18F-FDG PET/CT is rarely used to evaluate extramedullary disease (EMD) in leukemia, and the systemic assessment of non-central nervous system (non-CNS) EMD is unavailable for B-cell acute lymphoblastic leukemia (B-ALL). Here, we assessed EMDs in each patient with relapsed/refractory (r/r) B-ALL, regardless of whether the patient had EMD symptoms. Ninety-one adult patients were included, of them, 48 (52.7%) had undergone allogeneic hematopoietic cell transplantation (allo-HCT), and 17 (18.7%) had accepted chimeric antigen receptor-T (CAR-T) therapy. EMDs were evaluated using PET/CT (n = 50) or

Breast hematopoietic neoplasms (BHN) are rare and may either primarily (PBHN) or secondarily (SBHN) involve breast tissue. The widespread use of needle biopsy for diagnosis of breast lesions necessitates knowledge of their presentation, radiologic aspects, histomorphology, and outcomes for seeking appropriate hematopathology consultation. Herein, we present our clinical experience as an academic institution and referral center of BHN in the past 20 years. We identified 59 patients diagnosed at the University of Chicago Medical Center between 2002-2021. Demographic, pathologic, radiologic, ther

Interferon regulatory factors (IRFs) are innate immune transcription factors responsible for inducing the expression of type I interferons (IFN-I), which combat pathogen invasions via initiating the downstream Janus kinase signal transducer and activator of transcription (JAK-STAT) pathway. Among them, IRF7 plays a central role by forming heterodimers with IRF3 to initiate IFN-I production and is therefore frequently targeted by viruses to evade immune detection. Contrary to this common paradigm, we show that IRF7 is activated and upregulated by the retrovirus human T-cell leukemia virus type

This study investigates T-cell antigen expression and functional subsets in T-cell large granular lymphocytic leukemia (T-LGLL) cells by flow cytometry. The leukemic T-LGLs frequently showed expressions of CD57, dim CD2, CD5 and CD7, and more likely CD16 and HLA-DR, compared with reactive/benign T-LGLs. The leukemic T-LGLs were composed predominantly of CD8+ effector memory with re-acquired CD45RA cells (TEMRA, median: 89.6%), with TEMRA cells in >70% of the T-LGLs in 82% of the cases. A small subset of T-LGLL cases showed predominance of CD8+ naïve cells (TN, 2%), CD8+ effector memory cells (

The presence of lymphoid cells with flower-like nuclei in peripheral blood is an uncommon morphologic finding, classically associated with Human T-lymphotropic virus type 1 (HTLV-1) infection related Adult T-cell Leukemia/Lymphoma (ATLL). However, similar cytologic features may be observed in other hematolymphoid malignancies. To describe cases of hematologic neoplasms presenting with flower cells in the peripheral blood and to highlight their broader diagnostic spectrum. We report a series of patients with distinct hematologic malignancies who demonstrated flower cells on peripheral blood sme

Children with B-cell acute lymphoblastic leukemia (B-ALL) are increasingly treated with immunotherapeutic agents such as blinatumomab and inotuzumab. Thus, those who present in relapse for chimeric antigen receptor (CAR) T-cell therapy are likely to have prior exposure to these other immunotherapies. The effect of blinatumomab and inotuzumab exposure on cellular therapy, particularly, cell collection and the composition of apheresis products, is unknown. In this study, we analyzed the apheresis composition of 113 pediatric and young adult patients with relapsed or refractory B-cell malignancie

Autologous chimeric antigen receptor (CAR)-T cell therapy is effective in relapsed or refractory B-cell acute lymphoblastic leukemia (R/R B-ALL), but its clinical application is restricted by manufacturing failures, high costs, and limited accessibility. Gene-edited allogeneic CAR-T cells offer an off-the-shelf alternative, yet concerns remain regarding genomic instability and graft-versus-host disease (GVHD). We conducted a phase 1 study to assess the safety and efficacy of ThisCAR-T, a non-gene-edited, CD19-directed allogeneic CAR-T cell product, in patients with R/R B-ALL. Eleven patients w

UNLABELLED: Assessments of chimeric antigen receptor (CAR) T-cell pharmacokinetics by flow cytometry (FC) and a droplet digital PCR (ddPCR) assay were compared in adult patients with relapsed/refractory (R/R) B-cell acute lymphoblastic leukemia (B-ALL) following treatment with obecabtagene autoleucel (obe-cel) in the phase Ib/II FELIX study (NCT04404660). CAR T-cell persistence and B-cell aplasia (BCA) were then correlated with event-free survival (EFS). Peripheral blood (PB) samples collected from 127 obe-cel-infused patients were tested by FC and ddPCR. The Spearman correlation coefficient w

Brexucabtagene autoleucel (brexu-cel) produces high rates of measurable residual disease-negative (MRD-) complete response (CR) in adult patients with Philadelphia chromosome-positive (Ph+) acute lymphoblastic leukemia (ALL); however, subsequent relapses remain frequent. It is unclear whether maintenance with tyrosine kinase inhibitors (TKIs) can enhance remission durability. We evaluated outcomes among adult patients with relapsed/refractory Ph+ ALL who received commercial brexu-cel and achieved MRD- CR across 19 US institutions. Considering TKI maintenance as a time-varying covariate, we ana

BACKGROUND: Although inotuzumab ozogamicin (InO) has been approved as an effective agent for patients with refractory/relapsed (r/r) B-cell acute lymphoblastic leukemia (B-ALL), data in the context of post chimeric antigen receptor (CAR)-T cell therapy and allogeneic hematopoietic stem cell transplantation (allo-HSCT) relapse are lacking.
CASE: We report on a series of three heavily pretreated r/r B-ALL patients who relapsed after CAR-T therapy and allo-HSCT and were salvage treated with InO. All achieved bone marrow (BM) remission with negative minimal residual disease (MRD) after the first

BACKGROUND: The historical results in adults with Philadelphia chromosome (Ph)-positive acute lymphoblastic leukemia (ALL) in second relapse treated with third-line therapy (Salvage 2) are extremely poor. Using standard chemotherapy resulted in complete response rates of 20% and a median overall survival of 2 to 3 months. This may be changing with the recent availability of novel therapeutic strategies, including targeted antibodies and engineered chimeric antigen receptor T-cell therapy, as well as more potent BCR::ABL1 tyrosine kinase inhibitors.

