Functional characterization of neutrophils in patients with multiple myeloma.

Multiple myeloma is a hematologic cancer caused by abnormal plasma cell proliferation in the bone marrow and is associated with significant immune dysfunction. Neutrophils, essential components of the innate immune response, may function poorly in patients with multiple myeloma, increasing the risk of infection and affecting disease progression. This study aimed to systematically evaluate the extent of neutrophil dysfunction in patients with newly diagnosed and relapsed multiple myeloma. The study employed a cross-sectional analytical design and recruited 72 participants: 30 multiple myeloma p

Myeloid-derived suppressor cells in hematologic malignancies: A current state of the field.

Myeloid-derived suppressor cells are a pivotal component of the immunosuppressive tumor microenvironment in hematologic malignancies. Through diverse mechanisms, they promote disease progression, therapeutic resistance, and immune evasion. This comprehensive review synthesizes current evidence on the roles of myeloid-derived suppressor cells across major hematologic cancers and evaluates their prognostic significance, highlighting implications for patient stratification and potential therapeutic targeting.

Comparison of non-cryopreserved and cryopreserved autologous hematopoietic stem cell transplantation among patients with myeloma: single-center experience.

BACKGROUND: Non-cryopreserved hematopoietic stem cells (NCP HSCs) may simplify autologous stem cell transplantation (ASCT) by avoiding cryopreservation and dimethyl sulfoxide exposure. We compared the clinical outcomes of NCP HSCs and cryopreserved HSCs (CP HSCs) in patients with multiple myeloma. METHODS: This retrospective, single-center cohort study included 200 consecutive patients with multiple myeloma who underwent ASCT between 2014 and 2024: 147 received NCP HSCs, and 53 received CP HSCs. Preservation practices changed over time, and NCP HSC use increased from 46.6% during 2014-2019 to

[AL amyloidosis associated with multiple myeloma revealed by cholestatic hepatomegaly: A case report].

INTRODUCTION: AL amyloidosis is a rare disorder characterized by the extracellular deposition of immunoglobulin light chain fibrils produced by a clonal plasma cell population. Hepatic involvement as the initial manifestation is rare, and forms associated with jaundice and cholestasis carry a poor prognosis. CASE REPORT: We report the case of a 53-year-old man from Togo presenting with massive hepatomegaly, cholestatic jaundice, and weight loss. After a negative etiological workup, diffuse hepatic infiltration associated with a plasma cell dyscrasia raised suspicion of AL amyloidosis. Periphe

A natural product-derived degrader (COT-01): cortisone-based CRBN recruitment for cyclin D2 degradation in multiple myeloma.

Overexpression of Cyclin D2 drives aberrant cell cycle progression in multiple myeloma (MM). However, its lack of intrinsic enzymatic activity renders it an "undruggable" target for conventional small-molecule inhibitors. Here, we report the identification of COT-01, a natural product-derived cortisone analog, as a CRBN-dependent molecular glue degrader of Cyclin D2. COT-01 exhibited CRBN-binding activity with an apparent IC50 of 2.5 μM in an HTRF-based competitive assay, and BLI analysis independently supported concentration-dependent binding of COT-01 to recombinant human CRBNmidi, with a ki

Biparatopic targeting of CD38 using CD38-specific trimeric antibody recruiters improves cytotoxicity against multiple myeloma.

RATIONALE: Antibody recruiters are an innovative class of immunotherapeutics, comprising a tumor-binding module and an antibody-binding module to redirect endogenous antibodies toward target cells. Recruited antibodies can then induce Fc-dependent effector functions to lyse tumor cells. The clinical success of CD38-directed monoclonal antibodies daratumumab and isatuximab has established CD38 as a validated target for the treatment of multiple myeloma (MM). Since daratumumab treatment can result in antigen loss or mutation of the binding site, novel immunotherapeutics should explore different

Renal rescue in myeloma: How deep does the hematologic response need to be?

The choice of first-line therapy is exceptionally challenging when multiple myeloma is associated with advanced renal failure. In this issue of Med, Xiao and colleagues report on results from a randomized phase 2 study1 indicating that the pomalidomide, bortezomib, and dexamethasone triplet leads to better myeloma disease control, thereby increasing renal function after four cycles.

MAIA frailty subgroup analysis: long-term follow-up with daratumumab plus lenalidomide/dexamethasone in transplant-ineligible newly diagnosed myeloma.

In MAIA (Daratumumab, lenalidomide, and dexamethasone versus lenalidomide and dexamethasone alone in newly diagnosed multiple myeloma), daratumumab plus lenalidomide/dexamethasone (D-Rd) improved progression-free survival (PFS) and overall survival (OS) versus Rd in transplant-ineligible newly diagnosed multiple myeloma (NDMM). Here, we report an updated subgroup analysis by frailty status. We retrospectively performed frailty assessments using age, Charlson Comorbidity Index, and baseline Eastern Cooperative Oncology Group performance status score; patients were classified as fit, intermediat

T-cell-redirecting therapies in early relapsed multiple myeloma: a systematic review and network meta-analysis.

BACKGROUND: Chimeric antigen receptor T cells and bispecific antibodies, are moving from late-line salvage into early relapsed multiple myeloma. Indirect comparisons in heavily pretreated patients have favored cell therapy, but whether that hierarchy holds in early relapse is unknown. METHODS: We analyzed randomized phase 3 trials comparing a T-cell-redirecting therapy against a daratumumab-pomalidomide-dexamethasone-type control after at least one prior line. Pseudo-individual patient time-to-event data were reconstructed from published survival curves. Regimens were compared indirectly in B

Investigating Denosumab Interference in Serum Protein Electrophoresis.

BACKGROUND: Therapeutic monoclonal antibodies, widely used in multiple myeloma (MM), can interfere with the interpretation of serum protein electrophoresis (SPEP) and immunofixation electrophoresis results, challenging the differentiation from endogenous monoclonal (M) proteins associated with the disease. Although this interference has been well established for several therapeutic monoclonal antibodies, the potential impact of denosumab, an IgG2(κ) antibody used to treat MM-related bone disease, remains unclear. METHODS: We characterized the electrophoretic migration of denosumab using in vi

Targeting Exportin-1 in Cancer: From Nuclear Export to Oncogenic Transcriptional Hubs.

Maintaining cellular homeostasis requires precise coordination of molecular trafficking between the nucleus and the cytoplasm. Central to this process is the nucleocytoplasmic transport system, which governs the spatial distribution of proteins and RNA. Exportin-1 (XPO1), also known as chromosome region maintenance 1 (CRM1), is the principal nuclear export receptor responsible for the transport of hundreds of cargo proteins bearing leucine-rich nuclear export signals. In a wide range of malignancies, XPO1 is frequently dysregulated through overexpression or recurrent somatic mutations. This dy

Translocation-induced enhancer rewiring reveals cryptic oncogenic circuits in multiple myeloma.

Immunoglobulin heavy chain (IGH) enhancer translocations drive multiple myeloma (MM), but routine diagnostics resolve only 32 to 38% of cases. Here, we introduce TransFinder, a multiomic pipeline integrating SMRT long-read sequencing (LRS), Hi-C, H3K27ac ChIP-seq/CUT&Tag, and RNA-seq to systematically resolve cryptic translocations and their hijacked oncogenes by deconvoluting enhancer-promoter neo-loops at rearrangement breakpoints. Applied to MM, TransFinder identified that t(16;22)(q23;q11), a canonical translocation known to activate MAF, also hijacks enhancers to activate RAB36. It furthe
