Adenopathy and Extensive Skin Patch Overlying a Plasmacytoma (AESOP) Syndrome Associated With a Solitary Bone Plasmacytoma of the Left Scapula: A Rare Paraneoplastic Presentation Managed With High-Precision Radiotherapy.

Adenopathy and extensive skin patch overlying a plasmacytoma (AESOP) syndrome is a rare paraneoplastic manifestation characterized by three cardinal components: the underlying pathology (plasma cell dyscrasia), the cutaneous lesion (vascular cutaneous lesion), and the skeletal/nodal manifestation (solitary plasmacytoma/regional lymphadenopathy). Scapular involvement is exceptionally uncommon and may represent an early indicator of progression within the polyneuropathy, organomegaly, endocrinopathy, monoclonal plasma cell disorder, and skin changes (POEMS) spectrum. A 47-year-old male with esse

Diagnostic challenges posed by the wide morphological and immunophenotypic spectrum of plasma cell neoplasms.

Plasma cell neoplasms (PCNs) are a group of malignant neoplasms originating from the monoclonal expansion of immunoglobulin-secreting, terminally differentiated B cells, i.e., plasma cells (PCs). PCNs include monoclonal gammopathy of undetermined significance (MGUS), plasmacytoma (solitary plasmacytoma of the bone and extramedullary plasmacytoma), multiple myeloma (MM)/plasma cell myeloma, and rare PCNs associated with paraneoplastic syndromes. MM is a multifocal, bone marrow-based clonal PCN associated with end-organ damage and a monoclonal paraprotein (M protein) in the serum and/or urine. M

Development and validation of a machine learning-based postoperative prognostic model for plasma cell neoplasia with spinal lesions as initial clinical manifestations: a single-center cohort study.

BACKGROUND: Spinal multiple myeloma (MM) and solitary plasmacytoma of bone (SPB), both plasma cell neoplasms, greatly affect patients' quality of life due to spinal involvement. Accurate prediction of surgical outcomes is crucial for personalized patient care, but systematic treatment guidelines and predictive models are lacking. OBJECTIVE: This study aimed to develop and validate a machine learning (ML)-based model to predict postoperative outcomes and identify prognostic factors for patients with spinal MM and SPB. METHODS: A retrospective analysis was conducted on patients diagnosed with

Extramedullary plasmacytoma of the colon manifesting with intussusception.

The WHO categorises plasma cell tumours into two primary types: multiple myeloma and plasmacytoma. Plasmacytoma is further classified into solitary extramedullary plasmacytoma (EMP) and solitary plasmacytoma of bone. The upper respiratory tract and nasopharynx are the sites most commonly affected by EMP. Gastrointestinal tract involvement occurs in approximately 10% of EMP cases, while colonic involvement-particularly when associated with intussusception-is exceedingly rare. We present the case of a middle-aged man in his 40s who reported haematochezia, fatigue and weight loss over a 6-month p

Efficacy of radiotherapy and role of adjunctive immunochemotherapy in POEMS syndrome with solitary plasmacytoma.

Radiotherapy is a definitive treatment for polyneuropathy, organomegaly, endocrinopathy, monoclonal protein, skin changes (POEMS) syndrome with solitary plasmacytoma. However, data on its efficacy and the role of immunochemotherapy are limited. This study evaluated the treatment outcomes of patients with POEMS syndrome receiving radiotherapy as their primary therapy and compared the results of those treated with adjunctive immunochemotherapy with those without. A retrospective analysis of patients with POEMS syndrome referred to the University College London Hospital from 1998 to 2023 was cond

Solitary plasmacytoma of the tibia: literature review and case report.

Solitary plasmacytoma (SP) is seldom encountered. It can affect any bone in the body, but is more common in the spine, especially in the thoracic region, while SP in the tibia is relatively rare. Herein, we report the case of a 34-year-old woman who visited our hospital with right calf pain for over a month. The X-ray, computed tomography (CT), and magnetic resonance imaging (MRI) revealed a tumor growing along the longitudinal axis of her right tibia, which was suspected to be malignant. The patient subsequently underwent a puncture biopsy, and the pathological results revealed a plasma cell

Surgical Treatment after Radiotherapy for Solitary Plasmacytoma of the Cervical Spine: A Case Report.

Solitary plasmacytoma is a hematologic malignancy in which about 50% of cases progress to multiple myeloma within 10 years. Local radiation therapy is effective for solitary plasmacytoma treatment, but there are cases of spinal cord paralysis due to osteolytic pathological fractures in the spine. Here, we report a 63-year-old male with spinal cord injury caused by pathologic fracture progression after radiotherapy. A 63-year-old male presented with neck pain and left hemiplegia. Imaging studies revealed a pathological fracture of the C5 vertebral body, and biopsy confirmed the diagnosis of sol

Solitary Lumbar Extradural Plasmacytoma With Minimal Vertebral Involvement: A Rare Presentation.

Solitary plasmacytoma of the spine is an uncommon plasma cell neoplasm that typically arises from the vertebral body and is associated with osteolytic changes. Presentation as a predominantly extradural mass with minimal osseous involvement is rare and poses a diagnostic challenge due to nonspecific imaging features. We present the case of a 52-year-old female who presented with severe low back pain and bilateral radiculopathy without motor deficit or sphincter dysfunction. MRI revealed a well-defined, homogeneously enhancing anterior extradural mass at the L5 level, causing significant thecal

Solitary Plasmacytoma and its Progression into Multiple Myeloma - A Case Report.

