Validation of prognostic indices in adult T-cell leukemia/lymphoma and their prognostic value at the start of second-line treatment.

Adult T-cell leukemia/lymphoma (ATL), a peripheral T-cell neoplasm associated with human T-cell leukemia virus type 1 (HTLV-1), carries a poor prognosis. We first confirmed the applicability of the simplified ATL-prognostic index (sATL-PI) and the Japan Clinical Oncology Group prognostic index (JCOG-PI) in the cohort of patients with aggressive ATL diagnosed between January 2009 and March 2022 at Kagoshima University Hospital. The power of stratification of survival at the start of second-line treatment, namely sATL-PI-2 and JCOG-PI-2, were then evaluated in 53 and 54 patients. The sATL-PI-2 c

T-Cell Receptor Clonotypes and Aggressive Subtypes in Cutaneous T-Cell Lymphoma.

IMPORTANCE: T-cell receptor (TCR) clonotype patterns across disease stages and histologic subtypes in mycosis fungoides (MF) and Sézary syndrome (SS) remain poorly characterized, limiting their use in risk stratification. OBJECTIVES: To assess the association of TCR β (TCRB) and γ (TCRG) clonotypes with disease stage, folliculotropism, large-cell transformation, and overall survival (OS) as well as clonal abundance (percentage of total reads) with immune checkpoint expression. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study conducted at City of Hope (Duarte, California) in

Comparative Analysis of T-Cell and Bone Marrow Chimerism for Relapse Prediction in Acute Leukaemia Post-Transplantation.

Allogeneic haematopoietic stem cell transplantation is a critical treatment for acute myeloid leukaemia (AML) and acute lymphoblastic leukaemia (ALL), yet the risk of treatment failure still persists. Chimerism analysis serves as a potential tool for predicting disease recurrence and survival rates, but its specific role has not yet been clearly defined. This study aimed to explore the role of decreased T-cell and bone marrow (BM) chimerism in predicting post-transplant relapse and survival in patients with acute myeloid leukaemia (AML) and acute lymphoblastic leukaemia (ALL). The study subjec

Gamma delta T cell reconstitution in Acute Lymphoblastic Leukemia patients post allogeneic HSCT predicts clinical outcome and overall survival.

Predicting the transplant outcome of Acute Lymphoblastic Leukemia patients undergoing allogeneic hematopoietic stem cell transplantation (allo-HSCT) is a challenge. It is necessary to have prognostic biomarkers that can be monitored in these patients. In the present single center, long term follow-up study, immune cell composition or the immunophenotype of Acute Lymphoblastic Leukemia patients (n = 14) was analyzed at various intervals following allogeneic HSCT and correlated with clinical outcomes. Long-term survivors post-HSCT showed distinct immune cell profiles compared to non-survivors. L

Evaluating the Role of AI Assistants in Accelerating Neurodegenerative Disease Research: Opportunities and Translational Limitations.

Neurodegenerative diseases including Alzheimer's disease and Parkinson's disease remain among the most challenging disorders to study, diagnose and treat. Despite rising prevalence with population aging, disease-modifying therapies remain scarce and research progress is hindered by biological complexity, patient heterogeneity, and incomplete experimental systems. Artificial intelligence (AI) has emerged as a transformative approach given the high-dimensional and multimodal data generated in this field. Traditional machine learning and deep learning have advanced imaging biomarker detection, di

Safety and efficacy of chidamide for maintenance therapy after allogeneic hematopoietic stem cell transplantation in patients with T-ALL/T-LBL.

Post-transplant relapse remains a primary obstacle to long-term survival in patients with T-cell acute lymphoblastic leukemia/lymphoma (T-ALL/T-LBL), underscoring the need for effective maintenance therapies. Although chidamide, a selective histone deacetylase inhibitor, has shown activity in T-cell malignancies, its role as post-transplant maintenance has been scarcely studied. To evaluate the efficacy and safety of chidamide as maintenance therapy in T-ALL/T-LBL patients after allogeneic hematopoietic stem cell transplantation (allo-HSCT). We retrospectively analyzed 75 T-ALL/T-LBL patients

Blinatumomab Utilization in Pediatric B-Cell Acute Lymphoblastic Leukemia: Experience From the Mountain West.

BACKGROUND: Blinatumomab is a bispecific T-cell engager approved for the treatment of pediatric B-cell acute lymphoblastic leukemia (B-ALL). Outpatient home infusion reduces hospitalization burden and optimizes resource utilization, but is logistically challenging. METHODS: Our institution developed a multidisciplinary outpatient blinatumomab protocol. The workflow included early prior authorization, home healthcare (HH) partnerships, and standardized procedures for outpatient management. Clinical and logistical data were prospectively collected from August 2024 to August 2025. RESULTS: Sinc

PD-L1-armored CD19/CD22 dual-targeted CAR-T cell co-infusion bridging to allogeneic hematopoietic stem cell transplantation achieves 7-year sustained remission in an adult patient with early relapsed, chemorefractory B-cell acute lymphoblastic leukemia: a case report.

BACKGROUND: Antigen escape and PD-L1/PD-1 axis-mediated immunosuppression in the tumor microenvironment (TME) are the predominant drivers of treatment failure in adult patients with relapsed/refractory (R/R) B-cell acute lymphoblastic leukemia (B-ALL). CD19-targeted CAR-T monotherapy has limited ability to overcome these two core resistance mechanisms, and achieving long-term durable remission remains a critical unmet clinical need in adults with high-risk, chemorefractory B-ALL. METHODS: This is a single-center, retrospective case report of an adult patient with early-relapsed, chemorefracto

Blinatumomab consolidation for high-risk Ph- B-cell acute lymphoblastic leukemia: the GRAALL-2014/B-QUEST study.

