An increase in basophils in a case of acute myelomonocytic leukaemia associated with marrow eosinophilia and inversion of chromosome 16.

A patient with acute myelomonocytic leukaemia (M4 subtype) with pericentric inversion of chromosome 16, inv(16)(p13q22), and a marked basophilia is described. Results from transmission electron microscopy suggested that the basophils were immature. Clinically, although leukaemic cells had a tendency to infiltrate a number of tissues and organs, such as skin, gingiva, liver and lung, the patient was successfully treated with chemotherapy. The increase in basophils accompanied by marrow eosinophilia may be the result of abnormal differentiation of leukaemic cells that have the capacity to differ

Lymphokine activated killing of fresh human leukaemias.

The relative susceptibility of 10 human leukaemias comprising acute phase leucocytes from 5 acute myeloid and 5 lymphoid neoplasms, and 2 immunoblastic lymphomas to killing by peripheral blood mononuclear cells (PBMC), before and after target cell treatment with phytohaemagglutinin (PHA), and by interleukin-2 (IL-2) activated peripheral blood lymphocytes (PBL) was investigated in short term 51Cr release assays using effector cells from 10 allogeneic donors. Optimal lectin-dependent cellular cytotoxicity (LDCC) was verified against K562 and L1210 cells and lymphokine-activated killing (LAK) aga

Acquired inversions in human leucocytes.

Peri- and paracentric inversions are observed in human leukocytes at various rates. Four categories are proposed, in relation to the frequency of occurrence, although it may vary with time for a same inversion. Category 1 corresponds to isolated, thus non recurrent inversions. Category 2 (f congruent to .001) corresponds to inv(14)(q12qter) and inv(7)(p14q35) in individuals with presumably normal genetic constitution. Category 3 (f congruent to .01) corresponds to inv(7)(p14q35) in patients affected by ataxia telangiectasia (AT). This inversion, when it is frequent, indicates an abnormal genet

Acute myelomonocytic leukaemia and acquired haemophilia A with severe cutaneous haemorrhage.

A case of severe cutaneous haemorrhage due to thrombocytopenia in combination with an acquired haemophilia A is reported. The thrombocytopenia was due to acute myelomonocytic leukaemia (FAB M4). A factor VIII:C specific anticoagulant was also found. Chemotherapy led to complete remission with normal blood counts and coagulation-test results.

Gangliosides in acute myeloid leukaemia (AML) and non-Hodgkin's lymphoma (NHL).

Gangliosides of human acute myelomonocytic leukaemia (AMMoL), acute monocytic leukaemia (AMoL) and lymphoblastoid (pre-B) lymphoma cells were analysed by overpressured thin-layer chromatography (OPTLC) followed by scanning densitometry. AMMoL cells were found to contain four gangliosides, viz. GM3 (47.2%), GM1 (31.8%), GD1a (7.5%) and an unidentified compound migrating between GM1 and GD1a (13.5%). In AMoL cells, six components were identified (GM3:51.3%, GM1:13.4%, GD3:7.8%, GD1a:4.7%, GD1b:6%, and an unidentified compound migrating between GM2 and GM1). The total gangliosides extracted from

Granulocytic sarcoma of the kidney in a patient with acute myelomonocytic leukaemia. A case report.

A 26-year-old man with acute myelomonocytic leukaemia had been in remission for 16 months when he developed a painful swelling of the right testis. Intravenous pyelography showed a non-functioning right kidney. Surgical exploration revealed a granulocytic sarcoma of the right kidney and a right nephrectomy was performed. The patient refused further treatment and relapsed 3 months after the nephrectomy. Autopsy revealed a large greyish-white tumour mass occupying the pelvis and retroperitoneal tissue. Special techniques with immunoperoxidase and naphthol-ASD chloro-acetate esterase staining con

Cytogenetic studies on acute myelomonocytic leukaemia (M4) with eosinophilia.

Cytological and cytogenetic studies on 17 acute myelomonocytic leukemia with bone marrow eosinophilia (M4EO) are reported. Cytological criteria include an unusual high proportion of eosinophilic cells containing abnormal granules. Abnormal karyotypes have been found in 12 patients but chromosome 16 abnormalities were present in only 9. In two of them only one such mitosis was detected whereas in 7 others inv(16)(p13q22) and/or del (16)(q22) clones were present. However in 16 cases normal karyotypes were also present. Other abnormal clones coexisted in three patients, suggesting that chromosome

Acute median nerve compression by haemorrhage from acute myelomonocytic leukaemia.

