TET2 germline mutation in a patient with sequential lymphoid malignancies: a novel case report.

We report a case of a 30-year-old male patient with a germline heterozygous TET2 R1354fs mutation, who successively developed three distinct lymphoid malignancies: mixed cellularity classical Hodgkin lymphoma (MCCHL), angioimmunoblastic T-cell lymphoma (AITL), and T-cell acute lymphoblastic leukemia (T-ALL). Sequencing analysis of the oral mucosa and bone marrow specimens during the remission phase confirmed the germline origin of the TET2 mutation. This represents the first documented case of the sequential transformation across three distinct lymphoid malignancies attributable to a germline

Co-occurrence of angioimmunoblastic T-cell lymphoma and aggressive-refractory plasma-cell neoplasm: Two new cases and literature review.

Angioimmunoblastic T-cell lymphoma (AITL) is a rare and aggressive peripheral T-cell lymphoma of T-follicular helper (TFH) cell origin, characterized by systemic manifestations, immune dysregulation, and frequent secondary B-cell proliferations. While Epstein-Barr virus (EBV)-related diffuse large B-cell lymphoma develops in up to 20% of AITL cases, the occurrence of overt plasma cell neoplasms such as multiple myeloma (MM) has been documented only in isolated reports. We describe two new patients with AITL complicated by highly aggressive, treatment-refractory plasma-cell neoplasia, including

A Case of Angioimmunoblastic T-Cell Lymphoma Misdiagnosed as Eczema-Like Dermatitis in an Elderly Patient.

Angioimmunoblastic T-cell lymphoma (AITL) is a clinically heterogeneous subtype of peripheral T-cell lymphoma that frequently presents with nonspecific cutaneous manifestations. Approximately 40%-50% of patients initially develop diffuse erythema or papular eruptions that may mimic benign inflammatory or allergic dermatoses, posing a diagnostic challenge in early disease stages. We report the case of an 81-year-old man who presented with generalized pruritic papules and was initially diagnosed with an eczema-like inflammatory dermatitis. Although the skin lesions showed transient improvement w

Risk Factors of Disseminated Tumor Cells in the Bone Marrow of Patients With Angioimmunoblastic T-Cell Lymphoma and Their Impact on Prognosis.

INTRODUCTION: Angioimmunoblastic T-cell lymphoma (AITL) is a mature T-cell proliferative tumor characterized by an aggressive clinical course and low incidence. The risk factors for the presence of disseminated tumor cells (DTCs) in the bone marrow (BM) of patients and their impact on prognosis remain incompletely understood. In this study, we used multiparametric flow cytometry (MFC) to detect DTCs in the BM of AITL patients. We aimed to assess risk factors for the presence of DTCs, investigate correlations between the immunophenotype of DTCs and clinical indicators or efficacy, and identify

Next-generation sequencing for lymphoid neoplasms: Real-world utility from a clinical assay.

Although next-generation sequencing (NGS) has become the standard of care in myeloid neoplasms, the utility of lymphoma NGS (LNGS) in the clinical setting has not been well established. The goal of this study was to document the real-world use of LNGS panels and assess their prognostic, predictive, and diagnostic value. A total of 384 cases were retrospectively reviewed in conjunction with pathology reports, including 121, 183, and 80 tested for the limited B-cell lymphoma panel (25 genes), expanded B-cell lymphoma panel (46 genes), or T-cell lymphoma panel (22 genes), respectively. Phage Display Driven Identification and Computational Mapping of Macrocyclic Peptides Targeting RhoA G17V.

Mutant RhoA G17V is a clinically significant yet historically undruggable oncogenic GTPase that drives angioimmunoblastic T-cell lymphoma through a neomorphic interaction with the guanine nucleotide exchange factor Vav1. Its rigid GTPase fold, absence of deep binding pockets, and transient protein-protein interfaces have hindered conventional small-molecule approaches, creating a critical need for alternative therapeutic modalities. Here, we report a systematic strategy to target RhoA G17V using macrocyclic peptides. Two complementary phage-displayed cyclic peptide libraries, an AcrK-mediated

Case report of durable responders to sintilimab plus second-line chemotherapy in relapsed/refractory angioimmunoblastic T-cell lymphoma.

