Natural Killer Cells Significantly Target and Eliminate Chronic Myelogenous Leukemia via Direct Cytotoxicity or Antibody-Dependent Cellular Cytotoxicity.

This study was conducted to investigate whether natural killer (NK) cells can kill and/or inhibit the tumor growth of hematological tumors. When K562, the chronic myeloid leukemia cell line, was treated with the culture media collected from interleukin-2 (IL-2) and anti-CD16 mAbs-treated NK cells, increased surface expression levels of differentiation antigens MHC-class I, CD54, and PD-L1 were observed on K562. This finding validated our previous reports demonstrating that the NK cells induce tumor differentiation via their secreted factors. Next, we investigated NK cell-mediated cytotoxicity

Hsa_circ_0029325 drives imatinib resistance via the CDCA4/AKT axis in chronic myeloid leukemia.

BACKGROUND: Imatinib (IM) resistance poses a major unresolved clinical challenge in chronic myeloid leukemia (CML) treatment, significantly compromising patient prognosis. This study aims to elucidate the role of cell division cycle-associated protein 4 (CDCA4) in IM resistance and the underlying molecular mechanisms in CML. METHODS AND RESULTS: Pan-cancer analysis revealed that high CDCA4 expression was significantly associated with shorter overall survival in multiple cancer types. Quantitative real-time PCR demonstrated upregulated CDCA4 expression in both peripheral blood samples from CML

Tyrosine kinase inhibitor use and risk of neurodegenerative disease: A Swedish population-based cohort study.

INTRODUCTION: Effective disease-modifying therapies remain unavailable for neurodegenerative diseases (NDDs). Tyrosine kinase inhibitors (TKIs) indicated for chronic myeloid leukemia (CML) have been hypothesized to modulate the pathological mechanisms underlying neurodegeneration. Therefore, we aimed to explore the association between initiation of six different TKIs and the subsequent risk of developing NDDs in Swedish nationwide registries (2005-2024). METHODS: CML patients ≥50 years at diagnosis initiating TKI therapy were identified, alongside five comparators without CML matched on sex,

A Peripheral Blood Circular Ribonucleic Acid-Based Nomogram for Risk Stratification in Chronic Myeloid Leukemia.

BACKGROUND: Current prognostic tools for chronic myeloid leukemia (CML) rely primarily on clinical parameters and lack reliable molecular biomarkers for dynamic risk assessment. Circular RNAs (circRNAs) have emerged as promising liquid biopsy biomarkers because of their stability in peripheral blood. However, their prognostic value in CML remains underexplored. METHODS: In this retrospective cohort study, peripheral blood samples were collected from 417 patients newly diagnosed with CML and 45 healthy controls. Differentially expressed circRNAs were identified by high-throughput sequencing an

Pharmacokinetic profiling of imatinib in relation to CYP3A4 activity in leukaemia patients.

Imatinib pharmacokinetics exhibit large interindividual variability because of differences in CYP3A4 activity-the main imatinib-metabolizing enzyme. While therapeutic drug monitoring is effective, it requires steady-state conditions and frequent sampling. The endogenous biomarker 4β-hydroxycholesterol (4-β-OHC) has been proposed to reflect CYP3A4 activity, enabling proactive dose optimization. This study investigated 4-β-OHC as a CYP3A4 biomarker in leukaemia patients and its relationship with imatinib pharmacokinetics METHODS: A pharmacokinetic study included 19 patients with chronic myeloid

Diabetic Patients With Newly Diagnosed Chronic Myeloid Leukemia: TKI Choice and Early Adverse Events. A Real-Life Survey Within the Italian Chronic Myeloid Leukemia Network.

Limited data are available regarding patients with newly diagnosed chronic myeloid leukemia (CML) and concomitant diabetes mellitus (DM). Hence, we retrospectively analyzed 2149 patients with newly diagnosed chronic phase (CP) CML to compare clinical characteristics, choice of frontline therapy, early adverse events, and molecular responses between patients with or without DM. Among this cohort of 2149 patients, 241 patients (11.2%) had DM at CML diagnosis. Diabetic patients were older (median age 69.6 vs. 58.2 years, p < 0.001), showed a higher Sokal/ELTS risk and had more comorbidities, resu

Impact of Tyrosine Kinase Inhibitors on Estimated Glomerular Filtration Rate in Chronic Myeloid Leukemia Patients.

INTRODUCTION: Tyrosine kinase inhibitors (TKIs) are the cornerstone of therapy for chronic myeloid leukemia (CML). Although nephrotoxicity has been reported, existing evidence remains inconsistent, with limited data from the Gulf region. This study aimed to evaluate the impact of TKIs on renal function among patients with CML treated at Hamad Medical Corporation (HMC), Qatar. METHODS: This retrospective, single-center study included adults with CML who received a TKI between 2016 and 2023. The primary outcome was the change in estimated glomerular filtration rate (eGFR) over time. Secondary o

Prices and prescribing of competing cancer medicines in Dutch hospitals after imatinib loss of exclusivity: a retrospective claims analysis.

