CAR HEMATOTOX independently predicts outcomes after CD19 CAR-T therapy for acute lymphoblastic leukemia.

Chimeric antigen receptor (CAR) T-cell (CAR-T) treatment for B-cell acute lymphoblastic leukemia (ALL) induces high initial response rates, but most patients relapse. Low disease burden (often defined as <5% blasts in the bone marrow) is associated with better outcomes. CAR HEMATOTOX (HT) is a score using prelymphodepletion hematologic and inflammatory parameters to predict outcomes in lymphoma. Here, we assess its prognostic utility in a large multicenter adult B-cell ALL cohort. Patients who received brexucabtagene autoleucel across 33 centers in North America were included as part of the Re

Tumor burden-guided dosing contributes to mitigation of immunotoxicities following treatment with obecabtagene autoleucel in adult patients with relapsed/refractory B-cell acute lymphoblastic leukemia.

Obecabtagene autoleucel (obe-cel) is a CD19-targeted autologous chimeric antigen receptor T-cell therapy (CAR T) with a fast off-rate binding domain, administered as split-dose infusions guided by pre-lymphodepletion tumor burden (low-tumor- burden [TB] group: ≤20%; high-TB group: >20% bone marrow [BM] blasts). Obe-cel treatment in adult relapsed/refractory B-cell acute lymphoblastic leukemia (R/R B-ALL) was investigated in the phase Ib/II FELIX trial. Here, we report pharmacokinetics, safety, and efficacy outcomes in patients with low or high tumor burden and discuss the evidence/rationale ju

Blinatumomab exposure prior to allogeneic stem cell transplantation is associated with increased Epstein-Barr virus reactivation.

Blinatumomab, a CD19/CD3 bispecific T-cell engager, is frequently used for B-cell acute lymphoblastic leukemia (B-ALL) prior to allogeneic hematopoietic stem cell transplantation (allo-HSCT). Although its efficacy in achieving measurable residual disease negativity is well established, its profound B-cell depletion and potential immunomodulatory effects may exacerbate post-transplant viral reactivation risks, particularly in the context of the inherent immunosuppression associated with allo-HSCT. In this matched retrospective cohort study, we identified 97 consecutive B-ALL patients who receiv

Bridging With Chimeric Antigen Receptor T-Cell Therapy or Chemotherapy to Allo-HSCT in B-ALL: A Comparison of Treatment Complications and Survival.

OBJECTIVES: Comparisons of safety and efficacy of Allogeneic hematopoietic stem cell transplantation (allo-HSCT) following complete remission (CR) achieved by Chimeric antigen receptor T (CAR-T) cell therapy versus chemotherapy for B-cell acute lymphoblastic leukemia (B-ALL) have not been fully discussed. METHODS: We performed a comparison of transplant outcomes in 443 consecutive B-ALL patients who received allo-HSCT after achieving CR with CAR-T therapy (n = 50) or with chemotherapy (n = 393). The median follow-up time was 30 months (range: 1-62 months). RESULTS: The CAR-T group had a lowe

[A clinical experience with CAR-T cell therapy brexu-cel as salvage option for a Relapsed/Refractory patient with B-Cell Acute Lymphoblastic Leukemia in advanced disease stage].

Brexucabtagene autoleucel (brexu-cel), a CD19-directed chimeric antigen receptor (CAR) T-cell therapy, is a promising and available salvage option for relapsed/refractory philadelphia-positive B-Cell Acute Lymphoblastic Leukemia patients, potentially effective even after several previous lines of therapy and a previous allogeneic stem cell transplant (HSCT). This reported clinical case allowed us to highlight different relevant factors for the optimal, complex and multidisciplinary management of this subgroup of patients: bridging therapy selection, the need of reevaluating CD19 expression, di

[To transplant or not to transplant after CAR-T: that is the question!].

CAR-T cell therapy revolutioned the therapeutic landscape of the relapsed/refractory B-cell acute lymphoblastic leukemia (ALL-R/R), improving relapse-free survival and overall survival of patients. Optimal management of late complications, such as hematological toxicity as well as allogeneic transplantation role in the therapeutic sequence, are specific topics currently being debated. We report a case of a young woman with ALL-B R/R extramedullary relapsed after a first allogeneic transplant and second-line immunotherapy, successfully treated with brexu-cel obtaining a complete remission statu

Impact of Pretransplant Blinatumomab on Post-Transplant Outcomes in B Cell Acute Lymphoblastic Leukemia: A Systematic Review and Meta-Analysis.

B cell acute lymphoblastic leukemia (B-ALL) is an aggressive hematological malignancy that often requires allogeneic hematopoietic cell transplant (HCT) for long-term remission. Outcomes after HCT are affected by pretransplant disease burden, especially measurable residual disease (MRD) status. Blinatumomab, a CD19/CD3 bispecific T cell engager, has shown effectiveness in eliminating MRD in patients with B-ALL. Its effect on HCT outcomes is uncertain. To determine the impact of pretransplant blinatumomab on B-ALL outcomes after HCT. We performed a systematic review and meta-analysis of prospec

Homeodomain-driven oncogenic diversion to a γδTCR phenotype in T-cell acute lymphoblastic leukemia.

