Clinical features and incidence of central nervous system involvement in patients with adult T-cell leukaemia/lymphoma.

Central nervous system (CNS) involvement is a known complication of adult T-cell leukaemia/lymphoma (ATL), but its clinical characteristics and incidence still remain unclear. This study characterized the clinical features and incidence of CNS involvement in patients with ATL (CNS-ATL). Overall, 213 patients diagnosed with aggressive ATL were retrospectively analysed at the Kagoshima University Hospital between 2002 and 2021. Of which, 28 (13.1%) developed CNS-ATL. Patients with CNS-ATL observed at ATL diagnosis were classified as acute (n = 21, 75%), lymphoma (n = 1, 3.6%) or chronic (n = 5,

Multiomics Profiling of T-cell Leukemia and Lymphoma Enables Targeted Therapeutic Discovery.

UNLABELLED: T-cell leukemias and lymphomas (TCL) form a heterogeneous group of rare and often aggressive malignancies. Because of the rarity and heterogeneity of TCL subtypes, clinical trials are challenging to conduct, making pharmacogenomic studies in cell line panels critical for the discovery of targeted therapeutics. The scarcity of data repositories with integrated multiomics and drug screening data hinders the preclinical evaluation of drug vulnerabilities and the identification of molecular markers predictive of responses to monotherapies and combinations. To address this gap, we condu

DNA methyltransferase inhibitors in hematological malignancies and solid tumors.

Epigenetic modifications such as DNA methylation play a fundamental role in oncogenesis and the progression of neoplasms neoplasias. DNA methyltransferase inhibitors (DNMTi) constitute a family of therapeutic agents that impede the methylation at the 5-position on cytosine nucleotides, thereby modulating the epigenetic regulation of tumor suppressor genes, oncogenes, and other key regulatory genes. The first-generation DNMTi azacitidine and decitabine have demonstrated substantial efficacy in the treatment of medically non-fit, older patients with acute myeloid leukemia (AML) or myelodysplasti

Cell State Transitions Drive the Evolution of Disease Progression in B-Lymphoblastic Leukemia.

UNLABELLED: Cancer stem cells (CSC) are hypothesized to promote tumor progression through innate chemoresistance and self-renewal. CSCs reside in the CD34+/CD38- immunophenotypic subpopulation of acute myeloid leukemia (AML). Isolation of CSCs from B-lymphoblastic leukemia (B-ALL) has proven difficult, and the cells of interest apparently are not isolated to the CD34+/CD38- compartment. This may be explained, in part, by temporal variations of CD34 and CD38 expression which result in stochastic cell state transitions (e.g., from CD34+/CD38+ to CD34+/CD38-). We present a mathematical model of t

Refined Continuous Risk Index for Accurately Predicting Outcomes of Patients With Chronic Lymphocytic Leukemia After Limited-Duration Therapy.

PURPOSE: Measurable residual disease (MRD) status after limited-duration treatment for chronic lymphocytic leukemia (CLL) strongly predicts survival times and is considered a surrogate end point. However, most prognostic indices, including the International Prognostic Index (CLL-IPI), rely solely on pretreatment features. To incorporate dynamic MRD data in prognostic models, the continuous individualized risk index (CIRI) was proposed in 2019. Since developed in the context of chemoimmunotherapy, we here propose a refined version of CIRI, in the context of fixed-duration targeted therapy, whic

Clinicopathological and Immunophenotypic Analysis of Early T-Cell Precursor Acute Lymphoblastic Leukemia With Application of the Tokyo Children's Cancer Study Group Flow Cytometry Scoring System.

INTRODUCTION: Early T-cell precursor acute lymphoblastic leukemia (ETP-ALL) is a distinct subtype of T-ALL defined by an immature immunophenotype and unique molecular features. It is often associated with chemoresistance and poor outcomes. Accurate recognition is crucial for therapy optimization and consideration of hematopoietic stem cell transplantation. This study evaluated the Tokyo Children's Cancer Study Group (TCCSG) six-marker flow cytometric scoring system in identifying ETP-ALL and assessed its clinicopathological features and treatment outcomes. METHODS: All consecutive T-ALL cases

A new c.681dup RUNX1 variant in familial leukemia.

Constitutional RUNX1 gene variants are associated with Familial Platelet Disorder (FPD) and predispose to a variety of hematological malignancies, included Acute Myeloid Leukemia (AML) and, albeit less frequently, Acute Lymphoblastic Leukemia (ALL). In this study, we report on a proband with primary diagnosis of AML, followed by T-ALL after transplant, and a positive familial history for leukemia over three generations. A new heterozygous germline pathogenic RUNX1 (c.681dup, p.(Leu228ThrfsTer33)) variant was found in the proband and his affected mother.

Distribution of adult T-cell leukaemia/lymphoma subtypes and survival outcomes across geographical regions: An international retrospective cohort study.

Although adult T-cell leukaemia/lymphoma (ATL) has a higher burden in certain world regions, outcomes have not been directly compared across regions. We conducted a multicentre cohort study comparing the distribution of ATL subtypes and overall survival (OS) across 1090 adults (≥18 years) diagnosed with ATL during 2000-2023 in Japan (n = 366), South America (n = 364), the United States (n = 275) and the United Kingdom (n = 85). Survival was estimated using Kaplan-Meier methods. Acute ATL was more frequent in Japan (59%) than in the United States (45%), South America (36%) and the United Kingdo

The Role of Allogeneic Hematopoietic Cell Transplantation in the Management of Philadelphia Chromosome-Positive Acute Lymphoblastic Leukemia in the Era of Tyrosine Kinase Inhibitors.