BACKGROUND: When the first product in a therapeutic class loses market exclusivity, generic entry reduces price and spending for the off-patent medicine. In classes with close substitutes, a lower-priced benchmark may affect the value of remaining on-patent alternatives. Whether this leads to price convergence or prescribing shifts remains sparsely documented. OBJECTIVE: To describe spending and utilisation of competing tyrosine kinase inhibitors (TKI) in Dutch hospitals following loss-of-exclusivity (LoE) of imatinib, the first-in-class drug to treat chronic myeloid leukaemia. METHODS: We a

Recent Advances in Chronic Myeloid Leukaemia (CML): Emerging Molecular and Immunotherapeutic Targets.

Chronic myeloid leukaemia (CML), a distinct myeloproliferative neoplasm, results from the malignant reprogramming of haematopoietic stem cells (HSCs) into leukaemia stem cells (LSCs), primarily driven by breakpoint cluster region and Abelson (BCR-ABL1) fusion oncogene. Although the advent of tyrosine kinase inhibitors (TKIs) has significantly transformed clinical management, leading to improved patient survival and quality of life, sustained treatment-free remission (TFR) remains achievable in only a minority of patients. This limitation is largely attributable to the persistence of LSCs and t

Extramedullary T-Lymphoblastic Blast Crisis in Chronic Myeloid Leukaemia: A Unique Cytologic Diagnosis Confirmed by Flow Cytometry.

BACKGROUND: Extramedullary blast proliferation (EBP) occurs in approximately 4%-16% of patients with chronic myeloid leukaemia (CML) in blast phase and may precede overt marrow blast transformation. Although myeloid lineage predominates, lymphoid blast crises account for approximately 20%-30% of cases, with T-lineage differentiation representing only a small minority. Accurate recognition is important because EBP defines blast-phase disease irrespective of the bone marrow blast percentage. CASE PRESENTATION: A 40-year-old man presented with generalized lymphadenopathy, hepatosplenomegaly, ana

TMEPAI Confers Paclitaxel Resistance in Triple-Negative Breast Cancer Cells by Promoting AKT Phosphorylation and Its Downstream Cascade.

OBJECTIVE: TMEPAI (transmembrane prostate androgen-induced protein) is one of the proteins associated with the resistance of triple-negative breast cancer (TNBC) to various cytotoxic medicines. However, it has remained uncertain how TMEPAI mechanistically contributes to TNBC resistance to paclitaxel. Thus, this study aimed to investigate the effect and possible mechanism of TMEPAI gene editing via CRISPR-Cas9 on the response of triple-negative breast cancer cells to paclitaxel. METHODS: The present study was conducted on wild-type triple-negative breast cancer cells (BT-549) and BT-549 cells

How I individualize frontline treatment for chronic-phase CML.

Chronic myeloid leukemia (CML) has served as a paradigm for the development of effective initial and next-generation targeted therapies. The availability of 5 effective and generally well-tolerated BCR::ABL1 tyrosine kinase inhibitors for the treatment of newly diagnosed chronic-phase CML offers patients and their treating physicians a welcome luxury of choice. The long-term outlook for patients with newly diagnosed chronic-phase CML is excellent, with expected survival similar to age-matched controls. However, most patients are expected to require lifelong treatment. As a result, important co

How I approach hematopoietic stem cell transplantation for CML in a TKI world.

After the introduction of tyrosine kinase inhibitors (TKIs), the number of patients undergoing allogeneic hematopoietic stem cell transplantation (allo-HSCT) for chronic phase chronic myeloid leukemia (CP-CML) has dramatically decreased. Imatinib was the first TKI introduced into the clinical arena, predominantly used in the first-line setting. In cases of insufficient response, resistance, or intolerance, patients with CML can subsequently be treated with a second-, third-, or fourth-generation TKI. However, despite the approval of first-, second-, third-, and fourth-generation TKIs, allo-HSC

How I evaluate and treat resistance and relapse in CML.

As evidenced by the excellent survival outcomes, chronic myeloid leukemia treatment in the era of tyrosine kinase inhibitors (TKIs) is often successful. However, when response milestones are not met or lost, treatment decision-making may be challenging. The availability for first-, second-, or subsequent-line use of 6 different TKIs, each with definite and often nonoverlapping features in terms of mechanism of action, potency, activity against resistance mutations, and tolerability profile provides a reassuring opportunity to rescue an optimal response, but it must be exploited carefully to av

How I manage chronic myeloid leukemia during pregnancy.

Chronic myeloid leukemia (CML) represents a paradigm of success in targeted therapy, with tyrosine kinase inhibitors (TKIs) revolutionizing patient outcomes. This progress has extended to the management of pregnancy in women with CML, a complex scenario requiring a balance between disease control and fetal safety. Because TKIs are contraindicated during the first trimester due to their teratogenic potential, treatment must be stopped as soon as pregnancy is confirmed, necessitating careful preconception planning and alternative management strategies. This article uses illustrative clinical cas
