Targeting vascular inflammation and endothelial dysfunction with nanobodies: emerging therapeutic strategies for cardiovascular disease.

Despite a myriad of medicinal breakthroughs in lipid-lowering and antithrombotic therapies, cardiovascular diseases (CVDs) are still the main killers worldwide. According to the consolidated scientific evidence, atherosclerosis, ischemic heart disease, and other vascular pathologies are attributed to endothelial dysfunction and vascular inflammation. The standard treatments' failure to adequately control the inflammatory and endothelial pathways has led researchers to biologics that are molecularly specific. Nanobodies (Nbs) are single-domain antibody fragments obtained from camelid heavy-chai

Venetoclax-based therapy in high-risk patients with relapsed/refractory multiple myeloma harboring t(11;14): a case report and review of the literature.

INTRODUCTION: Multiple myeloma is a malignant plasma cell proliferative disease that is still incurable. Individualized targeted precision therapy is the key to prolonging survival. Patients with multiple myeloma are highly heterogeneous, and patients with high-risk factors have a short median survival and poor prognosis. T(11;14) is the most common immunoglobulin heavy-chain translocation in multiple myeloma. Although it is still classified as a standard prognostic risk stratification factor based on cytogenetics, such patients have special cell biological behaviors, and the efficacy and prog

Nanobody Nb07 mitigates sepsis by blocking the PFKM-p53-PD-1 axis to enhance macrophage phagocytosis.

Rationale: Macrophage phagocytosis is essential for pathogen clearance during sepsis. We previously demonstrated that the glycolytic enzyme 6-phosphofructokinase, muscle type (PFKM), modulates macrophage functions and its deficiency alleviates sepsis in mice. However, the function of PFKM in regulating macrophage phagocytosis remains unclear. Methods: CD14+ monocytes were sorted by flow cytometry from healthy volunteers and septic patients, and the subcellular localization of PFKM was assessed by immunofluorescence. Nuclear translocation mechanisms and PFKM-p53 interaction were identified by C

Multiomics analysis of red blood cells reveals thalassemia severity beyond globin gene mutations.

Hemoglobinopathies are the most common monogenic genetic disorders, primarily managed through blood transfusions or bone marrow transplantation. Clinical severity other than mutational effect is not well investigated and still unknown. This study aimed to identify dysregulated molecular pathways in red blood cells (RBCs) contributing to thalassemia severity. From a cohort of 285 patients with hemoglobinopathy, 10 age-matched individuals with identical compound heterozygous mutations (IVS 1-5 G>C and CD 26 G>A) were screened. Five had severe thalassemia requiring regular transfusions, whereas 5

A SARS-CoV-2 variant-induced NTD-targeting antibody enhances viral infection via a distinctive binding mode.

SARS-CoV-2 infection elicits both neutralizing and non-neutralizing monoclonal antibodies (mAbs), primarily targeting to the N-terminal domain (NTD), receptor-binding domain (RBD), and S2 subunit of the spike protein. Notably, a unique subset of NTD-targeting mAbs isolated from prototype Wuhan-Hu-1 strain infected donors displayed a capacity of facilitating the viral infection independent of the fragment crystallizable (Fc) region in vitro. However, the rapid evolution of SARS-CoV-2 variants, particularly with NTD mutations, has led to widespread immune evasion. Whether SARS-CoV-2 variants cou

Reprogramming CD22 CAR-T cells in vivo using CD8-targeted mRNA-LNPs to treat hematological malignancies.

Ex vivo chimeric antigen receptor (CAR) T cell therapy has proven successful in patients with B cell hematologic malignancies. However, current approaches are limited by the requirement for personal manufacturing processes and by barriers such as limited efficacy against solid tumors, treatment-associated toxicities, insufficient CAR-T cell trafficking to the tumor microenvironment, on-target off-tumor effects, and tumor antigen escape. Here, we describe a novel delivery platform that overcomes many of these barriers by employing targeted lipid nanoparticles (LNPs) to reprogram circulating hum

Evaluation of genetic studies on camels in Saudi Arabia: traits, tools, and applications.

