Rituximab combined with lenalidomide for the treatment of marginal zone lymphoma with IgM kappa positivity and cold agglutinin syndrome: a case report.

Cold agglutinin disease (CAD) and cold agglutinin syndrome (CAS) are currently recognized as distinct clinical entities. CAD is defined as a distinct clonal B-cell lymphoproliferative disorder of the bone marrow. CAD is also a subgroup of autoimmune hemolytic anemia, triggered by cold-reactive immunoglobulin targeting red blood cell surface antigens in its pathogenesis. CAS exhibits similar cold hemolytic anemia; however, it is secondary to hematological malignancies or infectious diseases. For lymphoma-associated CAS, specific types of antilymphoma chemotherapy are required. We report a man i

Comparative Risk of Adverse Cardiovascular Outcomes With Zanubrutinib Versus Ibrutinib in B-Cell Malignancies: A Large Real-World Propensity-Matched Analysis.

Bruton tyrosine kinase inhibitors (BTKis) are foundational therapies for B-cell malignancies but are associated with clinically important cardiovascular toxicities. Ibrutinib has been linked to atrial fibrillation/flutter (AF/AFL), bleeding, and ventricular arrhythmias, whereas second-generation BTKis such as Zanubrutinib may confer improved cardiovascular safety. In this study, we aimed to compare adverse cardiovascular outcomes between zanubrutinib and ibrutinib in a large real-world cohort. We conducted a retrospective, multicenter cohort study using the TriNetX Global Collaborative Network

Plasmablastic Lymphoma Presenting With Multiple Cutaneous Nodules Mimicking Primary Cutaneous Marginal Zone Lymphoma.

Plasmablastic lymphoma (PBL) is a rare, high-grade large B-cell lymphoma characterized by large atypical cells with plasmablastic or immunoblastic morphology and a terminal B-cell differentiation phenotype. PBL often occurs in patients with immune deficiency or dysregulation, most commonly HIV infection. Cutaneous involvement is rare and occurs most frequently in those with immune deficiency. We report a 73-year-old immunocompetent female presenting with multiple cutaneous lesions and cervical lymphadenopathy. Nasopharyngeal and cutaneous biopsies confirmed EBV-associated PBL with plasmacytic

Acquired bone marrow aplasia and long-lasting complete remission with Loncastuximab Tesirine in a patient with transformed marginal zone lymphoma: a case report.

Marginal zone lymphomas (MZL) are indolent lymphomas that may undergo transformation into aggressive lymphomas, most frequently diffuse large B-cell lymphoma (DLBCL). Loncastuximab tesirine (LONCA) is an antibody-drug conjugate used in the treatment of relapsed/refractory large B-cell lymphomas. Here we present the clinical case of a patient with nodal transformed DLBCL and concomitant bone marrow involvement by indolent lymphoma successfully treated with LONCA. However the therapy was discontinued after six cycles as the patient developed treatment-related bone marrow aplasia. This case raise

Decoding IGLL5 Mutation-Mediated BCR Signaling: A Novel Mechanism of CD8+ T Cell Exhaustion and Ocular MALT Lymphoma Progression.

Ocular adnexal mucosa-associated lymphoid tissue lymphoma (OAML) is driven by both genetic and immune microenvironmental factors, yet its pathogenic mechanisms remain incompletely understood. We previously identified IGLL5 as a recurrently mutated gene associated with poor prognosis in OAML. Functional and mechanistic analyses focusing on the S47G and A54G mutants show that these variants enhance association with the CD79A/CD79B complex, leading to persistent B-cell receptor (BCR) signaling. This signaling is accompanied by upregulation of CXCL10 and CXCL11, increased CD8+ T cell recruitment,

Pulmonary Marginal Zone B-Cell Lymphoma of Mucosa-Associated Lymphoid Tissue (PMZL-MALT) Diagnosed by Radial Probe Endobronchial Ultrasound-Guided Biopsy: A Case Report.

This case report presents a rare case of primary pulmonary marginal zone B-cell lymphoma of mucosa-associated lymphoid tissue (PMZL-MALT). Follow-up lung imaging performed 17 months after the initial presentation showed persistent pulmonary consolidation. After a radial probe endobronchial ultrasound (RP-EBUS)-guided transbronchial lung biopsy, histopathological analysis confirmed the diagnosis of primary PMZL-MALT. Notably, while RP-EBUS is commonly used for solitary peripheral pulmonary lesions, its value in asymptomatic, bilateral, nonspecific consolidations-an easily misdiagnosed scenario

Case Report: Extranodal marginal zone B-cell lymphoma in the lateral ventricle: literature review.

Primary central nervous system (CNS) mucosa-associated lymphoid tissue (MALT) lymphoma is a rare, low-grade B-cell lymphoma that typically arises in the dura, and its occurrence within the ventricular system is exceedingly rare. We report a case of primary CNS MALT lymphoma located in the right lateral ventricle of a 62-year-old man. The patient underwent gross total resection, followed by postoperative treatment with ifosfamide combined with a Bruton's tyrosine kinase (BTK) inhibitor, and adjuvant radiotherapy. At the latest follow-up, no evidence of recurrence was observed. A review of the l

IL-16 production is a mechanism of resistance to BTK inhibitors and R-CHOP in lymphomas.

