Isolated Neutropenia as a Sentinel of High-Risk Clonal Evolution: Acute Myeloid Leukemia With Myelodysplasia-Related Changes Harboring TP53 Deletion via Isochromosome 17q and Deletion 20q Mimicking a Myeloproliferative Neoplasm.

Isolated neutropenia is often deemed benign in elderly patients, frequently attributed to age-related marrow changes, medications, or nutritional deficiencies. However, persistent and unexplained neutropenia may signal early clonal hematopoiesis or evolving myeloid malignancy. While acute myeloid leukemia with myelodysplasia-related changes (AML-MRC) usually arises from multilineage cytopenia, presentation with isolated neutropenia and myeloproliferative neoplasia (MPN)-like features is exceedingly rare. This case underscores the importance of early, risk-adapted assessment in such scenarios.

Analysis of Myelodysplastic Neoplasia: Epidemiological and Biological Characteristics Based on 20 Clinical Cases and a Literature Review.

Myelodysplastic neoplasia (MDS) is a hematological malignancy characterized by inefficient hematopoiesis, responsible for quantitative and qualitative abnormalities of the three myeloid cell lineages, classically revealed by peripheral cytopenias and an increased risk of transformation into acute myeloid leukemia (AML). The diagnosis of these conditions is sometimes difficult, complex, and time-consuming, potentially requiring multiple examinations, relying primarily on complete blood count (CBC) data and the results of semi-quantitative and qualitative analysis of the bone marrow aspirate. Therapy-related myelodysplastic syndrome rapidly evolving to acute myeloid leukemia following CAR-T cell therapy in a patient with refractory diffuse large B-cell lymphoma: a case report.

BACKGROUND: Persistent cytopenia following chimeric antigen receptor T-cell (CAR-T) therapy is an increasingly recognized complication in patients with relapsed or refractory diffuse large B-cell lymphoma (DLBCL). While most cases stem from transient, immune-mediated mechanisms or depleted bone marrow reserves, secondary therapy-related myeloid neoplasms (t-MN) must be strongly suspected in patients presenting with prolonged, transfusion-refractory cytopenias. CASE PRESENTATION: We report the case of a 76-year-old woman diagnosed with refractory DLBCL (non-germinal center B-cell subtype, stag

Impact of Systemic Inflammatory and Autoimmune Diseases on Outcomes in Patients With Myelodysplastic Syndrome and Chronic Myelomonocytic Leukemia.

OBJECTIVE: Systemic inflammatory and autoimmune diseases (SIADs) are frequently reported in association with myeloid neoplasms; however, their prognostic relevance in myelodysplastic syndromes (MDS) and chronic myelomonocytic leukemia (CMML) remains unclear. This study aimed to evaluate the frequency of SIADs and their impact on survival outcomes in patients with MDS and CMML. MATERIALS AND METHODS: We retrospectively analyzed patients diagnosed with MDS, CMML, or acute myeloid leukemia transformed from MDS between 2000 and 2020. Medical records were reviewed for the presence of SIADs, demogr

Myelodysplastic Neoplasm with Biallelic TP53 Inactivation in a Well-Controlled HIV Patient.

BACKGROUND: Human immunodeficiency virus (HIV) infection represents a chronic disease that must be treated with lifelong antiretroviral therapy (ART). Individuals with HIV are at increased risk of developing some cancers, including hematolymphoid malignancies. However, knowledge of myelodysplastic neoplasm (MDS) in HIV patients remains limited. Several studies have shown that HIV-positive MDS patients present at a younger age, progress more rapidly to acute myeloid leukemia (AML), and have poorer overall survival. Moreover, these patients have an increased prevalence of high-risk cytogenetic a

Myelodysplastic Neoplasm with Increased Blasts-1 Presenting with Concurrent Diagnostic and Prognostically Relevant Morphologic Features: A Case Report.

