Successful Endoscopic Submucosal Dissection for Primary Esophageal Mucosa-Associated Lymphoid Tissue Lymphoma.

Primary esophageal extranodal marginal zone lymphoma of mucosa-associated lymphoid tissue (MALT lymphoma) is extremely rare, and its optimal management has not been established. We report a case of primary esophageal MALT lymphoma that was successfully treated by endoscopic submucosal dissection (ESD). A 70-year-old woman presented with an elevated lesion in the lower esophagus that was detected incidentally on computed tomography. Endoscopy and endoscopic ultrasonography revealed a subepithelial lesion confined to the submucosal layer without apparent invasion of the muscularis propria. Altho

Comparison of heart-sparing efficacy between deep inspiration and end-expiration breath-hold in gastric MALT lymphoma radiotherapy: Spatial separation versus margin reduction.

PURPOSE: This study aims to explore whether spatial separation in deep inspiration breath-hold (DIBH) or margin reduction in end-expiration breath-hold (EEBH) achieves heart-sparing efficacy in the radiotherapy (RT) of gastric mucosa-associated lymphoid tissue (MALT) lymphoma patients. METHODS AND MATERIALS: 51 gastric MALT lymphoma patients treated between 2023-2025 were enrolled and trained for either DIBH or EEBH. Dosimetric parameters (heart and left ventricle V5-V30 and mean doses) were compared. Geometric relationships between the heart and target volume were characterized using distanc

A Dual Response to CD20xCD3 Bispecific Therapy: Remission of Relapsed Diffuse Large B-cell Lymphoma and Improvement in Immune Thrombocytopenia.

Immune thrombocytopenia (ITP) is an autoimmune cytopenia that may occur in association with lymphoproliferative disorders. In such cases, treatment of the underlying malignancy may influence platelet recovery. CD20xCD3 bispecific antibodies have become important options for relapsed or refractory diffuse large B-cell lymphoma (DLBCL), but their effect on coexisting autoimmune cytopenias is not well defined. We report the case of an 81-year-old woman with relapsed DLBCL arising from an underlying marginal zone lymphoma and longstanding ITP. She achieved complete metabolic remission with initial

Auer rod-like inclusions in marginal zone lymphoma: a potential diagnostic pitfall.

A 47-year-old man presented with leukocytosis and absolute lymphocytosis. Peripheral blood smear examination revealed atypical lymphoid cells containing striking Auer rod-like cytoplasmic inclusions, creating a strong morphologic impression of acute promyelocytic leukemia. However, flow cytometry demonstrated a mature monoclonal B cell population lacking myeloid differentiation, and bone marrow biopsy established the diagnosis of marginal zone lymphoma. This rare case highlights a major diagnostic pitfall and underscores that Auer rod-like inclusions are not exclusively restricted to myeloid n

Primary Lymphomas of the Female Genital Tract: Recognizing the Rare Mimicker of Gynecologic Malignancy.

Primary lymphomas of the female genital tract (PLFGT) are extremely rare neoplasms, representing a minor fraction of both extranodal lymphomas and gynecologic malignancies. Due to their nonspecific clinical presentation and overlapping imaging features with more common gynecologic tumors, diagnosis is often delayed or missed. This narrative review aims to synthesize current evidence on the clinical characteristics, histologic subtypes, diagnostic approaches, treatment strategies, and prognostic factors of PLFGT, emphasizing recent developments that may influence clinical practice. A comprehens

Challenges in Differential Diagnosis and Management of Lymphoepithelial Sialadenitis (LESA): A Scoping Review.

Background: Lymphoepithelial sialadenitis (LESA) is a chronic lymphoid-rich inflammatory disorder of the salivary glands that is strongly associated with Sjögren's disease and may overlap clinically, radiologically, and histopathologically with IgG4-related sialadenitis, HIV-associated lymphoepithelial lesions, chronic sialadenitis, salivary gland tumors, and extranodal marginal zone B-cell lymphoma of mucosa-associated lymphoid tissue (MALT lymphoma). Objective: This scoping review mapped current evidence on diagnostic challenges, differential diagnostic criteria, management strategies, and s

Unmasking a Composite Lymphoma: Follicular Lymphoma Emerging After Bispecific Antibody Treatment of Marginal Zone Lymphoma.

