Treatment patterns in relapsed/refractory marginal zone lymphoma: real-world evidence from seven countries.

BACKGROUND: Given the rarity of marginal zone lymphoma (MZL), real-world data on relapsed/refractory (R/R) MZL are limited. METHODS: This retrospective panel-based chart review study examined patient characteristics and treatment patterns among patients with R/R MZL who received a second or third line (2L or 3L) of therapy between 1 January 2014 and 31 December 2021 in 7 countries. Treatment patterns were summarized overall and by MZL type. RESULTS: The study included 526 patients, 513 with 2L and 13 with 3L as index treatment. The median age of 2L patients was 67.0 years. Most patients had

Marginal Zone Lymphoma With T-Follicular Helper Hyperplasia: Paving the Way for New Concepts in Cellular Plasticity and Unveiling Histopathologic Pitfalls.

The expression of T-follicular helper (TFH) markers, initially described in nodal T-cell lymphomas arising from TFH-cells, has been reported in several non-TFH lymphomas, including B-cell lymphomas. It has also been observed in marginal zone lymphomas (MZL), whereas it is poorly characterized in nodal MZL (NMZL). The present study, therefore, aimed to characterize MZL with TFH cell expansions, by providing an extensive description of their histologic and molecular features. Thirty-two MZL with nodal involvement (25 primary nodal, 4 extranodal, and 3 primary splenic) showing TFH hyperplasia and

Gastric plasmacytoma with loss of Cd138 expression mimicking extranodal marginal zone lymphoma of mucosa-associated lymphoid tissue: a case report.

INTRODUCTION: Multiple myeloma (MM) is a malignant plasma cell neoplasm, while extramedullary plasmacytoma (EMP) represents a rare manifestation and is frequently associated with an adverse prognosis. Gastric involvement by EMP is exceedingly uncommon and may present significant diagnostic challenges, particularly in cases with atypical immunophenotypic features that resemble B-cell lymphomas, most notably extranodal marginal zone lymphoma of mucosa-associated lymphoid tissue (MALT lymphoma). CASE PRESENTATION: We report a 64-year-old man who had a large gastric tumor and was diagnosed with M

Peripheral CD8⁺ T-cell clonality and NK-cell expansion mark early response to mosunetuzumab in indolent B-cell lymphoma.

Mosunetuzumab is highly effective against many B-cell lymphomas, but longitudinal immune changes induced by intermittently dosed CD20×CD3 bispecific antibodies remain incompletely characterized. We performed immune profiling of peripheral blood samples from patients receiving fixed-duration (6 months) subcutaneous mosunetuzumab for previously untreated follicular or marginal zone lymphoma on a phase 2 clinical trial (NCT04792502). Analyses included flow cytometry, cytokine profiling, and RNA-seq of isolated CD8+ T-cells with TCR repertoire inference. Early (at 3 weeks) on-treatment samples sho

Identification of Novel Risk Loci for Common B-Cell Lymphoma Subtypes Through Cross-Trait Analysis with Idiopathic Inflammatory Myopathies.

Objectives: The genetic architecture of B-cell lymphomas is not fully characterized. We aimed to identify novel candidate variants associated with four common B-cell lymphoma subtypes-diffuse large B-cell lymphoma (DLBCL), follicular lymphoma (FL), chronic lymphocytic leukemia (CLL), and marginal zone lymphoma (MZL)-through cross-trait analyses with correlated idiopathic inflammatory myopathies subtypes, dermatomyositis (DM) and polymyositis (PM). Methods: Leveraging shared genetic susceptibility with DM and PM, we applied a pleiotropy-informed conditional false discovery rate (condFDR) method

Clinicopathologic spectrum and diagnostic strategies of intrahepatic neoplasms with prominent inflammatory background.

Intrahepatic neoplastic lesions with prominent inflammatory background (HNLwPIBs) constitute a heterogeneous group of entities that frequently pose substantial diagnostic challenges because of overlapping morphological features. To characterize their clinicopathological spectrum, diagnostic pitfalls, and key distinguishing clues, we retrospectively analyzed 60 cases of HNLwPIBs diagnosed at our institution over a 11-year period (2015-2025). The cohort comprised 8 distinct entities, including lymphoepithelioma-like cholangiocarcinoma (LEL-ICC), lymphocyte-rich hepatocellular carcinoma (LR-HCC),

Clinicopathologic spectrum and outcomes of primary ovarian lymphoma: a retrospective case series.

