Combating CD24 to overcome CD19 CAR-T resistance via induction of innate and adaptive immunity in B-cell malignancies.

CD24 is highly expressed in aggressive B-cell malignancies and represents a novel immunologic vulnerability to overcome resistance to CD19 chimeric antigen receptor T-cell (CAR-T) therapy. We developed a CD24 × CD3 bispecific antibody (CD24 BsAb) and CXCR3-armed CD24 CAR-T cells, both exhibiting potent antitumor activity in lymphoma and leukemia models. CD24 BsAb and CD24 CAR-T overcome CD19 CAR-T resistance driven by antigen loss and apoptosis defects in vitro and in vivo. Beyond direct cytotoxicity, CD24-targeted T cells reprogram macrophages toward an M1-like phenotype and enhance phagocyti

Peripheral CD4+ T-Cell Reserve as a Potential Biomarker of CD20 × CD3 Bispecific Antibody Outcomes and BTK Inhibitor-Associated Immune Modulation in B-Cell Lymphoma.

Bispecific antibodies (BsAbs) targeting CD20 × CD3 have achieved remarkable efficacy in relapsed or refractory (R/R) B-cell lymphoma, yet clinical responses remain heterogeneous and predictive biomarkers are lacking. We retrospectively evaluated associations between peripheral immune profiles and outcomes in 40 patients treated with CD20 × CD3 BsAbs and explored BTK inhibitor-associated immune changes using a paired BTK inhibitor cohort, functional assays, mouse models, and single-cell RNA sequencing. Higher baseline CD4+ T-cell counts correlated with superior response rates and prolonged prog

All-Oral PI3Kδ and HDAC Inhibition with Parsaclisib Plus Chidamide and Step-Down Maintenance in Relapsed or Refractory Peripheral T-Cell Lymphoma: A Phase Ib Trial.

Single-agent targeted therapies for relapsed or refractory peripheral T-cell lymphoma (R/R peripheral T-cell lymphoma [PTCL]) produce modest, short-lived responses, underscoring the need for tolerable, chemotherapy-free combinations. In an open-label, single-arm Phase Ib trial at two tertiary hospitals, we evaluated parsaclisib plus chidamide with step-down maintenance. Adults with R/R PTCL received parsaclisib 10-20 mg once daily plus chidamide 20 mg twice weekly for 8 weeks using 3+3 escalation; patients without progression or unacceptable toxicity continued parsaclisib 2.5 mg once daily plu

Advances in diagnosis and treatment of angioimmunoblastic T-cell lymphoma.

Angioimmunoblastic T-cell lymphoma (AITL) is a peripheral T-cell lymphoma (PTCL) that originates from follicular helper T cells (Tfh) and represents a rare subtype of non-Hodgkin lymphoma (NHL). The disease is characterized by unique morphological, molecular, and clinical features, and exhibits aggressive behavior. Currently, there is no standard treatment protocol, and patients treated with first-line CHOP-like chemotherapy show low remission rates, high recurrence rates, and poor overall prognosis. In recent years, with the in-depth study of the molecular genetic mechanism of AITL, various n

Graft-Versus-Tumour Effect Following Allogeneic Haematopoietic Stem Cell Transplantation in a Paediatric Patient With Mogamulizumab-Treated Primary Cutaneous Peripheral T-Cell Lymphoma.

INTRODUCTION: Primary cutaneous peripheral T-cell lymphoma, not otherwise specified (pcPTCL-NOS), is exceptionally rare in children, and evidence guiding diagnosis and treatment is limited. CASE PRESENTATION: A 10-year-old boy with pcPTCL-NOS refractory to chemotherapy received mogamulizumab prior to allogeneic haematopoietic stem cell transplantation (allo-HSCT). Despite a 60-day washout period, he developed prolonged steroid-refractory acute graft-versus-host disease requiring ruxolitinib and extracorporeal photopheresis. Concurrently, transient skin lesions developed at previous lymphoma s

Immunophenotyping of CD8+ T Cells in Classical Hodgkin Lymphoma Reveals a Shift Toward Intermediate Exhaustion Following PD-1 Blockade.

The immune microenvironment of classical Hodgkin lymphoma (cHL) is characterized by rare Hodgkin-Reed-Sternberg (HRS) cells surrounded by dysfunctional lymphocytes. HRS cells exploit the PD-L1/PD-1 axis to suppress antitumor immunity. Although PD1 blockade is effective in some relapsed cHL patients, the mechanisms of resistance remain unclear. We investigated T-cell dynamics in cHL using whole-exome sequencing (WES) of plasma circulating tumor DNA and longitudinal single-cell RNA and T-cell receptor sequencing of peripheral blood lymphocytes from a relapsed patient before and after anti-PD1 th

Regulation of follicular helper T cell subset differentiation in health and disease.

Follicular helper T (TFH) cells play a crucial role in adaptive humoral immunity. They provide help to B cells in the germinal centers (GCs), driving the affinity maturation of antigen-specific antibodies and class switch recombination. TFH cells possess profound heterogeneity in spatial distribution (e.g., GC-TFH, follicular mantle-TFH, circulating TFH, peripheral TFH) and phenotypic characteristics (e.g., TFH1, TFH2, TFH17), which enable TFH cells to respond to diverse immune challenges. This review summarizes the recent progress, including our data, on the TFH regulatory network driven by B

Impact of Residual Disease Localization Before Allogeneic Hematopoietic Stem Cell Transplantation on Outcomes in Adult T-Cell Leukemia-Lymphoma.