Angioimmunoblastic T-cell lymphoma (AITL) is a highly aggressive lymphoma characterized by complex and unique clinicopathological and biological features, as well as a generally poor prognosis. First-line treatment typically consists of anthracycline-based chemotherapy regimens, yet the long-term outcomes remains unsatisfactory, with most patients eventually experiencing disease progression. For patients with relapsed/refractory (R/R) AITL, standard treatment options are limited, and enrollment in clinical trials is often prioritized in practice. Here, we report two cases of R/R AITL patients

When Vasculitis Is Not Vasculitis: A Case Report in Which Dynamic Immunophenotyping Revealed Hidden Angioimmunoblastic T-cell Lymphoma.

BACKGROUND/AIM: Angioimmunoblastic T-cell lymphoma (AITL) is a rare peripheral T-cell lymphoma characterized by diverse and aggressive clinical manifestations that frequently mimic autoimmune disorders. Cutaneous presentations are common and may lead to diagnostic delay. CASE REPORT: A 67-year-old woman first presented with non-blanchable skin rash, with normal platelet count and negative autoimmune markers. The rash improved with corticosteroids but recurred two months later, accompanied by fever, night sweats, limb edema, diarrhea, and cervical lymphadenopathy. Skin biopsy reported vasculit

B-cell and plasma cell proliferations associated with T follicular helper cell lymphomas: clinical-pathological and genetic features.

Nodal T-follicular helper cell lymphomas (nTFHL) are aggressive peripheral T-cell lymphomas with unique clinical-biological features. B-cell and/or plasma cell proliferations (B/PCP) are frequently reported in nTFHL, yet their histological spectrum remains incompletely defined. This study aims to delineate the clinical, pathological and molecular features of a retrospective series of nTFHL-associated B/PCP, with particular emphasis on recurrent disease patterns. To this aim, we retrospectively analyzed 39 nTFHL with 45 synchronous and/or metachronous B/PCP, collected from five Italian referral

RCLARITY: A Prospective Phase II Study of Rituximab Combined With Lenalidomide and Chidamide (RLC) for the Treatment of Relapsed/Refractory Angioimmunoblastic T-Cell Lymphoma.

Relapsed/refractory (r/r) angioimmunoblastic T-cell lymphoma (AITL) is associated with a dismal prognosis, with historical overall survival (OS) < 6 months, underscoring the urgent need for novel therapies. We conducted the RCLARITY trial to evaluate the efficacy and safety of a chemotherapy-free regimen in this patient population. This single-arm, multicenter, prospective Phase II trial enrolled adult patients with r/r AITL. Patients received rituximab (375 mg/m2 intravenously on Day 1), lenalidomide (15 mg orally on Days 1-21), and chidamide (30 mg orally twice weekly) in 28-day cycles for u

Marginal Zone Lymphoma With T-Follicular Helper Hyperplasia: Paving the Way for New Concepts in Cellular Plasticity and Unveiling Histopathologic Pitfalls.

The expression of T-follicular helper (TFH) markers, initially described in nodal T-cell lymphomas arising from TFH-cells, has been reported in several non-TFH lymphomas, including B-cell lymphomas. It has also been observed in marginal zone lymphomas (MZL), whereas it is poorly characterized in nodal MZL (NMZL). The present study, therefore, aimed to characterize MZL with TFH cell expansions, by providing an extensive description of their histologic and molecular features. Thirty-two MZL with nodal involvement (25 primary nodal, 4 extranodal, and 3 primary splenic) showing TFH hyperplasia and

Incidence and Survival of Patients With Angioimmunoblastic T-Cell Lymphoma: A Nationwide Population-Based Study Using English Cancer Registration Data.

Angioimmunoblastic T-cell lymphoma (AITL) is a rare, aggressive subtype of peripheral T-cell lymphoma. Whole population studies of AITL are infrequent and often grouped along with other T-cell lymphomas. We undertook a population-based study using ICD-O3-coded cases of AITL diagnosed 2013-2019 in order to establish AITL epidemiology in England, along with survival outcomes and associated diagnoses. We identified 993 cases, with a median age of 71 years. Overall incidence was 2.57 cases/million person-years (95% CI 2.41-2.74). The incidence increased with age, ranging from 0.24 cases/million pe

The risk and predictors of second primary malignancies in survivors of angioimmunoblastic T-cell lymphoma: a population-based competing risk analysis.

