Phage-based diagnostic assay for melioidosis using multiplex PCR targeting Burkholderia phage terminase genes.

Conventional diagnostic approaches involve identifying the bacterial pathogen from patient samples, which can delay results and miss low-bacteremia cases. Herein, we demonstrate that Burkholderia phages circulating in patient blood can serve as a powerful diagnostic biomarker. Building on this, a multiplex PCR targeting the Burkholderia pseudomallei phage terminase gene was developed to improve melioidosis diagnosis. The assay provided 97.2% sensitivity and 100% specificity, with results obtainable within 6 h. The strength of this PCR assay is the amplification of four different multicopy Burk

The IrlS2-IrlR2 two-component system is a global regulator of biofilm formation, stress adaptation, and virulence in Burkholderia pseudomallei.

UNLABELLED: Burkholderia pseudomallei, which causes melioidosis, is an adaptable pathogen that survives in diverse environments. Two-component systems (TCSs) play pivotal roles in bacterial signal transduction and adaptation, yet the functions of most TCSs in B. pseudomallei remain poorly characterized. Here, we identified and functionally characterized a previously unstudied TCS, IrlS2-IrlR2, which shares partial homology with the IrlS-IrlR system but exhibits distinct regulatory roles. Knockout of irlR2 (ΔirlR2) did not affect growth in nutrient-rich medium but led to enhanced biofilm format

Emergence of Human and Animal Melioidosis in Southern Africa, 2018-2021.

Melioidosis is increasingly recognised in tropical and subtropical regions worldwide as a serious and potentially fatal bacterial infection affecting humans and animals, acquired from the environment. Until now, human cases of melioidosis had not been reported in Southern Africa. Over a four-year period, we identified three human and two animal cases of melioidosis in South Africa and Namibia. Burkholderia pseudomallei isolates were investigated by matrix-assisted laser desorption/ionisation time-of-flight mass spectrometry (MALDI-TOF MS) and whole-genome sequencing (WGS). Phylogenetic analysi

Diagnosing melioidosis and tracking treatment outcomes using breath.

Melioidosis is a life-threatening infectious disease caused byBurkholderia pseudomallei(Bp). Rapid diagnosis and appropriate antimicrobial treatment are critical to reduce mortality, yet diagnosis is hindered by diverse clinical manifestations, mimicry with other diseases, and reliance on slow culture-based methods. Detecting volatile compounds offers a non-invasive approach for rapid infection detection. In this study, we aim to identify volatile compounds in patients' breath that can aid in diagnosing melioidosis and indicating response to treatment. Breath samples were collected from 17 pat

Emergence of distinct Burkholderia pseudomallei lineages in Brazil: Phenotypic, genotypic, and phylogenetic agreement.

Burkholderia pseudomallei is a Gram-negative facultative intracellular bacterium that causes melioidosis, a neglected tropical disease that spreads beyond traditional endemic regions. This study examined B. pseudomallei isolates obtained from five melioidosis cases in Piauí, Brazil, using phenotypic, molecular, and genomic methods. Isolates were identified as B. pseudomallei using VITEK 2 system and MALDI-TOF mass spectrometry. Conventional PCR targeting the orf11, burk17, and burk475 gene regions confirmed species-level identification, with sequences showing 98.78-100% identity to GenBank B.

Association between HbA1c Levels and Clinical Outcomes in Patients with Diabetes Mellitus and Melioidosis.

Diabetes mellitus (DM) is the major clinical risk factor for melioidosis, though the effect of improved glycemic control on clinical outcomes is unknown. We investigated whether glycemic control affects clinical outcomes by analyzing a composite outcome of death or intensive care unit admission among patients from the Darwin Prospective Melioidosis Study, stratified by their glycosylated hemoglobin (HbA1c). A total of 292 patients with melioidosis and DM were identified with mean HbA1c of 10.2% (95% CI: 9.8-10.4). The composite outcome occurred in 74 (24.3%) patients, but it was not associated

Neuromelioidosis causing rapidly progressing neurological syndromes mimicking stroke and demyelination: Insights from a case series.

Melioidosis (Whitmores disease) is caused by Burkholderia pseudomallei, a Gram-negative saprophytic bacterium. In addition to systemic involvement, this neurotropic pathogen can invade the meninges, cranial nerves, brain, and spinal cord, resulting in diverse neurological manifestations. We describe four cases of neuromelioidosis encountered at our urban tertiary institute over a one-year period. All presented with rapidly progressive neurological symptoms mimicking stroke and acute disseminated encephalomyelitis (ADEM). The case series included three males and one female, aged between 32 and

Global animal melioidosis prevalence: a systematic review and meta-analysis.

