A Unique Case of Concomitant T-Prolymphocytic Leukemia and B-Cell Acute Lymphoblastic Leukemia.

T-prolymphocytic leukemia (T-PLL) is a rare lymphoid malignancy with a poor prognosis. B-cell acute lymphoblastic leukemia (B-ALL) also confers a poor prognosis, especially in patients with high-risk features without an option for transplant. Here, we present a case of a patient with T-PLL initially treated with multi-agent chemotherapy who then developed B-ALL, the management strategies, and possible pathogenesis of two concurrent rare malignancies. One proposed mechanism for the development of both hematologic malignancies in this patient is the acquisition of a KMT2A rearrangement, raising

How We Evaluate and Treat Leukemic Presentations of Mature T-Cell Lymphomas.

T-cell non-Hodgkin lymphomas, which arise from post-thymic mature T cells, constitute approximately 10-15% of all non-Hodgkin lymphomas. Their leukemic presentations, referred to here as mature T-cell leukemias, are relatively uncommon and present significant diagnostic and therapeutic challenges requiring an informed approach to diagnosis and management. The initial presentation is often persistent T-cell lymphocytosis that must be distinguished from reactive (non-malignant) causes. Unlike B-cell lymphocytosis, where clonality usually indicates malignancy, T-cell clonality can be detected in

Optical genome mapping reveals multiple apoptotic and cell-cycle pathway aberrations in B-cell prolymphocytic leukemia: a report of three cases.

B-cell prolymphocytic leukemia (B-PLL) is a rare B-cell neoplasm that presents splenomegaly, lymphocytosis, minimal or absent lymphoadenopathy, at least 55% of prolymphocytes in peripheral blood and a variable clinical course. Complex/composite karyotype and recurrent structural variants (SVs), including TP53 aberrations (mutations/deletion) and MYC abnormalities (translocation or gain) are genetic features typically seen in B-PLL. We applied the genome-wide technology of optical genome mapping (OGM) in 3 cases with B-PLL, finding multiple genomic aberrations, including SVs, copy number variat

Treatment Outcomes and Overall Survival of Patients With B-Cell Prolymphocytic Leukemia.

INTRODUCTION: B-cell prolymphocytic leukemia (B-PLL) is a rare, aggressive leukemic B-cell malignancy historically associated with poor outcomes and recently reclassified in the WHO Fifth Edition but retained as a distinct entity in the ICC. METHODS: We analyzed 950 patients labeled as B-PLL in the National Cancer Database (2004-2019), identified using ICD-O-3 morphology code 9833/3. RESULTS: Median age was 72 years, and 72% received systemic therapy at diagnosis. Median overall survival (OS) was poor (2.8 years) and did not improve over time. Patients initially managed with active surveilla

Prolymphocytic Progression in Chronic Lymphocytic Leukemia Without Richter Transformation: An Eighteen-Year Clinical-Diagnostic Correlation.

Chronic lymphocytic leukemia (CLL) is an indolent neoplasm of small, mature B lymphocytes characterized by clumped chromatin and scant cytoplasm. A minor subset of neoplastic cells may exhibit prolymphocytic morphology, with prominent nucleoli and moderate cytoplasm. An increased proportion of prolymphocytes in peripheral blood often correlates with an accelerated or aggressive disease course, distinct from Richter transformation (RT), which more commonly manifests as diffuse large B-cell lymphoma and requires intensified therapy. We report an 18-year clinical course of CLL in a 57-year-old ma

Splenic B-Cell Lymphoma/Leukemia With TCL1 Gene Rearrangement.

BACKGROUND: Splenic B-cell lymphoma/leukemia includes a group of indolent small B-cell lymphomas/leukemias that share overlapping morphological and immunophenotypic features, making accurate diagnosis challenging. Specific gene rearrangements have not been commonly associated with splenic B-cell lymphomas/leukemias, and the TCL1 gene rearrangement-characteristic of T-cell prolymphocytic leukemia-has not been previously reported in B-cell lymphomas/leukemias. Here, we report two unique cases of splenic B-cell lymphoma/leukemia harboring an IGH::TCL1 rearrangement. CASE PRESENTATION: Patient #1

An archive that will have moved: on an exemplary artistic intervention in the archive Fritzpunkt, its relation to the Marianne Fritz archive in Vienna and its conceptual ramifications.

