Immunoproliferative small intestinal disease in Basenji dogs: morphologic observations.

Eleven Basenjis with chronic diarrhea were evaluated for pathologic changes. Light microscopic and ultrastructural changes were characterized by lymphoplasmacytic infiltration of the lamina propria of the intestine and stomach and by evidence of degranulation of mast cells in close association with immunocytes. Other changes included hyperplasia of the thyroid parafollicular cells, thyroid follicular atrophy, epidermal atrophy, and ulceration of the pinna. Similarities of the condition in Basenjis and immunoproliferative small intestinal disease and Menetrier's disease in persons are presented

Immunoproliferative small intestinal disease in Algerians. II. Ultrastructural studies in alpha-chain disease.

The plasma cell infiltrate of the small intestine in alpha-chain disease has been studied ultrastructurally in an attempt to determine whether there is a significant nuclear-cytoplasmic asynchrony that could be used as evidence for the neoplastic nature of the disease, even in its early stages. No such asynchrony was identified. In the early stages of the disease, the infiltrate was mainly of slightly immature plasma cells indistinguishable from those of coeliac disease. Later stages were marked by the presence of less differentiated immunoblastic cells arising in the deep mucosa and infiltrat

[Lymphatic diseases specific to the tropics (Burkitt's lymphoma, tropical splenomegaly and Mediterranean lymphoma)].

Certain lymphoproliferative disorders (Burkitt's lymphoma, tropical splenomegaly, and Mediterranean lymphoma) are more frequently observed in the tropical or subtropical areas than in other parts of the world. The polymicrobial exposure of human beings in those areas to viral, bacterial and protozoal infections led to the assumption of a specific connection between these conditions and the lymphoproliferative disease. Some evidence has accumulated that the Epstein-Barr virus might be an oncogenic virus connected with the development of Burkitt's lymphoma. Repeated plasmodial infections were mo

Alpha-chain disease without qualitative serum IgA abnormality. Report of two cases, including a "nonsecretory" form.

Two cases of alpha-chain disease (alpha-CD) without detectable amounts of alpha-CD protein in serum are reported. Both patients presented with the usual epidemiologic, clinical and pathologic features of alpha-CD, including a predominantly plasmacytic infiltration of the whole small intestine, alpha-CD Protein was found in the jejunal fluid in one case but not in the other, the latter presenting as a "nonsecretory" form of alpha-CD. In both cases, immunofluorescence study of the small bowel mucosa showed that most of the infiltrating cells were positive for alpha chains and negative for other

Immunoproliferative small intestinal disease in Algerians. I. Light microscopic and immunochemical studies.

Twenty-two cases of immunoproliferative small intestinal disease (IPSID) were studied with an immunoperoxidase technique. Sixteen of these had serologic evidence of alpha heavy chain disease (AHCD) and have a characteristic immunochemical pattern that allows a diagnosis on jejunal biopsy. kappa-light chain was found in a case of AHCD in the benign appearing plasma cell proliferation and in the associated malignant lymphoma suggesting that failure of light chains is not constant in AHCD. Four cases of IPSID showed secretion of polyclonal IgA in the plasma cell infiltrate providing evidence that

In vitro immune cell function in six cases of immunoproliferative small intestinal disease after long term remission.

We have studied several parameters of in vitro immune cell function in peripheral blood mononuclear cells from six patients with immunoproliferative small intestinal disease after long term remission. We have observed two groups of patients with different patterns of response. (a) After stimulation with pokeweed mitogen (PWM) and Staphylococcus aureus, three patients showed a significant reduction of the Ig synthesis (A, G and M) and the proliferative response. In two of them, we found increased spontaneous suppressor T cell activity. In the third case, the diminished response could not be att

Immunoproliferative small-intestinal disease: follow-up of an alpha-chain negative, lymphoma-free group.

Six patients had the clinical and pathological features of immunoproliferative small-intestinal disease (IPSID). All were female with malabsorption or growth retardation in whom testing for alpha-heavy chains was repeatedly negative. They were followed for a mean of 7.5 years, during which time no significant alteration in the jejunal biopsy appearances of five patients has occurred, none have developed lymphomas.

Lymphocyte transformation and humoral immune factors in Basenji dogs with immunoproliferative small intestinal disease.

Serum protein concentrations and 4 immunologic factors were determined in 5 Basenji dogs with immunoproliferative small intestinal disease. There was no correlation between the total serum proteins, immunoglobulin G and immunoglobulin M concentrations, and physical health status of the animals. The severity of clinical signs correlated roughly with decrease in albumin and increase in globulin concentrations. The main changes were detected in beta- and fast gamma-globulins. The total hemolytic complement levels were decreased in the 2 most severely affected animals below the minimal laboratory

Intestinal absorption in Saudi Arabia: an evaluation of the one hour blood xylose test.

