INTRODUCTION: Adult T-cell leukemia/lymphoma (ATL) exhibits marked clinical heterogeneity; however, current classification primarily rely on morphology and laboratory findings and require further advancement. This study aimed to evaluate the roles of human T-lymphotropic virus type 1 (HTLV-1)-infected cell (CD4-positive T cell) analysis system using flow cytometry (HAS-Flow) and absolute CD4 count in prognostic assessment of nonaggressive ATL.
METHODS: We retrospectively analyzed 227 individuals with HTLV-1 infection in Japan (derivation cohort) and 81 in the United Kingdom (validation cohort

Exhausted T cells are phenotypically and functionally heterogeneous, from progenitor- to terminally-exhausted T cells. We evaluated gene expression profile of CD8+ T cells in acute leukemia to characterize the phenotype of exhausted T cells. Blood samples were collected from acute lymphoblastic leukemia (ALL) and acute myeloid leukemia (AML) patients prior to treatment and from control subjects. Additionally, samples were obtained from ALL patients after induction therapy. TCF7, NFATc1, IRF4, and BATF gene expression was then evaluated in isolated CD8+ T cells. CD8+ T cells from ALL patients s

Multiantigen targeting chimeric antigen receptor (CAR) T cells have emerged as a strategy to mitigate antigen escape observed after single antigen targeting therapy. Our initial experience with a bivalent CD19.22.BBζ CAR T-cell construct in children, adolescents and young adults (CAYA) with B-cell acute lymphoblastic leukemia (B-ALL) demonstrated limitations in CD22 recognition, but a tolerable safety profile and efficacy prompted further evaluation. This trial enrolled patients between the ages of 3-39 with relapsed/refractory B-ALL. Following dose-escalation, patients who enrolled at the rec

Post-transplant relapse remains a major challenge in T-ALL, with limited effective salvage options. We report on eight T-ALL patients who received low-dose chidamide (5 mg once weekly) as maintenance therapy after allogeneic HSCT. With a median follow-up of 22.7 months, the estimated 2-year OS and EFS are both 87.5%. Seven patients (87.5%) remain in continuous complete remission with sustained MRD negativity. Grade 3-4 neutropenia occurred in 57.1% of evaluable patients but was clinically manageable. This case series demonstrates that a low-dose chidamide maintenance regimen is feasible, well

Human T-cell Leukemia Virus type 1 (HTLV-1) drives Adult T-cell Leukemia/Lymphoma (ATLL) through sustained expression of the viral oncoprotein HBZ. Although HBZ is known to reshape host transcriptional programs, its role in post-transcriptional regulation in ATLL remains poorly understood. Here, we uncover a mechanism by which HBZ reprograms circular RNA (circRNA) biogenesis to support leukemic cell survival. Comprehensive circRNA profiling across ATLL subtypes revealed extensive circRNA remodeling, with distinct signatures associated with aggressive ATLL subtypes. Among these, circAFF2(3) is

Human T-cell leukemia virus-1 (HTLV-1) is the etiological agent of a series of chronic inflammatory diseases such as HTLV-associated myelopathy/Tropical spastic paraparesis (HAM/TSP), uveitis, dermatitis, and pneumonitis, and, importantly, of a T-cell lymphoproliferative neoplasm designed adult T-cell leukemia/lymphoma (ATL). Two viral proteins, Tax-1 and HBZ, are crucially involved in HTLV-1 infectivity and in ATL by altering key pathways of cell homeostasis. A fundamental distinction between the expression of the two oncoproteins exists, witnessed by the fact that Tax-1 is expressed in early

In the single-arm Phase Ib/II FELIX study (NCT04404660), obecabtagene autoleucel (obe-cel; CD19-directed autologous CAR T-cell therapy) demonstrated high overall remission rates (ORR) and a favorable safety profile in adults with relapsed/refractory (R/R) B-cell acute lymphoblastic leukemia (B-ALL). To contextualize results from FELIX, we compared the efficacy and safety of obe-cel with matched external control arms (ECA) derived from historical trials using propensity score matching. ECAs represented standard of care (SoC) non-CAR T-cell therapies: blinatumomab, inotuzumab ozogamicin, and con

BACKGROUND: Chimeric antigen receptor (CAR) T-cell therapy has transformed the treatment of hematologic malignancies, but its success depends on obtaining sufficient CD3+ T-cell yields during leukapheresis. This can be difficult in heavily pretreated patients, who often show leukopenia and reduced T-cell fitness.
METHODS: We analyzed 166 leukapheresis products from 154 patients undergoing manufacturing of CD19-directed CAR T-cells, including 146 from non-Hodgkin's lymphoma (NHL) and 20 from acute lymphoblastic leukemia (ALL). Collections were performed for commercial CAR T-cell products (axi-

BACKGROUND: Regulatory T cells (Tregs) may influence prognosis in leukemia. This study aimed to explore the effects and mechanisms of Treg subclusters on the prognosis in B-cell acute lymphoblastic leukemia (B-ALL) using single-cell RNA sequencing (scRNA-Seq).
METHODS: scRNA-seq was performed on sorted T cells from adults with newly diagnosed (Dix) B-ALL and healthy donors (HD) to establish the training cohort. Treg phenotypes, functions, and prognostic relevance were evaluated. A scRNA-seq dataset from the GEO database, including Dix and relapsed B-ALL, served as the validation cohort. Key f

