Screening of promising chemotherapeutic candidates from plants against human adult T-cell leukaemia/lymphoma (X): withanolides from Physalis pruinosa.

Adult T-cell leukaemia/lymphoma (ATL) is a malignancy of mature peripheral T-lymphocytes caused by human T-cell lymphotropic virus type I (HTLV-I). Administering conventional chemotherapeutic regimens used against malignant lymphomas to patients with ATL results in poor therapeutic outcomes. Previously, we isolated withanolides from the aerial parts of Physalis pruinosa and observed potent activity against leukaemia cell lines. The most effective compound showed selective toxicity against HTLV-1-infected T-cell lines compared with normal peripheral blood mononuclear cells following analysis of

Are CHOP-plus really CHOP-minus propositions in the treatment of PTCL? A comprehensive assessment of the strategy.

Peripheral T-cell lymphomas (PTCL) are a rare, heterogeneous subset of non-Hodgkin lymphomas (NHL) that are biologically and clinically distinct from B-cell lymphomas. They are typically resistant to chemotherapy and associated with poor outcomes compared to most B-cell malignancies. Despite this, CHOP (cyclophosphamide, doxorubicin, vincristine, and prednisone) or CHOP-like regimens remain the standard frontline therapy, even though pivotal studies establishing CHOP in NHL included only B-cell patients, with no evidence for PTCL. For decades, efforts to improve outcomes have focused on "CHOP-

Treosulfan/fludarabine versus thiotepa/busulfan/fludarabine for allogeneic haematopoietic cell transplantation in lymphoma in the post-transplant cyclophosphamide era: A GETH-TC study.

Allogeneic haematopoietic cell transplantation (alloHCT) remains a potentially curative strategy for relapsed or refractory lymphoid malignancies, even in the post-chimeric antigen receptor T-cell and bispecific antibody era. While reduced-intensity conditioning regimens offer lower non-relapse mortality (NRM), relapse rates remain high, and optimal conditioning strategies in the setting of post-transplant cyclophosphamide (PTCy) prophylaxis remain undefined. In this retrospective, international multicentre study, the primary end-point was NRM. We compared treosulfan/fludarabine (Treo/Flu) ver

FORTRESS-CART: Focused Observation and Risk Testing for Robust Evaluation of Safety in Chimeric Antigen Receptor T-Cell Therapy (CAR-T): Real-World Predictors of Second Primary Malignancies (SPMs) Among Patients Treated With CAR-T.

BACKGROUND: Chimeric antigen receptor-T (CAR-T) cell therapies are highly effective in treating hematologic malignancies. Cases of second primary malignancies (SPMs) have been reported post-treatment, although risk factors remain unclear. This real-world study evaluated predictors of SPMs among patients treated with CAR-T. METHODS: Patients with multiple myeloma (MM) or lymphoma/leukemia treated with CAR-T were identified from the Komodo Research Database (January 1, 2016-June 30, 2024). Incidences of SPMs and hematologic SPMs were evaluated following CAR-T infusion (index date) and predictor

Synthetic lethality of MCL-1 inhibition and CAR-T therapy in aggressive B-cell lymphoma.

Aggressive B-cell lymphomas, driven by MYC overexpression, exhibit rapid progression, resistance to therapies, and poor survival. While chimeric antigen receptor (CAR)-engineered T cells have demonstrated remarkable clinical efficacy in B-cell lymphomas, nearly half of patients who initially respond to CAR-T therapy eventually develop resistance and disease progression. In this study, we report the presence of residual drug-tolerant persister (DTP) and resistant lymphoma cells remaining within a highly immunogenic tumor microenvironment (TME) induced by the MCL-1 inhibitor (MCL-1i) S63845. Ibrutinib exposure correlates with improved efficacy of CAR T cells in patients with mantle cell lymphoma.