INTRODUCTION: Composite indolent B-cell lymphomas may remain unrecognized when one component dominates the clinical presentation. Apparent relapse after therapy may, therefore, represent the emergence of a biologically distinct lymphoma rather than the recurrence of the original disease. CASE PRESENTATION: An 82-year-old woman presented with systemic low-grade B-cell lymphoma that was most consistent with marginal zone lymphoma (MZL). Staging demonstrated peripheral blood involvement and multifocal FDG-avid disease. Treatment with the CD20×CD3-bispecific antibody mosunetuzumab resulted in com

The role of notch signaling in tumor angiogenesis in hematological malignancies.

The evolutionarily conserved Notch signaling pathway is a contact-dependent communication mechanism regulating cell fate, tissue patterning, and cellular metabolism. Beyond canonical NICD-RBPJ transcription, Notch engages in non-canonical crosstalk with networks like Wnt and NF-κB. In tumor angiogenesis, the endothelial Foxc1/2-DLL4-Notch1 axis coordinates tip/stalk cell plasticity, while restricting glycolytic flux via PFKFB3 downregulation to promote vessel maturation. Consequently, pathway inhibition triggers non-productive hypersprouting and vascular collapse, whereas Notch activation stra

Oncogenic Role of BCL6 Rearrangement: A Focus on Follicular and Marginal Zone Lymphomas.

The BCL6 gene is a transcriptional regulator essential for germinal center B-cell function, modulating their activation, survival, cell cycle, and differentiation into plasma or memory B-cells. In indolent B-cell lymphomas, particularly follicular lymphoma and a subset of marginal zone lymphomas, BCL6 rearrangements are increasingly recognized as recurrent but heterogeneous events. In follicular lymphoma, their clinicopathologic significance differs according to the BCL2 status. While BCL6 rearrangements have limited impact in conventional BCL2-rearranged follicular lymphoma, BCL2-negative/BCL

Mosunetuzumab With Response-Adapted Polatuzumab Vedotin and Obinutuzumab in Untreated Indolent B-Cell Non-Hodgkin Lymphoma.

PURPOSE: Chemoimmunotherapy has been the standard approach for untreated indolent B-cell lymphoma. We hypothesized that initial use of the CD3/CD20 bispecific mosunetuzumab followed by response-adapted polatuzumab vedotin and obinutuzumab would yield an optimized chemotherapy-free approach. METHODS: Previously untreated patients with follicular lymphoma (FL) and marginal zone lymphoma (MZL) with indication for treatment received eight cycles of mosunetuzumab. Those not achieving a complete response (CR) by positron emission tomography-computed tomography (PET-CT) following mosunetuzumab could

Lymphoma as a Rare Cause of Ureteral Obstruction: A Case Report.

Extranodal marginal zone lymphoma involving the urinary system is a rare manifestation of lymphoma. Ureteric obstruction by this subtype of lymphoma can mimic other more common genitourinary or retroperitoneal conditions. The current case describes a 42-year-old female who presented with abdominal pain and hydronephrosis without findings supportive of renal or ureteric calculus. Diagnostic work-up was unremarkable, even with the employment of endoscopic evaluation. The patient's ureter was dilated and she underwent both stenting and eventual percutaneous nephrostomy tube placement. Ultimately,

Isolated levator palpebrae superioris enlargement as the sole harbinger of primary orbital lymphoma.

We report a unique case of 59-year-old man, who presented with a 3-month history of progressive, painless right blepharoptosis with poor levator function. Magnetic resonance imaging demonstrated enlargement of the right levator palpebrae superioris (LPS) with sparing of the superior rectus and other extraocular muscles. The muscle was isointense on T1 and T2-weighted images with homogenous contrast enhancement and a low apparent diffusion coefficient. Incisional biopsy via upper lid skin crease incision revealed a thickened levator muscle, and histopathology showed a low-grade B-cell non-Hodgk

Nodal marginal zone lymphoma: genetic analysis and the differential diagnosis from other small B-cell lymphomas.

