The goal of the "Spot On CML" program is to provide diagnostic and monitoring tests to chronic myeloid leukemia (CML) patients in low- and middle-income countries (LMICs). Previously, we demonstrated reproducible BCR::ABL1 transcript quantification using dried blood spots (DBS). We have now optimized methods of DNA and RNA extraction from DBS, allowing the detection of myeloid gene variants, including ABL1 tyrosine kinase domain mutations. Among 177 CML patients from nine countries, ABL1 mutations were identified in 61 (34%) patients, with multiple mutations present in some cases. The most com

This manuscript presents the development of an electrochemical biosensor designed to detect K-562 chronic myeloid leukemia (CML) cells. The biosensor was made of highly oriented pyrolytic graphite (HOPG), functionalized with -OH and -COOH groups by surface etching with strong acids, and subsequently coated with modified titanium dioxide (TiO2-m). TiO2-m is TiO2 modified during its synthesis process using carbon nanotubes functionalized with -OH and -COOH groups. These changes improve the electron transfer kinetics and physicochemical properties of the electrode surface. TiO2-m improves the sen

Sensorineural hearing loss (SNHL) is an underrecognized manifestation in chronic myeloid leukemia (CML), particularly in the setting of hyperleukocytosis. Its prevalence, pathophysiology, and response to tyrosine kinase inhibitor (TKI) therapy remain poorly characterized. To assess the prevalence, severity, and reversibility of SNHL in treatment-naïve CML patients and evaluate associations with leukocyte count and disease risk. In this prospective study, 102 newly diagnosed CML patients with total leucocyte count (TLC) > 50 × 109/L underwent audiological evaluation at baseline, 3 months, and 6

Circular RNAs (circRNAs) are a novel class of RNA transcripts that regulate important cellular functions in health and disease. In this study, we report on the functional relevance of a protein-coding circRNA, circPCMTD1, in breakpoint cluster region protein-ABL proto-oncogene 1 (BCR/ABL1)-positive myeloid leukemias. In screening experiments, we found that circPCMTD1 depletion strongly inhibited the proliferative capacity of leukemic cells with BCR/ABL1 translocations. RNA sequencing and mass cytometry experiments identified aberrant activation of the DNA damage response (DDR) pathway as a dow

BACKGROUND: Chronic myeloid leukemia (CML) is a myeloproliferative neoplasm typically defined by BCR::ABL1 p210 fusions isoforms.
OBSERVATIONS: An 8-year-old female CML patient harbored a typical t(9;22)(q34;11) Philadelphia chromosome and BCR::ABL1 fusion by conventional karyotyping and FISH but lacked BCR::ABL1 p210 fusion transcripts by RT-PCR. Targeted next-generation sequencing (NGS) revealed BCR::SPECC1L::ABL1 fusion transcripts predicted to encode an in-frame BCR-exon-8::SPECC1L-exon-4::ABL1-exon-2 fusion protein retaining the tyrosine kinase domain. FISH and NGS were used for therapeu

INTRODUCTION: Nilotinib has been shown to be highly effective for the treatment of chronic myeloid leukemia (CML) and advancing patients to treatment-free remission (TFR). However, its efficacy and safety may be compromised by nonadherence due to its twice-daily fasting requirement. A reduced-dose nilotinib tablet may address these limitations.
AREAS COVERED: PubMed search (1966 to July 2026) for papers related to the pathology, etiology, and treatment of CML, pivotal clinical trials and real-world studies for tyrosine kinase inhibitors (TKI) focusing on nilotinib, and recent pharmacokinetic

Risk stratification to predict treatment failure in chronic-phase chronic myeloid leukemia (CP-CML) remains an important challenge in the tyrosine kinase inhibitor (TKI) era. We conducted a pooled analysis of 710 newly diagnosed CP-CML patients enrolled in the prospective JALSG CML212 and New TARGET studies, approximately 80% of whom received second-generation TKIs (2G-TKIs) as first-line therapy, to identify predictors of failure-free survival (FFS). In this study, we incorporated the ELN 2025-defined "Warning" category as an initial adverse state in the definition of FFS events and evaluated

Ligase chain reaction (LCR) is a promising nucleic acid amplification technique widely applied in molecular diagnosis. However, a critical limitation of LCR is target-independent amplification, primarily caused by the blunt-end ligation activity of thermophilic DNA ligases, which results in false positives and compromises sensitivity. To overcome this challenge, we developed a thermostable flap endonuclease 1 (FEN1)-aided LCR (FALCR) employing DNA probes with non-phosphorylated sticky ends. In each amplification cycle, FEN1 selectively cleaves the 5' DNA flap of the downstream probe in the pre

A 34-year-old man underwent unrelated bone marrow transplantation (BMT) for blast-phase chronic myeloid leukemia (CML). Molecular remission was achieved on day 29. Around day 600 after BMT, the patient developed blurred vision in the right eye. Peripheral blood major mRNA was undetectable on day 606. An ophthalmologic examination on day 613 identified circumferential iris thickening and anterior chamber hypopyon-like lesions in the right eye. Betamethasone and levofloxacin ophthalmic solutions were started on the same day. On day 624, the white lesion in the anterior chamber had resolved, but