T-cell acute lymphoblastic leukemia (T-ALL) results from the malignant transformation of thymocytes blocked in their differentiation. Surface expression of the γδ T-cell receptor (γδTCR) is surprisingly frequent in T-ALLs, questioning the susceptibility of the γδ-lineage to leukemogenesis. Among 1233 T-ALLs phenotyped in our center, 33% (n = 403) expressed a TCR, of which 47% (n = 191; 113 adults, 78 children) were positive for γδTCR (γδTCR+). Using a comprehensive analysis, we were able to delineate 2 distinct γδTCR+ T-ALL subtypes, with distinct physiological counterparts. The first (75% of

Targeting PRMT5 in Adult T-Cell Leukemia/Lymphoma: Opportunities and Challenges.

Adult T-cell leukemia/lymphoma (ATLL) is an aggressive T-cell malignancy caused by persistent infection with human T-cell leukemia virus type 1 (HTLV-1). ATLL remains difficult to treat despite intensive chemotherapy, antiviral therapy, and hematopoietic stem cell transplantation. The limited durability of current treatment strategies highlights the need for mechanism-based therapeutic approaches. Protein arginine methyltransferase 5 (PRMT5) is a type II arginine methyltransferase that regulates transcription, RNA splicing, DNA damage responses, and immune signaling through symmetric dimethyla

Composite lymphoma comprising adult T-cell leukemia/lymphoma and Epstein-Barr virus-positive primary central nervous system lymphoma.

A 73-year-old woman presented with cervical lymphadenopathy. A biopsy of the affected lymph node showed the diagnosis of adult T-cell leukemia/lymphoma (ATLL). While treatment was planned, she developed rapid-onset consciousness disturbance and gait disorder 1 week before the initiation of therapy, leading to emergency hospitalization. Computed tomography showed a hypodense lesion in the midbrain, and a cerebrospinal fluid analysis showed ATLL cells, resulting in a diagnosis of central nervous system infiltration of ATLL. High-dose methotrexate plus cytarabine therapy was administered. However

Post-translational modifications of triosephosphate isomerase reveal metabolic vulnerabilities in T-ALL: effect of combining dichloroacetic acid and the PPI rabeprazole.

Acute lymphoblastic leukemia, particularly the T-cell subtype, remains associated with poor outcomes in relapsed and adult patients, highlighting the need for novel therapeutic strategies. Metabolic reprogramming, especially glycolytic dependence, represents a promising target. Triosephosphate isomerase (TPI), a key glycolytic enzyme, undergoes cancer-associated post-translational modifications (PTMs), including deamidation and phosphorylation. Here, we evaluated the potential of proton pump inhibitors (PPIs), particularly rabeprazole (Rbz), to selectively target PTM-bearing TPI isoforms in Ju

Clathrin-associated SCYL2 contributes to the activation of PI3K/AKT signaling and tumorigenesis through PTEN phosphorylation in adult T-cell leukemia/lymphoma.

Inactivation of PTEN by post-translational modifications causes aberrant amplification of the PI3K/AKT signaling pathway in many tumors. PTEN is a tumor suppressor phosphatase that is frequently phosphorylated at conserved serine/threonine residues (S380, T382, and T383 clusters) in the C-terminal tail of ATL and various solid cancer cells. Here, we identify SCY1-like protein 2 (SCYL2), with a protein kinase-like domain, as a novel PTEN-binding protein; however, the mechanism by which SCYL2 regulates PTEN phosphorylation remains unclear. SCYL2-associated complex phosphorylates PTEN at STT, and

HDAC3 inhibition as a therapeutic strategy in T-cell acute lymphoblastic leukemia via the TYK2-STAT1-BCL2 signaling pathway.

INTRODUCTION: Few advances have been made in the treatment of T-cell acute lymphoblastic leukemia (T-ALL). Approaches targeting histone deacetylases (HDAC) have not been thoroughly investigated in T-ALL. However, the underlying molecular mechanism of HDAC inhibition remains to be fully elucidated. OBJECTIVES: The study aimed to evaluate the clinical outcome of chidamide (an oral selective HDAC inhibitor for HDAC1, HDAC2, HDAC3, and HDAC10) in combination with chemotherapy in relapsed or refractory T-ALL and explore the underlying molecular mechanism of HDAC inhibition in T-ALL. Blinatumomab-driven T-cell activation in αβ and γδ T-cell subsets: insights from in vitro assays.

INTRODUCTION: Blinatumomab (BLN) is a bispecific T-cell engager that has revolutionized the treatment of B-cell precursor acute lymphoblastic leukemia (BCP-ALL), significantly improving outcomes in both adults and children. By simultaneously binding to CD19 on B cells and CD3 on T cells, BLN triggers target cell-dependent T-cell activation, resulting in the cytolysis of CD19+ BCP-ALL cells. Despite the remarkable clinical advancements achieved with BLN, the immunological mechanisms underlying treatment response or failure remain poorly characterized. γδ T cells are attractive candidates for ad

Inotuzumab Ozogamicin in Clinical Practice: an Overview of Efficacy, Safety, and Real-World Applications.

PURPOSE OF REVIEW: Acute lymphoblastic leukemia (ALL) is a rare hematologic malignancy with a bimodal distribution of incidence in both pediatric/young adult and elderly patient populations. Despite the high complete remission rate, there is a high rate of relapse necessitating a need for therapy options in the relapsed/refractory setting. Given this, treatment paradigms for ALL have shifted towards targeted therapies and away from high-intensity chemotherapy. The efficacy of inotuzumab ozogamicin (InO) in the relapsed/refractory setting for pediatric and adult populations has led to incorpora
