BACKGROUND AND PURPOSE: Neuro-melioidosis is an uncommon but potentially fatal manifestation of Burkholderia pseudomallei infection with diverse and often under-recognized imaging features. Existing imaging literature is limited to isolated reports and small series. We aimed to characterize the craniospinal MRI spectrum of neuro-melioidosis in a multicentre Indian cohort and identify recurrent imaging patterns associated with diagnostic delay.
MATERIALS AND METHODS: In this retrospective multicentre case series, 33 patients with microbiologically confirmed neuro-melioidosis from seven Indian

Melioidosis, caused by Burkholderia pseudomallei, is a severe infectious disease with high mortality. Diagnostic delays due to conventional culture and serology limitations impact patient outcomes. This narrative review synthesizes evidence on next-generation sequencing (NGS) in melioidosis. NGS technologies encompass two main applications: metagenomic NGS (mNGS), which enables culture-independent detection directly from clinical samples, and whole-genome sequencing (WGS), which provides outbreak tracing and source attribution from cultured isolates. Resistance profiling detects antimicrobial

BACKGROUND: The One Health (OH) approach recognizes the interconnectedness of human, animal, and environmental health, particularly in Southeast Asia, where rapid environmental change and intensive human-animal interactions increase the risk of emerging infectious diseases. Wildlife interaction and environmental exposures are important but underexplored pathways for zoonotic disease transmission, especially in rural and ethnically diverse communities. Understanding these interconnected exposures is essential for informing effective disease prevention and control strategies.

BACKGROUND: Pediatric melioidosis is an under-recognised but potentially severe infection caused by Burkholderia pseudomallei. Evidence is largely fragmented across case reports and small series, limiting understanding of disease patterns, severity, and outcomes in children.
METHODS: A scoping review was conducted in accordance with PRISMA-ScR 2020 guidelines. PubMed and Google Scholar were searched up to December 2025 for studies reporting culture-confirmed melioidosis in children (≤18 years). Case reports, case series, and observational studies were included. Given the heterogeneity of avai

OBJECTIVES: Melioidosis, caused by Burkholderia pseudomallei, is increasingly recognized as a travel-associated infection, yet the extent of exposure among international travelers remains unknown. We aimed to determine B. pseudomallei seroprevalence among travelers to Southeast Asia and explore risk factors.
METHODS: We conducted a cross-sectional study at the Hospital for Tropical Diseases, Bangkok, enrolling travelers aged ≥18 years from non-endemic regions who had spent ≥1 month in Southeast Asia. Sera were tested by indirect enzyme-linked immunosorbent assay using two B. pseudomallei anti

Melioidosis is a potentially fatal endemic infection caused by the Gram-negative bacterium Burkholderia pseudomallei. The disease may present with a wide spectrum of clinical manifestations, including pneumonia, visceral abscesses, and septicemia. Neurological complications are uncommon but have been reported, with dural venous sinus thrombosis being a particularly rare manifestation. We describe a case of a 55-year-old Malay woman with underlying diabetes mellitus and hypertension who presented with a one-week history of fever, confusion, back pain, and dyspnoea. Blood cultures grew Burkholde

BACKGROUND: Melioidosis is a tropical infectious disease caused by Burkholderia pseudomallei. Although vascular involvement is rare, melioidosis-associated abdominal aortic pseudoaneurysm can rapidly progress and carries a high risk of rupture and mortality. Due to nonspecific early symptoms and the lack of specific guidelines, the optimal timing of surgery remains unclear.
CASE REPORT: A 62-year-old female farmer from Hainan Province, China, presented with persistent lower abdominal pain for over 10 days, followed by low back pain and fever (maximum 38.0 °C). She was diagnosed with type 2 di

BACKGROUND: Melioidosis, caused by the environmental saprophyte Burkholderia pseudomallei, is acquired from soil and surface water and rises sharply in incidence after heavy rainfall and flooding-something that extreme-weather trends under climate change are expected to make more frequent and more widespread. The disease presents with a broad clinical spectrum, but eschars or ecthyma gangrenosum-like necrotic skin lesions are very rarely reported and may be mistaken for scrub typhus in coendemic regions.
CASE PRESENTATION: A 66-year-old woman from northeast Thailand with no known comorbidity

UNLABELLED: The incidence of melioidosis, a disease caused by Burkholderia pseudomallei, has been increasing in India over the past decade. Cardiac melioidosis is a rare complication of melioidosis, which typically presents as pericarditis, myocarditis, or endocarditis. We report a very rare and probably the first of its kind presentation of cardiac melioidosis as a right atrial mass. A 24-year-old man presented with fever, cough with expectoration, and right-sided multiple joint pain. Burkholderia pseudomallei was isolated from blood cultures, and culture-specific antibiotic treatment was sta

