Extended follow-up of in vivo BCMA CAR-T therapy in relapsed/refractory multiple myeloma.

Data on long-term outcomes of in vivo chimeric antigen receptor T cell (CAR‑T) therapy remain scarce. We previously reported preliminary safety and efficacy data of in vivo B-cell maturation antigen (BCMA) CAR‑T (ESO-T01), in four patients with relapsed/refractory multiple myeloma, without leukapheresis and lymphodepletion, with up to 3 months of follow-up. The primary endpoints included safety and tolerability, while the secondary endpoints comprised efficacy, pharmacokinetics and pharmacodynamics. All patients developed dual-phase cytokine release syndrome (grades 1-3) and hematotoxicities (

Carfilzomib preserves dendritic cell phenotype and function compared with bortezomib.

INTRODUCTION: Despite major therapeutic advances, multiple myeloma (MM) remains largely incurable. Moreover, side effects and toxicities of treatments compromise patients' quality of life. Dendritic cell (DC)-based cancer vaccines can induce long-term tumor-specific immune responses and are generally well tolerated. To improve the potency of this immunotherapeutic approach, researchers are now exploring combination strategies that integrate approved agents with DC-based vaccines. Proteasome inhibitors (PIs), including bortezomib and carfilzomib, are the standard of care for MM. PIs have been s

Increased Out-of-Hospital Cardiac Arrest Risk in Cancer: Evidence From 800,000 Patients and 3.7 Million Controls.

BACKGROUND: Although cancer patients face an increased risk of cardiovascular disease, their risk of out-of-hospital cardiac arrest (OHCA) remains poorly characterized. OBJECTIVES: The objective of the study was to investigate the hazard of OHCA among patients with cancer and identify clinical factors associated with increased hazard compared with cancer-free controls. METHODS: This population-based, matched cohort study included adults (≥18 years) diagnosed with cancer from 2010 to 2022, matched 1:5 on age, sex, and region, to cancer-free controls. Using 7 national registers, we assessed th

Outpatient step-up dosing of teclistamab and talquetamab in relapsed or refractory.

BACKGROUND: Bispecific T-cell-engaging antibodies against BCMA and GPRC5D demonstrate high efficacy in relapsed/refractory multiple myeloma (RRMM) but are associated with cytokine release syndrome (CRS) and immune effector cell-associated neurotoxicity syndrome (ICANS), traditionally prompting inpatient monitoring when commencing therapy. Contemporary step-up dosing and prophylactic strategies may enable safe outpatient administration. PATIENTS AND METHODS: We conducted a single-center retrospective study evaluating outpatient step-up dosing (SUD) of teclistamab or talquetamab in adults with

Navigating the Immune Landscape: Balancing Efficacy and Toxicity of BCMA-Targeting Agents in Multiple Myeloma.

BACKGROUND: Multiple myeloma is a plasma cell malignancy with a complex genomic landscape, and relapse remains common despite therapeutic advances. B-cell maturation antigen (BCMA), selectively expressed on malignant plasma cells, has emerged as a pivotal immunotherapeutic target. BCMA-directed therapies, including chimeric antigen receptor T cells (CAR-T) and bispecific antibodies, have achieved unprecedented response rates in relapsed/refractory Multiple myeloma (RRMM), yet their application into clinical practice is challenged by unique toxicities and resistance mechanisms. SUMMARY: This r

IMiD/CELMoD resistance in myeloma is associated with changes in lipid metabolism that may be targetable.

Immunomodulatory drugs (IMiDs) are a cornerstone of multiple myeloma treatment, and newer cereblon E3 ligase modulatory drugs (CELMoDs) are in clinical trials. However, a major barrier to improving patient outcomes is the inevitable development of resistance to these agents. To explore the mechanisms underlying IMiD/CELMoD resistance and identify novel targets for treatment, human cell line models of acquired IMiD/CELMoD resistance were generated. A quantitative proteome analysis of these models identified common changes in lipid synthesis proteins, and glucose-labeling experiments confirmed a

Emergency diagnosis of multiple myeloma: a SEER-Medicare study.