INTRODUCTION: Myelodysplastic neoplasms (MDS) are clonal hematopoietic disorders characterized by ineffective hematopoiesis, cytopenias, and morphologic dysplasia, with a variable risk of progression to acute myeloid leukemia. Despite advances in molecular diagnostics, bone marrow morphology remains a cornerstone of diagnosis and risk stratification. CASE PRESENTATION: We report the case of a 72-year-old woman who presented with progressive fatigue and pancytopenia. Bone marrow examination revealed prominent dyserythropoiesis, dysgranulopoiesis, numerous micromegakaryocytes with unilobed nucl

Amendment-driven changes in nutrient availability and forage uptake in a recently converted boreal soil.

Increasing evidence suggests that long noncoding RNAs (lncRNAs) influence the pathogenesis and progression of hepatocellular carcinoma (HCC). The authors of the present study previously reported that abnormal upregulation of lncRNA DQ786243 (lncDQ) was associated with poor prognoses for patients with HCC. However, the elucidation of underlying mechanisms which influenced these results was not completed. Thus, the current study aimed to characterize the mechanisms and functions of lncDQ that facilitate its promotion of HCC progression. lncDQ, miR‑15b‑5p and Wnt3A expression levels were characte

Humanized mouse models in MDS.

Myelodysplastic syndromes (MDS) are heterogeneous hematopoietic stem cell disorders defined by ineffective hematopoiesis, multilineage dysplasia, and risk of progression to acute myeloid leukemia. Improvements have been made to identify recurrent genetic mutations and their functional roles, but translating this into preclinical models is still difficult. Traditional murine systems lack the human-specific cytokine support and microenvironmental support that is necessary to reproduce MDS pathophysiology. Humanized mouse models, particularly those incorporating human cytokines (e.g., MISTRG, NSG

A Heart of Myeloid: Extramedullary Chronic Myelomonocytic Leukemia-2 Presenting as a Myeloid Sarcoma of the Pericardium Causing Recurrent Pericardial Effusions.

Chronic myelomonocytic leukemia (CMML) is a rare neoplasm that has a roughly 15-30% chance of transforming into acute myeloid leukemia (AML). Acute leukemias have been known to cause pleural or pericardial effusions but having CMML transform into AML while presenting as a cardiac myeloid sarcoma causing pericardial effusions makes this case unique. A 59-year-old patient presented to the emergency room with shortness of breath and was found to be in cardiac tamponade requiring urgent care. The thoracic surgery team performed a pericardiectomy and placed a window drain. Cardiac tissue biopsy pro

Donor-Derived SF3B1-Mutated Myelodysplastic Neoplasm/Syndrome.

We report a rare case of donor-derived SF3B1-mutated myelodysplastic syndrome (MDS) arising in a 45-year-old woman following haploidentical allogeneic hematopoietic stem cell transplantation (allo-HSCT) for acute myeloid leukemia. Initial remission was achieved post-induction, and transplantation resulted in full donor chimerism. However, routine post-transplant surveillance revealed a novel SF3B1 K666N mutation, evolving to overt MDS with ring sideroblasts and multilineage dysplasia. Persistent 100% donor chimerism confirmed the donor-derived nature of the neoplasm. The patient's course was c

Unilateral Exophthalmos as the Initial Presentation of Acute Myeloid Leukemia in a Pediatric Patient: A Report of a Rare Case.

Acute myeloid leukemia (AML) is a malignant hematological condition characterized by the uncontrolled proliferation of immature myeloid cells in the bone marrow, which disrupts the normal production of blood cells. Although it is a relatively rare subtype of pediatric acute leukemia, it is a significant cause of leukemia-related mortality in children. Ocular involvement in acute leukemias, though uncommon, can be a revealing sign of the disease, sometimes presenting as either an initial or secondary manifestation, especially during relapse. We report the case of a five-year-old child presentin

Artificial intelligence-based prediction models for acute myeloid leukemia using real-life data: A DATAML registry study.