Allogeneic hematopoietic stem cell transplantation (allo-HCT) is the only curative approach for adult T-cell leukemia-lymphoma (ATL). However, preventing post-transplant relapse remains challenging. While pre-transplant disease status is known to be associated with post-transplant outcomes, the impact of residual disease (RD) localization at the time of allo-HCT has not been well described. In this retrospective study of 33 patients with aggressive ATL who underwent allo-HCT, patients were classified into two risk groups based on RD sites: a low-risk group (no lymph node [LN] or extranodal [EN

Microneedle-Assisted Cell Delivery and Therapy.

STUDY DESIGN: Secondary analysis. OBJECTIVE: To investigate the Fear-Avoidance Beliefs Questionnaire (FABQ) for its ability to predict 6-month outcomes for patients with low back pain (LBP) participating in physical therapy clinical trials. BACKGROUND: Consistent evidence suggests that fear-avoidance beliefs are predictive of short-term outcomes for patients with LBP. However, proposed cut-off scores have not been widely investigated for longer-term outcomes in samples of patients receiving physical therapy. METHODS AND MEASURES: Subjects (n = 160) were participants in 2 separate randomized

Identification and Validation of BATF3 as a Promising Biomarker Gene for Peripheral T-cell Lymphoma.

BACKGROUND: Peripheral T-cell lymphoma (PTCL) is a rare and heterogeneous group of hematological malignancies. Treatment options are limited and often unsatisfactory, leading to a poor prognosis in most subtypes. OBJECTIVE: This study aimed to identify potential biomarker genes for PTCL and to explore the underlying mechanisms by integrating machine learning, Mendelian Randomization (MR), and experimental validation. METHODS: Microarray datasets (GSE6338, GSE14879, and GSE59307) were downloaded from the Gene Expression Omnibus database. Differential expression analysis was conducted to ident

The Characteristics of Peripheral Blood Lymphocyte Subsets in HIV-related Diffuse Large B-cell Lymphoma Patients and Their Impact on Treatment Efficacy.

INTRODUCTION: Peripheral blood lymphocyte subsets have been shown to influence prognosis in HIV-associated Diffuse Large B-Cell Lymphoma (HIV-DLBCL), a rare and highly aggressive form of non-Hodgkin's lymphoma linked to immunosuppression and abnormal B-cell proliferation. To lay the foundation for individualized therapy based on factors such as CD4+/CD8+ ratio and Treg/NK cell characteristics, this retrospective study was conducted to explore the variations in lymphocyte subset levels. METHODS: Overall, 51 HIV-DLBCL patients, 50 DLBCL patients, and 42 Healthy Donors (HD) were enrolled in the

Beyond Thresholds: CMV Replication Kinetics as Early Predictors of Clinical Risk in Transplant Recipients.

Cytomegalovirus (CMV) is a pervasive beta herpes virus which establishes latency after primary infection. CMV infection after solid organ transplant is common and associated with negative outcomes for both the allograft and patient. The medications that are available can be associated with significant toxicity and do not provide a cure. Reliable protection from recurrence and freedom from antiviral therapy requires development of CMV-specific cell-mediated immunity (CMI). This can be difficult to achieve after transplant in the setting of immunosuppressive medications due to the complexity of

Recent advances and future perspectives in frontline treatment of peripheral T-cell lymphoma.

Peripheral T-cell lymphoma (PTCL) represents a heterogeneous group of lymphomas with a generally poor prognosis. With recent advances in our understanding of the cellular origins and genomic profiles of PTCL, lymphomas of follicular helper T-cell (TFH) origins with distinct markers and gene mutation profiles are categorized as a single entity, nodal TFH lymphomas-angioimmunoblastic type, follicular type, and not otherwise specified, in the fifth edition of the WHO classification. The standard treatment for PTCL has been CHOP or CHOP-like regimens for decades. The ECHELON-2 trial marked a signi

IL4/IL13 Inhibition via Dupilumab Reduces Malignant T-cell Proliferation and Promotes Antitumor Immunity in Sézary Syndrome.

Patients with Sézary syndrome, the aggressive leukemic variant of cutaneous T-cell lymphoma, have few therapeutic options and a poor prognosis. We previously showed that the IL4/IL13 signaling pathway affects Sézary syndrome tumorigenesis. In this study, we investigated the potential therapeutic effect of REGN668 (dupilumab), an mAb that blocks the IL4/IL13 pathway by targeting the receptors' common IL4Rα subunit. We used single-cell RNA sequencing coupled with T-cell immune repertoire analysis to define the transcriptional changes and molecular mechanisms associated with REGN668 treatment in

A phase 1 study of blinatumomab/lenalidomide in relapsed/refractory B-cell lymphoma: toxicity, efficacy, and correlative analysis.

Despite a recent increase in therapeutic options, patients with relapsed/refractory B-cell non-Hodgkin lymphoma (R/R B-NHL) eventually require novel therapies. We conducted a phase 1 trial of blinatumomab and lenalidomide in R/R B-NHL. Three dose levels representing 2 schedules were explored. The primary end points were adverse events (AEs) and determining the maximum tolerated dose (MTD)/recommended phase 2 dose (RP2D). Thirty-five patients were enrolled, and 34 patients initiated treatment with a median number of prior regimens of 3 (range, 2-8). There were no dose-limiting toxicities (DLTs)

Long-term follow-up of gastrointestinal CAR T-cell lymphoma: homing, clonal expansion, and response to cyclosporine.

Chimeric antigen receptor (CAR) T-cell therapy has emerged as a transformative treatment for hematological malignancies, yet its potential to drive lymphomagenesis poses significant clinical concerns. In this study, we investigated the mechanisms underlying CAR T-cell-associated lymphomagenesis in the gastrointestinal (GI) tract on a single case, focusing specifically on the role of integrin α4β7 expression and a predisposing somatic SH2B3 mutation. We observed oligoclonal CAR T cells homing to, and clonally expanding in, the GI tract, with the dominant expanded clone harboring both a pathogen
