Hypoxemia and pulmonary infiltrates during T-cell-engaging bispecific antibody therapy: A pulmonary and critical care framework.

T-cell-engaging bispecific antibodies (BsAbs) have rapidly expanded treatment options for relapsed or refractory multiple myeloma, B-cell lymphomas, and small-cell lung cancer. Their off-the-shelf availability, step-up dosing, repeated administration, and migration into outpatient and community settings distinguish them from autologous chimeric antigen receptor (CAR) T-cell therapy. These same features increasingly bring patients to pulmonary and critical care teams. Patients receiving BsAbs may present with fever, hypoxemia, pulmonary infiltrates, pneumonia, cytokine release syndrome (CRS), s

The prognostic value of KRAS, NRAS, and BRAF genes mutations in patients with newly diagnosed multiple myeloma.

BACKGROUND: KRAS, NRAS, and BRAF (RAS/RAF) genes mutations play a role in the pathogenesis of multiple myeloma (MM). We conducted this study to investigate whether RAS/RAF genes mutations have a prognostic value in patients with MM. MATERIAL AND METHODS: 117 newly diagnosed MM patients underwent the FISH technique to detect high-risk cytogenetic abnormalities and next-generation sequencing (NGS) to detect KRAS, NRAS, BRAF and TP53 mutations. RESULTS: In the study patient group, patients with the KRAS mutation had significantly longer OS and PFS compared to those with the TP53 mutation; patie

Aortic Dissection Incidentally Detected on PET/CT During Evaluation for Suspected Multiple Myeloma.

The identification and diagnosis of aortic dissection on PET/CT remain challenging without the use of contrast agents. Such cases are prone to misdiagnosis or being overlooked, particularly when the examination is focused on an unrelated primary clinical objective. We present a case of thoracic aortic dissection that was incidentally detected on initial PET/CT during the further evaluation of suspected multiple myeloma. The FDG-avid lesion was initially mistaken for an esophageal mass, with differential diagnoses including extramedullary plasmacytoma associated with multiple myeloma and primar

The EF1α promoter of cilta-cel drives a highly effective GZMK+ transcriptional program.

Ciltacabtagene autoleucel (cilta-cel) and idecabtagene vicleucel (ide-cel) are approved BCMA-directed chimeric antigen receptor (CAR) T cells approved for the treatment of multiple myeloma. Although the higher efficacy of cilta-cel compared to ide-cel may be related to its unique nanobody binding domain, the constructs also differ in their use of EF1α and MND promoters, respectively, to drive CAR expression. To elucidate the basis of cilta-cel's efficacy, we analyzed CAR T cells from 87 patients with multiple myeloma (50 cilta-cel, 37 ide-cel) using flow cytometry and single-cell multi-omics.

Targeting lactate-driven protein lactylation overcomes proteasome inhibitor resistance in multiple myeloma.

Acquired resistance to proteasome inhibitors (PIs) remains a major challenge limiting the long-term efficacy of multiple myeloma (MM) therapy, while the metabolic-epigenetic mechanisms underlying PI resistance remain incompletely defined. Using clinical bone marrow specimens, paired parental and PI-resistant MM cell models, xenografts, and integrated lactylome, CUT&Tag, and RNA-seq profiling, we investigated the role of lactate-associated lactylation in acquired PI resistance. Bone marrow lactate levels, lactate dehydrogenase (LDH) activity, and global protein lactylation were elevated in rela

Impact of extramedullary plasmacytoma on outcomes in patients with relapsed refractory multiple myeloma treated with talquetamab: U.S. Myeloma Immunotherapy Consortium.

Talquetamab, a GPRC5D-targeting bispecific antibody, shows promising activity in relapsed-refractory multiple myeloma. However, real-world outcomes in patients with extramedullary disease (EMD) remain poorly described. We studied 360 patients treated with talquetamab at 15 U.S. centers by Dec 2024. Soft tissue plasmacytomas (STP) were classified as EMD (not contiguous with bone) or paraskeletal disease (PSD; contiguous with bone). If both were present, patients were assigned to the EMD category. Of those, 97 (27%) had EMD, 22 (6%) had PSD, and 241 (67%) had No-STP. Median follow-up was 12.8 mo

Intelligent immunotherapy: AI-driven optimization of CAR-T therapy in hematologic malignancies.

Chimeric antigen receptor (CAR)-T cell therapy has transformed outcomes in selected hematologic malignancies, yet its delivery remains constrained by variable efficacy, acute and prolonged toxicities, and complex manufacturing logistics. Artificial intelligence (AI) is being applied across the CAR-T continuum at three distinct levels of evidentiary maturity. Externally validated parsimonious models built on routine clinical and laboratory data including CAR-HEMATOTOX, endothelial activation and stress index (EASIX) and m-EASIX, MyCARe, and gradient-boosting cytokine release syndrome predictors

Efficacy analysis and adverse event management of selinexor in the treatment of relapsed/refractory multiple myeloma.