In Saudi Arabia, the dromedary camel (Arabian camel) has enormous cultural, economic, and ecological importance. Nevertheless, camel genetic studies and breeding have lagged behind those of other livestock species in the past. This review summarizes findings from recent research on the genetic diversity, population structure, and functional traits of Saudi Arabian camels. It presents the application of molecular technologies, including microsatellite markers, mitochondrial DNA, whole-genome sequencing, and multi-omics approaches, in studying camel phenotypes such as milk yield, growth rate, an

A Case Report of Thrombocytopenia Caused by a May-Hegglin Anomaly in a Young Saudi Female.

Low platelet count is rarely caused by inherited thrombocytopenia. May-Hegglin anomaly is an uncommon condition that falls under the umbrella of familial thrombocytopenia. The condition is under-reported in Saudi Arabia; therefore, we report the current case. This is a 24-year-old Saudi lady, presented to the emergency room with vaginal bleeding. No bleeding occurred at any other sites. She has a positive family history of thrombocytopenia among her father and 2 of her siblings. Her platelet count was 16 × 103/µL with normal other blood count as well as renal and liver panels. She was admitted

Immunoglobulin Light Chain Amyloidosis: 2026 Update on Diagnosis, Prognosis, and Treatment.

DISEASE OVERVIEW: AL amyloidosis is a clonal plasma cell disorder in which fragments of immunoglobulin light or heavy chain are deposited in tissues. Clinical features depend on organs involved but can include HFpEF, nephrotic syndrome, hepatic dysfunction, peripheral/autonomic neuropathy, and "atypical smoldering multiple myeloma or MGUS." DIAGNOSIS: Tissue biopsy stained with Congo red demonstrating amyloid deposits with apple-green birefringence is required. Organ biopsy is not required in 85% of patients. Verification that amyloid is composed of immunoglobulin light chains is mandatory.

A loss of inter-α-inhibitor Heavy Chain 5 (ITIH5) leads to corneal thinning and increased corneal inflammation following injury and early onset geriatric decline.

BACKGROUND: Inter-alpha trypsin inhibitor (IαI) proteins contain heavy chains (ITIHs, traditionally HCs), and ITIH5 is the most highly expressed ITIH isoform in the cornea. In skin, ITIH5 is upregulated during inflammatory skin diseases, however little is known about the biological functions of ITIH5 in other tissues. The aim of this study was to characterize the role of ITIH5 in the cornea during homeostasis, in corneal wound healing and in aging using mice lacking ITIH5 (Itih5-/-). METHODS: The ocular surface of Itih5-/- mice were analyzed at 8 weeks (adult) and 1 year (aging) and following

Structural basis for antibody cross-neutralization of Dengue and Zika viruses.

Safe and effective vaccines against co-circulating mosquito-borne orthoflaviviruses such as Zika virus (ZikV) and the four serotypes of Dengue virus (DenV1-4) must elicit broadly neutralizing antibodies (bnAbs) to prevent the risk of enhancement of infection by non-neutralizing antibodies. We recently discovered new orthoflavivirus-directed bnAbs, including F25.S02, which neutralizes DenV1-4 and ZikV with comparable or superior potency to the previously characterized E dimer epitope (EDE) bnAbs. Here, we used cryoEM and X-ray crystallography to understand the basis of cross-neutralization of F

Recurrent SARS-CoV-2 Omicron broadly neutralizing humanized antibodies in different single human VH1-2-rearranging mouse models.

During V(D)J recombination, antibody diversity is enhanced by nontemplated junctional modifications that generate immensely diverse heavy chain (HC) and light chain (LC) complementarity-determining 3 antigen-contact regions (CDR3s). We previously developed a mouse model that generates diverse antibody repertoires by rearranging a single human VH1-2 and Vκ1-33, associated with highly diverse CDR3s generated by V(D)J recombination with mouse Ds and/or Js. Immunization of this model with SARS-CoV-2 D614G spike elicited an antibody that potently neutralized SARS-CoV-2 variants through Omicron BA.2

Immunogenetic Architecture of Chronic Lymphocytic Leukemia at Early Stage: Insights from the O-CLL1 Cohort.

