From development to clinical success: the journey of established and next-generation BTK inhibitors.

Over the past decade, Bruton's tyrosine kinase (BTK) has emerged as a pivotal therapeutic target for B-cell malignancies and autoimmune diseases, given its essential role in B-cell development and function. Dysregulation of BTK signalling is implicated in a range of hematologic cancers, including Waldenström's macroglobulinaemia (WM), mantle cell lymphoma (MCL), and chronic lymphocytic leukaemia (CLL). The development of BTK inhibitors (BTKIs), starting with ibrutinib, has revolutionized the treatment of these malignancies by inhibiting B-cell receptor (BCR) signalling and inducing apoptosis i

Covalent Bruton tyrosine kinase inhibitors across generations: A focus on zanubrutinib.

Bruton tyrosine kinase (BTK), the primary target of BTK inhibitors, is a key enzyme in the proliferation and survival pathway of neoplastic B-cells. BTK inhibitors are approved in many hematologic malignancies: chronic lymphocytic leukaemia, mantle cell lymphoma, marginal zone lymphoma, Waldenström macroglobulinaemia and follicular lymphoma. Second-generation BTK inhibitors display high target selectivity thus resulting in a reduction in off-target and off-tissue effects, better therapeutic index and improved tolerability. This paper summarizes the mechanisms of action of first and second gene

Advances in Bruton tyrosine kinase (Btk) inhibition are steered by Bruton tyrosine kinase phylogeny.

Bruton tyrosine kinase (Btk) has long been known to play a key role in chronic lymphatic leukaemia, Waldenström macroglobulinaemia and other B-cell proliferative disorders. An impressive programme of drug discovery and clinical development led to the approval of covalent and non-covalent Btk inhibitors that became pillars of treatment of such malignancies. However, both a risk of cardiovascular events and the emergence of an elusive mutational landscape seem to complicate the clinical use of each Btk inhibitor. In this plain language mini-review, we show that the search for better Btk inhibito

Zanubrutinib for the treatment of Bing-Neel syndrome.

Infiltration of the central nervous system by malignant lymphoplasmacytic cells is a rare complication of Waldenström macroglobulinaemia (WM) and is referred to as Bing-Neel syndrome (BNS). Traditionally, the treatment of BNS included chemotherapy, but in more recent years, the oral Bruton tyrosine kinase (BTK) inhibitor ibrutinib has become a standard therapy for WM and the most common therapy for BNS due to the drugs tolerability and ability to cross the blood-brain barrier. Zanubrutinib, a second-generation covalent BTK inhibitor with fewer off-target effects, is also effective in WM. In th

Bortezomib, Rituximab and Dexamethasone Regimen (BDR) in Waldenström Macroglobulinaemia: A Retrospective Real-World Analysis.

INTRODUCTION: We retrospectively analysed bortezomib-dexamethasone-rituximab (BDR) combination in patients with Waldenström macroglobulinaemia (WM) in a real world setting. METHODS: A total of 87 patients were included: 49 patients (56%) were treated in frontline, 22 (25%) in second line and 16 (19%) in third or further line settings. A log-rank test was used to compare overall and event-free survival (OS and EFS) whereas a Gray's test was performed to compare cumulative incidence of deaths and relapse (CID and CIR) according to the IPSS-WM groups, MYD88/CXCR4 mutational status and line of th

Quality-of-life endpoints collection, reporting, and framing in randomised trials of indolent lymphomas: a systematic review.

Indolent lymphomas are generally incurable, with protracted disease courses. The approval of drug treatment options often relies on surrogate endpoints (eg, progression-free survival), which do not capture patient-centred outcomes such as quality of life (QOL). This systematic review characterises the use of QOL as an endpoint in randomised controlled trials (RCTs) of indolent lymphomas, and the association of QOL with survival outcomes. ClinicalTrials.gov was searched from database inception to May 20, 2024, for phase 3 RCTs of indolent lymphomas, including follicular lymphoma, marginal zone

Development and characterization of the novel MYD88 mutated, 6q deleted BCWM.2 cell line for Waldenström macroglobulinaemia.