Nodal marginal zone lymphoma (NMZL) is a rare indolent B-cell lymphoma without a disease-defining phenotype. The distinction from other small B-cell lymphomas is challenging, especially from t(14;18)-negative follicular lymphoma (FL). To find out whether molecular analysis could help in separating NMZL from other small B-cell lymphomas, we analyzed a cohort of 52 cases with the diagnosis or differential diagnosis of NMZL using targeted next generation sequencing and gene expression profiling (GEP). After morphological and mutational analysis, 38 (73%) cases were classified as NMZL, 8 (15%) cas

Hematolymphoid neoplasms involving the breast: A single institution clinicopathologic study of 59 patients.

Breast hematopoietic neoplasms (BHN) are rare and may either primarily (PBHN) or secondarily (SBHN) involve breast tissue. The widespread use of needle biopsy for diagnosis of breast lesions necessitates knowledge of their presentation, radiologic aspects, histomorphology, and outcomes for seeking appropriate hematopathology consultation. Herein, we present our clinical experience as an academic institution and referral center of BHN in the past 20 years. We identified 59 patients diagnosed at the University of Chicago Medical Center between 2002-2021. Demographic, pathologic, radiologic, ther

Cross-tissue atlas of mucosa-associated lymphoid tissue lymphomas reveals intratumoral heterogeneity and microenvironmental subtypes.

Mucosa-associated lymphoid tissue (MALT) lymphoma, a common primary extranodal B cell lymphoma that arises in diverse mucosal tissues, has a pathogenesis closely linked to the supportive lymphoma microenvironment (LME). However, its microenvironmental heterogeneity and therapeutic implications remain unclear. Here, we report a cohort of 89 MALT lymphoma samples from multiple anatomical sites, profiled by bulk (n = 89), single-nucleus (n = 16), and spatial (n = 9) RNA sequencing. Integrative analysis of 120 bulk transcriptomes from our cohort and two independent cohorts, guided by cell-type-res

A Recently Recognized and Underdiagnosed Entity of Splenic Diffuse Red Pulp Small B-Cell Lymphoma: A Report of Two Cases.

Splenic diffuse red pulp small B-cell lymphoma (SDRPL) is an extremely rare entity newly individualized in the WHO 2022 classification, following provisional recognition in 2008. It remains underrecognized in routine practice. Its diagnosis is challenging because it overlaps with other splenic lymphomas with villous lymphocytes, particularly splenic marginal zone lymphoma (SMZL) and hairy cell leukemia variants, with only a few cases reported due to recent recognition. We present two patients illustrating their distinctive diagnostic and therapeutic phenotypes. Case 1 was a 65-year-old woman d

18 F-FAPI-04 PET/CT Outperforms 18 F-FDG in Detecting Multiple Muscle and Subcutaneous Adipose Tissue Involvement of Marginal Zone Lymphoma.

Marginal zone lymphoma (MZL) involving multiple muscles and subcutaneous adipose tissue is extremely rare. We present PET/CT findings of pathologically confirmed MZL with muscle and subcutaneous involvement in a 63-year-old man. 18 F-FAPI-04 PET/CT demonstrated significantly higher radiotracer uptake and better lesion detectability than 18 F-FDG PET/CT in all detectable lesions and, in addition, identified occult right neck muscle involvement not clearly appreciable on 18 F-FDG PET/CT. Model-Based Analysis with Mechanistic Insights to CAR-T-Cell Therapy Kinetics: Case Study with Axicabtagene Ciloleucel and Brexucabtagene Autoleucel.

Model-informed drug development is increasingly used to support chimeric antigen receptor (CAR)-T-cell therapy programs. However, most population pharmacokinetic (PK) CAR-T-cell models available in literature are variations of an empirical piecewise-linear model, which accurately describes the observed data but lacks a mechanistic foundation. This limits its use for simulations of scenarios beyond the observed data. This work presents a mechanism-based population PK framework that aims to bridge existing empirical and mechanistic CAR-T-cell modeling approaches. The model was developed using a