Fusion genes are a series of typical tumor biomarkers that can induce dysregulated gene expression and generate oncogenic proteins, both of which contribute to malignant transformation. Consequently, their detection is crucial for early cancer diagnosis, treatment selection, and prognostic evaluation. However, the existing fusion gene detection techniques remain constrained by time-consuming protocols, labor-intensive sample processing, and dependence on sophisticated instrumentation. To overcome these challenges, we present a rapid and portable photothermal biosensing platform utilizing DNA s

OBJECTIVE: Chronic myeloid leukemia (CML) is characterized by the translocation of chromosomes 9 and 22, resulting in the BCR-ABL fusion gene. Tyrosine kinase inhibitors (TKIs) have markedly improved CML outcomes. Yet, resistance may develop due to mutations in the BCR-ABL kinase domain, particularly in the ATP-binding loop (P-loop), activation loop (A-loop), catalytic domain, and direct binding site. This study aimed to evaluate the relationship between BCR-ABL kinase domain mutations, hematological response, and overall survival among CML patients receiving TKI therapy at Wahidin Sudirohusod

Data and guidelines for a second attempt at treatment-free remission (TFR) in patients with chronic myeloid leukemia in the chronic phase (CML-CP) are limited. We retrospectively analyzed the clinical data of six patients with CML-CP who attempted a second discontinuation of tyrosine kinase inhibitors (TKIs) for TFR at Peking University People's Hospital from October 2006 to October 2024. Before the first TKI discontinuation, all six patients had received imatinib treatment for a median of 82.5 months (range, 40-87 months) and maintained a sustained deep molecular response, specifically MR(4.5

BACKGROUND AND OBJECTIVES: The efficacy of asciminib was proven in newly diagnosed (first-line, 1L) patients with Philadelphia-positive chronic myeloid leukemia in chronic phase (Ph+ CML-CP) and in patients treated with at least two prior tyrosine kinase inhibitors (third-line, 3L+). Given that no randomized controlled trial has been conducted for second-line (2L) patients with CML, this analysis aims to infer the efficacy of asciminib in the 2L setting using the 80 mg once-daily dosing regimen and to support the use of asciminib in patients with CML-CP irrespective of line of therapy.

The clinical implementation of BCR::ABL1 tyrosine kinase inhibitors (TKI) for the treatment of chronic myeloid leukemia (CML) represents one of the big successes of mechanism-based cancer therapy. In 2025, the survival of patients who start TKI therapy while in the chronic phase is approaching that of age-matched controls. Despite this paradigm shift, significant challenges remain. Some patients still develop overt TKI resistance and progress to blast phase, and the majority continue to harbor residual leukemia and require life-long TKI therapy. Growth and survival signals arising from the mic

Resistance to tyrosine kinase inhibitors (TKIs) remains a major challenge in breakpoint cluster region (BCR)::Abelson 1 (ABL1)-driven leukaemias. Asciminib offers a novel therapeutic option; however, resistance continues to emerge. We hypothesised that targeting proteostasis and epigenetic regulation with bortezomib and panobinostat could eliminate TKI-refractory cells via TKI-independent mechanisms. We profiled parental and TKI-resistant chronic myelogenous leukaemia (CML) and Ba/F3 models. Viability, cytotoxicity, and caspase-3/7 activity were assessed following single-agent treatment with a

Promyelocytic blast crisis (BC) of CML is an extremely rare event, with only seven cases described as arising during therapy with TKIs. We present a 68-year-old male who developed promyelocytic blast crisis 12 months after CML diagnosis and start of Imatinib therapy, confirmed by the concomitant presence of the t(15;17) and t(9;22) translocations in the leukemic cells. Molecular remission for the PML-RARA clone was achieved with standard acute promyelocytic leukemia induction therapy with ATRA and idarubicin. However, BCR-ABL showed resistance to first-line Dasatinib and second-line Ponatinib,

The discovery of biologically active compounds from plants is an emerging challenge in natural pharmaceutical development. Here, we report the comprehensive strategy for undescribed compounds discovery targeting p53 pathway from Hypericum lancasteri against chronic myeloid leukemia (CML). The UHPLC-Q-TOF-MS analysis of H. lancasteri obtained 44 compounds information in positive and negative ion modes. Besides, three new compounds (1-3) together with two known derivatives (4 and 5) were obtained and identified by further phytochemical research. The dichloromethane extraction part (DEP) of H. la

Currently there is no established prognostic scoring system for patients with chronic myeloid leukemia (CML) in blast phase (BP). This study aimed to identify prognostic factors of CML-BP in a large cohort of prospectively and retrospectively collected patients and to develop a readily available prognostic scoring system at the onset of BP to enable future comparison between different trials and series. The analyses were based on 275 patients from thirteen countries, collected within the European LeukemiaNet Blast Phase Registry with a median observation time of 45 months and a median OS of 18

Chronic myeloid leukemia (CML) is driven by the BCR-ABL fusion oncogene and progresses from chronic phase (CP) to blast phase (BP). While tyrosine kinase inhibitors (TKIs) effectively control CML-CP, CML-BP remains a therapeutic challenge characterized by treatment resistance and poor survival. Accumulating evidence indicates that aberrant activation of epigenetic regulatory elements remodels the transcriptome in cancer, creating dependencies on specific transcriptional regulators that drive cancer progression. However, whether specific transcriptional mechanisms promote the transition from CM