Cell lines have enabled a comprehensive understanding of disease biology and the advancement of new therapeutics in Waldenström macroglobulinaemia (WM). Herein, we report the development of BCWM.2, a novel WM cell line derived from an untreated symptomatic WM patient. Immunophenotypic and genomic analyses confirmed its origin from primary bone marrow WM cells. Flow cytometry revealed expression of CD19, CD20, CD23, CD38, CD45RA, CD52, immunoglobulin M (IgM) heavy chain and λ light chain on BCWM.2 cells. Whole genome sequencing demonstrated that, unlike BCWM.1, MWCL-1 and RPCI-WM1, which harbou

Report of Consensus Panel 1 from the 12th International Workshop on the management of patients with IgM and Waldenstrom's Macroglobulinemia related neuropathy.

The IgM-related peripheral neuropathies (IgM-PN) are a group of chronic disorders characterized by the presence of monoclonal IgM that may be associated with one of several diseases affecting the peripheral nerves. In many cases, there is a monoclonal IgM associated with activity against neural targets, leading to progressive peripheral nerve demyelination. Neurological symptoms in this setting can also result from direct invasion of the peripheral or central nervous system by lymphoplasmacytic cells (neurolymphomatosis and Bing-Neel syndrome respectively) or via other mechanisms (for example

Phenotypic Profile of Waldenström Macroglobulinaemia B-Cells: Establishment of a Diagnosis Scoring System and Clinico-Biological Correlations.

Waldenström Macroglobulinaemia (WM) is sometimes difficult to differentiate from marginal zone lymphoma (MZL), two entities with overlapping features for which no single marker assessed by multiparameter flow cytometry (MFC) is specific. The aim of this work was to establish a diagnostic phenotypic score for bone marrow (BM) and peripheral blood (PB) WM samples, to differentiate it from MZL and to improve the detection of small circulating WM clones. This study revealed a distinct phenotypic profile between WM and MZL B-cells. WM B-cells showed decreased expression of CD19, FMC7, CD22, CD27 an

Escherichia coli Cellulitis in a Patient With Waldenström Macroglobulinaemia: A Case Report.

Escherichia coli cellulitis is rarely reported, typically affecting immunocompromised individuals. The management of such cases presents significant challenges, as these patients often exhibit resistance to the empirical antibiotics commonly used for cellulitis. Furthermore, identifying E. coli in blood cultures necessitates additional investigations to rule out alternative sources of infection, such as biliary, urinary, or deep-seated collections. We present the case of a 79-year-old woman with Waldenström macroglobulinaemia, AL amyloidosis, and secondary immunosuppression who developed E. co

BCL-2 inhibition in Waldenström macroglobulinaemia and marginal zone lymphoma.

Marginal zone lymphoma (MZL) and Waldenström macroglobulinaemia (WM) are B-cell lymphomas characterized by an indolent disease course. However, both are incurable with current standard treatments. Therefore, additional therapies are certainly needed. BCL-2 inhibition has been a welcome addition to the treatment armamentarium for haematological malignancies. The BCL-2 inhibitor venetoclax is approved by the Food and Drug Administration and the European Medicines Agency for the treatment of chronic lymphocytic leukaemia and acute myeloid leukaemia and is endorsed by the National Comprehensive Ca

Patient Perception of the Relevance of Quality-of-Life Questionnaires in Waldenström Macroglobulinaemia: An International Survey.

Waldenström Macroglobulinaemia is an indolent disease which is recognised as contributing to a diverse range of disabling symptoms which can impact quality of life. Little data are available in evaluating patient and healthcare professional views on quality-of-life questionnaires (QLQ) used to evaluate quality of life in Waldenström Macroglobulinaemia. We aimed to assess patient perception of their disease and the relevance of the currently employed QLQs in practice by patients and healthcare professionals. One-hundred-and-twenty patients (22% treatment-naïve) and thirty-four healthcare profes
