The spectrum of tonsillar involvement in chronic lymphocytic leukemia (CLL).

Tonsillar enlargement in chronic lymphocytic leukemia (CLL) either with or without symptoms has been rarely reported. We describe 22 cases of CLL with tonsillar involvement and demonstrate this is a CLL disease site that may be under-recognized in routine practice. Tonsillar involvement can be divided into three categories: (1) symptomatic CLL tonsillar involvement with upper airway obstruction (UAO) requiring therapy managed with: (a) tonsillectomy without systemic therapy when tonsillar enlargement is disproportionate to overall disease state, (b) tonsillectomy and systemic CLL therapy, and

Dynamic feedback control of oncogenic tyrosine kinase signaling in acute leukemia.

CD25 is a subunit of the interleukin-2 (IL-2) receptor on T cells and natural killer (NK) cells. Acute leukemias with oncogenic tyrosine kinases often include CD25+ leukemia subpopulations, which portend poor clinical outcomes for patients; however, acute leukemia cells do not respond to IL-2. Here, we identified CD25 and its phosphorylation by protein kinase Cδ (PKCδ) as central elements of a feedback loop that stabilized fluctuations in oncogenic tyrosine kinase signaling in acute lymphoblastic and myeloid leukemia. Genetic ablation of CD25 in murine and patient-derived xenograft (PDX) model

Real-world treatment utilization patterns, discontinuation and healthcare resource utilization of first-line Bruton tyrosine kinase inhibitors among elderly patients ≥65 years in chronic lymphocytic leukemia.

BACKGROUND: Chronic lymphocytic leukemia (CLL) commonly affects older patients, who often endure greater health disparities and treatment burdens. This study assessed treatment utilization patterns, discontinuation, and healthcare resource utilization (HCRU) of first-line (1L) use of Bruton tyrosine kinase inhibitors (BTKis) in elderly patients with CLL. RESEARCH DESIGN AND METHODS: A retrospective observational study using the Symphony Integrated Dataverse identified CLL patients aged ≥65 years initiating a 1L BTKi (zanubrutinib, acalabrutinib, ibrutinib; 01/2022-11/2024). Baseline character

Chronic lymphocytic leukemia with IGH/BCL2 fusion and clonal heterogeneity: phenotypic and molecular profiling analysis of a rare case.

Clonal heterogeneity within chronic lymphocytic leukemia (CLL) has been increasingly recognized as a factor that may influence disease progression, treatment response, and prognosis. Here, we report a rare case of CLL with two distinct B-cell populations displaying distinct immunophenotypic characteristics, genetic mutations, and genetic evolution. The two aberrant mature B cell populations were identified in peripheral blood (PB) and bone marrow (BM) through multiparametric flow cytometry (MFC) with identical light chain restriction but distinct immunophenotypes. The first population (CD5 + C

A Brainstem-Thalamic-Cortex Glutamatergic Neuron Pathway Modulates Propofol Anesthesia.

BACKGROUND: The precise mechanisms underlying the modulation and transformation of consciousness during propofol anesthesia remain inadequately understood. Both the paraventricular thalamus (PVT)-medial prefrontal cortex (mPFC) pathway and the parabrachial nucleus (PB) are implicated in the regulation of sleep-wake cycles. Consequently, this study aimed to explore whether projections from the PB to the PVT-mPFC pathway are involved in the effects of propofol anesthesia. METHODS: Anterograde and retrograde viral tracing techniques were employed to map the projections from the PB to the PVT and

Long-Read Whole-Transcriptome Sequencing and Selective Gene Panel Profiling Enable Sensitive Detection of Fusion Oncogenes in Pediatric B-Cell Acute Lymphoblastic Leukemia.

Long-read whole-transcriptome sequencing (WTS) has the potential to precisely characterize fusion oncogenes that drive leukemia and other cancers. Although there is a variety of general-purpose fusion detection algorithms that use modern long-read sequencing data, they show poor sensitivity for precision diagnostics in B-cell acute lymphoblastic leukemia (B-ALL) and do not robustly assess technical and analytical parameters (eg, sequencing depth) to reliably detect fusion transcripts. FUSILLI (FUSions In Leukemia Long-read sequencing Investigator) is a novel long-read fusion detection algorith

Cutaneous Adverse Effects in Patients Treated with BTK Inhibitors.