Primary ovarian lymphoma (POL) is rare and often presents as an adnexal mass, creating diagnostic and management challenges. We performed a retrospective cohort study of 13 women diagnosed with POL between January 2018 and December 2023. Median age was 44 years (range 13-69), with 69.2% being premenopausal. LDH was elevated in 9/13 (69.2%) and CA-125 in 8/10 (80.0%) while CEA and CA19-9 were negative. Histology was heterogeneous: diffuse large B-cell lymphoma (DLBCL, 46.2%), extranodal NK/T-cell lymphoma (23.1%), Burkitt lymphoma (15.4%), marginal zone lymphoma (7.7%), and follicular lymphoma

Outcomes of CAR T-Cell Therapy in transformed indolent Non-Hodgkin Lymphomas and de novo DLBCL: A comparative analysis from the Italian CAR T-SIE study.

BACKGROUND: Transformed indolent non-Hodgkin lymphoma (TiNHL) generally has poorer outcomes than de novo diffuse large B-cell lymphoma (dnDLBCL). Although anti-CD19 CAR T-cell therapy is standard for relapsed/refractory (R/R) large B-cell lymphoma (LBCL), its prognostic role across histologies remains unclear. METHODS: We conducted a multicenter, retrospective/prospective observational study comparing R/R TiNHL and R/R dnDLBCL patients treated with commercial CAR T-cell therapy and enrolled in the Italian CART-SIE study. FINDINGS: Among 438 patients, 107 had TiNHL (96 transformed follicular

Beyond the Battlefield: Lymphoma Outcomes in Post-9/11 Service Members and Veterans.

Background: Non-Hodgkin lymphoma (NHL) is the most common hematologic malignancy among U.S. military service members (SMs), yet the impact of deployment and deployment-related factors on survival remains unclear. Modern military service involves potential exposure to environmental hazards and trauma that could plausibly influence lymphoma biology, treatment tolerance, or long-term outcomes. Methods: Tricare beneficiaries with NHL diagnosed between 2001 and 2022 were identified from the Defense Health Agency Cancer Registry. Deployment history, demographics, lymphoma subtype, and mortality data

[Diagnostic approach to non-cutaneous T-cell lymphomas in 2025: What pitfalls and how should they be classified?].

Peripheral T-cell lymphomas are a heterogeneous group of neoplasms derived from mature T or NK lymphocytes. With the exception of primary cutaneous T-cell lymphomas, these lymphomas are rare (<7% of lymphomas in France). Peripheral T-cell lymphomas derived from follicular helper T cells (TFH) account for more than 40% of cases. TFH lymphoma has numerous diagnostic pitfalls, including classical Hodgkin lymphoma, diffuse large B-cell lymphoma, and marginal zone lymphoma, which are attributable to the "follicular T helper" function of the tumor cells. Anaplastic large cell lymphomas (ALCL) are cl

Genetically Predicted Gut Microbiota and Lymphoma Risk: A Mendelian Randomization Study.

INTRODUCTION: Growing evidence links gut microbiota (GM) to hematological malignancies; however, its role in lymphoma remains unclear. This study aimed to investigate the potential causal relationships between genetically predicted gut microbial taxa and lymphoma subtypes using a Mendelian randomization (MR) framework. METHODS: Using genome-wide association study (GWAS) summary data for 211 gut microbial taxa and 10 lymphoma subtypes, we performed bidirectional Mendelian randomization (MR) and sensitivity analyses to assess causality. Reverse MR was also used to evaluate reverse causation. St

Glofitamab in combination with immunochemotherapy in patients with relapsed/refractory B-cell non-Hodgkin lymphoma: Phase 1b dose-escalation study.

Glofitamab was investigated with obinutuzumab/rituximab plus cyclophosphamide, doxorubicin, vincristine and prednisone (G/R-CHOP) in patients with relapsed/refractory B-cell non-Hodgkin lymphoma (B-NHL) who had ≥1 prior obinutuzumab/rituximab-containing therapy (NCT03467373). The primary end-point was to determine the optimal biological dose of glofitamab with R-CHOP. Using a modified continual reassessment method, glofitamab dose escalation was administered from cycle (C) 2 day (D)8 (70 μg, 1800 μg, 10 mg, 30 mg), with G/R- CHOP for 6-8 21-day cycles. The 70 and 1800 μg cohorts received fixed

Indolent primary cutaneous B-cell lymphomas resemble persistent antigen reactions without signs of dedifferentiation.

Primary cutaneous B-cell lymphoma encompass clinically heterogeneous entities. While primary cutaneous diffuse large B-cell lymphoma, leg type (pcDLBCL-LT) is aggressive, primary cutaneous follicle centre lymphoma (pcFCL) and primary cutaneous marginal zone lymphoma (pcMZL) typically follow an indolent course. To clarify their pathophysiological basis, we perform single-cell RNA sequencing on pcFCL, pcMZL, and pcDLBCL-LT, alongside reactive B-cell rich lymphoid proliferations (rB-LP), gastric mucosa-associated lymphoid tissue (MALT) lymphoma, and systemic counterparts. Here we show that the in