The pivotal ZUMA-2 trial evaluated the chimeric antigen receptor (CAR) T-cell therapy brexucabtagene autoleucel (brexu-cel) for the treatment of relapsed/refractory (R/R) mantle cell lymphoma (MCL), and ultimately led to the approval of brexu-cel for R/R MCL. Here we report a retrospective analysis of data and samples from ZUMA-2, analyzed by exposure to class of Bruton tyrosine kinase inhibitor (BTKi; first-generation ibrutinib vs second-generation acalabrutinib) in a treatment line before brexu-cel. We found that the ibrutinib-exposed group had significantly greater duration of progression-f

Case Report: Extracorporeal photopheresis for cutaneous lupus erythematosus induces putatively atheroprotective B and T cell responses.

Extracorporeal photopheresis (ECP) involves the reinfusion of autologous peripheral blood lymphocytes rendered apoptotic by in vitro exposure to psoralen and ultraviolet A light. Antigenic determinants presented by apoptotic lymphocytes, primarily T cells, elicit immunomodulatory responses that have shown therapeutic benefit in several conditions, including cutaneous T-cell lymphoma, graft-versus-host disease, and various inflammatory/autoimmune disorders. We treated with ECP a 41-year-old woman diagnosed with cutaneous lupus erythematosus and concomitant hypercholesterolemia, achieving a mark

Patterns of care and clinical outcomes in systemic peripheral T-cell lymphoma: the LEO-MER prospective cohort study.

Few prospective benchmark studies exist to characterize the evolving contemporary real-world practice for peripheral T-cell lymphoma (PTCL). We report the patterns of first-line care and outcomes for 720 patients with systemic PTCL enrolled in 2 related prospective cohort studies, Lymphoma Epidemiology of Outcomes (LEO) from 2015 to 2020 and Molecular Epidemiology Resource (MER) from 2002 to 2015, both followed to 2024. The primary end points were event-free survival (EFS) and overall survival (OS) using Kaplan-Meier estimator and Cox regression model. Secondary end points included correlation

Combined microbial inoculation improves plant growth, nutrient acquisition, and soil biological functioning in Opuntia ficus-indica under water deficit.

UNLABELLED: The productivity of Opuntia ficus-indica (L.) Mill. in arid and semi-arid environments is strongly constrained by water scarcity and limited soil biological activity. This study evaluated the effects of plant growth-promoting rhizobacteria (PGPR) consortia (Cs1 and Cs2), arbuscular mycorrhizal fungi (AMF), and their combined inoculations (Cs1&AMF and Cs2&AMF) on plant growth, nutrient acquisition, soil physicochemical properties, enzymatic activities, and microbial functional diversity under non-stressed and water-stressed conditions (95% and 20% of field capacity, respectively). Autologous stem cell transplantation in major T-cell lymphoma entities: An analysis by the EBMT Lymphoma Working Party.

Autologous stem cell transplantation (auto-SCT) is an established treatment for peripheral T-cell lymphoma (PTCL) to consolidate first remission and for patients with relapsed/refractory (r/r) disease. We aimed to examine the outcomes of patients with PTCL NOS, AITL, and ALK-negative ALCL undergoing auto-SCT between 2010 and 2022 and reported to EBMT. Adult patients with major T-cell entities who had received auto-SCT either up-front or for r/r disease were included. 2082 patients underwent up-front and 1249 salvage auto-SCT. Three-year progression-free (PFS) and overall survival (OS) after up

From Cell Lines to Avatars: Charting the Future of Preclinical Modeling in T-Cell Malignancies.

T-cell malignancies represent a complex spectrum of clinically and biologically heterogeneous diseases. Effective translational research and drug development are critically dependent on preclinical models that faithfully recapitulate this diversity. This review analyzes the current preclinical landscape, identifying a profound disparity between the clinical spectrum of T-cell neoplasms and the available in vitro tools. We demonstrate that the existing armamentarium of cell lines is heavily skewed, with an abundance of models for T-cell lymphoblastic leukemia/lymphoma (T-ALL), cutaneous T-cell

At the Crossroads of Lineage: Secondary Malignancies After CAR-Based Immunotherapy.

