Radiotherapy in Cervical Myeloid Sarcoma: A Case Report and Literature Discussion.

BACKGROUND: Myeloid sarcoma (MS) is a rare extramedullary tumor formed by the abnormal proliferation of immature myeloid cells. It can occur concurrently with or secondary to various myeloid neoplasms, most commonly acute myeloid leukemia (AML). Cervical involvement is extremely rare, and its clinical presentation often mimics that of primary gynecologic malignancies, typically manifesting as vaginal bleeding, a palpable cervical mass, or cervical enlargement. CASE PRESENTATION: We report the case of a 67-year-old woman with a history of AML who had received multiple lines of chemotherapy and

Extramedullary AML: Clinical and Molecular Features.

BACKGROUND: Acute myeloid leukemia with extramedullary disease (EMD-AML) represents a distinct clinical entity associated with diagnostic and therapeutic challenges, and its prognostic significance remains uncertain. METHODS: A retrospective study of 617 adults with newly diagnosed AML (2005-2018) was conducted, analyzing 246 patients with EMD-AML and 371 without EMD involvement. The clinical characteristics and treatment outcomes were analyzed. Propensity score matching (PSM) was applied to adjust for baseline confounders. RESULTS: Patients with isolated EMD-AML and those with concurrent bo

Leukemia Cutis in Acute Myeloid Leukemia: A Case Report.

Leukemia cutis is an uncommon extramedullary manifestation of acute myeloid leukemia (AML) caused by infiltration of the skin by leukemic blasts. Its clinical morphology is highly variable and may mimic infectious, inflammatory, or other neoplastic dermatoses, making tissue diagnosis essential. We report a 49-year-old man who presented with fever, epistaxis, and multifocal nodular and ulcerated skin lesions involving the right wrist, chest, and scalp, as well as gingival hyperplasia. Skin biopsy was reported to confirm cutaneous myeloid sarcoma, consistent with leukemia cutis in the setting of

RAS Pathway Activation and Microenvironmental Adaptation as Hallmarks of Myeloid Sarcoma.

UNLABELLED: Myeloid sarcoma, an aggressive extramedullary subtype of acute myeloid leukemia (AML), occurs in approximately 20% of patients and remains strikingly understudied in large-scale genomic and multiomic investigations. The key drivers of its tumor evolution are largely unknown; timely detection in asymptomatic patients poses a clinical challenge, and effective treatment options are limited, as patients are often excluded from clinical trials, rendering it a largely neglected disease entity. In this study, we demonstrate that myeloid sarcoma evolves from medullary AML but exhibits dist

Brugada Phenocopy Associated With the Tyrosine Kinase Inhibitor Avapritinib During Febrile Illness.

BACKGROUND: Brugada phenocopy is a reversible Brugada pattern on an electrocardiogram (ECG) caused by identifiable clinical conditions (such as fever, electrolyte abnormalities, or drug exposure) in patients without true congenital Brugada syndrome, which is an inherited arrhythmogenic disorder often associated with SCN5A mutations. CASE SUMMARY: A 53-year-old man with metastatic myeloid sarcoma presented with high-grade fever and chest pain. He was previously treated with imatinib, discontinued because of severe skin rash, followed by dasatinib, and most recently avapritinib, initiated 3 wee

Primary isolated lumbar spine myeloid sarcoma: a rare case report and review of the literature.

Myeloid sarcoma is a rare extramedullary malignancy of immature myeloid cells that is most commonly associated with acute myeloid leukemia, while primary isolated spinal involvement without systemic disease is exceedingly uncommon and diagnostically challenging. We report a rare case of isolated lumbar myeloid sarcoma in a 39-year-old non-leukemic man who presented with an eight-month history of low back pain and radiculopathy, normal neurological and laboratory findings, and imaging suggestive of a benign nerve sheath tumor. Surgical decompression and resection were performed, and histopathol

Case Report: Iliac perivascular myeloid sarcoma presenting with iliofemoral deep vein thrombosis and hydronephrosis.

Myeloid sarcoma (MS) is a rare extramedullary tumor of myeloid blasts that may occur before, concurrently with, or after acute myeloid leukemia (AML) (1-4). We report a 53-year-old man with progressive left lower-limb pain and swelling, initially admitted to the vascular surgery service, with imaging revealing left external iliac vein and proximal common femoral vein occlusive thrombosis, a peri-iliac mass encasing the iliac vessels, and ipsilateral hydronephrosis. Biopsy of the left inguinal lesion showed MPO positivity, CD34 positivity, partial TdT positivity, and a Ki-67 index of about 80%,

Identification of an NFIA::CBFA2T3 fusion in cerebrospinal fluid confirms the diagnosis of pediatric CNS myeloid sarcoma with erythroid differentiation: a case report and literature review.

Myeloid sarcoma with erythroid differentiation represents a mass-forming presentation of acute erythroid leukemia. This entity is exceptionally rare in the pediatric population, with only sporadic reports of de novo cases predominantly involving the central nervous system or orbit. Diagnosing myeloid sarcoma with erythroid differentiation poses significant clinical and pathological challenges, particularly in cases without bone marrow involvement. Here, we report the case of a 1-year-old boy with myeloid sarcoma with erythroid differentiation exhibiting diffuse parenchymal brain infiltration w

Rare and complex three-way t(8;11;21) translocation in core-binding factor acute myeloid leukemia transforming into refractory mediastinal myeloid sarcoma: cytogenetic and molecular insights.

