Central nervous system involvement in CLL: an international retrospective study by ERIC, the European Research Initiative on CLL.

Central nervous system involvement (CNSi) of chronic lymphocytic leukemia (CLL) is a rare condition with no consensus on diagnostic criteria and limited evidence for management and outcome. Here, we report an international, multicenter, retrospective study conducted by the European Research Initiative on CLL. The study defined CNSi of CLL by the following: (1) detection of CLL cells in the cerebrospinal fluid or confirmation of CLL infiltration of the CNS based on a tissue biopsy, (2) clinical or radiographic evidence of neurologic disease, and (3) the absence of other explanations for the neu

Multiomics Profiling of T-cell Leukemia and Lymphoma Enables Targeted Therapeutic Discovery.

UNLABELLED: T-cell leukemias and lymphomas (TCL) form a heterogeneous group of rare and often aggressive malignancies. Because of the rarity and heterogeneity of TCL subtypes, clinical trials are challenging to conduct, making pharmacogenomic studies in cell line panels critical for the discovery of targeted therapeutics. The scarcity of data repositories with integrated multiomics and drug screening data hinders the preclinical evaluation of drug vulnerabilities and the identification of molecular markers predictive of responses to monotherapies and combinations. To address this gap, we condu

DNA methyltransferase inhibitors in hematological malignancies and solid tumors.

Epigenetic modifications such as DNA methylation play a fundamental role in oncogenesis and the progression of neoplasms neoplasias. DNA methyltransferase inhibitors (DNMTi) constitute a family of therapeutic agents that impede the methylation at the 5-position on cytosine nucleotides, thereby modulating the epigenetic regulation of tumor suppressor genes, oncogenes, and other key regulatory genes. The first-generation DNMTi azacitidine and decitabine have demonstrated substantial efficacy in the treatment of medically non-fit, older patients with acute myeloid leukemia (AML) or myelodysplasti

Cell State Transitions Drive the Evolution of Disease Progression in B-Lymphoblastic Leukemia.

UNLABELLED: Cancer stem cells (CSC) are hypothesized to promote tumor progression through innate chemoresistance and self-renewal. CSCs reside in the CD34+/CD38- immunophenotypic subpopulation of acute myeloid leukemia (AML). Isolation of CSCs from B-lymphoblastic leukemia (B-ALL) has proven difficult, and the cells of interest apparently are not isolated to the CD34+/CD38- compartment. This may be explained, in part, by temporal variations of CD34 and CD38 expression which result in stochastic cell state transitions (e.g., from CD34+/CD38+ to CD34+/CD38-). We present a mathematical model of t

Refined Continuous Risk Index for Accurately Predicting Outcomes of Patients With Chronic Lymphocytic Leukemia After Limited-Duration Therapy.

PURPOSE: Measurable residual disease (MRD) status after limited-duration treatment for chronic lymphocytic leukemia (CLL) strongly predicts survival times and is considered a surrogate end point. However, most prognostic indices, including the International Prognostic Index (CLL-IPI), rely solely on pretreatment features. To incorporate dynamic MRD data in prognostic models, the continuous individualized risk index (CIRI) was proposed in 2019. Since developed in the context of chemoimmunotherapy, we here propose a refined version of CIRI, in the context of fixed-duration targeted therapy, whic

Ten-year experience of CD22 CAR T cells for children and young adults with B-cell acute lymphoblastic leukemia.

In 2014, the first patient received a CD22 chimeric antigen receptor (CAR) T-cell product. As one of the earliest approaches targeting an antigen other than CD19, this trial addressed an unmet need while providing insights into CAR T-cell efficacy, impact of manufacturing changes, and management of inflammatory toxicities over its 10-year span. This final chapter provides a comprehensive review of the collective findings. Across 78 patients with B-cell acute lymphoblastic leukemia who received infusion, cytokine release syndrome was observed in 66 (84.6%) patients, whereas 28 (35.9%) had immun

Menin inhibitors for adult acute myeloid leukemia: 2025 update.

INTRODUCTION: Menin inhibitors are a relatively new class of drugs that reverse oncogenesis and induce leukemic cell differentiation by blocking the interaction between the MEN1 gene product (menin) and lysine (K)-methyltransferase 2 (KMT2A). This is relevant in 50-70% of acute myeloid leukemia (AML) subsets driven by overexpression of the HOXA9/MEIS1 pathway. These patients have dismal outcomes with current options, especially in the relapsed/refractory setting. AREAS COVERED: Here, we discuss menin pathobiology, the available and emerging clinical data for menin inhibitors, clinical nuance

The role of pharmacogenetics in the therapy regimen of acute lymphoblastic leukemia in children: recent discoveries and future perspectives.

Acute lymphoblastic leukemia (ALL) is the most frequent malignancy in children. Although survival rates have improved in recent decades, relapse and therapy-related toxicities remain major challenges in pediatric ALL. Pharmacogenetics is an approach with the potential to refine precision medicine in pediatric ALL. This review synthesizes current knowledge on pharmacogenetic markers of drug efficacy, toxicity, and adverse effects for drugs used in childhood ALL, including glucocorticosteroids, vincristine, asparaginase, anthracyclines, 6-mercaptopurine, and methotrexate. Clinical outcomes of modified busulfan plus cyclophosphamide versus total body irradiation plus cyclophosphamide for T-cell acute lymphoblastic leukemia/lymphoma.

T-cell acute lymphoblastic leukemia/lymphoblastic lymphoma (T-ALL/LBL) remains a challenging malignancy with poor prognosis. Allogeneic hematopoietic stem cell transplantation (allo-HCT) remains a key curative option, but the optimal conditioning regimen is unclear. We retrospectively analyzed 93 T-ALL/LBL patients undergoing allo-HCT between January 2010 and July 2023, including 72 with modified busulfan plus cyclophosphamide (mBuCy) and 21 with total body irradiation plus cyclophosphamide (TBI-Cy). Propensity score matching (PSM) was applied to adjust baseline differences. Prior to PSM, surv

Hidden in Plain Sight: Unmasking Hairy Cell Leukemia Through Structured Clinical Reasoning.