Survivors of angioimmunoblastic T-cell lymphoma (AITL) face a substantial burden of second primary malignancies (SPMs). Using the SEER database (2000-2022), we identified 2217 AITL patients for analysis. The overall SPM risk was significantly elevated versus the general population (SIR = 1.60), driven predominantly by a marked excess of acute myeloid leukemia (SIR = 22.27). Competing risk analysis estimated the cumulative incidence of SPMs at 4.1, 6.5, and 9.6% by 2, 5, and 10 years, respectively. Multivariable analysis confirmed age 55-74 years as an independent risk factor, whereas advanced

Single-cell transcriptomic mapping of immune heterogeneity across angioimmunoblastic T-cell lymphoma subtypes.

Relapse and treatment resistance remain critical obstacles in the clinical management of angioimmunoblastic T-cell lymphoma (AITL), limiting the success of current therapeutic strategies. Therefore, a comprehensive characterization of the tumor microenvironment (TME) in AITL is essential to enhance treatment efficacy. This study analyzed samples from 68 patients with AITL, stratified into three molecular subtypes based on immunoglobulin (IG) gene rearrangement and flow cytometry results. Utilizing single-cell RNA sequencing, the TME was profiled across subtypes A, B, and C, sampling multiple d

Shared clonal origin of angioimmunoblastic T-cell lymphoma and Burkitt lymphoma arising through divergent evolution from a common precursor.

BACKGROUND: Nodal T follicular helper cell lymphoma, angioimmunoblastic type (AITL), is a common peripheral AQ1T-cell lymphoma characterized by recurrent mutations in TET2, IDH2, DNMT3A, and RHOA, with TET2 and DNMT3A mutations frequently arising in early hematopoietic stem and progenitor cells (HSPCs) as clonal hematopoiesis (CH). In addition to its well-established association with secondary myeloid neoplasms, AITL is complicated by secondary B-cell lymphomas in up to 10-23% of cases, often associated with EBV infection. However, the clonal relationship between AITL, CH, and associated B-cel

TFH phenotype and response to tucidinostat in relapsed/refractory PTCL: analysis of patients from a phase IIb trial.

T follicular helper (TFH)-derived peripheral T-cell lymphomas (PTCLs) harbor frequent mutations in epigenetic regulators and are sensitive to epigenetic therapies. The histone deacetylase inhibitor tucidinostat demonstrated efficacy in relapsed/refractory PTCL, especially angioimmunoblastic T-cell lymphoma (AITL), in a phase IIb trial; however, the lack of TFH phenotyping has prevented assessment of the predictive value of this phenotype across a broader PTCL spectrum. Therefore, we retrospectively analyzed patients originally diagnosed with AITL or PTCL not otherwise specified (NOS) based on

Multiple subcutaneous nodules as the primary presentation of angioimmunoblastic T-cell lymphoma: a case report and literature review.

Angioimmunoblastic T-cell lymphoma (AITL) frequently involves the skin, typically presenting as rash or pruritus. However, AITL presenting primarily with multiple subcutaneous nodules is exceedingly rare and poses significant diagnostic challenges, often leading to misdiagnosis. We reported a 53-year-old man who presented with widespread subcutaneous nodules. He had a 1-month history of multiple painless subcutaneous nodules on the trunk and upper limbs. Physical examination revealed multiple firm nodules measuring 0.5-2 cm, accompanied by dark red plaques. Laboratory investigations showed mil

Autologous Stem Cell Transplant Consolidation Is Associated With Improved Overall Survival in Angioimmunoblastic T-Cell Lymphoma: A Real-World National Cancer Database Study.

Angioimmunoblastic T-cell lymphoma (AITL) is an aggressive peripheral T-cell lymphoma with poor outcomes, and the role of autologous stem cell transplantation (ASCT) as consolidation after frontline therapy remains controversial. We conducted a retrospective cohort study using the National Cancer Database, including adults diagnosed with AITL between 2004 and 2020 who received frontline systemic therapy. Patients were categorized as chemotherapy alone (Chemo) or chemotherapy followed by ASCT (Chemo + ASCT). To mitigate immortal time bias, prespecified landmark analyses were performed, with a 6
