Distinguishing T-cell from NK-cell neoplasms can occasionally be challenging, as neoplastic T cells can lose T-cell markers such as surface CD3 (sCD3) and CD5 while acquiring NK-cell markers such as CD16, CD56, and CD94. In this study, we present a series of 7 mature T-cell lymphoma/leukemia cases with NK-like immunophenotypes to clarify diagnostic approaches for lineage determination. The cohort included 3 cases of peripheral T-cell lymphoma, not otherwise specified, 2 cases of T-large granular lymphocytic leukemia, and 2 cases of hepatosplenic T-cell lymphoma. In all cases, flow cytometry im

Purtscher-like retinopathy (PLR) is a secondary, non-traumatic occlusive microvascular retinal disease characterized by retinal leukoderma, hemorrhage, and cotton wool spots. It is commonly associated with conditions such as pancreatitis, renal disease, and infections including COVID-19 but is rarely reported in patients with hematologic malignancies, particularly following hematopoietic stem cell transplantation (HSCT). This article reports a case of relapsed B-cell acute lymphoblastic leukemia (B-ALL) in a patient who underwent multiple lines of immunotherapy, including CD19- and CD22-target

Human T-lymphotropic virus type 1 (HTLV-1) infection is asymptomatic in most carriers; however, approximately 5% of infected individuals may develop adult T-cell leukemia/lymphoma (ATLL), a rare and highly aggressive malignancy with poor prognosis. We report a case of acute-type ATLL in an elderly patient from the Brazilian Amazon that was initially suspected to be T-cell lymphoblastic lymphoma, resulting in diagnostic delay. Peripheral blood and bone marrow examinations revealed marked lymphocytosis with characteristic "flower cells," prompting serological testing for HTLV-1. Infection was co

BACKGROUND AND PURPOSE: Follicular mucinosis is characterized by deposits of mucin in the sebaceous gland and outer root sheath of hair follicles. The diagnosis may occur without another cutaneous disease ("idiopathic" follicular mucinosis) or in association with hematologic malignancies, most often cutaneous T-cell lymphoma, and other skin conditions.
PATIENT: We present a case of follicular mucinosis occurring in an African male with adult T-cell leukemia-lymphoma.
RESULTS AND INTERPRETATION: Four cases of follicular mucinosis in the setting of adult T-cell leukemia-lymphoma have been repo

We study the numerical approximation of a time-dependent variational mean field game system with local couplings and either periodic or Neumann boundary conditions. Following a variational approach, we employ a finite-difference discretization and solve the resulting finite-dimensional optimization problem using the Chambolle-Pock primal-dual algorithm. As this involves computing proximal operators and solving ill-conditioned linear systems at each iteration, we embed within our solver a general class of parallel-in-time preconditioners based on suitably-chosen diagonalization techniques, appl

BACKGROUND: Adult T-cell leukemia/lymphoma (ATL) is a rare and aggressive type of peripheral T-cell malignancy caused by the human T-lymphotropic virus type 1 (HTLV-1). Approximately 5 to 10 million people are infected with HTLV-1 worldwide, with high prevalence in Japan. This observational study analyzed patient characteristics, treatment patterns, healthcare resource utilization (HRU), and costs of ATL in Japan.
METHODS: A retrospective analysis of ATL patients from the Medical Data Vision database, Sept. 1, 2012-Aug. 31, 2022, was conducted.
RESULTS: Among 1572 ATL patients, 384 received

BACKGROUND: Relapsed and refractory (R/R) T-cell acute lymphoblastic leukemia/lymphoma (T-ALL/T-LL) remains a therapeutic challenge. Allogeneic hematopoietic stem cell transplantation (HSCT) after achieving complete remission (CR) offers the only chance of cure, but effective modalities to achieve CR are limited. Activation of mTOR and PI3K pathways has been well described in T-ALL and monotherapy with mTOR inhibitors has shown efficacy signals used in relapsed disease.
METHODS: This multisite Phase 1 study evaluated the safety, tolerability, and anti-leukemic activity of everolimus with NEC

