CAR-T versus allogeneic transplantation as consolidation for B-cell acute lymphoblastic leukemia in remission: a propensity-score matched study.

BACKGROUND: Despite being a standard consolidation treatment, allogeneic hematopoietic stem cell transplantation (allo-HSCT) for B-cell acute lymphoblastic leukemia (B-ALL) in remission is still hampered by relapse and complications. While CAR-T therapy is revolutionizing later-line treatment, its role as a consolidation strategy in remission, particularly for patients ineligible for transplantation, remains unexplored. This study compared the efficacy of CAR-T therapy with that of allo-HSCT as consolidation for B-ALL patients in complete remission (CR). METHODS: A retrospective, propensity-s

Treatment outcome in pediatric patients with T-ALL in Brazil.

BACKGROUND: Several studies conducted in high-income countries (HICs) have demonstrated that treatment intensification in pediatric T-cell acute lymphoblastic leukemia (T-ALL) improved survival outcomes. However, studies in low- and middle-income countries (LMICs) remain scarce. In this study, we evaluated survival rates and their associations with clinical factors within the context of the Brazilian Unified Health System. METHODS: Data from 86 patients aged 0-19 years, diagnosed with T-ALL at one of four pediatric oncology reference centers of the Brazilian Unified Health System, were includ

Diagnostic Concordance and Superiority of Flow Cytometry Over Immunohistochemistry in Acute Leukemia Immunophenotyping: A Single-Center Study.

BACKGROUND Early diagnosis is crucial for effective treatment and management of acute leukemia. Flow cytometry (FC) is a fast and reliable immunophenotyping technique. This study aimed to compare the diagnostic agreement between FC and histopathological evaluation using shared parameters from bone marrow samples. MATERIAL AND METHODS A total of 144 patients presenting to Adult Hematology, Pediatric Hematology, and Pediatric Oncology clinics at Çukurova University Faculty of Medicine were enrolled; bone marrow samples were processed at the Central Laboratory FC Unit. The cohort included 70 pati

Long-term remission of chronic-type adult T-cell leukemia/lymphoma associated with Graves' disease treated with methimazole.

Adult T-cell leukemia/lymphoma (ATL) is a rare T-cell malignancy, where the chronic type often progresses to an acute form in the presence of poor prognostic factors. We report a remarkable case of chronic-type ATL that achieved long-term hematologic and molecular remission following methimazole treatment for concomitant Graves' disease. A 58-year-old woman was diagnosed with chronic-type ATL in 2022. Her disease showed gradual progression with elevated soluble interleukin-2 receptor (sIL-2R) levels and lactate dehydrogenase. In 2023, she developed Sjögren's syndrome and Graves' disease. Follo

Therapeutic Advances in Adult B-cell Acute Lymphoblastic Leukemia with KMT2A Rearrangements.

B-cell acute lymphoblastic leukemia (ALL) with KMT2A rearrangements (KMT2Ar B-ALL) represents a distinct, high-risk subtype in adults, characterized by aggressive disease biology, high relapse rates, and inferior long-term survival. Conventional intensive chemotherapy, even when consolidated with allogeneic hematopoietic stem cell transplantation (allo-HSCT), yields suboptimal outcomes, underscoring the need for novel therapeutic approaches. In recent years, substantial progress has been made with the introduction of antibody-based immunotherapies, cellular immunotherapies, and small molecular

Real-time diagnosis of adult T-cell leukemia/lymphoma using HBZ RNA in situ hybridization.

OBJECTIVES: Diagnosing adult T-cell leukemia/lymphoma (ATLL) remains challenging due to its clinical and histopathologic heterogeneity. While HTLV-1 serology is a key diagnostic criterion, it is often unavailable at the time of diagnosis and does not confirm viral integration in tumor cells. The HTLV-1-encoded HBZ RNA is a reliable diagnostic biomarker for ATLL. We evaluated the diagnostic performance of RNAscope in situ hybridization (ISH) for HBZ RNA in formalin-fixed, paraffin-embedded (FFPE) tissues to distinguish ATLL from other T-cell lymphomas. METHODS: HBZ ISH was evaluated in 72 case

Prognostic factor analysis of CD19 CAR-T therapy followed by hematopoietic stem cell transplantation in relapsed/refractory B-ALL.

To evaluate the long-term prognosis of patients with relapsed/refractory B-cell acute lymphoblastic leukemia (R/R B-ALL) who received sequential allogeneic hematopoietic stem cell transplantation (allo-HSCT) following CD19 Chimeric Antigen Receptor T-cell (CAR-T) therapy, and to identify the risk factors influencing overall survival (OS) and the severity of acute graft-versus-host disease (aGVHD). A retrospective analysis was conducted on 63 patients with R/R B-ALL who received sequential allo-HSCT after CD19 CAR-T therapy. Cox regression analysis was used to identify factors influencing OS. Outcomes of CHOP versus CHOEP/EPOCH in aggressive adult T-cell leukemia/lymphoma: an international cohort analysis.

