Deciphering the molecular landscape of Sjögren's disease, mucosa-associated lymphoid tissue lymphoma, and thyroid cancer: unraveling the complexities of disease mechanisms and diagnostic biomarkers.

BACKGROUND: Sjögren's disease (SjD), mucosa-associated lymphoid tissue lymphoma (MALT lymphoma), and thyroid cancer (THCA) are clinically distinct yet immunologically intertwined diseases characterized by chronic inflammation and aberrant immune activation. However, the shared molecular basis linking autoimmunity and oncogenesis remains poorly defined. METHODS: We performed an integrative transcriptomic analysis across three datasets (TCGA-THCA, GSE40611/GSE127952 for SjD, and GSE25638 for MALT lymphoma), totaling 749 samples. Analyses included differential expression, Gene Ontology (GO)/Kyot

Primary ureteral extranodal marginal zone lymphoma with transformation to diffuse large B-cell lymphoma mimicking urothelial carcinoma: A case report.

Extranodal marginal zone lymphoma of mucosa-associated lymphoid tissue (MALT) involving the ureter is particularly uncommon, and histological transformation to diffuse large B- cell lymphoma (DLBCL) further complicates diagnosis and management. We report a case of a female patient presenting with obstructive uropathy caused by long-segment ureteral wall thickening radiologically suggestive of urothelial carcinoma. Endoscopic evaluation and biopsies were inconclusive, leading to nephroureterectomy. Histopathological examination unexpectedly revealed MALT lymphoma with areas of transformation to

Risk-stratified clinical management of primary colorectal mucosa-associated lymphoid tissue lymphoma based on endoscopic morphology.

PURPOSE: Primary colorectal mucosa-associated lymphoid tissue (MALT) lymphoma is a rare and indolent disease with no standardized treatment. Lacking clear risk stratification criteria, treatment strategies vary widely from observation to systemic therapy. This study aimed to identify predictors of advanced-stage disease to propose risk-stratified treatment. METHODS: Patients diagnosed with primary colorectal MALT lymphoma at Samsung Medical Center (Seoul, Korea) between January 2016 and October 2025 were enrolled. Clinical, laboratory and endoscopic features were analyzed. The primary outcome

Radiotherapy as a curative strategy for primary pleural indolent lymphomas: Analytical guidelines based on literature and an illustrative population-based case series.

BACKGROUND: Primary pleural indolent non-Hodgkin lymphomas, such as marginal zone lymphoma and follicular lymphoma, are rare and understudied. When truly localized, these entities are theoretically curable with radiotherapy, as is the case for other extranodal indolent lymphomas (e.g., orbit, stomach, lung). However, due to their low incidence and frequent diagnostic delays related to nonspecific clinical and radiologic presentations, no standard therapeutic recommendations currently exist. The role of radiotherapy in this setting remains undefined, even in expert guidelines. METHODS: We perf

[Combining clinical and pathological features to explore the diagnostic challenges of marginal zone lymphoma].

The diagnosis of marginal zone lymphoma (MZL) still relies on morphology, immunohistochemistry, and flow cytometry. Pathologists and clinicians have some difficulties in the diagnosis and differential diagnosis of MZL. The key difficulty lies in its diverse clinicopathological manifestations and the lack of characteristic biomarkers or molecular biological features. Combined with clinical and pathological features, this article focuses on some difficulties in the related differential diagnosis, such as the differentiation between extranodal MZL (EMZL) of mucosa associated lymphoid tissues and

Single-cell atlas of B cell heterogeneity in lacrimal IgG4-related disorders: Unraveling the transition from inflammatory exhaustion to clonal malignancy.

This study analyzed B-cell heterogeneity in lacrimal gland tissues from 4 patients with IgG4-related ophthalmic disease (IgG4-ROD) and 2 patients with IgG4-positive MALT lymphoma using single-cell transcriptome sequencing (scRNA-seq). The results revealed that while the B-cell differentiation trajectories have similarities between the two diseases, critical differences were evident. IgG4-ROD was predominantly composed of naïve B cells, with memory B cells mainly being the central memory type. These cells were enriched in hormone/innate immunity pathways, and the plasma cells exhibited features

Refining anisotropic PTV margins for gastric MALT lymphoma radiotherapy with breath-hold: fan beam CT-guided validation and adaptation.

