Burkholderia pseudomallei in Sarawak, Malaysian Borneo, Remains Highly Susceptible to Trimethoprim-Sulfamethoxazole Despite Resistance to Its Individual Components.

Burkholderia pseudomallei, the causative agent of melioidosis, is endemic in Sarawak, Malaysian Borneo, where it is represented by a unique gentamicin-susceptible population. Despite trimethoprim-sulfamethoxazole (co-trimoxazole) being the cornerstone of eradication therapy, emerging reports of elevated minimum inhibitory concentrations (MICs) among Sarawak isolates have raised concerns over its clinical efficacy. We performed a retrospective and comprehensive antibiotic susceptibility assessment of clinical B. pseudomallei isolates from hospitals across Sarawak. Susceptibility to trimethoprim

Burkholderia pseudomallei quorum sensing molecule 3-hydroxy-C10 HSL, triggers organelle stress and inflammatory responses in A549 cell line.

Burkholderia pseudomallei, the causative agent of melioidosis, is a recognised bioterrorism threat. This microorganism produces a key quorum molecule, 3-Hydroxy-C10 homoserine lactone (3-OH-C10 HSL), which has shown to modulate host immune responses. This study investigated the impact of 3-Hydroxy-C10 HSL on A549 cell line, with a focus on organelle stress and inflammatory responses. Treatment with 3-Hydroxy-C10 HSL (100 μM, 2 h) induces a significant elevation of cytosolic calcium and endoplasmic reticulum (ER) stress, evidenced by BiP upregulation and activation of the PERK-CHOP axis, indica

Genomic diversity and clade clustering of Burkholderia pseudomallei and B. thailandensis prophages with soil-derived phages.

Most studies on bacteriophages (phages) of the Gram-negative bacterium Burkholderia pseudomallei rely on in silico predictions and thus underestimate the true diversity of phages. Analysis of the whole genome sequences of culturable prophages induced from B. pseudomallei and B. thailandensis, along with their free Burkholderia phages isolated from soils in Thailand, identified six novel groups of Burkholderia phages, surpassing in silico expectations. The analysis also indicated that soil-dwelling phages may have originated from lysogenic B. pseudomallei strains. Free phages isolated from soil

Rapid diagnosis of skin and soft tissue melioidosis in children.

Melioidosis is an endemic infection caused by Burkholderia pseudomallei, found in tropical and subtropical regions. In resource-limited settings, culture-based diagnostics are often slow, delaying appropriate treatment, or unavailable. We conducted an interventional ambispective cohort study to assess the impact and diagnostic accuracy of the Active Melioidosis Detect Plus rapid diagnostic test (AMD-RDT) in Cambodian children with suspected skin and soft tissue melioidosis (SST-M). The retrospective cohort (July 2022-July 2023) received standard diagnostics; the prospective cohort (July 2023-D

Epidemiology, Management, and Outcomes of Patients Hospitalized With Community-Acquired Infection in a Resource-Limited Setting in Southeast Asia: A Prospective Observational Study.

BACKGROUND: In many resource-limited settings, hospitalization for community-acquired infection is common, but data regarding illness severity, etiology, and morbidity remain sparse. METHODS: We conducted a prospective observational study from May 2022 to August 2023 at 2 hospitals in northeast Thailand. Adults hospitalized with community-acquired infection were enrolled within 24 hours of admission and followed up to 28 days. We identified patients meeting sepsis criteria and assessed related epidemiology, management, and mortality risk factors. RESULTS: Of 1445 patients screened, 940 were

Melioidosis in sickle cell hemoglobinopathies in eastern India: Cavitating pneumonia and immune susceptibility - a case series-based insight.

Melioidosis, caused by Burkholderia pseudomallei, is an emerging infection in the tropics. Most of the melioidosis patients have underlying co-morbidities or occupational exposures. Hemoglobinopathies, such as thalassemia, are established risk factors for melioidosis; however, the association of SCD with melioidosis is infrequently reported. We report two culture-confirmed melioidosis cases from eastern India: a 58-year-old male with SCD on hydroxyurea and another 44-year-old with sickle cell trait, both presenting with cavitary lung lesions mimicking tuberculosis. SCD-related functional asple

Melioidosis in India: A systematic review of individual cases.

