Infusion products, peripheral blood, and bone marrow samples are obtained before and after CAR-T cell infusion. Single-cell analysis reveals that central memory CARpos T cells increase in long-term responders, whereas CXCR3+CD3
