Extramedullary T-Lymphoblastic Blast Crisis in Chronic Myeloid Leukaemia: A Unique Cytologic Diagnosis Confirmed by Flow Cytometry.

BACKGROUND: Extramedullary blast proliferation (EBP) occurs in approximately 4%-16% of patients with chronic myeloid leukaemia (CML) in blast phase and may precede overt marrow blast transformation. Although myeloid lineage predominates, lymphoid blast crises account for approximately 20%-30% of cases, with T-lineage differentiation representing only a small minority. Accurate recognition is important because EBP defines blast-phase disease irrespective of the bone marrow blast percentage. CASE PRESENTATION: A 40-year-old man presented with generalized lymphadenopathy, hepatosplenomegaly, ana

Extreme conformal prediction: Reliable intervals for high-impact events.

BACKGROUND: Prehospital (ambulance) care can reduce morbidity and mortality from trauma. Yet, there is a dearth of effective evidence-based interventions and implementation strategies. Emergency Medical Services Traumatic Shock Care (EMS-TruShoC) is a novel bundle of five core evidence-based trauma care interventions. High-Efficiency EMS Training (HEET) is an innovative training and sensitization program conducted during clinical shifts in ambulances. We assess the feasibility of implementing EMS-TruShoC using the HEET strategy, and feasibility of assessing implementation and clinical outcomes

TMEPAI Confers Paclitaxel Resistance in Triple-Negative Breast Cancer Cells by Promoting AKT Phosphorylation and Its Downstream Cascade.

OBJECTIVE: TMEPAI (transmembrane prostate androgen-induced protein) is one of the proteins associated with the resistance of triple-negative breast cancer (TNBC) to various cytotoxic medicines. However, it has remained uncertain how TMEPAI mechanistically contributes to TNBC resistance to paclitaxel. Thus, this study aimed to investigate the effect and possible mechanism of TMEPAI gene editing via CRISPR-Cas9 on the response of triple-negative breast cancer cells to paclitaxel. METHODS: The present study was conducted on wild-type triple-negative breast cancer cells (BT-549) and BT-549 cells

How I individualize frontline treatment for chronic-phase CML.

Chronic myeloid leukemia (CML) has served as a paradigm for the development of effective initial and next-generation targeted therapies. The availability of 5 effective and generally well-tolerated BCR::ABL1 tyrosine kinase inhibitors for the treatment of newly diagnosed chronic-phase CML offers patients and their treating physicians a welcome luxury of choice. The long-term outlook for patients with newly diagnosed chronic-phase CML is excellent, with expected survival similar to age-matched controls. However, most patients are expected to require lifelong treatment. As a result, important co

How I approach hematopoietic stem cell transplantation for CML in a TKI world.

After the introduction of tyrosine kinase inhibitors (TKIs), the number of patients undergoing allogeneic hematopoietic stem cell transplantation (allo-HSCT) for chronic phase chronic myeloid leukemia (CP-CML) has dramatically decreased. Imatinib was the first TKI introduced into the clinical arena, predominantly used in the first-line setting. In cases of insufficient response, resistance, or intolerance, patients with CML can subsequently be treated with a second-, third-, or fourth-generation TKI. However, despite the approval of first-, second-, third-, and fourth-generation TKIs, allo-HSC

How I evaluate and treat resistance and relapse in CML.

As evidenced by the excellent survival outcomes, chronic myeloid leukemia treatment in the era of tyrosine kinase inhibitors (TKIs) is often successful. However, when response milestones are not met or lost, treatment decision-making may be challenging. The availability for first-, second-, or subsequent-line use of 6 different TKIs, each with definite and often nonoverlapping features in terms of mechanism of action, potency, activity against resistance mutations, and tolerability profile provides a reassuring opportunity to rescue an optimal response, but it must be exploited carefully to av

How I manage chronic myeloid leukemia during pregnancy.

Chronic myeloid leukemia (CML) represents a paradigm of success in targeted therapy, with tyrosine kinase inhibitors (TKIs) revolutionizing patient outcomes. This progress has extended to the management of pregnancy in women with CML, a complex scenario requiring a balance between disease control and fetal safety. Because TKIs are contraindicated during the first trimester due to their teratogenic potential, treatment must be stopped as soon as pregnancy is confirmed, necessitating careful preconception planning and alternative management strategies. This article uses illustrative clinical cas

Insights from single-cell omics: cellular heterogeneity as a foundation of clinical outcome in chronic myeloid leukemia.

The BCR::ABL1 tyrosine kinase inhibitors (TKI) in chronic myeloid leukemia (CML) represent a paradigm for molecularly targeted therapy. However, clinical outcomes (rate/depth of response, treatment-free remission [TFR], progression to blast crisis [BC]) and adverse events vary among patients. While additional somatic mutations have been invoked to explain varying clinical outcomes, we here propose a complementary perspective based on single-cell omics (sc-omics) approaches that have enabled unprecedented resolution of the cellular ecosystems, including their composition, interactions, and acti

How I treat advanced phases of CML.

