CD4+CD56+ lineage negative hematopoietic neoplasm: so called blastic NK cell lymphoma.

Blastic natural killer (NK) cell lymphoma is a rare neoplasm characterized by blastoid tumor cells expressing CD4 and CD56, with predominant skin involvement. Although this tumor has been regarded as a neoplasm related to NK cell, recent studies suggested that it is derived from plasmacytoid dendritic cells, but not from NK cell. Herein we report 4 cases of CD4+CD56+ lineage marker-blastic NK cell lymphomas with a review of literatures. The patients were 3 men and one woman. Three of them were young (17, 18, and 22 yr old). Three patients had skin lesions, at initial presentation in two patien

NK-cell neoplasms in Japan.

Neoplasms putatively originating from precursor and mature natural killer (NK) cells are rare, and their clinical features are unclear. A nationwide survey was performed in Japan to clarify the clinical features of these neoplasms diagnosed between 1994 and 1998, and data for 237 patients who met the criteria for putative NK cell-lineage neoplasms were analyzed. Among them, 11 had myeloid/NK-cell precursor acute leukemia, 15 blastic NK-cell lymphoma, 21 precursor NK-cell acute lymphoblastic leukemia, 22 aggressive NK-cell leukemia/lymphoma, 149 nasal-type NK-cell lymphoma (123 nasal and 26 ext

Allogeneic haematopoietic stem cell transplantation as a promising treatment for natural killer-cell neoplasms.

The efficacy of allogeneic haematopoietic stem-cell transplantation (allo-HSCT) for natural killer (NK)-cell neoplasms is unknown. We investigated the results of allo-HSCT for NK-cell neoplasms between 1990 and 2003 through questionnaires. After reclassification by a haematopathologist, of 345 patients who underwent allo-HSCT for malignant lymphoma, 28 had NK-cell neoplasms (World Health Organization classification): extranodal NK/T-cell lymphoma (n=22), blastic NK-cell lymphoma (n=3), and aggressive NK-cell leukaemia (n=3). Twelve were chemosensitive and 16 chemorefractory. Twenty-two had mat

[Agranular CD4+/CD56+ haematodermic neoplasm (Precursor haematologic neoplasm)].

The agranular CD4+/CD56+ haematodermic neoplasm (so-called blastic NK-cell lymphoma) represents a distinct clinicopathologic entity and it is characterised by its clinical presentation (skin tropism, bone marrow involvement with or without leukemic phase, very poor prognosis) and the common expression of the T helper CD4 as well as the NK cell marker CD56. The authors present an 86 year old male patient with hemorrhagic macules, plaques and hemorrhagic flat nodules mainly on the trunk without any complaints. The skin biopsy revealed an agranular CD4+/CD56+ hematodermic tumor. During the medica

Long-term remission of blastic natural killer-cell lymphoma after autologous peripheral blood stem-cell transplantation.

We report here a case of blastic natural killer (NK)-cell lymphoma treated successfully with autologous peripheral blood stem-cell transplantation (APBSCT). A 57-year-old man had skin tumors and was diagnosed as having blastic NK-cell lymphoma by tumor biopsy. Skin, lymph nodes, left tonsil, and bone marrow were involved at presentation. Chemotherapy led to complete remission (CR). To sustain CR, the patient underwent high-dose chemotherapy in combination with 12 Gy of total-body irradiation (TBI) followed by autologous peripheral blood stem-cell rescue. He showed rapid recovery of hematopoies

CD56-positive acute lymphoblastic leukemia.

We report a patient with lung cancer who developed CD56-positive acute lymphoblastic leukemia. He was referred to our hospital for thrombocytopenia. Atypical cells were found in the blood and the bone marrow. These cells were immunophenotypically positive for CD3epsilon, CD56, and terminal deoxynucleotidyl transferase, and negative for surface CD3, CD4, CD19, CD33, and myeloperoxidase. A small proportion of leukemic cells express CD13. There were no rearrangements of T-cell receptor (TCR)-beta, TCR-gamma, or immunoglobulin heavy chain. No Epstein-Barr virus was detected. [Morphological and immunohistochemical characteristics of blastic NK-cell lymphoma].

This is a review characterizing a rare skin tumor which in the WHO-EORTC classification of skin lymphoma is called CD4+/CD5+ hematodermal tumor (blastic NK-cellular lymphoma). Analysis of clinical picture and results of histological investigation of tumor nodules in the skin and bone marrow does not allow one to reveal specific signs sufficient for diagnosis. Immunohistochemical study with panel of markers including CD4, CD56, CD68, TdT is necessary for this tumor diagnosis.

The new World Health Organization-European Organization for Research and Treatment of Cancer classification for cutaneous lymphomas: a practical marriage of two giants.

Following consensus meetings of the two parent organizations, a new World Health Organization-European Organization for Research and Treatment of Cancer (WHO-EORTC) classification for primary cutaneous lymphomas has recently been published. This important development will now end the ongoing debate as to which of these was the preferred classification. The new classification will facilitate more uniformity in diagnosis, management and treatment of cutaneous lymphomas. In particular, it provides a useful distinction between indolent and more aggressive types of primary cutaneous lymphoma and pr

The pathology of NK-cell lymphomas and leukemias.

