FDXR upregulation by p63/p73 is a prognostic and therapeutic marker of adult T-cell leukemia/lymphoma.

Infection with HTLV-1 can lead to severe diseases, including adult T-cell leukemia/lymphoma (ATL), for which current therapies are inadequate. Selective elimination of chronically infected cells requires identifying cellular markers dysregulated by HTLV-1 that can be exploited therapeutically. Combining metabolomics and proteomics analyses, we identified increased expression of the mitochondrial enzyme ferredoxin reductase (FDXR) as a hallmark of HTLV-1-infected cells. In patient cells, FDXR upregulation mirrored disease severity, was enriched in proliferating tumour cells and predicted worse

Allogeneic stem cell transplantation in aggressive Adult T cell leukemia/lymphoma: a prospective multicenter study.

Adult T-cell leukemia/lymphoma (ATL) is associated with a dismal prognosis in aggressive subtypes, with a median overall survival (OS) of less than one year. In Japan, first line chemotherapy followed by allogeneic stem cell transplantation (allo-SCT) is recommended for eligible patients, yet this strategy is scarcely reported elsewhere, especially among ethnic minorities. We prospectively evaluated the feasibility and outcome of allo-SCT in this setting. Between 2016 and 2024, 80 ATL patients were enrolled; 74 (median age 50 years) received frontline chemotherapy (CHO(E)P regimen +/-Brentuxim

HTLV-1 and ATL: Decoding Molecular Pathogenesis for Next-Generation Therapies.

Adult T-cell leukemia/lymphoma (ATL) is an aggressive T-lymphocyte malignancy arising as a late consequence of human T-cell leukemia virus type 1 (HTLV-1) infection. Despite decades of research, ATL remains characterized by a dismal prognosis and high relapse rates, representing a significant clinical challenge. This review proposes a unifying model of ATL pathogenesis centered on the transition from a transient, Tax-driven early transformation to a constitutive, HBZ-mediated persistent state. We present this switch as a unifying conceptual framework, and explicitly distinguish its well-suppor

A novel severe toxicity-free, and progression-free survival endpoint predicts outcomes after CD19 chimeric antigen receptor-T cell therapy in large B-cell lymphoma.

CD19-directed chimeric antigen receptor-T cell (CAR-T) therapies have transformed treatment for relapsed/refractory large B-cell lymphoma (LBCL), yet heterogeneity in toxicity and efficacy persists. To address this, we developed a novel composite endpoint: severe toxicity-free and progression-free survival at 6 months (TPFS6), defined as absence of severe CRS, ICANS, progressive/stable disease (PD/SD), and non-relapse mortality (NRM) within 6 months of CAR-T therapy. In our cohort, 37% achieved TPFS6. Events contributing to TPFS6 failure included severe CRS (6.8%), ICANS (30.3%), NRM (1.7%), a

Comparative clinical outcomes and single-cell profiling of CD19 CAR-T versus blinatumomab for relapsed/refractory B-ALL.

BACKGROUND: CD19-targeting chimeric antigen receptor T-cells (CAR-T) and blinatumomab represent two leading immunotherapies for relapsed/refractory B cell acute lymphoblastic leukemia (r/r B-ALL). Despite sharing the same target, their differential clinical efficacy and underlying mechanisms remain unclear. METHODS: This retrospective study enrolled 106 r/r B-ALL patients (61 CD19 CAR-T vs 45 blinatumomab) between February 2020 and August 2023. The complete response (CR) rate, long-term survival, and therapeutic toxicities were compared between the two cohorts. Additionally, we conducted scRN

Impact of Residual Disease Localization Before Allogeneic Hematopoietic Stem Cell Transplantation on Outcomes in Adult T-Cell Leukemia-Lymphoma.

Allogeneic hematopoietic stem cell transplantation (allo-HCT) is the only curative approach for adult T-cell leukemia-lymphoma (ATL). However, preventing post-transplant relapse remains challenging. While pre-transplant disease status is known to be associated with post-transplant outcomes, the impact of residual disease (RD) localization at the time of allo-HCT has not been well described. In this retrospective study of 33 patients with aggressive ATL who underwent allo-HCT, patients were classified into two risk groups based on RD sites: a low-risk group (no lymph node [LN] or extranodal [EN

First human pleural infection caused by Naganishia globosa (formerly Cryptococcus saitoi).

Naganishia globosa (formerly Cryptococcus saitoi) is an environmental basidiomycetous yeast rarely associated with human disease. We report the first case worldwide of N. globosa isolated from pleural fluid in a patient with T-cell acute lymphoblastic leukemia (T-ALL), an immunocompromised host, hospitalized in Türkiye. The isolate was identified by MALDI-TOF MS and confirmed by molecular sequencing. The patient received antifungal therapy following diagnosis. Liposomal amphotericin B, isavuconazole, fluconazole, and voriconazole were used in the patient's treatment and were modified according

Lymphoblastic lymphoma in adults: diagnosis, prognosis and treatment.

