Curative combination chemotherapy is available for many patients with aggressive non-Hodgkin's lymphoma (NHL); however, treatment of elderly patients with these regimens is difficult due to excessive toxicity. From 1983 to 1988 the authors treated 26 patients 65 years and older with aggressive NHL with a novel 8-week chemotherapy regimen containing bleomycin, etoposide, cyclophosphamide, doxorubicin, methotrexate with leucovorin, and prednisone (BECALM), designed to preserve dose intensity and minimize toxicity. Small bowel obstruction (SBO) as a late complication of the treatment of Hodgkin's disease was found in 32 of 398 (8%) patients followed over a 20 year period. The vast majority of these patients had an initial staging laparotomy as part of staging protocols. The causes of mortality of 3,649 white and 397 non-white male U.S. embalmers and funeral directors, who had died between 1975 and 1985, were examined in a proportional mortality study. Of 95 young patients with non-Hodgkin's lymphoma treated at the National Cancer Institute on Pediatric Branch Protocol 7704, 18 (18.9%) had bone marrow involvement prior to treatment. Fine-needle aspirates from 12 cases of malignant non-Hodgkin's lymphoma of the oral cavity were cytologically classified according to the Kiel classification. Ten lymphomas were high-grade and two were classified as being low-grade malignancies. Of 95 young non-Hodgkin's lymphoma patients entered consecutively on the National Cancer Institute (NCI) Protocol 7704, 26 (27.4%) had involvement of one or more bones. Of 117 patients in remission for at least 12 months after chemotherapy for confirmed Burkitt's lymphoma, 14 subsequently relapsed.