Natural killer (NK) cell lymphomas and leukemias are a rare but clinically important group of neoplasms. Most of these tumors are aggressive, with a high rate of mortality. They include extranodal NK/T-cell lymphomas of nasal type and aggressive NK-cell leukemias. Both are Epstein-Barr virus (EBV) associated and show similar epidemiologic features. A closely related entity seen mainly in children is hydroa vacciniforme-like lymphoma, which also is EBV positive. EBV influences the pathophysiology of these tumors, through the induction of cytokines and chemokines. The differential diagnosis of N

Blastic CD56+ natural killer-cell lymphoma with primary cutaneous manifestation.

T/natural killer-cell lymphomas belong to a heterogeneous group of non-Hodgkin lymphomas with predominant extranodal, often cutaneous, manifestations. In contrast to B- and T-cell lymphomas, T/NK-cell lymphomas were only recently regarded as a distinct entity. These rather aggressive malignancies arise from cytotoxic T cells, NK-cells or NK-like T cells, which share several phenotypic and functional properties. We report a man with a blastic NK-cell lymphoma with nodular skin infiltrations as the leading clinical manifestation of the disease. Complicated of tuberculosis, the patient died withi

Fine-needle aspiration cytology of blastic natural killer-cell lymphoma (CD4+ CD56+ hematodermic neoplasm).

We describe fine-needle aspiration (FNA) cytology findings of a case of blastic natural killer (NK)-cell lymphoma. The patient was a 68-year-old man who was diagnosed with cutaneous blastic NK-cell lymphoma involving his forehead. He developed a 1.5 cm, right pre-auricular lymph node five months later. FNA cytology revealed a monomorphic population of medium-sized lymphoid cells with a dispersed single cell arrangement. The tumor cells contained eccentrically located nuclei with a plasmacytoloid appearance and moderate amount of grayish cytoplasm without azurophilic granules. The nuclei were r

Blastic NK-cell-like lymphoma with T-cell receptor gene rearrangement.

Blastic natural killer (NK) cell lymphoma is very rare but has been recently classified as a distinct entity in WHO classification. However, the classification remains controversial, and the clinicopathologic spectrum is not completely understood. We report a unique case of cutaneous CD4(+) CD56(+) malignancy with a typical clinical presentation and immunophenotype of blastic NK-cell lymphoma in a 15-year-old Guamanian girl. The skin was the only site involved by the lymphoma. Molecular study showed clonal T-cell receptor gamma gene rearrangement. The patient has been disease-free till now (mo

Childhood blastic NK cell leukemia successfully treated with L-asparagenase and allogeneic bone marrow transplantation.

Blastic NK cell lymphoma/leukemia is a rare and highly malignant neoplasia in both adults and children. It is characterized by lymphoblastoid morphology without cytoplasmic granules and immature NK cell immunophenotypes (CD56+, CD57-, CD16-). It has predilection for extranodal organ involvement, and the prognosis of affected patients is extremely poor under the current chemotherapy. We present a 14-year-old girl who was diagnosed as having blastic NK cell leukemia with mediastinal, pleural, and pericardial involvement. Immunophenotyping of her leukemic cells showed positive for CD2, CD5, CD7,

NK-cell related neoplasms in Osaka, Japan.

The frequency of NK-cell related neoplasms was estimated among lymphoproliferative diseases diagnosed and treated in Osaka, Japan, from 1999 to 2003. The total number of registered cases was 1,400, among which 1,092 patients were diagnosed as having malignant lymphomas. There were 987 cases of non-Hodgkin's lymphoma (NHL) and 105 (9.6%) of Hodgkin's lymphoma. Immunophenotypic analysis revealed that 743 patients had B-cell lymphomas and 209 T/NK-cell lymphomas. Among the T/NK-cell lymphomas, 40 showed positive immunoreactivity for CD56, and thus they were judged to be NK/T-cell lymphomas. They

Blastic natural killer (NK)-cell lymphoma: report of an unusual CD4 negative case and review of the CD4 negative neoplasms with blastic features in the literature.

Blastic Natural Killer (NK)-cell lymphoma is a relatively new entity which has been recently included in the WHO classification. CD4 expression is observed in most cases of blastic NK-cell lymphomas and has been related with skin tropism. We report an unusual CD4 negative blastic NK-cell lymphoma with primary presentation in the skin, subsequent infiltration of the bone marrow and aggressive behavior. It is emphasized that extensive immunophenotyping and EBER RNA in situ hybridization are required in order to establish the diagnosis of blastic NK-cell lymphoma. We also present a review of the

[Hematodermic CD4/CD56 neoplasm].

Hematodermic CD4/CD56 neoplasm is a recently described entity. This name has been initially proposed by the French Study Group on Cutaneous Lymphomas which established the primary anatomoclinical and pathogenic and cytogenic bases of the disease in 1999. This descriptive and provisional name allowed conceptualizing the entity by its main clinical and phenotypical characteristics. The first case in the literature goes back to 1994. Since that time, several other cases have been published. The expression of CD56 led most of the authors to propose an NK-cell lineage origin. In the last WHO classi