INTRODUCTION: To validate a fan-beam CT (FBCT)-guided Library of Plans (LoP) strategy for gastric MALT lymphoma treated under end-expiration breath-hold (EEBH), incorporating substructure-specific anisotropic margins to balance target coverage and organ-at-risk (OAR) sparing. METHODS: Two distinct cohorts were evaluated: a historical CBCT-IGRT cohort (n=28, 142 fractions) to establish baseline rigid setup deviations, and a prospective FBCT-IGRT cohort (n=33, 330 fractions) for dose recalculation, deformation analysis, and validation of the LoP strategy. Rigid setup deviations were quantified,

Functional Renal Assessment via Dynamic LAFOV 18F-FDG PET/CT in a Patient with Recurrent Extranodal Marginal-Zone Lymphoma.

We report the case of a 72-y-old man with recurrent extranodal marginal-zone B-cell lymphoma who underwent dynamic long-axial-field-of-view 18F-FDG PET/CT. In addition to detecting extensive recurrent disease, a review of a maximum-intensity-projection cine derived from the dynamic acquisition demonstrated delayed uptake and excretion from the left kidney, consistent with impaired function. This incidental functional renal assessment supplemented standard anatomic imaging and influenced treatment management. This case highlights the emerging capability of dynamic long-axial-field-of-view 18F-F

Gastrointestinal-Predominant ALK-Negative Systemic Anaplastic Large Cell Lymphoma Presenting as Fever of Unknown Origin: Case Report.

Anaplastic large cell lymphoma (ALCL) is a CD30-positive mature T-cell lymphoma. ALK-negative systemic ALCL typically affects older adults, with aggressive clinical behavior. Predominant gastrointestinal involvement is rare and may present with nonspecific systemic symptoms, posing diagnostic challenges, particularly when manifesting as fever of unknown origin. We report a case of a patient with ALK-negative systemic ALCL and predominant gastrointestinal involvement. The patient had a history of indolent cutaneous extranodal marginal zone B-cell lymphoma. The patient presented with a prolonged

A morphology-driven proof-of-concept study linking interphase nuclear abnormalities to dicentric-mediated genomic instability.

Genomic instability is a major driver of clinical heterogeneity and treatment resistance in B-cell malignancies. In chronic lymphocytic leukemia (CLL) complex karyotype (CK) and disruption of the TP53 pathway are key markers of poor prognosis. Here we investigated whether interphase nuclear abnormalities - nucleoplasmic bridges (NPB) and nuclear buds (NBUD) - visible on routine blood smears could serve as cytological surrogates of dicentric chromosomes and structural genomic instability. We analyzed 29 patients with indolent B-cell malignancies: CLL (n = 15) and splenic marginal zone lymphoma

Extranodal Marginal Zone Lymphoma: Integrating Etiology, Microenvironment, and Genetics Into Clinical Decision-Making.

Extranodal marginal zone lymphoma (EMZL) represents a unique paradigm among indolent B-cell neoplasms, in which lymphomagenesis is frequently driven by chronic antigenic stimulation within tissue-specific microenvironments. Persistent infectious or autoimmune triggers promote the development of ectopic lymphoid tissue and sustain B-cell activation, while recurrent genetic alterations-most prominently those that converge on NF-κB signaling-enable progressive escape from antigen dependence. This dual biological foundation explains both the indolent clinical course of most cases and the remarkabl

Primary Oral MALT Lymphoma Without Nodal or Systemic Involvement: A Rare Presentation.

Extranodal marginal zone lymphoma of mucosa-associated lymphoid tissue (MALT lymphoma) is a low-grade B-cell lymphoma, whose primary manifestation in the oral cavity is exceedingly rare. Owing to its nonspecific clinical presentation and histopathological overlap with benign inflammatory or reactive lesions, the diagnosis of MALT lymphoma can be challenging. We report the case of a middle-aged patient presenting with a slow-growing, asymptomatic oral lesion initially suspected to be an oral lipoma. Histopathological examination, complemented by immunohistochemical analysis, confirmed the diagn

Utility of MEF2B immunohistochemistry in distinguishing follicular lymphoma and marginal zone lymphoma in diagnostically challenging cases.