OBJECTIVES: Assessing the stage of maturation is critical for the classification of T-cell neoplasms and is helpful in minimal/measurable residual disease (MRD) testing for T-lymphoblastic leukemia (T-ALL). This study analyzes the utility of CD48 in characterizing T-cell neoplasms at diagnosis and posttherapy.
METHODS: This retrospective review characterizes the expression of CD48 in T-ALL and mature T-cell neoplasms at diagnosis and posttreatment. CD48 intensity was assessed qualitatively by flow cytometry relative to the background T cells. Potential confounders were also evaluated.

T-cell non-Hodgkin lymphomas, which arise from post-thymic mature T cells, constitute approximately 10-15% of all non-Hodgkin lymphomas. Their leukemic presentations, referred to here as mature T-cell leukemias, are relatively uncommon and present significant diagnostic and therapeutic challenges requiring an informed approach to diagnosis and management. The initial presentation is often persistent T-cell lymphocytosis that must be distinguished from reactive (non-malignant) causes. Unlike B-cell lymphocytosis, where clonality usually indicates malignancy, T-cell clonality can be detected in

Talicabtagene autoleucel (tali-cel) is the first humanized CD19-directed CAR-T cell therapy approved in India for relapsed/refractory (r/r) B-cell acute lymphoblastic leukemia (B-ALL) and B-cell non-Hodgkin lymphoma (B-NHL). Here, we integrated clinical and manufacturing operations through a centralized coordination unit (CCU) and evaluated tali-cel implementation. Patients with r/r B-ALL (n = 105) and r/r B-NHL (n = 145) who underwent leukapheresis between 15 November 2023 and 31 January 2025 across 56 treatment centers were included. The CCU enabled timely delivery, with manufacturing slot a

Viruses induce approximately 12% of human cancers, including lymphomas. In the case of T/NK cell neoplasms, human T-cell leukemia virus type I (HTLV-1) causes adult T-cell leukemia-lymphoma (ATL), and Epstein-Barr virus (EBV) is associated with extranodal NK/T-cell lymphoma (ENKTCL) and chronic active Epstein-Barr virus disease (CAEBV). Common mechanisms for lymphoma development have been proposed. Viral genes, such as tax and HTLV-1 bZIP factor (HBZ) of HTLV-1, and latent membrane protein 1 (LMP1) and BamHI A rightward transcript microRNA (miRNA-BART) of EBV, contribute to host immune evasion

OBJECTIVES: Adult T-cell leukemia (ATL), caused by human T-cell leukemia virus type 1 (HTLV-1), has a poorer prognosis than other peripheral T-cell lymphomas. Phosphorylation of highly expressed in cancer 1 (Hec1) by NIMA-related kinase 2 (Nek2) is essential for mitosis. This study evaluated the therapeutic potential of Nek2 and Hec1 inhibitors in ATL.
METHODS: Cell proliferation, survival, cell cycle progression, apoptosis, mitochondrial membrane potential, and reactive oxygen species (ROS) were assessed. Expression of Nek2, Hec1, and related signaling proteins was also analyzed.
RESULTS: N

Among the metal-derived complexes, recently, tin derivatives have been investigated as promising anti-cancer drug candidates. Our previous study showed that the tin-based compound Bu3SnOCOCF3 (TBT) exerts cytotoxic activity on solid tumor cell lines. In the present study, the effects of TBT were evaluated in vitro on HTLV-1-infected human lymphocytic cell lines at different stages of viral transformation, consisting of IL-2-dependent (PB2/IL-2) and IL-2-independent (PB2/NO-IL-2) cells, generated in our laboratory by HTLV-1 in vitro infection of lymphocytes from the same donor, and the C91/PL c

The chemokine system, comprising a network of chemokines and their receptors, orchestrates leukocyte migration and plays a central role in immune surveillance and inflammation. Targeting this system has emerged as a promising strategy in cancer immunotherapy and the treatment of immune-related disorders. C-C chemokine receptor 4 (CCR4), the receptor for chemokines CCL17 and CCL22, is a clinically validated therapeutic target for adult T cell leukemia/lymphoma, cutaneous T cell lymphoma, and other malignancies; however, the molecular mechanisms underlying CCR4 inhibition remain poorly understoo

BACKGROUND: The human T-cell leukemia virus type 1 (HTLV-1) is a neglected oncogenic retrovirus responsible for adult T-cell leukemia/lymphoma (ATLL) and autoimmune diseases that disproportionately affects marginalized populations worldwide. Peru reports the highest global ATLL incidence, yet comprehensive genomic studies remain limited. We aimed to characterize the virological, immunological, and host genetic landscape of HTLV-1 infection to identify population-specific high-risk features.
METHODS: We conducted a prospective cohort study (2017-2018) of 67 HTLV-1-infected individuals from Lim