Resistance to Bruton tyrosine kinase (BTK) inhibitors remains a major clinical challenge in B-cell lymphomas and often occurs in the absence of BTK or PLCG2 mutations. Here, we investigated nongenetic mechanisms of ibrutinib resistance in marginal zone lymphoma (MZL) and their broader therapeutic implications. Continuous ibrutinib exposure generated a resistant MZL model that showed cross-resistance to BTK inhibitors and degraders, without evidence of multidrug resistance or genetic alterations. Integrated transcriptomic, epigenetic, and proteomic analyses revealed extensive reprogramming, inc

Long-term outcomes and cardiac mortality after radiotherapy for primary pulmonary mucosa-associated lymphoid tissue lymphoma.

PURPOSE: Pulmonary mucosa-associated lymphoid tissue (MALT) lymphoma is a rare but indolent B-cell lymphoma, most often presenting as localized disease. Radiotherapy is a curative option, but its proximity to the heart raises concerns about long-term cardiac toxicity, which remains poorly documented. The purpose of this work was to evaluate the potential risk of cardiac toxicity following radiotherapy in patients with pulmonary MALT lymphoma. PATIENTS AND METHODS: This retrospective, population-based study used Surveillance, Epidemiology, and End Results (SEER) data (17 registries, 2000-2022;

CXCR4-targeted [68Ga]Ga-PentixaFor PET/CT improves staging and alters management in marginal zone lymphoma: a prospectively recruited head-to-head study with [18F]FDG.

BACKGROUND: Marginal zone lymphoma (MZL) exhibits substantial clinical heterogeneity, necessitating accurate whole-body tumor burden assessment and early risk stratification. However, conventional [18F]FDG PET/CT is limited in both anatomical staging and in vivo biological characterization. This prospectively recruiting study conducted a head-to-head comparison of the CXCR4-targeted tracer [68Ga]Ga-PentixaFor and [18F]FDG in treatment-naïve patients to evaluate their complementary value in improving staging accuracy and their association with early treatment response. METHODS: Forty-one treat

Antimicrobial Peptides Induce Cell Death in Marginal Zone Lymphoma Models Resistant to Targeted Therapies.

INTRODUCTION: Marginal zone lymphoma (MZL) is an indolent yet incurable B-cell malignancy in which targeted agents such as BTK and PI3K inhibitors frequently fail due to resistance or toxicity. Antimicrobial peptides (AMPs), evolutionarily conserved effectors of innate immunity, possess selective cytotoxicity against malignant cells by exploiting tumor-specific membrane alterations. METHODS: Peptides were synthesized and tested for their anti-proliferative activity in MZL cell lines. RESULTS: We evaluated the antitumor activity of seven natural AMPs, including Antarctic fish-derived trematoc

[Treatment options for marginal zone lymphoma].

Marginal zone lymphoma (MZL) is a group of indolent B-cell lymphomas with high biological and clinical heterogeneity, including mucosa-associated lymphoid tissue, nodal, and splenic MZL. Despite diverse first-line treatment options, significant differences exist between guidelines and clinical practices, lacking a unified and standardized treatment pathway. Therefore, this article elaborates on the assessment of treatment indications of MZL, the initial therapies for different MZL subtypes, and summarizes the treatment for relapsed/refractory MZL, the selection of maintenance therapy, and manag

[Current status and progress of staging and efficacy evaluation of marginal zone lymphoma].

Marginal zone lymphoma (MZL) is characterized by high clinical heterogeneity, making individualized precision therapy highly contingent upon accurate staging and dynamic response assessment. This review systematically summarizes the current status and progress in the evaluation of splenic MZL (SMZL), mucosa-associated lymphoid tissue (MALT) lymphoma, and nodal MZL (NMZL). In terms of staging modalities, selection should be based on subtype-specific characteristics; notably, as a novel radiotracer, 68Ga-Pentixafor positron emission tomography/computed tomography (PET/CT) provides crucial comple

[Current status and progress of prognostic evaluation in marginal zone lymphoma].

Marginal zone lymphoma (MZL) is an indolent B-cell non-Hodgkin lymphoma with marked heterogeneity. Currently, histological subtype, clinical stage, biochemical parameters, and clinical scoring systems represented by the MZL-international prognostic index (MZL-IPI) remain the principal basis for risk stratification; however, their differentiation power is limited, particularly for intermediate-risk groups, and they can not fully reflect the biological heterogeneity of the disease. Molecular genetic studies have revealed the significant prognostic impact of mutations in genes such as NOTCH2, KLF

SPEN Loss Drives Extrafollicular Diffuse Large B-cell Lymphoma with Female-Specific Lethality and Therapeutic Vulnerabilities.

UNLABELLED: Diffuse large B-cell lymphomas (DLBCL) are genetically and phenotypically heterogeneous, making diagnosis and treatment challenging. Current models suggest DLBCLs derive from follicular B cells engaged in adaptive immune responses. By studying cooccurring truncating mutations in SPEN and NOTCH2 in the BN2-DLBCL subtype, our data suggest a previously unrecognized extrafollicular trajectory. Using animal models and human specimens, we find that this cooperative mutational axis supports expansion of putative clonal precursors with features of marginal zone, memory, and a distinct, aut