Donor-derived anti-CD19 chimeric antigen receptor T-cell (CAR T-cell) therapy has emerged as one of the most promising treatments for relapsed/refractory (R/R) B-cell acute lymphoblastic leukemia (B-ALL). In this retrospective study, we evaluated the safety and efficacy of donor-derived anti-CD19 CAR T cells manufactured at our local GMP-compliant production facility. Eleven patients with R/R B-ALL received donor-derived anti-CD19 CAR T-cell therapy. Seven of the eleven patients had previously undergone allogeneic hematopoietic stem cell transplantation (allo-HSCT). All eleven patients achieve

Acute leukemia is a hematologic malignancy that affects people of all ages. Immunotherapies, including T-cell engagers, antibody-drug conjugates, and chimeric antigen receptor T-cell therapies have contributed to a paradigm shift in the treatment of patients with acute leukemia. Combined with advancements in allogeneic hematopoietic stem cell transplant and targeted therapy development, patients with acute leukemia have additional effective treatment options, providing the possibility of long-term remission. However, these advances have resulted in a complex and dynamic treatment landscape for

T-lymphoblastic leukemia/lymphoma (T-ALL/LBL) is an aggressive neoplasm of immature T-lymphoid precursors that is classified as T-cell acute lymphoblastic leukemia when the primary site of involvement is the bone marrow and peripheral blood and as T-cell lymphoblastic lymphoma when it presents as a solid mass, most commonly in the anterior mediastinum. The neoplastic cells are defined by expression of T-lineage antigens (cytoplasmic or surface CD3) together with one or more markers of immaturity (terminal deoxynucleotidyl transferase, CD1a, CD34, CD99, or CD117) and must not fulfill criteria f

Adult T-cell leukemia/lymphoma (ATLL) is an aggressive HTLV-1-associated T-cell neoplasm, and durable control of relapsed/refractory disease remains difficult without allogeneic hematopoietic stem cell transplantation. Mogamulizumab, an afucosylated anti-C-C chemokine receptor type 4 (CCR4) monoclonal antibody, and tucidinostat, an oral histone deacetylase inhibitor, have activity in this setting, but the immunovirological consequences of sequential CCR4-directed therapy and epigenetic modulation are unclear. We describe two patients with relapsed or refractory ATLL who achieved prolonged dise

Acute promyelocytic leukemia (APL) predominantly affects younger patients with significantly prolonged survival. Therefore, controlling the incidence of secondary hematological malignancies represents a crucial therapeutic goal. Secondary, acute T-lymphoblastic leukemia (T-ALL) following APL is rare and confers a poor prognosis, with only 2 cases reported to date. Herein, we present a case of a young female with APL who attained complete remission (CR) after receiving dual induction therapy with all-trans retinoic acid (ATRA) and arsenic trioxide (ATO), followed by standard consolidation and m

Replication stress (RS) represents a major vulnerability of cancer cells treated with nucleoside analogs and related antimetabolites; however, tumors frequently acquire tolerance mechanisms that permit survival despite persistent DNA lesions. This review examines molecular determinants of RS tolerance, focusing on human T-cell leukemia virus type 1 (HTLV-1)-mediated adult T-cell leukemia/lymphoma (ATL) as a model of virus-mediated rewiring of DNA damage responses. Chain-terminating nucleoside analogs generate aberrant replication intermediates, including blocked 3' DNA termini, mis-incorporate

HTLV-1 is a retrovirus associated with adult T cell leukemia/lymphoma (ATL) and inflammatory diseases, including HTLV-1-associated myelopathy (HAM) and HTLV-1-associated bronchopneumonopathy (HAB). Although pulmonary complications are common in HTLV-1-associated diseases, the underlying mechanisms remain unclear. We compared HTLV-1 proviral load (PVL) and chemokine receptor expression in bronchoalveolar lavage (BAL) cells and peripheral blood mononuclear cells (PBMCs) from asymptomatic carriers (ACs) and patients with HAB, HAM, or ATL. T cell subsets were analyzed by flow cytometry, and the ex

Elranatamab, a BCMA-CD3 bispecific antibody, has demonstrated robust activity in relapsed/refractory multiple myeloma (RRMM), but real-world outcomes remain poorly defined. We conducted a multicenter retrospective study of 130 patients treated with commercial elranatamab across nine U.S. academic centers. The cohort was heavily pretreated (91% triple-class refractory, 49% penta-refractory), with 49% previously exposed to BCMA-targeted therapies. Only 22% would have met eligibility for MagnetisMM-3 cohort A. The overall response rate (ORR) was 65%, including ≥CR in 36%. Median progression-free