Prolymphocytic leukemia (PLL) is a rare lymphoproliferative disorder characterized by marked leukocytosis and splenomegaly. PLL accounts for approximately 2% of chronic lymphoid leukemias. The clinical course is progressive in the majority of cases due to the resistance of the disease to conventional chemotherapy. The disease is divided according to the cell of origin into the B- (B-PLL) and T-cell (T-PLL) types. T-PLL and B-PLL are morphologically identical, but lymphadenopathy and skin involvement are more common in T-PLL than in B-PLL. Approximately 80% of cases are of B-cell phenotype. T-P

A Rare Case of B-cell Prolymphocytic Leukemia: The First Documented Diagnosis in Georgia With a Comprehensive Literature Review.

B-cell prolymphocytic leukemia (B-PLL) is a rare and aggressive malignancy of mature B-cells associated with poor prognosis, limited treatment options, and a median survival of approximately three years. We report the first documented case of B-PLL in the country of Georgia, involving a 64-year-old male presenting with fever, night sweats, weight loss, and generalized lymphadenopathy. Initial investigations revealed significant lymphocytosis and a predominance of prolymphocytes on the peripheral smear. The diagnosis was confirmed through bone marrow aspirate analysis and flow cytometry, demons

Burkitt lymphoma in patients with chronic lymphocytic leukemia and other indolent B-cell lymphoid malignancies.

The most common form of Richter transformation, observed in 90-95% of patients, is the transformation of chronic lymphocytic leukemia (CLL) into diffuse large B-cell lymphoma (DLBCL). CLL has also been found to transform into Hodgkin lymphoma, prolymphocytic leukemia, and, in extremely rare cases, into other hematological malignancies, including Burkitt lymphoma (BL). This manuscript discusses a case of CLL transforming into BL and reviews a rare collection of cases of Burkitt lymphoma or Burkitt-like lymphoma associated with CLL,B-cell prolymphocytic leukemia, mantle cell lymphoma, follicular

Nucleolated cells in extranodal marginal zone lymphoma: a case report and discussion of circulating lymphoma cells with prominent nucleoli.

Various types of lymphoma/leukemia can present with leukemic involvement by neoplastic cells with prominent nucleoli. Patients with chronic lymphocytic leukemia (CLL) most commonly can present in this fashion but other, less common types include mantle cell lymphoma, splenic marginal zone lymphoma, splenic diffuse red pulp small B-cell lymphoma, and splenic B-cell lymphoma/leukemia with prominent nucleoli (SBLPN). The latter is a newly proposed category in the fifth edition of the World Health Organization classification of hematolymphoid neoplasms. This category includes most cases that were

Lymphoid Malignancies in Ataxia-Telangiectasia: Experience with Novel Targeted Therapies.

Patients with ataxia-telangiectasia (AT) face unique challenges in managing lymphoid malignancies due to heightened sensitivity to chemotherapy and radiation, requiring alternative treatment strategies. This report presents three cases demonstrating the successful integration of targeted therapies: ibrutinib for diffuse large B-cell lymphoma, alemtuzumab for T-cell prolymphocytic leukemia, and venetoclax for early T-cell precursor acute lymphoblastic leukemia. These cases highlight the potential of targeted agents to improve outcomes while minimizing toxicity. Our findings underscore the need

Unusual Clinical Presentation of B-Cell Prolymphocytic Leukemia Cases as Splenic Marginal Zone Lymphoma Effectively Treated With Rituximab Monotherapy.

BACKGROUND/AIM: B-cell prolymphocytic leukemia (B-PLL) is a rare B-cell neoplasm, historically classified as a distinct entity, but recently removed from the World Health Organization (WHO) classification due to its overlap with other B-cell chronic leukemic lymphoproliferative disorders (B-CLD). B-cell prolymphocytic leukemia: an enduring bona fide entity.