BACKGROUND: Distinguishing follicular lymphoma (FL) from marginal zone lymphoma (MZL) can be challenging, as the morphology and immunophenotype can overlap particularly in cases with marginal zone differentiation (MZD). MEF2B (Myocyte enhancer binding factor B) is a transcription factor that controls BCL6 expression in germinal center (GC) B-cells. Previous studies reported this stain to have a high sensitivity and specificity for FL compared to other GC markers. PURPOSE: We investigated the expression of MEF2B in small B cell lymphomas (SBCL) to evaluate its utility in classification of diag

Hepatitis B Virus Reactivation During Rituximab Therapy Despite Anti-HBs Positivity: A Case Report.

Hepatitis B virus (HBV) reactivation is a well-recognized complication of immunosuppressive therapy, particularly with B-cell-depleting agents such as rituximab. Anti-HBs positivity may reflect previous immune control, but it does not eliminate the risk of reactivation in patients with previously resolved HBV infection. We report the case of a 63-year-old man with splenic marginal zone lymphoma treated with rituximab-based chemotherapy. Before treatment, HBV testing showed negative HBsAg, positive anti-HBc, positive anti-HBs, and undetectable HBV DNA. No antiviral prophylaxis was initiated bef

Primary Spinal Marginal Zone Lymphoma Presenting With Spinal Cord Compression: A Case Report.

Extranodal marginal zone lymphoma (EMZL) is an indolent B-cell lymphoma that typically arises in mucosal or extranodal tissues. Spinal involvement is exceptionally rare and may present with spinal cord compression, posing diagnostic and therapeutic challenges. We report the case of a 70-year-old man who presented with progressive paraparesis following a four-month history of lumbosciatic pain. MRI revealed an epidural and paravertebral mass extending from L2 to L4 with vertebral body destruction and spinal canal invasion. Urgent decompressive laminectomy with subtotal resection was performed.

Hairy cell leukemia in a 51-year-old Syrian male: a case report.

INTRODUCTION: Hairy cell leukemia (HCL), a rare B-cell lymphoproliferative disorder, originates from the splenic marginal zone B cell. However, diagnosis can be particularly challenging in resource-limited settings where advanced tests are not readily available. Misclassification may result in non-selective chemotherapy exposure and increased toxicity. Reporting such cases is important to highlight diagnostic challenges in resource-limited countries. CASE PRESENTATION: A 51-year-old male presented with abdominal discomfort, weight loss, and fatigue. Examination revealed splenomegaly. Initial

[Proliferative Glomerulonephritis with Monoclonal Immunoglobulin Deposits (PGNMID) Associated with Marginal Zone B-cell Lymphoma: A Case of MGRS in a Patient with Nephrotic Syndrome and Rapidly Progressive Renal Failure].

Monoclonal gammopathies of renal significance constitute a heterogeneous spectrum of nephropathies characterised by renal deposition of monoclonal immunoglobulins or their fragments, produced by clonal proliferation of B lymphocytes or plasma cells, in the absence of diagnostic criteria for symptomatic multiple myeloma or overt lymphoma. In recent years, the growing recognition of these nosological entities has highlighted how early diagnosis and the implementation of clone-directed therapeutic strategies can significantly alter the natural history of the disease and improve long-term renal ou

Dose-reduced (24 Gy) involved-site radiotherapy combined with rituximab in early-stage non-gastric mucosa-associated lymphoid tissue lymphoma: a prospective phase II trial.

While involved-site radiotherapy (ISRT) is the standard first-line treatment in localized non-gastric mucosa-associated lymphoid tissue (MALT) lymphoma, the cumulative risk of distant relapse poses a persistent clinical challenge. We conducted a prospective phase II trial evaluating rituximab with 24 Gy ISRT, aiming to reduce distant relapse and enhance long-term survival. By October 2025, 60 patients with early-stage non-gastric MALT lymphoma were enrolled. Among the per-protocol efficacy-evaluable cohort (n = 55), the combined immunoradiotherapy regimen achieved a complete response rate of 1