We designed artificial intelligence-based prediction models (AIPM) using 52 diagnostic variables from 3687 patients included in the DATAML registry treated with intensive chemotherapy (IC, N = 3030) or azacitidine (AZA, N = 657) for an acute myeloid leukemia (AML). A neural network called multilayer perceptron (MLP) achieved a prediction accuracy for overall survival (OS) of 68.5% and 62.1% in the IC and AZA cohorts, respectively. The Boruta algorithm could select the most important variables for prediction without decreasing accuracy. Thirteen features were retained with this algorithm in the

Patterns of lower risk myelodysplastic syndrome progression: factors predicting progression to high-risk myelodysplastic syndrome and acute myeloid leukemia.

The patterns of low-risk myelodysplastic syndrome (MDS) progression and the clinical and molecular features of those patterns have not been well described. We divided our low-risk (LR) MDS patients (N=1,914) into 4 cohorts: 1) patients who remained LR-MDS (LR-LR; N=1,300; 68%), 2) patients who progressed from LR to high-risk (HR) MDS (LR-HR) without transformation into acute myeloid leukemia (AML) (N=317; 16.5%), 3) patients who progressed from LR to HR MDS and then AML (LR-HR-AML; N=124; 6.5%), and 4) patients who progressed from LR MDS directly to AML (LR-AML; N=173; 9%). Acute myeloid leukemia with ETV6::CHIC2 fusion gene: 'Pitfalls' in diagnosis.

OBJECTIVES: Acute myeloid leukemia (AML) with ETV6::CHIC2 and basophilia is rare in hematologic malignancies with poor prognosis. Due to the small number of clinical cases, it is misdiagnosed and missed frequently, and it is necessary to explore laboratory detection for accurate diagnosis. METHODS: We report a case of AML with ETV6::CHIC2 and basophilia by morphological screening, immunotyping with precise gating, interpretation of FISH results, and RNA transcriptome sequencing, thus laying the accurate diagnosis for clinical treatment. RESULTS: We confirmed a rare case of AML with ETV6::CHI

Data-driven, harmonised classification system for myelodysplastic syndromes: a consensus paper from the International Consortium for Myelodysplastic Syndromes.

The WHO and International Consensus Classification 2022 classifications of myelodysplastic syndromes enhance diagnostic precision and refine decision-making processes in these diseases. However, some discrepancies still exist and potentially cause inconsistency in their adoption in a clinical setting. We adopted a data-driven approach to provide a harmonisation between these two classification systems. We investigated the importance of genomic features and their effect on the cluster assignment process to define harmonised entity labels. A panel of expert haematologists, haematopathologists, a

Whole Exome Sequencing of Intermediate-Risk Acute Myeloid Leukemia without Recurrent Genetic Abnormalities Offers Deeper Insights into New Diagnostic Classifications.

Two new diagnostic classifications of acute myeloid leukemia (AML) were published in 2022 to update current knowledge on disease biology. In previous 2017-edition categories of AML with myelodysplasia-related changes, AML was not otherwise specified, but AML with mutated RUNX1 experienced profound changes. We performed whole exome sequencing on a cohort of 69 patients with cytogenetic intermediate-risk AML that belonged to these diagnostic categories to correlate their mutational pattern and copy-number alterations with their new diagnostic distribution. Our results show that 45% of patients c

The spectrum of GATA2 deficiency syndrome.

Inherited or de novo germ line heterozygous mutations in the gene encoding the transcription factor GATA2 lead to its deficiency. This results in a constellation of clinical features including nontuberculous mycobacterial, bacterial, fungal, and human papillomavirus infections, lymphedema, pulmonary alveolar proteinosis, and myelodysplasia. The onset, or even the presence, of disease is highly variable, even in kindreds with the identical mutation in GATA2. The clinical manifestations result from the loss of a multilineage progenitor that gives rise to B lymphocytes, monocytes, natural killer

Isocitrate dehydrogenase 2 (IDH2) mutations occur in more than 15% of cytogenetically normal acute myeloid leukemia (CN-AML) but comparative studies of their roles in leukemogenesis have been scarce. We generated zebrafish models of IDH2R172K and IDH2R140Q AML and reported their pathologic, functional and transcriptomic features and therapeutic responses to target therapies.
