Inotuzumab Ozogamicin in Clinical Practice: an Overview of Efficacy, Safety, and Real-World Applications.

PURPOSE OF REVIEW: Acute lymphoblastic leukemia (ALL) is a rare hematologic malignancy with a bimodal distribution of incidence in both pediatric/young adult and elderly patient populations. Despite the high complete remission rate, there is a high rate of relapse necessitating a need for therapy options in the relapsed/refractory setting. Given this, treatment paradigms for ALL have shifted towards targeted therapies and away from high-intensity chemotherapy. The efficacy of inotuzumab ozogamicin (InO) in the relapsed/refractory setting for pediatric and adult populations has led to incorpora

Characteristics and outcome determinants in children, adolescents and young adults who failed tisagenlecleucel for B-cell acute lymphoblastic leukemia.

Tisagenlecleucel (tisa-cel), an autologous anti-CD19 CAR T-cell therapy, has significantly improved outcomes in pediatric, adolescents and young adults with relapsed/refractory B-cell precursor acute lymphoblastic leukemia (R/R BCP-ALL). However, 30-50% experience early failure or relapse. We retrospectively analyzed 52 cases of early failures (n = 13) or relapses (n = 39), evaluating post-tisa-cel outcomes and prognostic factors. CD19 antigen loss was the only factor associated with a lower complete remission rate after salvage therapy (OR = 0.16, 95%CI [0.03-0.90], p = 0.04). Median overall

Real-world outcomes and toxicities of CAR-T in relapsed/refractory follicular lymphoma: a multicenter cohort study.

Chimeric antigen receptor T-cell therapy has revolutionized the treatment of relapsed/refractory (R/R) follicular lymphoma (FL). Real-world efficacy and toxicity data are needed because clinical trial populations are often unrepresentative. Therefore, we conducted a multicenter retrospective study of R/R FL patients undergoing commercial axicabtagene ciloleucel (axi-cel) or tisagenlecleucel (tisa-cel) between 2021 and 2024. End points included efficacy measures (overall response rate [ORR], complete response rate [CRR], progression-free survival [PFS], and overall survival [OS]) and toxicity (

Exploitation of CREB signaling by HTLV-1 and BLV: A central axis in viral persistence and leukemogenesis.

Deltaretroviruses, including Human T-cell Leukemia Virus type 1 (HTLV-1) and Bovine Leukemia Virus (BLV), are oncogenic pathogens that exploit host transcriptional machinery to establish persistence and drive malignant transformation. Central to this process is the cAMP response element-binding protein (CREB), a transcription factor that regulates cell survival, proliferation, and differentiation. Viral proteins, such as Tax and HBZ, directly interact with CREB and its cofactors (CBP/p300, TORC2), enabling the robust activation of viral long terminal repeats (LTRs) and the dysregulation of hos

BCL11B enhancer hijacking by t(14;16)(q32;q24) translocation defines a novel high-risk subtype of T-ALL.

The molecular classification of T-cell acute lymphoblastic leukemia (T-ALL) remains incomplete, limiting risk stratification and the development of targeted therapies. Enhancer hijacking is a critical oncogenic mechanism that deregulates proto-oncogenes by repositioning cisregulatory regions via structural variants. Here, we performed an integrated analysis of pediatric and adult T-ALL and mixed-phenotype acute leukemias (MPALs), using whole-genome and whole-transcriptome sequencing. This analysis identified a group of 14 patients with predominantly T-lineage neoplasms driven by a t(14;16)(q32

Long-term follow-up of CD19 chimeric antigen receptor T cell therapy in acute lymphoblastic leukemia patients relapsed after allogeneic hematopoietic stem cell transplantation.

Relapse after allogeneic hematopoietic stem cell transplantation (allo-HSCT) in patients with B-cell acute lymphoblastic leukemia (B-ALL) is associated with a poor prognosis. Although CD19 chimeric antigen receptor T (CAR-T) cell therapy has shown promise in this population, its long-term efficacy and safety remain unclear. To address this, we conducted a retrospective multicenter study to evaluate the long-term survival outcomes of autologous and allogeneic CD19 CAR-T cell therapies in 55 B-ALL patients who relapsed after allo-HSCT. Following CAR-T cell infusion, 37 patients (67.3%) achieved

Synergistic effects of HDAC inhibitor tucidinostat and ENT inhibitor dipyridamole in T-cell malignancies.

Achieving both selectivity and cytotoxicity against cancer cells remains a major challenge in cancer therapy. Histone deacetylase (HDAC) inhibitors, such as tucidinostat, suppress tumor growth by epigenetically regulating tumor suppressor gene expression and are approved for the treatment of hematological malignancies. Despite their promising efficacy, frequent and severe adverse effects, particularly hematological toxicities, often limit long-term treatment, underscoring the urgent need for safer therapeutic strategies. In this study, we show ex vivo that combining tucidinostat with dipyridam

CAR-T cells with the CD38-CD73-Tim-3-HLA-DR+ phenotype predict the efficacy of tisagenlecleucel as a treatment for B cell precursor ALL.