Philadelphia chromosome (Ph)-positive acute lymphoblastic leukemia (ALL), the most common subtype of adult ALL, has undergone dramatic transformation in prognosis over the past two decades. Introduction of tyrosine kinase inhibitors (TKIs) targeting BCR::ABL1 fusion protein has revolutionized frontline therapy, with successive generations of TKIs-from imatinib to dasatinib and most recently ponatinib-achieving progressively deeper and more durable molecular responses. Concurrently, the integration of immunotherapy, particularly blinatumomab, has enabled chemotherapy-sparing approaches further

Adult T-cell leukemia/lymphoma: molecular pathogenesis, emerging therapies, and future directions.

Adult T-cell leukemia/lymphoma (ATLL) is a rare and aggressive malignancy of mature T-cells caused by chronic infection with Human T-cell Leukemia Virus Type 1 (HTLV-1). Despite advances in understanding its pathobiology, ATLL remains associated with poor clinical outcomes, largely due to intrinsic chemoresistance and immune evasion mechanisms driven by viral oncogenes and acquired genetic alterations. The discovery of frequent mutations in TCR/NF-κB, JAK/STAT, and apoptotic pathways has unveiled novel therapeutic vulnerabilities, while agents such as mogamulizumab have demonstrated clinical e

Pharmacologic modulation of the SAT1-N1-acetylspermidine axis is associated with enhanced chemotherapy cytotoxicity in T-LBL/ALL cells.

Adult T-lymphoblastic lymphoma/acute leukemia (T-LBL/ALL) remains an aggressive malignancy for which biologically informative markers of treatment-associated states are limited. Emerging evidence suggests that tumor cells rewire their metabolism to buffer cytotoxic stress. In this study, we employed untargeted and targeted serum metabolomics to identify metabolic features associated with T-LBL/ALL and chemotherapy sensitivity. We found that serum N1-acetylspermidine (N1-AcSpd) was prominently elevated in patients compared with controls. Clinically, patients with high baseline N1-AcSpd exhibite

The tumor suppressor NDRG2 recruits protein phosphatase 2A to suppress STAT5 phosphorylation in adult T-cell leukemia/lymphoma.

Adult T-cell leukemia/lymphoma (ATL) is an aggressive T-cell malignancy with a poor prognosis that is caused by human T-cell leukemia virus type 1 infection. We previously demonstrated that N-myc downstream-regulated gene 2 (NDRG2) is significantly downregulated in ATL, resulting in aberrant activation of the signal transduction pathways through the dissociation of serine/threonine protein phosphatase 2A. To identify potential targets of NDRG2, we performed comprehensive mass spectrometry of differentially phosphorylated peptides in ATL cells with overexpression of NDRG2 using a TiO2-based enr

Heterogeneity of HTLV-1 proviral integration sites and internal structures in the ATL cell line MT-1.

Human T-cell leukemia virus type 1 (HTLV-1) causes adult T-cell leukemia (ATL). The ATL-derived cell line MT-1 is used to study HTLV-1 biology and leukemogenesis. However, the proviral integration sites and internal structures of HTLV-1 are not comprehensively characterized in MT-1 cells. We analyzed two independently maintained MT-1 cell lines (MT-1 J and MT-1 M) using long PCR, quantitative PCR-based proviral load analysis, inverse long PCR, inverse PCR, integration site-specific PCR, direct sequencing, and Rapid Amplification of Integration Site without Interference by Genomic DNA contamina

HTLV-1-Driven Clonal Evolution and Immune Escape in Adult T-Cell Leukaemia: From Viral Persistence to Therapeutic Failure.

Adult T-cell leukaemia/lymphoma (ATL) is an aggressive CD4 +T-cell malignancy caused by the human T-cell leukaemia virus type 1 (HTLV-1). Approximately 3%-5% of infected individuals develop ATL after a prolonged latency of 30-50 years, during which a complex interplay between viral oncoproteins and host genomic alterations drives the transition from viral persistence to overt malignancy. This transformation is orchestrated by the viral transactivator Tax, which initiates cellular transformation, and the HTLV-1 basic leucine zipper factor (HBZ), which maintains the malignant phenotype and promo

Development of Chimeric Antigen Receptor-Modified NK Cells for the Immunotherapy of Adult T-Cell Leukemia/Lymphoma.

Adult T-cell leukemia/lymphoma (ATL) is an aggressive hematological malignancy with a poor prognosis and limited therapeutic options. ATL cells exhibit aberrant surface glycosylation patterns resulting from dysregulated glycosyltransferase expression, representing a potentially tumor-specific therapeutic target. We previously isolated an ATL-specific single-chain variable fragment (scFv), S1TSCFR3-1, that selectively binds to ATL cell surface glycans. In this study, we engineered natural killer (NK) cells to express a chimeric antigen receptor (CAR) incorporating S1TSCFR3-1 and evaluated their
