Acute myelomonocytic leukaemia following atypical congenital rubella.

The child of a woman immunised against rubella presented at 5 months with developmental delay and recurrent infection; she was shown to have congenital rubella. At 15 months she developed acute myelomonocytic leukaemia (AMML). Rubella is difficult to diagnose after immunisation. AMML has not been previously described in association with congenital rubella, as far as is known.

Nucleolar organizer regions in acute and chronic leukaemias.

A silver staining technique for nucleolar organizer regions (NORs) has been applied to bone marrow biopsies of various types of acute and chronic leukaemias. This method could be easily evaluated on resin-embedded bone marrow obtained from acute lymphocytic leukaemia (n = 12), acute myelogenous (n = 16), chronic lymphocytic (n = 16) and chronic granulocytic (n = 20) leukaemia. A significant difference (p < or = 0.1) was only found between the AgNOR numbers in nuclei of lymphocytes from acute and chronic leukaemia (mean of 1.23 to 1.40 and 1.58) and those of cells from acute and chronic myeloge

Granulocytic differentiation of the human myelomonocytic leukaemia cell line ME-1 in serum-free culture.

We have recently established ME-1, a human myelomonocytic leukaemia cell line derived from acute myelomonocytic leukaemia with eosinophilia (M4E0). When ME-1 cells were cultured in serum-free medium, they stopped proliferating and began to differentiate morphologically, functionally and phenotypically to mature granulocyte-like cells. The protein kinase inhibitor, 1-(5-isoquinolinyl-sulphonyl)-2-methylpiperazine (H-7) enhanced this differentiation dose-dependently. Upon addition of fetal calf serum (FCS) to the serum-free medium, the differentiation of ME-1 cells into granulocyte-like cells wa

[Cytogenetic marker del(12p) in a case of acute myeloid leukemia M4 with eosinophilia and inv(16)].

A 35-year-old Spanish female having acute myelomonocytic leukaemia with marrow eosinophilia is reported. The karyotype revealed the specific chromosomal abnormality associated with this disorder, inv(16), and a less-specific myelodysplastic cytogenetic marker, del(12p). We have reviewed the published cases in order to elucidate the possible significance of presenting a 12p deletion (nonrandomly associated with chronic myelomonocytic leukaemia) together with other chromosomal rearrangements.

Acute myelomonocytic leukaemia with infiltrative peripheral neuropathy.

The case of a 71-year-old woman with acute myelomonocytic leukaemia and severe pains in her arms, legs and back during treatment is described. Signs of a generalized polyneuropathy subsequently developed. Autopsy revealed massive leukaemic infiltration of peripheral nerves. Peripheral nerve involvement in acute leukaemia appears to be rare as judged from the very few previously reported cases. The condition should be suspected whenever inexplicable pain problems and signs of peripheral neuropathy occur in patients with acute leukaemia.

Granulocytic sarcomas of small intestine and brain are associated with acute myelomonocytic leukaemia with abnormal eosinophils and inversion of chromosome 16.

We report two cases of acute myelomonocytic leukaemia with abnormal eosinophils (M4Eo) in which the presenting feature was small bowel obstruction. We suggest there is a unique clinicopathological association between small intestine involvement with leukaemia and the M4Eo subtype. Central nervous system involvement by myeloblastoma occurred in one of the two cases which is a recognised feature of M4Eo and should necessitate prophylaxis with intrathecal therapy. Inversion of chromosome 16 which is a cytogenetic marker for M4Eo was demonstrable in one of the two cases.

Multiple myeloma and chronic myelomonocytic leukaemia developing in a patient with autoimmune disease.

A 64-year-old lady with a personal and family history of autoimmune disease developed chronic myelomonocytic leukaemia and multiple myeloma simultaneously. Her sister died of acute myelomonocytic leukaemia, but showed no evidence of autoimmune disease. It is possible that chronic immunological stimulation, perhaps by an autoantigen, may predispose toward malignant transformation in both plasma cell and monocyte series. However, the present observations raise the alternative possibility of a primary disorder of monocytes that predisposes toward both autoimmune disease and a clonal disorder of p

The effect of monocytic involvement of hyperaldolasaemia in patients with acute myelogenous leukaemia.

Serum aldolase concentrations were determined in 78 patients with acute myelogenous leukaemia (AML). Mean serum aldolase concentration (+/- SD) at the time of diagnosis was 28.3 U/L (+19.4). 71.8% of AML patients had increased serum aldolase concentrations. The highest hyperaldolasaemia were observed in patients with acute myelomonocytic leukaemia and acute monoblastic leukaemia. The monocytic involvement in AML patients was significantly related to serum aldolasaemia. Human aldolase gene has been located on chromosome 16, whose structural aberrations are frequent in AML patients with monocyti

[Simultaneous presentation of multiple myeloma and acute myelomonocytic leukemia in the absence of previous chemotherapy. Description of a case and review of the literature].