Anti-CD19 chimeric antigen receptor T cell (CAR-T) therapy is highly effective for B cell precursor acute lymphoblastic leukemia (BCP-ALL); however, approximately half of the patients relapse. Thus, there is an urgent need to identify factors that improve efficacy. This study enrolls 19 patients with BCP-ALL (16 children and 3 young adults) who receive tisagenlecleucel. Infusion products, peripheral blood, and bone marrow samples are obtained before and after CAR-T cell infusion. Single-cell analysis reveals that central memory CARpos T cells increase in long-term responders, whereas CXCR3+CD3

[Application of blinatumomab in adult acute B-lymphoblastic leukemia: a comprehensive strategy from remission induction to post-transplantation maintenance].

Allogeneic hematopoietic stem cell transplantation (allo-HSCT) is an important treatment option for adult patients with relapsed/refractory acute lymphoblastic leukemia (ALL) , and achieving complete remission (CR) is a critical step before allo-HSCT. Recent studies have shown that immunotherapy with the bispecific T-cell engager (BiTE) blinatumomab can eliminate residual chemotherapy-resistant B-ALL cells, significantly improve remission rates in patients with relapsed/refractory B-ALL and those with positive minimal residual disease (MRD) , successfully bridge patients to allo-HSCT, and prov

Outcomes of pediatric and adult patients with relapsed/refractory cortical (CD1a+) T-cell acute lymphoblastic leukemia. The Spanish experience from SEHOP and PETHEMA groups.

Patients with relapsed/refractory T-cell acute lymphoblastic leukemia (R/R T-ALL) have very poor prognosis. CAR T-cell therapy is being explored in these patients with encouraging results. Specifically, CD1a-directed CAR T-cells are being explored in clinical trials to treat cortical T-ALL (CD1a+), but there are no data on outcome in this subgroup of patients before this therapy. This retrospective, observational study aimed to describe the characteristics and outcomes of patients with R/R CD1a + T-ALL. We included pediatric and adult patients diagnosed with R/R CD1a + T-ALL in 25 sites of Spa

Incorporating ALL Biology into Treatment Decisions: Next Steps toward Targeted Therapy in Adult ALL.

UNLABELLED: Acute lymphoblastic leukemia (ALL) management is evolving from a generalized approach toward a precision medicine paradigm driven by molecular heterogeneity. This review highlights how genomic profiling identifies high-risk subtypes, including KMT2A-rearranged, IKZF1-plus BCR::ABL1-positive, BCR::ABL1-like, and TP53-mutant B-cell ALL, which necessitate tailored therapies. We explore novel strategies such as BH3 mimetics, menin inhibitors, pre-T-cell receptor signaling blockade, and targeted immunotherapy, which show promise in relapsed/refractory and high-risk groups. We also addre

Comparative efficacy and long-term survival of CD19/22 versus CD19 CAR-T immunotherapy in Relapsed/Refractory B-ALL with TP53 alterations.

BACKGROUND: Patients with refractory/relapsed (R/R) B-cell acute lymphoblastic leukemia (B-ALL) harboring TP53 alterations have a dismal prognosis due to profound chemoresistance. While CD19 chimeric antigen receptor T-cell (CAR-T) therapy has shifted the treatment landscape, long-term efficacy in this high-risk subset remains limited. The dual-target CD19/22 CAR-T has been developed to enhance anti-tumor activity; however, its comparative efficacy and long-term survival relative to CD19 CAR-T in this high-risk population remain unclear and require further elucidation to inform clinical decisi

Molecular variants, clonal evolution and clinical relevance in pediatric and adult T-cell lymphoblastic neoplasia.

T-cell lymphoblastic lymphoma (T-LBL) and T-cell acute lymphoblastic leukemia (T-ALL) originate from thymic T-cell precursors, with ongoing debate on whether they are variants of the same disease or distinct entities. For 211 patients, including pediatric and adult T-ALL and T-LBL cases, targeted next-generation sequencing and SNP-arrays were performed, and single-nucleotide variants, indels and copy-number variants (CNVs) were analyzed. We aimed to assess genetic differences between T-ALL and T-LBL across age. Generally, mutational landscape analysis identified mutated PHF6 being associated w

Impact of TP53 alterations on outcomes in pediatric and young adult patients with relapsed / refractory B-cell acute lymphoblastic leukemia after CD19-CAR T-Cell therapy.

Relapse remains the leading cause of treatment failure in pediatric and young adult patients with relapsed/refractory B-cell acute lymphoblastic leukemia (r/r B-ALL) undergoing chimeric antigen receptor T-cell (CAR-T) therapy. While prognostic factors for relapse have been identified, further refinement is needed. We conducted a single-center retrospective study of 69 patients treated with Tisagenlecleucel, to evaluate the prognostic impact of TP53 alterations (TP53Alt), including mutations and/or deletions. Among 49 patients with available samples, 17 (34.7%) had TP53Alt. These patients showe

Central nervous system involvement in CLL: an international retrospective study by ERIC, the European Research Initiative on CLL.

Central nervous system involvement (CNSi) of chronic lymphocytic leukemia (CLL) is a rare condition with no consensus on diagnostic criteria and limited evidence for management and outcome. Here, we report an international, multicenter, retrospective study conducted by the European Research Initiative on CLL. The study defined CNSi of CLL by the following: (1) detection of CLL cells in the cerebrospinal fluid or confirmation of CLL infiltration of the CNS based on a tissue biopsy, (2) clinical or radiographic evidence of neurologic disease, and (3) the absence of other explanations for the neu