Lymphoblastic lymphoma is classified within the spectrum of acute lymphoblastic leukemia, though it possesses distinct clinical and biological characteristics that distinguish it from the latter. Prognostic factors are primarily based on genetic alterations associated with high-risk disease, as well as treatment response evaluated by PET-CT and measurable residual disease in the bone marrow. However, these data are derived from retrospective studies, and prospective validation is necessary. Treatment is based on protocols identical to those used for acute lymphoblastic leukemia, as they provid

Simultaneous Determination of Venetoclax and Posaconazole in Human Plasma by UPLC-MS/MS: Application to Therapeutic Drug Monitoring in Acute Myeloid Leukemia Patients.

BACKGROUND: Due to the high risk of invasive fungal disease (IFD) in acute myeloid leukemia (AML) patients, the antifungal drug posaconazole is often co-administered with venetoclax. As posaconazole is a potent inhibitor of CYP3A4, the standard dosing regimen may lead to the elevated plasma concentrations of venetoclax due to potential drug-drug interactions (DDIs). Therefore, therapeutic drug monitoring (TDM) is necessary to optimize the dosage. METHODS: This study developed and validated a rapid and sensitive ultra performance liquid chromatography tandem mass spectrometry (UPLC-MS/MS) meth

Immunotherapy in the up-front treatment of adult B-cell precursor acute lymphoblastic leukemia: when, how, who?

Acute lymphoblastic leukemia (ALL) is a hematologic malignancy characterized by the proliferation of immature lymphoid cells. Despite advances in chemotherapy, a significant proportion of patients continue to have poor outcomes. Immunotherapy has emerged as a transformative approach in the treatment of ALL, offering new hope for these patients. This advances viewpoint article provides a comprehensive overview of the role of immunotherapy in ALL, focusing on the mechanisms of action, the timing of its use, the patient populations most likely to benefit, and drug resistance. We discussed the maj

Outcomes of patients with HTLV-1 adult T-cell leukemia/lymphoma (ATLL) after allogeneic SCT: results of the EBMT LWP.

Adult T-cell leukemia/lymphoma (ATLL) is a rare HTLV-1-associated malignancy with a dismal prognosis due to intrinsic chemoresistance and immunosuppression. This retrospective EBMT registry analysis evaluated outcomes of allogeneic stem cell transplantation (allo-SCT) in 73 ATLL patients transplanted between 2004 and 2021. With a median follow-up of 3.9 years, 2-year overall survival (OS) and progression-free survival (PFS) were 49% (95% CI 37-60) and 43% (95% CI 31-54), respectively. Non-relapse mortality was 7% at 2 years, while relapse incidence (RI) was 49.8%. Multivariate analysis identif

CAR HEMATOTOX independently predicts outcomes after CD19 CAR-T therapy for acute lymphoblastic leukemia.

Chimeric antigen receptor (CAR) T-cell (CAR-T) treatment for B-cell acute lymphoblastic leukemia (ALL) induces high initial response rates, but most patients relapse. Low disease burden (often defined as <5% blasts in the bone marrow) is associated with better outcomes. CAR HEMATOTOX (HT) is a score using prelymphodepletion hematologic and inflammatory parameters to predict outcomes in lymphoma. Here, we assess its prognostic utility in a large multicenter adult B-cell ALL cohort. Patients who received brexucabtagene autoleucel across 33 centers in North America were included as part of the Re

Tumor burden-guided dosing contributes to mitigation of immunotoxicities following treatment with obecabtagene autoleucel in adult patients with relapsed/refractory B-cell acute lymphoblastic leukemia.

Obecabtagene autoleucel (obe-cel) is a CD19-targeted autologous chimeric antigen receptor T-cell therapy (CAR T) with a fast off-rate binding domain, administered as split-dose infusions guided by pre-lymphodepletion tumor burden (low-tumor- burden [TB] group: ≤20%; high-TB group: >20% bone marrow [BM] blasts). Obe-cel treatment in adult relapsed/refractory B-cell acute lymphoblastic leukemia (R/R B-ALL) was investigated in the phase Ib/II FELIX trial. Blinatumomab exposure prior to allogeneic stem cell transplantation is associated with increased Epstein-Barr virus reactivation.

Blinatumomab, a CD19/CD3 bispecific T-cell engager, is frequently used for B-cell acute lymphoblastic leukemia (B-ALL) prior to allogeneic hematopoietic stem cell transplantation (allo-HSCT). Although its efficacy in achieving measurable residual disease negativity is well established, its profound B-cell depletion and potential immunomodulatory effects may exacerbate post-transplant viral reactivation risks, particularly in the context of the inherent immunosuppression associated with allo-HSCT. In this matched retrospective cohort study, we identified 97 consecutive B-ALL patients who receiv