B-cell prolymphocytic leukemia (B-PLL) was recognized as a distinct entity in the fourth edition of the World Health Organization (WHO) classification for hematolymphoid neoplasms (WHO-HAEM4); however, its de novo presentation has been removed from the upcoming 5th edition classification (WHO-HAEM5). We present a case of a 65-year-old man with leukocytosis, fatigue, and no organomegaly by imaging. Bone marrow examination showed a prolymphocytoid population comprising 78% of the marrow elements. After thorough exclusion of other entities by clinical parameters and ancillary methods, we conclude

Optimizing drug combinations for T-PLL: restoring DNA damage and P53-mediated apoptotic responses.

T-prolymphocytic leukemia (T-PLL) is a mature T-cell neoplasm associated with marked chemotherapy resistance and continued poor clinical outcomes. Current treatments, that is, the CD52-antibody alemtuzumab, offer transient responses, with relapses being almost inevitable without consolidating allogeneic transplantation. Recent more detailed concepts of T-PLL's pathobiology fostered the identification of actionable vulnerabilities: (1) altered epigenetics, (2) defective DNA damage responses, (3) aberrant cell-cycle regulation, and (4) deregulated prosurvival pathways, including T-cell receptor

Alemtuzumab monotherapy for T-cell prolymphocytic leukemia: an observational study in Japan.

Alemtuzumab is recommended as first-line and second-line therapies for T-cell prolymphocytic leukemia (T-PLL). This study retrospectively evaluated the efficacy and safety of alemtuzumab in nine Japanese patients with T-PLL at five participating institutions who were treated between January 2015 and August 2023. The median age at first administration of alemtuzumab was 72 years (range, 39 to 78). Two patients were treatment naïve, and seven had been treated with a median of one (range, 1 to 3) prior systemic therapy. Six patients were refractory to their most recent therapy. Three patients com

Splenic marginal zone lymphoma with prolymphocytic transformation and cyclin D1 expression in the absence of CCND1 rearrangement.

Splenic marginal zone lymphoma (SMZL) is one of the most common B-cell lymphomas that affect the spleen. We report a case with splenomegaly and lymphocytosis that showed a clonal B-cell population lacking CD5 and CD10 expression. Notably, the atypical lymphoid cells showed prolymphocytoid morphology and expressed cyclin D1. Fluorescence in-situ hybridization was negative for CCND1/IgH rearrangement. The prolymphocytoid morphology and cyclin D1 expression present a diagnostic pitfall. The clinical presentation, morphology, immunophenotype, and molecular genetic findings are most consistent with

Epigenetic features support the diagnosis of B-cell prolymphocytic leukemia and identify 2 clinicobiological subtypes.

The recognition of B-cell prolymphocytic leukemia (B-PLL) as a separate entity is controversial based on the current classification systems. Here, we analyzed the DNA methylome of a cohort of 20 B-PLL cases diagnosed according to the guidelines of the International Consensus Classification/Fourth revised edition of the World Health Organization Classification, and compared them with chronic lymphocytic leukemia (CLL), mantle cell lymphoma (MCL), splenic marginal zone lymphoma (SMZL), and normal B-cell subpopulations. Unsupervised principal component analyses suggest that B-PLL is epigeneticall

Understanding splenic B-cell lymphoma/leukaemia with prominent nucleoli: Diagnosis, underpinnings for disease classification and future directions.

The 5th edition of the WHO classification of haematolymphoid tumours (WHO-HAEM5) introduced a new category, splenic B-cell lymphoma/leukaemia with prominent nucleoli (SBLPN). The diagnostic entity B-cell prolymphocytic leukaemia (B-PLL) has been discontinued and the category of hairy cell leukaemia variant (HCLv) has been conceptually reframed. B-PLL and HCLv diagnoses were uncommon. Overlap existed between B-PLL and other indolent lymphomas like chronic lymphocytic leukaemia/small lymphocytic lymphoma (CLL/SLL). HCLv lacked consistent cytomorphological, immunophenotypic and genetic features.