Bruton's tyrosine kinase (BTK) inhibitors have revolutionized the treatment landscape for patients with indolent lymphoid malignancies such as chronic lymphocytic leukemia (CLL) and mantle cell lymphoma (MCL). The most common adverse events include cardiac arrhythmia, bleeding, infection, diarrhea, arthralgias, hypertension, and skin changes. Second-generation BTK inhibitors, e.g., acalabrutinib and zanubrutinib and the non-covalent BTK inhibitor pirtobrutinib, are less toxic than the first-generation BTK inhibitor ibrutinib. The most common toxic skin symptoms related to BTKi treatment includ

Expression and signaling patterns of CD180/MD-1 and CD150 in Chronic Lymphocytic Leukaemia (CLL) cells.

Chronic lymphocytic Leukaemia (CLL) is a B-cell malignancy marked by biological heterogeneity, variable progression and survival outcomes. Orphan toll-like receptor CD180 (RP105) complexed with the accessory protein MD-1 and CD150, which belongs to the signalling lymphocyte activation molecule family (SLAMF1), have emerged as an important modulators of CLL cell signaling and fate. However, their functional interplay, remains insufficiently understood. This study investigated the surface and intracellular expression of CD180, CD150, and MD-1 in primary CLL cells and the MEC-1 cell line, alongsi

Aberrant signaling in tonsillar B cells producing pathogenic O-glycoforms of IgA1 in IgA nephropathy.

IgA nephropathy (IgAN) is a mesangioproliferative glomerulonephritis characterized by IgA1-containing immune-complex deposits wherein IgA1 is enriched for galactose-deficient IgA1 (Gd-IgA1) glycoforms. IgAN pathogenesis involves mucosal immune system, as IgAN onset and activity are associated with infections of the upper-respiratory tract, i.e., synpharyngitic hematuria. Current four-hit hypothesis postulates that multiple events, starting with the production of Gd-IgA1, in genetically susceptible individuals lead to the formation of nephritogenic immune complexes and development of IgAN. Automated CLL cell population detection using a weakly supervised approach and CLL MRD flow cytometry data.

Minimal/measurable residual disease detection is routinely performed as part of post-diagnostic treatment plans for many types of cancer, for which multiparameter flow cytometry is one possible modality frequently used. We propose a machine learning approach for binary prediction of minimal residual disease status with flow cytometry data. Our method involves the projection of cells from the original feature space to a low-dimensional embedding in which cells are clustered by similarity, and the cluster-wise cell proportions are used for prediction as well as regression. This is a weakly super

Hi-C for genome-wide detection of enhancer-hijacking rearrangements in routine lymphoid cancer biopsies.

Standard techniques for detecting genomic rearrangements in formalin-fixed paraffin-embedded (FFPE) biopsies have important limitations. We performed FFPE-compatible Hi-C on 44 clinical biopsies comprising large B cell lymphomas (n = 18), plasma cell neoplasms (n = 14), and other diverse lymphoid cancers, identifying consistent topological differences between malignant B cell and plasma cell states. Hi-C detected expected oncogene rearrangements at high concordance with fluorescence in situ hybridization (FISH) and supported enhancer hijacking in recurrent rearrangements of BCL2, CCND1, and MY

t(14;19)(q32;q13) breakpoints telomeric to BCL3 on 19q13.32 in B-cell lymphomas other than chronic lymphocytic leukemia and long-range structures of der(19)t(14;19) based on optical genome mapping.

t(14;19)(q32;q13) is found in a fraction of chronic lymphocytic leukemia (CLL) patients and creates the IGH::BCL3 fusion gene, but this translocation has been observed in B-cell lymphomas other than CLL (non-CLL BLs). Ultra-high molecular weight DNA from liquid nitrogen-frozen leukemia cells of one CLL patient and OCT compound-embedded cryopreserved biopsies from two non-CLL BL patients, all of whom carried cytogenetic t(14;19), were subjected to optical genome mapping. In the CLL patient, t(14;19) resulted in fusion between IGHA1 and BCL3, whereas in the non-CLL BL patients, breakpoints on 19

Survival disparities in Philadelphia chromosome-positive vs. Philadelphia chromosome-negative B-cell acute lymphoblastic leukemia in the era of modern therapeutic approaches: a decade-long surveillance, epidemiology, and end results (SEER) data based investigation (2010-2021).