BACKGROUND/OBJECTIVES: The immunoglobulin heavy-chain variable (IGHV) gene repertoire represents a characteristic feature of chronic lymphocytic leukemia (CLL), although its configuration is not well defined at the early disease stages. The IGHV repertoire of a cohort of early CLL patients was analyzed and compared to that of a "real-world" reference cohort. METHODS: Patients from the O-CLL1 observational protocol, which enrolled only Binet stage A cases within twelve months from diagnosis, were studied. IGHV/IGHJ rearrangements were sequenced and annotated following ERIC recommendations, and

Development of canine parvovirus-neutralizing monoclonal antibodies from natural host and their germline gene usage.

Canine parvovirus 2 (CPV-2) remains a leading cause of acute infectious gastroenteritis with high global morbidity in dogs. While murine neutralizing monoclonal antibodies (mAbs) are widely used for antiviral therapy, their efficacy is limited by immune rejection in canine recipients. Here, we developed an efficient single B-cell cloning platform to generate canine-derived neutralizing mAbs against CPV-2 and characterized their germline gene usage patterns. Specifically, using biotinylated CPV-2 virions as bait, CPV-2-binding B cells were singly isolated via fluorescence-activated cell sorting

BAG3 modulates clathrin-mediated endocytosis and tau uptake in astrocytes.

Astrocytes maintain neuronal homeostasis by removing extracellular disease-relevant proteins, such as tau, to prevent their uptake by neurons. In Alzheimer's disease (AD), this astrocytic function is impaired, contributing to pathological tau accumulation. Many AD-associated risk genes are linked to endocytosis pathways, suggesting their role in AD pathogenesis. Although astrocytes can internalize, degrade, and release tau, the mechanisms governing these processes remain unclear. Bcl2-associated athanogene 3 (BAG3), a multifunctional protein regulating vacuolar processes, interacts with compon

Selection of anti-CD38 antibodies for flow cytometric detection of myeloma cells treated with daratumumab or isatuximab.

Treatment with the anti-CD38 therapeutic monoclonal antibodies (mAbs) daratumumab (DARA) and isatuximab (ISA) achieves deep responses in patients with multiple myeloma (MM), often resulting in measurable residual disease (MRD) negativity. CD38 is one of the key surface markers used for flow cytometric MRD assessment in MM; however, DARA is known to interfere with CD38 detection by conventional anti-CD38 mAbs. The impact of ISA on CD38 detection remains unclear. This study aimed to develop optimized methods for accurate detection of CD38 on MM cells by flow cytometry. The anti-CD38-variable hea

Chronic lymphocytic leukemia with IGH/BCL2 fusion and clonal heterogeneity: phenotypic and molecular profiling analysis of a rare case.

Clonal heterogeneity within chronic lymphocytic leukemia (CLL) has been increasingly recognized as a factor that may influence disease progression, treatment response, and prognosis. Here, we report a rare case of CLL with two distinct B-cell populations displaying distinct immunophenotypic characteristics, genetic mutations, and genetic evolution. The two aberrant mature B cell populations were identified in peripheral blood (PB) and bone marrow (BM) through multiparametric flow cytometry (MFC) with identical light chain restriction but distinct immunophenotypes. The first population (CD5 + C

B-cell repertoire sequencing reveals frequent rearrangements of IGHD5-5 in patients with systemic sclerosis.

Systemic sclerosis (SSc) is an autoimmune disorder marked by fibrosis of the skin and internal organs, with B cells increasingly recognized as key players in its pathogenesis. However, the characteristics of the B cell receptor (BCR) repertoire in SSc remain insufficiently defined. In this study, we performed high-throughput sequencing of immunoglobulin heavy chain genes in 15 female anti-centromere antibody (ACA)-positive SSc patients and five age-matched healthy female controls to explore disease-specific repertoire biases. A total of 2,597,460 in-frame sequence reads and 384,111 unique read

[The application of thrombopoietin receptor agonists in the treatment of non-muscle myosin heavy chain 9-related disease].

MYH9-related disease (MYH9-RD) is an autosomal dominant disorder caused by mutations in the MYH9 gene, characterized by macrothrombocytopenia, neutrophil inclusions, and other features. Due to its low incidence and nonspecific clinical manifestations, many patients are misdiagnosed or undiagnosed. Clinical management often includes platelet transfusion, pro-hemostatic or antifibrinolytic agents, avoidance of antiplatelet drugs, and hemorrhage prevention. Hematopoietic stem cell transplantation may be a treatment option for severe cases. For a long time, platelet transfusion has been the most c