OBJECTIVES: Melioidosis is increasingly recognized in India, but available evidence remains fragmented across small reports. We aimed to synthesize national case-based data to describe the clinical spectrum and identify factors associated with mortality. METHODS: We systematically searched PubMed, Embase, and Web of Science from inception to February 5, 2025. Case reports, case series, and observational cohorts with individual patient-level culture- or molecular-confirmed data from India were included. Two reviewers independently screened studies, extracted data using a standardized form, and

Expanding the molecular epidemiology of melioidosis in North Central Vietnam.

Cases of melioidosis have been recorded for many years in Vietnam though it is still not a nationally reportable disease in Vietnam. More research is needed to understand the disease ecology and public health impacts of melioidosis in the country. To this aim, broadening the knowledge base of strains and epidemiology of infections in relation to genotypes present in the soil reservoir can tell us about the propensity of Burkholderia pseudomallei genotypes to transmit from soil to humans. Thirty-five clinical B. pseudomallei isolates, ten from soil, one from swine, and one from a bear were coll

Environmental Presence of Burkholderia pseudomallei at Military Sites in an Endemic Region: Implications for Future Deployments and Studies.

The footprint of melioidosis is expanding globally, but its historical roots are in Southeast Asia and northern Australia. Melioidosis has long been described in military personnel deployed to melioidosis-endemic regions; however, the magnitude of the risk has not been quantified, and the nature of infecting events remains speculative. As with infections in the endemic population, the greatest concern is for inhalational melioidosis. Soil and air at four military locations in the highly melioidosis-prevalent environment of Darwin, Northern Territory, Australia, were sampled. Burkholderia pseud

In-Silico Characterisation of Burkholderia pseudomallei K96243 Pathogenic Islands: Unveiling Novel Targets for Therapeutic Development.

Burkholderia pseudomallei is a deadly bacterium responsible for melioidosis, which is challenging to treat because of its antibiotic resistance and ability to evade the immune response. This study is focused on in silico analysis to identify novel drug targets. The B. pseudomallei K96243 strain, we have identified seven pathogenicity islands with 138 genes. Subsequent filtering based on criteria such as the data present, essentiality, lack of human homology, and uniqueness narrowed this to 24 promising targets, with eight top candidates. These include proteins involved in energy production (ct

Successful treatment of pulmonary melioidosis in an Australian dog.

A 4-year-old female spayed Siberian Husky developed dyspnoea and lethargy during a flood event affecting southeast Queensland in 2022. Initial assessments revealed pyrexia, reduced respiratory function markers, neutrophilia, thrombocytopaenia and consolidation of the left caudal lung lobes. Despite several days of supportive care and broad-spectrum antimicrobials, the dog remained pyrexic, then deteriorated on day 5, developing left-sided pneumothorax. Computed tomography (CT) confirmed left caudal lung lobe consolidation with multiple gas foci, partial left cranial lung lobe consolidation and

Atypical Presentations in Melioidosis: A Case-Based Review from Endemic Regions.

BACKGROUND: Melioidosis, caused by Burkholderia pseudomallei, is a severe and often underdiagnosed infection endemic to South Asia, Southeast Asia, and northern Australia. While pneumonia and sepsis are the classical presentations, the disease is increasingly recognized for its diverse and atypical clinical manifestations. OBJECTIVE: The objective is to improve diagnostic accuracy and increase clinical awareness in both endemic and non-endemic settings by reviewing and classifying atypical presentations of melioidosis that have been documented in the literature. METHODS: A narrative, case-ba

Innovative microparticle strategies: harnessing essential oil-loaded lecithin/chitosan systems against Burkholderia pseudomallei.

Burkholderia pseudomallei, the causative agent of melioidosis, presents high antimicrobial resistance, largely due to its biofilm-forming ability. Here, innovative lecithin/chitosan microparticles loaded with lemongrass (LEO) and geranium (GEO) essential oils (LEOLCN and GEOLCN) were developed and characterized as a novel strategy to combat B. pseudomallei. Microparticles were characterized and evaluated for their effects on planktonic growth and biofilm inhibition, as well as for potential synergistic interactions with antibiotics. Microparticles exhibited a spherical morphology, high encapsu