INTRODUCTION: Solitary plasmacytoma (SP) is a rare plasma cell dyscrasia characterized by localized monoclonal plasma cell proliferation without systemic involvement. Although radiotherapy remains the standard treatment, progression to multiple myeloma (MM) is a recognized risk. CASE REPORT: We report a case of SP initially presenting as a localized bone lesion, diagnosed through imaging, histopathology, and laboratory evaluation. The patient underwent standard treatment and was closely followed. Over time, the disease progressed to MM, confirmed by clinical deterioration, laboratory findings

Circulating Tumor Cells in Multiple Myeloma: From Peripheral Clues to Central Insights.

Circulating tumor cells (CTCs) have emerged as a key component of liquid biopsy in multiple myeloma (MM), reflecting the ability of malignant plasma cells to escape the bone marrow (BM) niche, disseminate systemically, and contribute to disease progression. Their detection in peripheral blood is now feasible across the disease spectrum, from MGUS and smoldering myeloma to solitary plasmacytoma, symptomatic MM, and plasma cell leukemia, providing a dynamic window into tumor biology. Technological advances, including high-sensitivity flow cytometry and next-generation single-cell sequencing, hav

Nasopharyngeal Solitary Extramedullary Plasmacytoma: A Rare Case and Review of Literature.

INTRODUCTION: Solitary extramedullary plasmacytoma (EMP) of the nasopharynx is a rare plasma cell neoplasm, representing a small subset of head and neck malignancies. Its nonspecific symptoms often lead to diagnostic delays. CASE REPORT: A 50-year-old female with non-specific progressive nasal obstruction and intermittent epistaxis, underwent complete endoscopic surgical excision using coblation. Post operative histopathology revealed Nasopharyngeal Extramedullary Plasmacytoma. The patient then received adjuvant radiotherapy (Volumetric Modulated Arc Therapy - VMAT), leading to a successful o

A multicenter study of progression risk and outcomes in solitary bone plasmacytoma with and without marrow involvement.

Solitary bone plasmacytoma (SBP) and SBP with minimal marrow involvement (SBPmm) are treated similarly with definitive radiation therapy (RT) followed by observation. However, differences in the natural disease course after RT between the two, are not well described. We performed a multicenter study assessing outcomes after RT in 141 patients with SBP and 102 with SBPmm diagnosed between January 2010 and December 2024. About 92% of patients with SBPmm progressed to multiple myeloma (MM) vs 76% of those with SBP (P = .01). Among patients with SBP, 21% progressed with a second solitary bone lesi

Solitary Plasmacytoma of the Mandible: A Rare Presentation.

Solitary plasmacytoma is a rare plasma cell neoplasm, most often arising in vertebrae and long bones; among these, mandibular involvement is exceptional. The following report documents a rare mandibular presentation in a 48‑year‑old female patient who presented with pain and swelling in the right lower jaw. Clinical examination, radiographic findings, and histopathology confirmed the diagnosis. The lesion was managed surgically with hemi‑mandibulectomy and reconstruction, with no recurrence to date. This case highlights the importance of early recognition of maxillofacial plasmacytomas by dent

Solitary Bone Plasmacytoma of Left Wrist Joint - A Case Report.

INTRODUCTION: Solitary Plasmacytoma is an infrequent form of plasma cell (PC) dyscrasia which presents as a single mass of monoclonal PCs, located either intraosseous or extramedullary. Solitary bone plasmacytoma (SBP) is characterized by a mass of neoplastic monoclonal PCs primarily occurring in the axial skeleton without systemic involvement. This case delineated an unusual appendicular-osteoarticular (wrist joint) involvement, which was diagnosed through an exhaustive set of investigations and managed by radiotherapy (RT), which is the standard of care. The rarity of the clinico-radiologica

A Rare Rescue Case of Post-biopsy Pseudoaneurysm Treated by Direct Percutaneous Glue Injection after Failed Embolization.

Post-biopsy pseudoaneurysm is a rare but potentially life-threatening complication, particularly in hypervascular spinal lesions. We report a 67-year-old man with a hypervascular vertebral lesion in the right L2 transverse process who developed a retroperitoneal hematoma after computed tomography-guided biopsy. Initial transarterial embolization provided only temporary hemostasis. Due to technical constraints, including the presence of a spinal artery variant, repeat transcatheter arterial embolization was not feasible. Fluoroscopic computed tomography-guided direct injection of n-butyl cyanoa

Case Report: A case of incidentally discovered isolated gastric plasmacytoma.

Extramedullary plasmacytoma (EMP) is a rare plasma cell malignancy, accounting for approximately 3% of all plasma cell neoplasms. Gastric plasmacytoma, a distinct subtype of EMP arising from B lymphocytes, is extremely uncommon, representing roughly 2% of EMP cases, and lacks specific clinical manifestations. Herein, we report a 53-year-old male presenting with persistent acid reflux and heartburn. Gastroscopy identified a flat, elevated lesion in the gastric body. Endoscopic mucosal resection (EMR) was performed to obtain the lesion specimen. The diagnosis of primary solitary gastric plasmacy