Intensified chemotherapy regimens have improved outcomes in adults with Philadelphia chromosome-negative (Ph-) B-cell acute lymphoblastic leukemia (B-ALL), yet relapse remains a major cause of treatment failure and death. Blinatumomab is a T-cell engager that demonstrated marked activity in relapsed and measurable residual disease-positive (MRD+) B-ALL, as well as in frontline consolidation for MRD- patients. The phase 2 GRAALL-2014/B-QUEST substudy evaluated the integration of blinatumomab into consolidation and maintenance therapy for adults with high-risk (HR) B-ALL. Between 2015 and 2020,

Pre-Transplant MRD Negativity and Age Under 40 Predict Superior Long-Term Survival in Relapsed/Refractory B-ALL After CAR-T Therapy Bridging to Haploidentical HSCT: A Multicenter Retrospective Study With 6-Year Follow-Up.

Sequential allogeneic hematopoietic stem cell transplantation (allo-HSCT) is a critical approach to enhance the efficacy of chimeric antigen receptor T cell (CAR-T) therapy for relapsed/refractory acute lymphoblastic leukemia (R/R ALL). The long-term survival rates following allo-HSCT after CAR-T cell therapy remain controversial. We previously reported the results of 122 patients with R/R B-ALL who achieved minimal residual disease negative (MRD-) complete remission (CR) after anti-CD19 CAR-T, including 67 patients without subsequent transplantation (non-transplant group) and 55 patients with

CD19/BCMA-Targeted Chimeric Antigen Receptor-T Cell Therapy for Anti-HLA Antibody Sensitisation in Patients With Acute Leukaemia: A Pilot Clinical Study.

Antibodies targeting foreign HLA molecules can be detrimental in transplantation. Nonetheless, existing desensitisation methods for anti-HLA antibodies frequently prove insufficient. We conducted a single-arm pilot study to evaluate CD19/B cell maturation antigen (BCMA) chimeric antigen receptor T (CAR-T) cell therapy for desensitising anti-HLA antibodies prior to transplantation. Seven patients diagnosed with acute leukaemia participated in the study, comprising four individuals with acute myeloid leukaemia (AML) and three with acute B lymphoblastic leukaemia (B-ALL). Among them, one patient

SOHO State of the Art Updates and Next Questions: Novel Therapies in T-ALL: From CAR-T to Novel Targets.

While outcomes for children and adolescents with T-cell acute lymphoblastic leukemia (T-ALL) have improved significantly with contemporary therapy, outcomes for newly diagnosed adults and all patients with relapsed or refractory (r/r) disease remain poor. Improved understanding of T-ALL genomics and the integration of novel agents into treatment have the potential to improve outcomes further by enhancing risk stratification and improving salvage rates for those with r/r disease. In this review, we will discuss landmark genomic studies that have identified therapeutic targets in T-ALL, shed fur

Epidemiology and Clinical Outcomes of HTLV-1: A Comprehensive Narrative Review of Endemic and Nonendemic Regions.

Human T-lymphotropic virus Type 1 (HTLV-1) is a globally distributed, oncogenic retrovirus endemic in specific regions, including southwestern Japan, sub-Saharan Africa, the Caribbean, parts of South America (notably Brazil), Iran, and Indigenous communities in Australia. Although most infections are asymptomatic, a minority of carriers develop severe, life-altering conditions: adult T-cell leukemia/lymphoma (ATL) or HTLV-1-associated myelopathy/tropical spastic paraparesis (HAM/TSP). This narrative review presents a comprehensive analysis of epidemiological studies, clinical reports, and publ

Dinaciclib reduces MCL-1 levels and triggers apoptosis in adult T-cell leukemia/lymphoma.

Adult T-cell leukemia/lymphoma (ATL) is a rare T-cell malignancy caused by long-term HTLV-1 infection. The disease is aggressive, with a median survival of 8-10 months, and no new treatments are currently available. Dinaciclib is a selective small-molecule drug that targets CDK1, CDK2, CDK5, and CDK9. This study assessed dinaciclib's ability to inhibit ATL cells. It significantly suppressed ATL proliferation at low nanomolar concentrations. The antiproliferative effect was due to induced apoptotic cell death, as shown by FACS analysis. Annexin/PI staining revealed morphological signs of apopto

Anti-thymocyte globulin (ATG)- or alemtuzumab-based graft-versus-host disease prophylaxis in reduced-intensity conditioning allogeneic hematopoietic cell transplantation (HCT) for patients 40 years and older with acute lymphoblastic leukemia in first complete remission: a study from the EBMT Acute Leukemia Working Party.

For patients with high-risk acute lymphoblastic leukemia (ALL), allogeneic hematopoietic cell transplantation (HCT) remains standard of care. In the setting of an HLA-matched unrelated donor HCT, in vivo T-cell depletion (TCD) for prophylaxis of graft versus host disease (GVHD) relies on anti-thymocyte globulin (ATG) in Europe and alemtuzumab in the UK. In a retrospective study from the EBMT registry, we pair-matched 90 ALL patients aged ≥40 years transplanted in CR1 according to age (median 56 years) and ALL subtype (37.8% Ph-negative B-ALL, 46.7% Ph-positive B-ALL, 15.6% T-ALL).