The treatment of relapsed/refractory multiple myeloma (RRMM) remains a significant clinical challenge, particularly for patients with multidrug resistance, who have a poor prognosis and limited therapeutic options. Selinexor, as the world's first oral selective inhibitor of nuclear export (SINE), targets the XPO1 protein, offering a novel treatment mechanism distinct from traditional agents for RRMM. This narrative review systematically synthesizes the mechanism of action of Selinexor and critically evaluates evidence from key clinical studies (such as the STORM and BOSTON trials). It focuses

BANF1 at the crossroads of genome stability and innate immunity: emerging implications for hematologic malignancies.

Genomic instability and immune escape are linked features of hematologic malignancies, but the mechanisms that allow malignant cells to tolerate persistent nuclear and genotoxic stress while avoiding damaging DNA-sensing responses remain incompletely defined. Barrier-to-autointegration factor 1 (BANF1) is a small DNA-binding protein that bridges chromatin, couples it to nuclear-envelope proteins, supports postmitotic nuclear assembly and rupture repair, modulates several DNA-damage-response pathways, and restricts productive engagement of cyclic GMP-AMP synthase (cGAS) with self-DNA. Human BAN

Exploring Non-Covalent Pin1-PROTACs Insights Into Degradation Efficiency in Multiple Myeloma Cell Line.

Pin1 is a peptidyl-prolyl isomerase involved in key signalling pathways and upregulated in different cancers, including Multiple Myeloma (MM), where it is linked to poor prognosis/drug resistance. Despite its therapeutic relevance, Pin1 remains challenging to target with conventional inhibitors thereby motivating interest in Targeted Protein Degradation (TPD) strategies. All reported Pin1-PROTACs rely on covalent Pin1 ligands, leaving non-covalent degraders largely unexplored. Since PROTACs (PROteolysis TArgeting Chimeras) act catalytically/reversibly, non-covalent designs are attractive. Here

The 19S proteasome as a therapeutic target in hematologic malignancies.

The ubiquitin-proteasome system (UPS) is the primary intracellular pathway for maintaining protein homeostasis through the selective degradation of ubiquitinated proteins. Central to this process is the 26S proteasome, which orchestrates the turnover of regulatory and misfolded proteins and thereby controls critical cellular functions, including cell-cycle progression, apoptosis, DNA repair, signal transduction, and stress response pathways. Dysregulation of proteasome function contributes to many hallmarks of cancer, leading to malignant transformation, disease progression, and therapeutic re

Integrated transcriptomics reveals a TNFRSF-metabolism axis and a context-dependent BCMA signaling state in myeloma progression.

Multiple myeloma (MM) evolves from monoclonal gammopathy of undetermined significance (MGUS) and smoldering multiple myeloma (SMM) with extensive metabolic reprogramming and immune remodeling. We investigated whether coordinated activation of the tumor necrosis factor receptor superfamily (TNFRSF) aligns with metabolic state and prognosis across this continuum. We integrated bone-marrow single-cell RNA sequencing from healthy donors, MGUS, SMM, and MM with bulk RNA sequencing from the Multiple Myeloma Research Foundation (MMRF) CoMMpass cohort. At single-cell resolution, per-cell metabolic act

Denosumab Therapy Duration and Osteonecrosis of the Jaw and Skeletal Events in Bone-Metastatic Prostate Cancer.

IMPORTANCE: Denosumab reduces skeletal-related events in bone-metastatic prostate cancer but carries a cumulative risk of medication-related osteonecrosis of the jaw (ONJ). The treatment duration that best balances these risks has not been established in clinical settings. OBJECTIVE: To evaluate the association between duration of denosumab treatment and the 3-year cumulative incidence of ONJ and skeletal events. DESIGN, SETTING, AND PARTICIPANTS: This nationwide retrospective cohort study emulated a target trial using Korean Health Insurance Review and Assessment Service data from January 1

Purpureocillium lilacinum Fungemia Masquerading as an Unidentified Yeast in a Heavily Immunosuppressed Patient With Relapsed Multiple Myeloma and Therapy-Related Mixed-Phenotype Acute Leukemia: A Case Report.

Purpureocillium lilacinum (formerly Paecilomyces lilacinus) is an environmental saprophytic mold and an increasingly recognized opportunistic pathogen in immunocompromised hosts, most often causing cutaneous, ocular, or sinopulmonary disease; bloodstream infection (fungemia) is rare. The initial microscopic morphology on Gram stain of a positive blood culture can simulate yeast, delaying correct identification and appropriate antifungal therapy. We describe a 67-year-old man with relapsed multiple myeloma and therapy-related mixed-phenotype acute leukemia, further complicated by a prior Mycoba