The screening value of the one-hour blood xylose test, corrected for body surface area, was prospectively studied in Saudi Arabian adults and children under investigation for suspected intestinal malabsorption. Sensitivity of discrimination between patients with and without upper small bowel disease was 91%, compared to 85% for the five-hour urine xylose test. Primary small bowel disorder was rare. In a three-year review, no cases of adult coeliac disease or tropical sprue were found. The most common causes of malabsorption were intestinal tuberculosis, abdominal lymphoma and immunoproliferati

Endoscopic diagnosis of immunoproliferative small intestinal disease (IPSID).

Six patients with immunoproliferative small intestinal disease (IPSID), were examined endoscopically. The disease was found to involve the duodenum and jejunum and was characterized by the loss of the mucosal circular folds, a thickened cobblestone appearance of the mucosa and multiple sessile polyps. Ulceration of the mucosa was seen in one patient. A large ulcerated mass was seen in the third part of the duodenum in one patient.

Immunoproliferative small intestinal disease with duodenojejunal lymphoma: radiologic changes.

The radiologic changes in 1090 cases of lymphoma of the small bowel were analyzed and correlated with the pathologic findings. The material was collected in Iran, a high incidence area for immunoproliferative small bowel disease (IPSID). Of 100 cases of primary intestinal lymphoma associated with IPSID, 48 involved the duodenum and upper jejunum. They were associated with a characteristic spruelike radiologic pattern of the nonlymphomatous segments of the gut, the intensity of which decreased distally. However, the radiologic features of the lymphoma associated with IPSID were identical with t

HLA AW19, B12 in immunoproliferative small intestinal disease.

A group of 21 patients with immunoproliferative small intestinal disease (IPSID), 10 with alpha heavy chain paraproteinaemia and 11 without it, were HLA typed. The results were compared with 35 disease controls and 120 normal controls. A significant increase of Aw19 and B12 antigens was noted among the patients compared with the control subjects. The high rate of association of both AW19 and B12 antigens in patients as compared with controls suggests a possible haplotype (Aw19, B12) association with IPSID.

Primary intestinal lymphoma in South Africa.

All adult cases of primary intestinal lymphoma seen during the years 1953--77 at Groote Schuur Hospital, South Africa, were reviewed. Seventy percent of patients with solitary lymphoma and 80% with immunoproliferative small intestinal disease were mulatto. Patients in the latter group presented with malabsorption and those with a solitary lymphoma presented with intestinal obstruction. Four of 15 patients tested for the presence of alpha-heavy-chains were found to have alpha-heavy-chain disease. Family studies of potential genetic and immunologic factors showed some significant differences in

Idiopathic subclinical enteropathy in southern Iran.

A study of 100 normal subjects from villages in southern Iran revealed that almost one third of this population had one (or more) absorptive or morphological abnormality of the small-bowel mucosa. The finding of morphologically normal bowel mucosa in stillborn fetuses and in newborns who died within two weeks of birth was consistent with the possibility that the abnormalities noted in the village population were postnatally acquired. Finally, speculations are offered regarding the relationship of idiopathic enteropathy and "immunoproliferative small intestinal disease," which is often seen in

"Mediterranean abdominal lymphoma" or immunoproliferative small intestinal disease. Part II: pathological aspects.

The pathology of 25 cases of Mediterranean abdominal lymphoma, better designated as immunoproliferative small intestinal disease (IPSID), are reported from the American University of Beirut Hospital. The series includes nine cases with documented alpha heavy chain disease (alpha-HCD). The disease is characterized by the presence of a diffuse and compact bandlike lymphoplasmacytic infiltration of the proximal small intestinal mucosa. The presence of a concomitant malignant lymphoma in the intestine and/or mesenteric lymph nodes, and of alpha-heavy protein in the serum is commonly encountered. T

Primary upper small-intestinal lymphoma and alpha-chain disease. Report of 10 cases emphasizing pathological aspects.

Ten cases of primary upper small-intestinal lymphoma associated with alpha-chain protein in serum were discovered in a prospective study of the sera of patients with immunoproliferative small-intestinal disease (IPSID). Patients were mostly young males presenting with abdominal pain, weight loss, and diarrhea and showing laboratory evidence of carbohydrate, fat, and vitamin B12 malabsorption and hypoalbuminemia. The more frequently encountered pathologic abnormality was a diffusely nodular jejunal mucosa produced by a plasmacytic infiltrate of variable cell maturity involving a varible depth o