BACKGROUND: Burkholderia pseudomallei, the causative agent of melioidosis, infects both humans and a broad range of animal species. While human disease has been well characterized, animal data are dispersed across studies and lack a comprehensive global synthesis. This systematic review and meta-analysis estimated the pooled global prevalence of animal melioidosis and examined regional and diagnostic influences. METHODS: Following PRISMA 2020 guidelines, studies reporting B. pseudomallei detection in animals were retrieved from PubMed, Scopus, and Embase up to April 2025. Pooled prevalence wa

Whispers of decay: necrotic mediastinal lymphadenopathy due to melioidosis in a diabetic patient.

We report a case of melioidosis presenting as isolated necrotic mediastinal lymphadenopathy in a 60-year-old man from coastal Karnataka with poorly controlled diabetes (HbA1c 9.9%). Such a presentation, closely mimicking tuberculosis, is rarely described in the literature. Dual microbiological confirmation through sputum culture and endobronchial ultrasound (EBUS) guided lymph node aspiration established the diagnosis of Burkholderia pseudomallei. This case underscores the importance of considering melioidosis in the differential diagnosis of necrotic mediastinal lymphadenopathy, particularly

Combined use of methyl gallate and N-acyl homoserine lactonase YtnP to inhibit biofilm formation in Burkholderia thailandensis.

Biofilm formation in Burkholderia thailandensis, a model for the causative agent of melioidosis, is critically regulated by N-acyl homoserine lactone (AHL)-mediated quorum sensing (QS), contributing to its pathogenicity and treatment failure. This study investigates a novel combinatorial strategy to disrupt this process by employing methyl gallate, a plant-derived biofilm inhibitor, together with the AHL-lactonase YtnP. Our results demonstrate that methyl gallate not only attenuated B. thailandensis virulence in a Galleria mellonella infection model but also showed a markedly slower developmen

Development of an antigen detection test kit (Melioidosis-ATK) for point-of-care diagnosis of melioidosis.

Melioidosis is a fatal infectious disease caused by the environmental bacterium Burkholderia pseudomallei. Although it is highly endemic in tropical regions, melioidosis remains underdiagnosed. The disease presents with diverse clinical manifestations, but diagnosis relies on time-consuming bacterial culture. Many patients with poor outcomes are referred from community hospitals, where culture facilities are unavailable. Early and accurate diagnosis is critical for timely treatment and improved outcomes. We developed an antigen detection test kit (Melioidosis-ATK) that targets the 6-deoxy-hept

Sulforaphane Pre-Treatment Improves Alveolar Macrophage Killing After Alcohol-Induced Phagocytic Dysfunction In Vitro and in Galleria mellonella Larvae.

Background: Alcohol is associated with increased mortality and morbidity globally. Pulmonary infections with opportunistic pathogens can occur in healthy humans; however, binge alcohol intoxication (≥0.08% BAC) is a major risk factor. We have previously shown that a single dose of alcohol comparable to binge alcohol intoxication increases infection by reducing alveolar macrophage function in vivo. Sulforaphane (SFN), a phytonutrient, is a potent inducer of antioxidant production through the induction of nuclear factor erythroid 2-related factor 2 (Nrf2) and inhibition of the nuclear factor kap

Genetic Engineering of Lysogenic-Lytic Switch Genes Improves Burkholderia Phage Killing Efficacy.

Burkholderia pseudomallei, the causative agent of melioidosis, presents significant challenges in both treatment and environmental decontamination. Bacteriophages, or phages, are increasingly being explored as potential diagnostic, therapeutic, and biocontrol agents against this bacterial pathogen. Our recent investigation has shown that most B. pseudomallei genomes contained prophage(s) associated with specific tRNA gene loci, prompting us to explore these detectable prophages as sources of temperate phages for further applications. Rainfall-Driven Mobilisation of Clinically Relevant Burkholderia pseudomallei in a Groundwater-Connected Urban Creek, Northern Australia.

Burkholderia pseudomallei is a saprophytic environmental bacterium and the causative agent of melioidosis, a serious opportunistic infection in tropical regions, including northern Australia. Infection occurs following environmental exposure via percutaneous inoculation, ingestion, or inhalation; however, the environmental reservoirs and transmission pathways responsible for human disease remain poorly defined. Groundwater has been implicated as a potential source of infection, but the factors influencing the persistence and mobility of B. pseudomallei in surface waters in North Queensland are

Shotgun metagenome sequencing and informatics can accurately form a metagenome-assembled genome (MAG) of the bacterial tier 1 select agent Burkholderia pseudomallei for rapid public health response events.

Shotgun metagenomics, when sufficient read depth exists for each taxon, enables capturing metagenome-assembled genomes (MAGs) directly from a microbial community. In 2021, an aromatherapy spray contaminated with Burkholderia pseudomallei caused an outbreak of melioidosis in the United States. Metagenome-assembled genome binning depends in part on different nucleotide compositions, and the contaminated aromatherapy spray contained other bacteria, including related species (e.g., Burkholderia cepacia, Burkholderia cenocepacia, Burkholderia multivorans, Burkholderia pseudomultivorans, Cupriavidus