Hairy cell leukemia (HCL) is a rare, indolent B-cell neoplasm that often presents with nonspecific laboratory findings, which can delay diagnosis. We report the case of a 75-year-old male with incidentally detected isolated thrombocytopenia and relative lymphocytosis. Peripheral blood smear showed lymphoid cells with cytoplasmic projections suggestive of hairy cells. Flow cytometry confirmed a dominant clonal population consistent with classical HCL, along with a small CD5+/CD23+ monoclonal B-cell population compatible with a chronic lymphocytic leukemia-like clone. Abdominal CT also revealed

Sustained Benefit of Blinatumomab in Infants With KMT2A-Rearranged ALL: Long-Term Outcomes, Toxicity, and Pharmacokinetics.

KMT2A-rearranged infant ALL (KMT2A-r ALL) has a poor prognosis. Adding blinatumomab, a bispecific T-cell engager targeting CD19, to standard chemotherapy for infants with KMT2A-r ALL improved short-term outcomes. Here, we present long-term results, toxicity, and pharmacokinetics of blinatumomab from this study. Thirty infants received Interfant-06 protocol chemotherapy with one additional postinduction blinatumomab course. Disease-free survival (DFS) and overall survival (OS) were compared with a historical Interfant-06-selected cohort without blinatumomab. Infection and administration of intr

Geographic and Socioeconomic Determinants of Treatment Abandonment in Pediatric Acute Leukemia: A Cohort Study in South-Central Mexico.

IntroductionPediatric acute leukemia is the most common childhood malignancy and one of the leading causes of cancer-related mortality worldwide, particularly, in low- and middle-income countries (LMICs), where treatment abandonment remains a major barrier to survival. Geographic accessibility and socioeconomic conditions are recognized determinants, but their combined influence in Mexico remains understudied. This study evaluated the association between geographic accessibility, socioeconomic factors, and treatment abandonment among children with acute leukemia in south-central Mexico. Methods

Multi-scale transcriptomics analysis reveals MHC suppression in MEF2D fusion positive B-cell acute lymphoblastic leukemia.

MEF2D gene related fusions, define a unique B-cell precursor acute lymphoblastic leukemia (BCP-ALL) with poor prognosis. In this study, we performed single-cell RNA sequencing (scRNA-seq) on three MEF2D fusion cases and collected transcriptomic data of 81 BCP-ALL cases with MEF2D fusions from public datasets. By comparing scRNA-seq profiles with those of normal bone marrow cells and other BCP-ALL subtypes, we identified a characteristic pre-B cell developmental arrest specific to the MEF2D subtype. Single-cell analyses indicated a pronounced reduction of major histocompatibility complex (MHC)

The effect of IL-4 on CD180 expression and signalling in Chronic lymphocytic leukaemia (CLL).

Chronic lymphocytic leukaemia (CLL) progression is critically dependent on tumour microenvironment (TME) signals, including those regulated via toll-like receptors (TLRs) that drive CLL cell survival and proliferation. We have previously shown that CD180, an orphan TLR is expressed on the surface in approximately 60 % of CLL samples, and that heterogeneity of CD180 cell surface expression and signalling impacts CLL cell survival, proliferation, and apoptosis. Interleukin-4 (IL-4), a cytokine abundantly present in the CLL microenvironment, is a well-documented pro-survival factor for CLL cells.

Personalised multipeptide-based T-cell activator for chronic lymphocytic leukaemia: an open-label, single-centre, phase 1 study.

BACKGROUND: Therapeutic T-cell activation to induce tumour-specific immune responses promises sustainable cancer control. However, this treatment is not yet widely used because of the challenges of personalised drug design and a shortage of mutation-derived neoepitopes. This study aimed to evaluate the immunogenicity, safety, and toxicity of iTAC-XS15-CLL01, a personalised warehouse-based multipeptide T-cell activator combined with the Toll-like receptor 1/2 ligand XS15, in patients with chronic lymphocytic leukaemia who were undergoing Bruton's tyrosine kinase inhibitor (BTKi)-based regimes.

Prolonged progression-free survival with zanubrutinib in relapsed/refractory CLL: an indirect treatment comparison versus other BTK inhibitors using multilevel network meta-regression.

BACKGROUND: Bruton tyrosine kinase inhibitors (BTKis) are therapeutic agents for relapsed/refractory chronic lymphocytic leukemia (R/R CLL). Previous indirect treatment comparisons are limited in simultaneously comparing multiple interventions and adjusting for population differences. This study aimed to use a more rigorous approach called multilevel network meta-regression (ML-NMR) to estimate the relative treatment effects of zanubrutinib compared to acalabrutinib and ibrutinib in two target populations: a general R/R CLL population similar to the phase 3 ALPINE trial's intention-to-treat (I

Identification of cryptic KMT2A-PTD and other novel fusion genes by transcriptome sequencing alters molecular risk stratification in AML-NK.

Acute myeloid leukaemia with normal karyotype (AML-NK) is a cytogenetically cryptic yet heterogeneous clinical subgroup that lacks structural chromosomal abnormalities. Although classified as an intermediate-risk group, AML-NK exhibits variability in patient outcomes, underscoring the need for refined molecular stratification. In this study, high-throughput deep sequencing was performed on 51 AML-NK patients, with a comprehensive analysis of a Southeast Asian cohort to identify cryptic and novel fusion genes potentially implicated in disease pathogenesis and prognosis.