Advanced phases of Philadelphia chromosome-positive chronic myeloid leukemia (CML), classically encompassing de novo presentation of accelerated-phase disease and blast-phase disease (myeloid and lymphoid "blast crisis") and progression to these from antecedent chronic-phase disease, have diminished in incidence but remain a challenge. Despite continued development of additional and novel kinase inhibitors of BCR::ABL1, global limitations on access to diagnostics and therapy persist and account for continued incidence of advanced disease at initial presentation and progressive disease. Asciminib demonstrates superior efficacy and safety in newly diagnosed chronic myeloid leukemia in the ASC4FIRST trial.

Many patients receiving frontline tyrosine kinase inhibitors (TKIs) for chronic-phase chronic myeloid leukemia (CML-CP) experience inadequate disease control and/or adverse events (AEs) that impair quality of life. Treatments offering optimal efficacy, safety, and tolerability will support long-term therapy. In the primary analysis from the ASC4FIRST trial, a phase 3 randomized trial comparing asciminib with investigator-selected TKIs (IS-TKIs) in newly diagnosed CML-CP, asciminib demonstrated superior efficacy vs all IS-TKIs and vs imatinib in the imatinib stratum, meeting both primary object

Caffeic acid phenethyl ester induced apoptosis in chronic myeloid leukemia cells by inhibiting mitochondrial complex I.

Chronic myeloid leukemia (CML) is an oncogenic hematologic disorder defined by the BCR-ABL1 fusion gene, which initiates pathological proliferation of myeloid lineage cells. While tyrosine kinase inhibitors (TKIs) have substantially improved clinical outcomes, therapeutic resistance associated with the T315I mutation continues to present a significant obstacle. This research evaluates the efficacy of caffeic acid phenethyl ester (CAPE), a bioactive natural product, in countering TKI resistance mediated by this specific genetic alteration. The investigation employed a comprehensive methodology

Predictors of Response to Imatinib Therapy and Long-Term Outcomes in Paediatric and Adolescent Patients With Chronic Myeloid Leukaemia.

BACKGROUND: Chronic myeloid leukaemia (CML) is an infrequent myeloproliferative neoplasm in the paediatric population as compared to adults. Despite vast progress in understanding the disease biology and therapy of CML-chronic phase (CML CP), the applicability of risk scoring systems in practice and prognostic factors in children remain grey areas. Hence, we tried to analyse disease characteristics, molecular response to frontline imatinib therapy and its clinical predictors along with long-term outcomes in our population. MATERIALS AND METHODS: In this study, we retrospectively analysed 104

Specifically targeting the ABL myristoyl pocket: STAMP inhibitors for chronic myeloid leukemia.

INTRODUCTION: Chronic myeloid leukemia (CML) has been transformed by ATP-competitive BCR:ABL1 tyrosine kinase inhibitors (TKIs); however, resistance, intolerance, and long-term toxicity remain clinically relevant challenges, particularly in patients requiring prolonged therapy or multiple treatment lines. Asciminib, the first-in-class Specifically Targeting the ABL Myristoyl Pocket (STAMP) inhibitor, represents a paradigm shift by restoring physiologic ABL1 autoinhibition through allosteric binding. AREAS COVERED: This Special Report reviews the mechanistic basis, preclinical development, and

Quantitative reverse transcription PCR coupled with high-resolution melting for simultaneous detection and quantification of four isoforms of BCR::ABL1.

AIMS: The diagnosis and therapeutic decision-making in chronic myeloid leukaemia (CML) depend on the detection and quantification of BCR::ABL1. Currently, no clinical assay is used in routine practice to simultaneously quantify multiple BCR::ABL1 isoforms, requiring separate tests for each isoform. To address this challenge, we have developed and optimised an assay (BloodHound) that leverages quantitative reverse transcription PCR (RT-qPCR) and high-resolution melting (HRM) technologies to simultaneously detect and quantify four BCR::ABL1 isoforms (p190, p210, p230 and p203), with a limit of d

Real-World Outcomes With Low-Dose Dasatinib (50 mg) in Imatinib-Resistant Chronic Myeloid Leukemia in Chronic Phase: A Retrospective Analysis of Efficacy and Safety.

BACKGROUND: Dasatinib, a potent second-generation tyrosine kinase inhibitor (TKI), is highly effective in chronic myeloid leukemia in chronic phase (CML-CP) resistant to imatinib at standard dosing (100 mg daily), but is often limited by adverse events. Emerging evidence suggests low-dose dasatinib (50 mg daily) may maintain efficacy with improved safety, but data in imatinib-resistant CML-CP remain limited. AIMS: To evaluate the efficacy and safety of low-dose dasatinib (50 mg daily) in patients with imatinib-resistant CML-CP and to identify predictors of treatment response and disease progr

A selenium-containing selective estrogen receptor modulator to overcome drug resistance of chronic myeloid leukemia.

Chemotherapy failure caused by adriamycin (ADM) and imatinib (IM) resistance remains a critical challenge in the treatment of chronic myeloid leukemia (CML). In this study, a novel compound 4, 4’-(selenophene-2, 5-diyl)bis(3-fluorophenol) (Se-1) with estrogen receptor regulation and selenium anticancer activity was applied to reverse drug resistance of CML. Se-1 exhibited superior inhibitory activity against resistant K562/ADM cells compared to sensitive K562 cells. The growth of K562/ADM in xenograft mouse was suppressed by Se-1 treatment. The anti-leukemia mechanism of Se-1 was tested by wes