Core-binding factor (CBF) acute myeloid leukemia (AML) with t(8;21)(q22;q22)/RUNX1::RUNX1T1 is typically considered as a favorable-risk AML in the context of cytarabine-based intensive chemotherapy. However, in some situations such as additional adverse-risk mutations or cytogenetics, the prognosis and disease course may be more uncertain. Here, we report the case of a young patient diagnosed with CBF-AML and RUNX1::RUNX1T1 fusion gene, carrying a rare and complex three-way t(8;11;21)(q22;q13;q22) translocation, with mutated KIT, ASXL1 and TET2 genes, transforming into an aggressive and multi-

Myeloid sarcoma of the septum pellucidum causing hallucinations and personality changes in an elderly woman: a case report.

Myeloid sarcoma is an extramedullary tumor composed of myeloid blasts, most commonly involving the skin, lymph nodes, and soft tissues. We report a rare case of myeloid sarcoma of the central nervous system (CNS) causing neuropsychiatric symptoms in an elderly woman with a history of JAK2-positive polycythemia vera and acute myeloid leukemia (AML) in long-term remission. The patient presented with a one-month history of complex visual hallucinations, short-term memory loss, and personality changes. Magnetic resonance imaging revealed a 2.4-cm enhancing mass in the septum pellucidum extending b

Absent Cyclin D1 Expression in Myeloid Sarcomas Distinguishes From Malignant Histiocytic Neoplasms: When Morphologic Ambiguity is Deceptive.

Myeloid sarcoma (MS) is a tumor mass involving any extramedullary site composed of myeloid-lineage blasts with myeloid, myelomonocytic, or monocytic differentiation. Distinguishing MS from malignant histiocytic neoplasms (MHNs) is diagnostically challenging due to morphologic and immunophenotypic overlap, particularly for MS with monocytic differentiation, which can mimic MHNs, especially the subtypes of histiocytic sarcoma and interdigitating dendritic cell sarcoma, leading to potential misdiagnosis and inappropriate management. Cyclin D1 overexpression is implicated in MHNs, but its utility

Allogeneic hematopoietic stem cell transplantation with liposomal mitoxantrone conditioning for extramedullary leukemia.

Leukemia relapse after allogeneic hematopoietic stem cell transplantation (allo-HSCT) remains the leading death cause, with extramedullary disease history as an independent relapse risk factor in acute leukemia. We analyzed 10 patients with extramedullary disease history who received allo-HSCT with mitoxantrone liposome (Lipo-MIT)-containing conditioning at our center (Dec 2022-Jan 2025). Median age was 39.5 (14-63), 7 had acute myeloid leukemia (AML, including myeloid sarcoma), 2 T lymphoblastic lymphoma/leukemia, and 1 B-cell acute lymphoblastic leukemia. Pre-transplant, 4 were in complete r

Bilateral Myeloid Sarcoma of Breast: A Case Report and Discussion.

Myeloid sarcoma (MS) involving the breast is rare. We reviewed a case of a 21-year-old female with palpable masses in the bilateral breasts without medullary acute myeloid leukemia. Ultrasound revealed irregular, indistinct, and complex hypoechoic masses with internal blood flow and hyperechoic halos in the bilateral breasts. A biopsy was subsequently recommended, and MS was confirmed. Further cytogenetic evaluation of breast biopsy specimens demonstrated positive t(16;16)(p13.1;q22) translocation. The patient was admitted for chemotherapy. Subsequent follow-up breast ultrasound following chem

A tale of two sites - Distinct spatial differences between bone marrow and extramedullary AML.

Acute myeloid leukemia (AML) is the most common acute leukemia in adults and remains associated with poor overall survival. Beyond genetic factors in the bone marrow (BM) that drive clonal evolution, leukemic progression, immune evasion, and therapy resistance, far less is known about extramedullary (EM) AML, a clinically relevant manifestation characterized by leukemic infiltration of peripheral tissues and frequently associated with adverse outcomes. This review focuses on the spatial heterogeneity of AML, differentiating between BM and EM disease, elucidating how niche components shape leuk

Retroperitoneal and Pelvic Myeloid Sarcoma in a Patient With JAK2-Positive Polycythemia Vera: A Case Report.

Myeloid sarcoma (MS) is a rare extramedullary tumor composed of immature myeloid cells that may occur in association with acute myeloid leukemia or other myeloid neoplasms. We report a 75-year-old woman with JAK2-positive polycythemia vera who presented with abdominal pain, distension, and anorexia. Imaging revealed a large retroperitoneal and pelvic mass causing hydronephrosis. Biopsy demonstrated immature mononuclear cells positive for CD33, CD43, CD117, CD68, and CD163, consistent with MS with monocytic differentiation. Other malignancies were excluded, and flow cytometry was non-diagnostic

Myeloid sarcoma: Clinical, Pathologic, and Molecular Findings.

Myeloid sarcoma is defined as a neoplasm of myeloid blasts that involves an extramedullary site as a mass and effaces tissue architecture. Myeloid sarcoma is considered clinically to be equivalent to acute myeloid leukemia (AML) and requires appropriate therapy. As currently defined, extramedullary AML that does not form a mass is inconsistent with the diagnosis of myeloid sarcoma and the management of patients with extramedullary AML is less well defined. Furthermore, the pathogenetic relationship and clinical importance of extramedullary AML versus myeloid sarcoma is under explored. Recent s