Philadelphia chromosome (Ph) status is a critical prognostic marker in B-cell acute lymphoblastic leukemia (B-ALL). This study evaluates the impact of Ph-positive (Ph+) and Ph-negative (Ph-) status on overall survival (OS) and cancer-specific survival (CSS) while analyzing the role of demographic and treatment variables. A retrospective cohort study involving 14,175 patients diagnosed with B-ALL from 2010 to 2021 was analyzed in this investigation. Primary outcomes were OS and CSS, analyzed using hazard ratios (HRs) with 95% confidence intervals (CIs) and associated p-values. Subgroup analysis

Cost-Effectiveness of Venetoclax-Based Treatment in Treatment-Naïve Fit Patients With CLL Without TP53 Aberrations.

The GAIA-CLL13 trial showed that venetoclax-obinutuzumab (Ven-O) based regimens, with or without ibrutinib, offer superior efficacy compared to chemoimmunotherapy (CIT) with regards to progression free survival (PFS) in fit, treatment-naïve (TN) CLL patients. However, their higher costs warrant a cost-effectiveness evaluation. This study assessed the cost-effectiveness of Ven-O versus venetoclax with rituximab (Ven-R), venetoclax-obinutuzumab-ibrutinib (Ven-O-I), and CIT in fit, TN CLL patients without TP53 aberrations across the Netherlands, Norway, and Denmark. Improving acute lymphoblastic leukemia diagnosis through CBAM-enhanced VGG19 deep learning.

Acute Lymphoblastic Leukemia (ALL) is an aggressive blood cancer that requires rapid and accurate diagnosis. Manual review of blood and bone marrow smears is time-consuming and observer-dependent, underscoring the need for automated methods. This study presents a deep learning framework with attention mechanisms for automated detection and subtyping of ALL from microscopic bone marrow images, including healthy samples. The model combines a Convolutional Block Attention Module (CBAM) with a VGG19 backbone, forming a hybrid CBAM-VGG19 network that hierarchically enhances key morphological featur

When Targeted Therapy Falls Short: Unraveling Resistance Mechanisms in Chronic Lymphocytic Leukemia.

PURPOSE OF REVIEW: Targeted therapies have revolutionized the treatment of chronic lymphocytic leukemia (CLL), however the disease remains incurable. This is largely due to somatic mutations in proteins targeted by these therapies, like Bruton’s tyrosine kinase (BTK) and B cell lymphoma-2 (Bcl-2), or transcriptional rewiring of CLL cells. Here we review recent findings regarding mechanisms of resistance to targeted agents in CLL. RECENT FINDINGS: BTK inhibitor (BTKi) resistant CLL is potentiated by mutations disrupting covalent and non-covalent BTKi binding or those conferring loss of BTK’s ki

A Novel Modified Bu/Vp16/cy/Flu/Ara-C Conditioning Regimen Enhances Outcomes for High-Risk Acute Lymphoblastic Leukemia Patients Undergoing Allogeneic Hematopoietic Stem Cell Transplantation.

PURPOSE: Allogeneic hematopoietic stem cell transplantation (allo-HSCT) effectively treats high-risk acute lymphoblastic leukemia (ALL), yet challenges persist due to post-transplant relapse and conditioning regimen toxicities. The determination of an appropriate preconditioning regimen is critical to improving patient outcomes. In our transplant center, we have modified the traditional busulfan (Bu)/cyclophosphamide (Cy)/etoposide (Vp16) protocol by adding fludarabine (Flu) and cytarabine (Ara-C), while reducing the dosage of Cy. This novel modification seeks to enhance transplantation outcom