On the basis that T-cell acute lymphoblastic leukemia (T-ALL) cells overexpress IL7 receptor (IL7R), which promotes resistance to chemotherapy and disease relapse, here we develop IL7Rα (CD127)-targeted chimeric antigen receptor (CAR) T cells with low- and high-affinity single-chain variable fragments. We establish the antitumor efficacy of CD127 CAR against T-ALL cells in vitro, in female mouse models of T-ALL, and against blasts from patients with T-ALL using the patients' own T cells transduced with CD127 CAR. Antitumor efficacy is higher with low-affinity CAR T cells than high-affinity CAR

CD19-directed therapy remains the mainstay treatment for relapsed/refractory B-cell acute lymphoblastic leukemia (ALL). However, treatment patterns and outcomes following CD19-negative (CD19-) relapse remain poorly defined. We retrospectively analyzed 65 adult patients with ALL who developed CD19-relapse after CD19-directed therapy. TP53 mutations and BCR::ABL1-like ALL were each identified in 27.7% (n = 18) of patients. Forty-six patients (70.8%) received one CD19-targeted therapy, whereas 19 patients (29.2%) received ≥ 2 prior to CD19-relapse. Overall, 60 patients (92.3%) received blinatumom

Human T-cell leukemia virus type 1 (HTLV-1) is the causative agent of adult T-cell leukemia/lymphoma (ATLL) and the neuroinflammatory disease, HTLV-1-associated myelopathy/tropical spastic paraparesis (HAM/TSP). The HTLV-1 Tax regulatory protein plays a critical role in HTLV-1 persistence and pathogenesis; however, the underlying mechanisms are poorly understood. Here we show that Tax dynamically regulates mitochondrial reactive oxygen species (ROS) and membrane potential to trigger mitochondrial dysfunction. Tax is recruited to damaged mitochondria through its interaction with the IKK regulat

Perturbations in dendritic cells (DCs) in B/T cell acute lymphoblastic leukemia (ALL), their cause(s) and consequence(s) on antileukemia immunity, and patient outcomes remain poorly explored. We find that maturation of DC1-6 subsets is disrupted in children and adults with ALL. Conventional DC1 and DC2 subpopulations are reduced at the expense of the progenitors and the DC4 fractions in ALL. The potential to mature, present antigens, and produce cytokines for initiating T cell surveillance appears to be impaired in every ALL DC subset. Such DC subsets are accordingly unable to induce T cell pr

Background/Objectives: T-cell receptor gamma (TCRγ) clonality testing by multiplex polymerase chain reaction (PCR) is an important ancillary method in the diagnostic evaluation of T-cell lymphoproliferative disorders. Interpretation is particularly challenging in HTLV-1 infection, in which persistent antigenic stimulation may produce clonal or oligoclonal T-cell expansions overlapping with non-neoplastic patterns. This study aimed to assess the diagnostic performance and interpretative contribution of two EuroClonality/BIOMED-2 assays (TCRγ-A and TCRγ-B), considered complementary components of

BACKGROUND: Chimeric antigen receptor (CAR) T-cell therapy has expanded rapidly as an investigational treatment for advanced cancers. Studies in acute lymphoblastic leukemia (ALL) suggest health-related quality of life (HRQOL) may improve by 1-month post-infusion. However, patient-reported outcomes remain insufficiently characterized, particularly across the broader pediatric CAR T-cell population.
PROCEDURE: English- or Spanish-speaking patients (ages 8-25 years) receiving CAR T-cell therapy for any malignancy were recruited from three US pediatric cancer centers. Patient-reported HRQOL (Ped

Adult T cell leukemia/lymphoma (ATL) is an aggressive T cell malignancy with poor prognosis. Recurrent genetic alterations in T cell receptor (TCR) signaling components, including PLCG1, PRKCB, and CARD11, highlight the biological relevance of this pathway in ATL. We focused on mucosa-associated lymphoid tissue lymphoma translocation protein 1 (MALT1), a key regulator of TCR signaling that functions through complex formation with CARD11 and BCL10, and developed a potent and selective MALT1 protease inhibitor, CRD-1441551. CRD-1441551 exhibited variable antitumor effects across ATL models both

OBJECTIVE: To analyze the clinical characteristics and prognosis of adult T-cell leukemia/lymphoma (ATLL) patients.
METHODS: The clinical data of 32 newly diagnosed ATLL patients admitted to our hospital from January 1, 2014, to October 1, 2021 were retrospectively analyzed. The efficacy and the incidence of complications of different chemotherapy regimens were explored, and the factors that may affect patients' prognosis were analyzed.
RESULTS: Among the 32 enrolled ATLL patients, the male-to-female ratio was 1.3:1, and the median age was 53 (range 24-79) years. Of these patients, 21 were o