PURPOSE: Non-specific presenting symptoms make multiple myeloma (MM) one of the most difficult hematologic cancers to diagnose, increasing the risk of late diagnosis and emergency presentation (EP). We examined the frequency of EP in MM, estimated associations between EP and patient-level factors, and MM symptom-related healthcare utilization in the year before diagnosis. METHODS: We studied Medicare beneficiaries age ≥ 66 years diagnosed with MM between 2008 and 2017. Emergency presentation (EP) was defined as diagnosis preceded by a recent ED visit, sub-classified as: inpatients (EP-IP) for

Immunoglobulins as replacement therapy in hematological malignancies: from historical hypogammaglobulinemia to the era of T-cell-engaging therapies.

INTRODUCTION: Secondary immunodeficiency (SID) with hypogammaglobulinemia is a frequent complication of B-cell hematological malignancies, arising from the disease and from an expanding range of B-cell-depleting and T-cell-engaging therapies. Immunoglobulin replacement therapy (IgRT) is widely used to prevent infection, yet the evidence underpinning this practice is surprisingly weak. AREAS COVERED: The randomized trials, almost all conducted before 2000 and confined to chronic lymphocytic leukemia (CLL) and multiple myeloma (MM), show a reduction in clinically documented infection (pooled re

African Genetic Similarity Is Associated with Reduced Inflammation and Enhanced Adaptive Immunity in Multiple Myeloma.

The bone marrow immune microenvironment (BMME) is a critical factor in multiple myeloma (MM) pathogenesis. Despite higher incidence of MM among self-identified African Americans, the relationship between African genetic similarity and the BMME remains unclear. We present the largest analysis to date assessing the impact of Ancestry on the BMME in myeloma (N=320, including 64 with ≥50% African genetic similarity, AFR-High) leveraging our recently generated Immune Atlas. AFR-High was associated with enrichment of late T-effector cells, driven by rare cytotoxic CD4⁺ T cells, suggesting a distinct

Enriched Adaptive Immunity and Reduced Inflammatory Response in Plasma Cell Disorders of Self-Identified Black Patients.

The immune tumor microenvironment (iTME) plays a crucial role in disease biology of plasma cell (PC) dyscrasias, but remains poorly characterized in self-identified Black individuals, a population at increased risk. Mass cytometry and scRNA-seq of 128 bone marrow aspirates revealed significant changes in immune cell composition across the disease spectrum, including shifts from naïve to effector T cells with declines in B cells, MAIT, pDCs, and increases in monocytes with disease progression. Self-identified Black patients had higher relative proportions of terminally differentiated T cells an

Anticancer Drugs and Cancer-Associated Thrombosis.

The rapid development of novel cancer therapies has considerably improved the care of cancer patients and cancer-related survival; however, these therapies are associated with a broad range of adverse events that have become a major burden in this population. Alongside other cancer-related factors, vascular toxicity from anticancer therapies contributes to an increased risk of venous thromboembolism. Among anticancer drugs, chemotherapeutic agents (eg, anthracyclines, platinum-based drugs, antimetabolites), hormonal therapies (eg, selective estrogen receptor modulators), agents used for multip

Management of monoclonal gammopathy of renal significance: treatment standard.

Monoclonal gammopathy of renal significance (MGRS) is a heterogeneous group of clonal B cell or plasma cell proliferative disorders that cause kidney injury without meeting criteria for overt malignancy. Advances in diagnostic techniques have enhanced the identification and classification of MGRS lesions. Clone-directed therapies such as proteasome inhibitors, anti-CD38 and anti-CD20 monoclonal antibodies are now central to treatment, targeting a deep haematologic response that is strongly associated with improved renal outcomes and reduced risk of recurrence following kidney transplantation.

Proteasome Cap Targeting Chimeras for Ubiquitination-Independent Targeted Protein Degradation.

Selective degradation of a disease-associated protein of interest (POI) is a powerful therapeutic strategy. FDA-approved and investigational glue and degrader drugs function by recruiting a POI to an E3 ubiquitin ligase that mediates POI polyubiquitination and triggers proteasomal degradation. However, using E3 ligases as an intermediary and requiring POI polyubiquitination makes this mechanism of action complex and difficult to rationalize and optimize. These issues have led to interest in evaluating whether direct recruitment of non-ubiquitinated POIs to the proteasome might achieve the same