CD19-directed chimeric antigen receptor (CAR) T-cell therapies have been instrumental in improving outcomes of refractory or relapsed B-cell malignancies. However, there have been safety concerns due to recent reports of second primary malignancies (SPMs) related to CAR T-cell therapies. We reviewed articles from Embase, PubMed, and Cochrane Library records and included SPM case reports as well as cohort studies. Across published cohorts, secondary cutaneous or peripheral T-cell lymphoma (PTCL) after diffuse large B-cell lymphomas (DLBCLs) have been reported at low incidence (generally in the

CD19 chimeric antigen receptor T-cell therapy for the treatment of a patient with refractory Burkitt lymphoma after kidney transplant.

BACKGROUND AIMS: CD19 chimeric antigen receptor (CAR) T-cell therapy has shown promise in treating post-transplant lymphoproliferative disorder, but the risk of graft rejection is acknowledged and there is no consensus on the use of calcineurin inhibitors with CAR T-cell therapy. METHODS: We report a kidney transplant recipient with refractory Burkitt lymphoma who received cyclosporine A (CsA) following CD19 CAR T-cell infusion. Serial peripheral blood sampling was performed before and after CsA administration, and samples were analyzed via single-cell RNA sequencing. RESULTS: The patient ex

Beyond the Rash: Cutaneous Peripheral T-Cell Lymphoma as a Diagnostic Pitfall in Primary Care.

Cutaneous manifestations of T-cell lymphomas may resemble common dermatologic or infectious conditions, increasing the risk of delayed recognition. We report a 78-year-old woman with known peripheral T-cell lymphoma (PTCL) who presented with a rapidly progressive, diffuse erythematous rash accompanied by cervical lymphadenopathy. Initial evaluation prioritized more common etiologies; however, inpatient medicine broadened the differential diagnosis, prompting early hematology and oncology consultation. Contrast-enhanced CT of the neck demonstrated multiple enlarged, partially necrotic left cerv

Combined oral PI3Kδ inhibitor linperlisib and HDAC inhibitor chidamide in relapsed/refractory peripheral T-cell lymphoma: a multicenter phase 1 trial.

This prospective, phase I trial evaluated the safety and efficacy of linperlisib plus chidamide in patients with relapsed/refractory peripheral T-cell lymphoma (r/r PTCL). Linperlisib was administered at three dose levels (40, 60, and 80 mg) following a standard "3 + 3" scheme. Fourteen patients were enrolled, including six with peripheral T-cell lymphoma, not otherwise specified and eight with nodal T-follicular helper lymphoma. No dose-limiting toxicities occurred. Linperlisib 80 mg daily was selected as the recommended phase II dose. The most common grade ≥3 adverse events were hematologic.

Non-ICANS neurotoxicities CD19-directed CAR T-cell therapy and the emergence of movement and neurocognitive treatment-emergent adverse events: a case report.

We report a 63-year-old male patient with diffuse large B-cell lymphoma (DLBCL) who developed delayed-onset neurotoxicity on day +22 following CD19-directed CAR T-cell therapy with axicabtagene ciloleucel (axi-cel), after an initial episode of CRS and ICANS. The underlying disease had relapsed with secondary central nervous system (CNS) involvement, highlighting a high-risk setting for neurotoxicity. Unlike classical ICANS, the syndrome featured progressive gait ataxia and hypokinetic movement disturbances-clinical hallmarks of so-called movement and neurocognitive treatment-emergent adverse e

Survival Trends and Prognostic Modeling in ALK-Positive Anaplastic Large Cell Lymphoma: A Population-Based Study in the Brentuximab Vedotin Era.

BACKGROUND: ALK-positive anaplastic large cell lymphoma (ALK+ ALCL) is a rare subtype of peripheral T-cell lymphoma with traditionally favorable prognosis. The introduction of brentuximab vedotin (BV) has significantly impacted treatment outcomes, but the long-term survival trends and predictive factors for this population remain underexplored. METHODS: A total of 1548 patients diagnosed with ALK+ ALCL between 2004 and 2017 were identified from the Surveillance, Epidemiology, and End Results (SEER) database. Patients were categorized into two eras: pre-BV (2004-2010, n = 795) and post-BV (201
