Real-world effectiveness and safety of zanubrutinib in Spanish patients with relapsed/refractory marginal zone lymphoma.

There is no established standard treatment of relapsed/refractory (R/R) marginal zone lymphoma (MZL). Zanubrutinib was approved in this setting based on the phase 2 MAGNOLIA trial. We retrospectively evaluated the characteristics and outcomes of 118 real-world patients in Spain with R/R MZL treated with zanubrutinib after at least 1 previous anti-CD20-based regimen. The median age at zanubrutinib initiation was 75 years, and 56% were female. MZL subtypes included splenic (55.3%), mucosa-associated lymphoid tissue (18.9%), nodal (19.8%), and nonclassifiable (6%). The median number of previous l

Pathology, Clinical Features, and Natural History of Low-Grade B-Cell Lymphomas With Primary Osseous Presentation.

Primary bone lymphomas are rare, and most reported cases represent diffuse large B-cell lymphoma. Low-grade B-cell lymphomas presenting with primary osseous involvement (PB-LGBCLs) are far less common, and their clinicopathologic features and clinical course remain poorly defined. We retrospectively analyzed 32 PB-LGBCLs diagnosed at a single tertiary cancer center. Cases were included when the disease presented with bone-only involvement, with or without contiguous soft tissue extension, and without lymph node, spleen, or noncontiguous extranodal disease at initial diagnosis. Staging marrow a

Primary ocular adnexal MALT lymphoma involving the upper eyelid in a young woman: a case report and literature review.

Primary ocular adnexal MALT lymphoma (POAML) is a rare indolent small B-cell lymphoma that primarily involves the orbit, conjunctiva, and lacrimal gland, with a high risk of early misdiagnosis. Herein, we report a rare case of POAML arising in the right upper eyelid of a 35-year-old woman, who presented with progressive eyelid swelling. The diagnosis was established by surgical excision, histopathological examination, and immunoglobulin gene rearrangement analysis. No systemic involvement was identified, and the disease was staged as Ann Arbor IE. The patient received local radiotherapy (28 Gy

Primary Hepatic Mucosa-Associated Lymphoid Tissue Lymphoma and Primary Biliary Cholangitis: A Case Report of a Rare Association.

INTRODUCTION: Primary hepatic mucosa-associated lymphoid tissue (MALT) lymphoma is an exceedingly rare subtype of extranodal marginal zone B-cell lymphoma within the spectrum of non-Hodgkin lymphomas. Chronic inflammatory and autoimmune conditions are recognized risk factors for lymphomagenesis; however, the association between primary hepatic MALT lymphoma and primary biliary cholangitis (PBC) has been rarely reported. CASE PRESENTATION: We report the case of a 50-year-old woman with a 9-year history of non-cirrhotic PBC, treated with ursodeoxycholic acid and with only partial biochemical re

The Impact of Surgical Intervention on the Survival of Primary Head and Neck MALT Lymphoma.

INTRODUCTION: Primary head and neck mucosa-associated lymphoid tissue lymphoma (HN-MALT) is a rare B-cell lymphoma whose epidemiology, clinical features, and treatment strategies remain unclear. METHODS: We conducted a retrospective analysis of HN-MALT data from the Surveillance, Epidemiology, and End Results (SEER) database (2000-2021) and compared the incidence and mortality rates among age groups. The Kaplan-Meier was used to determine the overall survival (OS) and lymphoma-specific survival (LSS). Prognostic factors were identified using univariate and multivariate Cox regression models.

Survival outcomes and prognostic nomogram for primary pulmonary MALT lymphoma and diffuse large B-cell lymphoma: A SEER database study.

Primary pulmonary lymphoma is a rare extranodal malignancy, and robust population-based evidence regarding its treatment and outcomes remains limited. This study aimed to compare treatment patterns and survival outcomes between primary pulmonary mucosa-associated lymphoid tissue (MALT) lymphoma and primary pulmonary diffuse large B-cell lymphoma (DLBCL), as well as to develop an individualized prognostic model for pulmonary DLBCL. Patients diagnosed between 2010 and 2022 were identified from the Surveillance, Epidemiology, and End Results (SEER) database. Propensity score matching (PSM), Kapla

Clinicopathological analysis of oral-cavity B-cell neoplasms: Comparison between non-immunosuppressive and immunosuppressive patients.

No comparative analysis of the infrequently occurring B-cell neoplasms (BCNs) in the oral cavity has yet been reported in patients with non-immunosuppressiveness or immunosuppressiveness. To identify any differences in clinicopathological findings between patients with non-immunosuppressiveness and those with immunosuppressiveness, we compared the clinicopathological characteristics of oral-cavity BCNs in these patient groups. The clinical characteristics of 39 patients were analyzed from hospital records, including 24 with non-immunosuppressiveness and 15 with immunosuppressiveness. Histologi

Primary extranodal marginal zone lymphoma of gastric mucosa-associated lymphoid tissue in children: a case report and review of the literature.

BACKGROUND: Extranodal marginal zone lymphoma of mucosa-associated lymphoid tissue represents an important subtype of marginal zone lymphoma, a heterogeneous B-cell neoplasm originating from the marginal zone of lymphoid follicles. Therefore, gastric marginal zone lymphoma arising from mucosa-associated lymphoid tissue in immunocompetent children is particularly special. This article aims to report this rare case and systematically review the research progress of pediatric marginal zone lymphoma combined with its clinicopathological features. CASE PRESENTATION: A 13-year-old Chinese female pr

Fulminant Synchronous Epstein-Barr Virus-Positive MALT Lymphoma and Diffuse Large B-Cell Lymphoma Monomorphic Post-Transplant Lymphoproliferative Disorder in a Child Following Heart Transplant: A Case Report.