Aggressive adult T-cell leukemia/lymphoma (ATL) subtypes (i.e., acute and lymphomatous) are rare diseases, and the conventional management with cyclophosphamide, doxorubicin, vincristine, and prednisone (CHOP) has historically yielded poor outcomes. The benefit of etoposide-containing regimens (CHOEP/EPOCH) in the first-line setting is inconclusive. We compared the effectiveness of CHOEP/EPOCH versus CHOP in aggressive ATL. We conducted an international, multicenter retrospective cohort study among newly diagnosed patients with ATL during 2000-2023 across five Latin American countries and the

Central nervous system involvement and relapse in adult acute lymphoblastic leukaemia-a population-based analysis in 2007-2022.

Central nervous system (CNS) involvement and relapse remains a clinical challenge in the management of acute lymphoblastic leukaemia (ALL), and studies among adult ALL patients are scarce. We identified all adult ALL patients in Sweden between 2007 and 2022 (n = 838) from the population-based ALL register and evaluated factors associated with CNS involvement and relapse. CNS involvement at diagnosis was detected in 7.8% of patients, more often in patients with T-cell ALL (T-ALL) and with white blood cell (WBC) count >30 × 109/L, without affecting overall survival or risk of any relapse. The 5-

TET2 germline mutation in a patient with sequential lymphoid malignancies: a novel case report.

We report a case of a 30-year-old male patient with a germline heterozygous TET2 R1354fs mutation, who successively developed three distinct lymphoid malignancies: mixed cellularity classical Hodgkin lymphoma (MCCHL), angioimmunoblastic T-cell lymphoma (AITL), and T-cell acute lymphoblastic leukemia (T-ALL). Sequencing analysis of the oral mucosa and bone marrow specimens during the remission phase confirmed the germline origin of the TET2 mutation. This represents the first documented case of the sequential transformation across three distinct lymphoid malignancies attributable to a germline

Adult T-cell leukaemia/lymphoma with different histological features in a single lymph node.

We report a case of adult T-cell leukaemia/lymphoma (ATLL) that exhibited different pathological subtypes within a single lymph node. The patient was a 76-year-old Japanese man who presented with decreased appetite and anemia, and a thorough examination was conducted. A peripheral blood analysis and imaging studies, including ultrasound and computed tomography, suggested malignant lymphoma, and cervical lymph node biopsy was subsequently performed. In the excised lymph node, the normal architecture was completely effaced, and areas showing the dense proliferation of pleomorphic atypical lymphoc

The risk and predictors of second primary malignancies in survivors of angioimmunoblastic T-cell lymphoma: a population-based competing risk analysis.

Survivors of angioimmunoblastic T-cell lymphoma (AITL) face a substantial burden of second primary malignancies (SPMs). Using the SEER database (2000-2022), we identified 2217 AITL patients for analysis. The overall SPM risk was significantly elevated versus the general population (SIR = 1.60), driven predominantly by a marked excess of acute myeloid leukemia (SIR = 22.27). Competing risk analysis estimated the cumulative incidence of SPMs at 4.1, 6.5, and 9.6% by 2, 5, and 10 years, respectively. Multivariable analysis confirmed age 55-74 years as an independent risk factor, whereas advanced

Characterization of morphological and immunophenotypic heterogeneity in adult T-cell leukemia/lymphoma and development of a peripheral blood tumor load-based prognostic index.

We evaluated morphologic and immunophenotypic features of adult T-cell leukemia/lymphoma (ATLL) using morphology and flow cytometry (FCM), compared tumor cell proportions in bone marrow (BM) and peripheral blood (PB), and assessed whether these parameters could improve prognostication beyond existing indices for aggressive ATLL. We retrospectively analyzed PB and BM morphology, FCM, and clinical records of individuals newly diagnosed with ATLL at our institution from 2010 to 2024. Morphologic reevaluation was performed in 47 of 58 enrolled individuals, all with leukemic manifestations. ATLL ce

Temporal trends in relative survival and excess mortality among US adults with lymphoid malignancies.

BACKGROUND: Lymphoid malignancies comprise a heterogeneous group of diseases with diverse treatment approaches and outcomes. Therapeutic advances have been uneven across subtypes, and contemporary data on temporal changes in mortality remain limited. We assessed trends in survival and excess mortality using US population-based data. METHODS: This retrospective cohort study included patients newly diagnosed with lymphoid malignancies during 2001-2018 from 17 registries of the Surveillance, Epidemiology, and End Results program. We estimated 5-year relative survival and modeled excess mortality

Exploiting the Notch pathway in HTLV-1-driven adult T-cell leukemia/lymphoma: a mini-review.

Human T-cell leukemia virus type 1 (HTLV-1) is the etiological agent of adult T-cell leukemia/lymphoma (ATLL) and several inflammatory disorders. Despite extensive research, the molecular mechanisms underlying HTLV-1 pathogenesis remain incompletely defined. Among host pathways hijacked by the virus, Notch signaling has emerged as a critical regulator of T-cell biology, immune evasion, and leukemogenesis. The Notch family, comprising four receptors (NOTCH1-4) and five ligands (Jagged1/2, DLL1/3/4), regulates key cellular processes, including proliferation, differentiation, and survival. Eviden