A case of simultaneous occurrence of multiple myeloma and acute myelomonocytic leukaemia without previous chemotherapy is reported. A careful search of the literature disclosed 16 other cases of the same association. The possible relationships between these two haematopoietic neoplasms are discussed. On the literature and personal data, it is concluded that in course of multiple myeloma the association with an acute myeloblastic leukaemia is due to two possibilities: the first, more frequent, is the long-term effect of chemotherapy, while the second, rare, is that, by still unknown causes, a s

Inappropriate diagnosis of necrotizing arachnidism. Watch out Miss Muffet--but don't get paranoid.

A 28-year-old man presented with painful progressive necrosis of the skin of his left leg which was associated with general toxicity and a high fever. A working diagnosis of necrotizing arachnidism was made, but no improvement followed two weeks of therapy with analgesics, antibiotics and antihistamines. His symptoms responded rapidly to prednisolone therapy, and a diagnosis of pyoderma gangrenosum was confirmed by biopsy. This subsequently proved to be associated with early acute myelomonocytic leukaemia.

HRAS1 and INS genes are relocated but not structurally altered as a result of the t(7;11)(p15;p15) in a clone from a patient with acute myeloid leukaemia (M4).

A patient whose leukaemic cells carried the rare t(7;11)(p15;p15) was diagnosed as having acute myelomonocytic leukaemia (AML-M4), and supports the association of this specific translocation with forms of acute myeloid leukaemia showing differentiation. Blast phase chronic myeloid leukaemia was excluded by lack of involvement of the ABL and BCR genes. Chromosome in situ hybridization studies showed that both the HRAS1 and INS genes were present on the terminal part of chromosome 11p which was translocated to chromosome 7p. Neither HRAS1 nor INS were structurally rearranged. Field inversion gel

Diabetes insipidus associated with leukaemia.

Three cases of leukaemia complicated by pituitary diabetes insipidus are presented. In one patient diabetes insipidus developed five months before acute myelomonocytic leukaemia became apparent. The two other patients suffered from acute myelomonocytic leukaemia and from a blastic transformation of chronic myeloid leukaemia. In one case a transient cure of diabetes insipidus was obtained after intrathecal chemotherapy and irradiation of the skull.

Retinoic acid alone and in combination with cytosine arabinoside induces differentiation of human myelomonocytic and monoblastic leukaemic cells.

The effect of retinoic acid (RA) alone and in combination with cytosine arabinoside (Ara-C) on differentiation of fresh human myeloid leukaemic cells from patients with AML was studied. Cells from six patients: three with acute myelomonocytic leukaemia AMMoL and three with acute monoblastic leukaemia AMoL with a percentage of blasts greater than 70, were treated in an in vitro primary suspension culture with retinoic acid (10(-7) M), cytosine arabinoside (100 ng/ml) or both in combination. Non-adherent mononuclear cells were seeded at a concentration of 5 x 10(5) cells/ml in RPMI 1640 culture

Monoclonal antibody (Y1/82A) with specificity towards peripheral blood monocytes and tissue macrophages.

A new monoclonal antibody, Y1/82A, was raised against phytohaemagglutinin activated peripheral blood mononuclear cells. Using an immunohistochemical technique it was shown that Y1/82A reacts against peripheral blood and bone marrow monocytes and resident macrophages from essentially all human tissues. Y1/82A bound to determinants present in leukaemic cells from patients with acute myelomonocytic leukaemia and acute monocytic leukaemia, but not to neoplastic cells from patients with malignant lymphoproliferative disorders or malignant epithelial tumours. Y1/82A failed to react with other cell t

Lysosomal enzyme abnormalities in preleukaemic Sweet's disease: case report.

Quantitative and qualitative abnormalities in marrow lysosomal enzymes, suggestive of acute myeloid leukaemia, were detected in a patient with Sweet's disease and monocytosis 12 months before she presented with acute myelomonocytic leukaemia. Biochemical characterisation of blood and marrow cell extracts may help to identify those patients with Sweet's disease and other preleukaemic conditions who are most at risk of developing leukaemia.

Establishment of three permanent human leukaemia cell lines producing immunoreactive calcitonin.

Three permanent human leukaemia cell lines, designated EW2, LG3 and MS6, were established from bone marrow aspirates of a patient with acute myelomonocytic leukaemia (EW2), acute monocytic leukaemia (LG3) and acute myeloblastic leukaemia (MS 6). In vitro all lines exhibited a myelomonocytoid marker profile in terms of classical staining reactions and cell-surface antigen structure. In supernatants and extracts of EW2 and MS 6 cells immunoreactive human calcitonin was found in raised levels. No significant levels of immunoreactive human calcitonin were found in supernatants of LG3 cells, while

Detection of the Tn antigen in leukaemia using monoclonal anti-Tn antibody and immunohistochemistry.

Exposure of the normally cryptic Tn antigen on haemopoietic cells leading to erythrocyte polyagglutination has been reported in a few cases of malignant or premalignant haemopoietic disorders and has been attributed to a selective deficiency of the enzyme 3-beta-D-galactosyl-transferase. A male patient presented with acute myelomonocytic leukaemia with no evidence of Tn expression but, 16 months later, in the terminal stage of the disease, the majority of the erythrocytes were found to be polyagglutinable. Tn expression was confirmed by the use of lectins and by agglutination with a Tn-specifi