BACKGROUND: Post-transplant lymphoproliferative disorder (PTLD) is a devastating complication following solid-organ and hematopoietic stem cell transplantation. While PTLD is often effectively managed with immunotherapy or immuno-chemotherapy, rare cases present in a fulminant manner and are rapidly life-threatening. METHODS: We describe a case of an Epstein-Barr virus (EBV)-negative 16-year-old who developed EBV DNAemia and rapidly fulminant monomorphic PTLD within five months of receiving a heart transplant from an EBV-positive donor. RESULTS/IMPORTANT CLINICAL FINDINGS: Three months after

Primary pulmonary mucosa-associated lymphoid tissue lymphoma presenting as a right hilar mass: A case report.

RATIONALE: Primary pulmonary lymphoma (PPL) is an extremely rare malignant lung tumor that is frequently misdiagnosed as pneumonia or lung cancer due to its atypical radiological and clinical presentation. The rationale of this report is to elucidate the diagnostic value of combining computed tomography (CT) and 18F-fluorodeoxyglucose positron emission tomography/computed tomography (18F-FDG PET/CT) in evaluating a rare presentation of PPL. PATIENT CONCERNS: A 44-year-old male was admitted after an incidental lung lesion was discovered during routine screening. He was completely asymptomatic,

Concurrent presentation of distinct extranodal and splenic marginal zone lymphomas.

Marginal zone lymphomas (MZLs) encompass extranodal, splenic, and nodal subtypes that are thought to clonally arise from antigen-exposed B-cells. Here, we present the case of a patient with a concurrent diagnosis of extranodal marginal zone lymphoma (EMZL) of the ocular adnexa and splenic marginal zone lymphoma (SMZL). These were identified on a core biopsy of the ocular adnexa and a bone marrow biopsy, respectively. While sequential occurrences of MZLs have been previously documented, this represents a concurrent diagnosis of two distinct MZLs.

Diagnostic challenges posed by the wide morphological and immunophenotypic spectrum of plasma cell neoplasms.

Plasma cell neoplasms (PCNs) are a group of malignant neoplasms originating from the monoclonal expansion of immunoglobulin-secreting, terminally differentiated B cells, i.e., plasma cells (PCs). PCNs include monoclonal gammopathy of undetermined significance (MGUS), plasmacytoma (solitary plasmacytoma of the bone and extramedullary plasmacytoma), multiple myeloma (MM)/plasma cell myeloma, and rare PCNs associated with paraneoplastic syndromes. MM is a multifocal, bone marrow-based clonal PCN associated with end-organ damage and a monoclonal paraprotein (M protein) in the serum and/or urine. M

[Diagnosis/Treatment Status and Research Progress of Primary Thyroid Lymphoma--Review].

Primary thyroid lymphoma (PTL), a rare malignancy originating from thyroid lymphoid tissue, is strongly associated with Hashimoto's thyroiditis where accelerated lymphocyte proliferation and chronic antigen stimulation constitute key pathogenic mechanisms. The predominant pathological subtypes of PTL include diffuse large B-cell lymphoma and mucosa-associated lymphoid tissue lymphoma (MALTL). Current research on molecular alterations primarily focuses on MALTL, particularly pathogenic mutations in CD274 and TNFRSF14 . Diagnostic imaging modalities such as ultrasonography and computed tomograph

The treatment of marginal zone lymphoma.

The treatment landscape of B-cell non-Hodgkin lymphomas is rapidly evolving. However, few advances have occurred in marginal zone lymphoma (MZL), with a single US Food and Drug Administration-approved agent impacting the treatment landscape. Multiple factors are associated with this slower pace of progress, with a lower MZL incidence representing a significant factor. Pivotal randomized indolent lymphoma clinical trials analyzed MZL subsets without the appropriate power to capture differences between treatment arms. Furthermore, the current Lugano classification may not fully capture the prese

Drug development in MZL: caring for the forgotten child.

Marginal zone lymphoma (MZL) comprises a biologically heterogenous group of indolent B-cell lymphomas that remain substantially underrepresented in clinical research. Despite recent and significant therapeutic advances in B-cell malignancies, trial design in MZL continues to face persistent challenges, including diagnostic heterogeneity, inconsistent control arms, suboptimal end points, and economic barriers. In this narrative review, we examine these key obstacles and discuss emerging strategies to overcome them, such as the standardization of diagnostic criteria, implementation of subtype-sp

Sjögren Disease-B Cells at the Brink: From Autoimmunity to Lymphomagenesis and the Rise of Novel B Cell-Targeted Therapies.

Sjögren disease (SjD) is a common systemic autoimmune disorder characterized by inflammation of the exocrine glands, resulting in dryness. Patients frequently exhibit extraglandular manifestations affecting various organ systems. To date, there are no US Food and Drug Administration (FDA)-approved disease-modifying therapies for SjD. In this review, we explore the expanding field of SjD endotyping as a tool to enhance patient stratification, prognostication, and clinical decision-making. SjD endotypes driven by heightened B cell activity are linked to increased lymphoma risk. B cells play a ce

Venetoclax and ibrutinib induces durable clinical responses in marginal zone lymphoma.

Oral ibrutinib and venetoclax (I+V) demonstrate activity as monotherapy in marginal zone lymphoma (MZL), with low complete response (CR) rates. I+V has shown good clinical activity with acceptable tolerability in other malignancies. We conducted a single-site, phase 2 trial of daily I+V in patients with MZL. Ibrutinib commenced at 560 mg daily; after 4 weeks, venetoclax commenced with weekly dose escalation to 400 mg daily. Combination therapy continued until disease progression or toxicity. The primary end point was week 16 CR. Minimal residual disease (MRD) was assessed by flow cytometry in