A 61-year-old male receiving treatment for acute myelomonocytic leukaemia (AMML) developed weakness and sensory loss in the thumb, index and middle finger and half of the ring finger of the left hand as a result of a spontaneous haemorrhage into the carpal tunnel and forearm, an unusual cause of median nerve compression. These findings were confirmed by CT scanning and post-mortem examination. This is the first report of spontaneous haemorrhage occurring in a patient with AMML.

Metallothionein gene cluster is split by chromosome 16 rearrangements in myelomonocytic leukaemia.

The metallothioneins (MTs) are a family of proteins of low relative molecular mass which bind heavy-metal ions. MTs exist in several molecular forms (MT-I, MT-II) and are encoded by a multi-gene family containing at least 14 closely related genes and pseudogenes. These proteins function in the regulation of trace-metal metabolism, the storage of these ions in the liver, and as a protective mechanism against heavy-metal toxicity. Somatic cell hybridization has shown that most MT genes, including the functional MT genes (MT1A, MT1B, MT2A), lie on human chromosome 16. Using in situ hybridization,

Cytochemical, immunological and ANAE-isoenzyme studies in acute myelomonocytic leukaemia: a study of 39 cases.

This study reports the cytochemical, electrophoretic and immunological characteristics of blasts cells from 39 cases of acute myelomonocytic leukaemia (M4). The results indicate considerable cytochemical heterogeneity, particularly with respect to esterase (alpha naphthyl acetate and chloroacetate) activities and suggest that an increased serum lysozyme concentration is a more consistent feature. Investigations with a range of monoclonal antibodies also revealed some differences in expression of monocyte-associated determinants although it is considered that immunological assessments are more

Sequential karyotypic evolutions and bone marrow aplasia preceding acute myelomonocytic transformation from myelodysplastic syndrome.

Serial haematopathological and cytogenetic studies disclosed three distinct clinical phase in a case of refractory anaemia (RA), a subtype of myelodysplastic syndrome (MDS; FAB group, 1982): first, chronic MDS phase (1 year 10 months) with karyotypic abnormality (45, XY, --7) (Clone I); second, hypo-aplastic phase concurrent with first clonal evolution (45, XY, --7, 12p--) (Clone II); third, acute myelomonocytic leukaemia phase (6 months) with second clonal evolution (45, XY, --7,t (1q --; Bq+), Bq --, 12p --) (Clone III). In the second phase the bone marrow became almost aplastic as Clone II

An HLA-DR negative acute leukaemia which stimulates MLC and CMC responses.

We describe an acute myelomonocytic leukaemia (E72) devoid of cell-surface HLA-DR antigens, but capable of inducing cellular responses. Leukaemia E72 induced proliferation of normal lymphocytes in primary mixed lymphocyte culture (MLC), which was only weakly inhibited by anti-DR sera. Depletion of a small percentage (congruent to 4%) of DR+ cells on a cell-sorter failed to abrogate the capacity of E72 to stimulate MLC and cell-mediated cytotoxicity (CMC) responses. We found that normal lymphocytes primed with E72 responded in secondary MLC to lymphocytes and leukaemias, suggesting that the lym

Diabetes insipidus as a complication of acute myelomonocytic leukaemia.

A female patient, aged 44, with diabetes insipidus as a complication of acute myelomonocytic leukaemia (AMML) is described. She presented with bleeding, anaemia, polyuria and polydypsia. She was treated with intranasal vasopressin for diabetes insipidus and responded well to treatment. Chemotherapy was administered for the leukaemia and a full remission was achieved. The patient relapsed a few days before final admission to hospital and died of septicaemia 7 months after initial diagnosis. A short review of the literature related to this subject is also presented.

Stimulatory ability of human leukaemic cells in the allogeneic mixed leucocyte reaction.

Cryopreserved mononuclear cells from 34 patients with a high proportion of blasts in the peripheral blood were tested for their ability to stimulate normal allogeneic lymphocytes in the one-way mixed leucocyte reaction (MLR). Of these, more than half failed to stimulate allogeneic responders, despite the fact that many of them carried Ia-like antigens. In general, acute myeloid leukaemia cells were non-stimulatory whereas acute myelomonocytic leukaemia cells were effective stimulators. Thus, MLR stimulatory ability appears to be a differentiation marker in acute non-lymphocytic leukaemias. Acu