Effective control of tuberculosis, nontuberculous mycobacterial (NTM) disease, and melioidosis depends on rapid and accurate laboratory diagnosis. Mycobacterium tuberculosis complex (MTBC) remains a health priority, whereas NTM infections, particularly Mycobacterium avium complex (MAC), are increasingly recognized and may be misdiagnosed as tuberculosis. Melioidosis, caused by Burkholderia pseudomallei, is underdiagnosed in endemic regions. These infections have overlapping clinical presentations, and conventional diagnostics methods may not accurately differentiate among these diseases. We de

Hepatic melioidosis is a clinically significant manifestation of Burkholderia pseudomallei infection, increasingly recognised across South and Southeast Asia. It primarily affects diabetic and immunocompromised individuals and is characterised by a subacute to chronic presentation that often mimics other causes of liver abscess. The pathogenesis of hepatic melioidosis reflects the interplay between bacterial virulence mechanisms and impaired host immunity, leading to intracellular persistence and necrotising hepatic lesions. Characteristic imaging features, including the honeycomb and necklace

BACKGROUND: Sequential treatment of ankylosing spondylitis (AS) that includes tumour necrosis factor-α antagonists (anti-TNF agents) has been applied in most of the Western countries. Existing cost-effectiveness (CE) models almost exclusively presented the incremental CE of anti-TNF agents using a closed cohort while budget impact studies are mainly lacking. Notwithstanding, information on impact on total population health and societal budget as well as on actual incremental CE for a given decision time span are important for decision makers. This study aimed at quantifying, for different deci

Paracetamol (acetaminophen) poisoning remains the most common exposure reported to US poison information centres and the leading cause of poisoning-related fatalities, despite the availability of an effective antidote, acetylcysteine. Oral acetylcysteine solution has been approved for the management of acetaminophen poisoning in the US for four decades. Until the recent approval of intravenous acetylcysteine in the US, it was necessary to compound the oral solution for intravenous administration. The effectiveness and tolerability of oral and intravenous acetylcysteine for the prevention of he

A successful DNA vaccine for the treatment of tumors should break established immune tolerance to tumor antigen. However, due to the relatively low immunogenicity of DNA vaccines, compared to other kinds of vaccines using live virus or protein, a recombinant viral vector was used to enhance humoral and cellular immunity. In the current study, we sought to develop a novel anti-cancer agent as a complex of DNA and oncolytic adenovirus for the treatment of malignant melanoma in the C57BL/6 mouse model. MART1, a human melanoma-specific tumor antigen, was used to induce an increased immune reaction

BACKGROUND: Melioidosis is an infectious disease caused by Burkholderia pseudomallei. In infected mice, IFN-γ can provide protection against B. pseudomallei infection. Invariant Natural Killer T (iNKT) cells are a subpopulation of T lymphocytes, activated by recognition of glycolipid ligands such as α-Galactosylceramide presented by CD1d, produce and secrete several cytokines, including IFN-γ and IL-4. The response of iNKT cells in human melioidosis was then investigated.
OBJECTIVE: To determine the iNKT cells response in human melioidosis.
METHODS: The number of human iNKT cells and its act

BACKGROUND: NLRP12 has been shown to play an essential role as a negative regulator in several bacterial infection.
OBJECTIVE: The purpose of this study is to elucidate the role of NLRP12 in B. pseudomallei-infected RAW264.7 macrophages.
METHODS: The protein expression and the level of TNF-α production were determined by immunoblotting and ELISA assay, respectively.
RESULTS: The results demonstrated that unlike the LPS-mutant strain which lacks O antigenic polysaccharide, the wild-type B. pseudomallei was able to upregulate NLRP12 protein expression in RAW264.7 macrophages. NLRP12 expressio

BACKGROUND: The overlapping clinical and radiographic features of pulmonary melioidosis and lung cancer present diagnostic challenges to healthcare providers in endemic settings.
METHODS: We compared the clinical, laboratory and imaging characteristics of 19 pulmonary melioidosis cases with those of 15 cases of small cell lung cancer (SCLC) and 17 cases of non-small cell lung cancer (NSCLC).
RESULTS: Compared with SCLC/NSCLC cases, those with pulmonary melioidosis were more likely to have diabetes, have fever, neutrophilia and leukocytosis on presentation (p<0.05). While certain CT imaging f

INTRODUCTION: The tunnel sign (linear contrast enhancement along the corticospinal tract) can aid in the early diagnosis of neuromelioidosis, a potentially fatal condition requiring urgent diagnosis.
METHODOLOGY: We retrospectively analyzed the hospital data of all patients with culture-confirmed melioidosis between 2018 and 2023 in a tertiary care hospital in South India. MRI scans of patients with neuromelioidosis were reviewed for the presence of the tunnel sign.
RESULTS: Of the 177 patients with melioidosis, 21 had possible neurological involvement. Seven had confirmed neuromelioidosis.
