A case of chronic myelomonocytic related lysozyme-induced nephropathy.

BACKGROUND: Lysozyme-induced nephropathy (LyN) is a rare cause of acute kidney injury (AKI) in patients with chronic myelomonocytic leukemia (CMML). Diagnosing LyN is challenging due to the numerous potential and concurrent aetiologies. Previous case reports have demonstrated the diagnostic utility of serum and urinary lysozyme measurements, which may correlate with renal dysfunction; however, the role of renal imaging remains unclear and renal biopsy remains essential for diagnosis. CASE PRESENTATION: An independent 80-year-old male presented to an Australian tertiary hospital with stage 3 A

Chronic myelomonocytic leukaemia.

Chronic myelomonocytic leukaemia (CMML) is a morphological hybrid between myelodysplastic and myeloproliferative neoplasms and is characterized by clonal monocytosis. Myeloid neoplasm-associated somatic mutations are present in >90% of patients with CMML but are not diagnostically specific but a subset are prognostically relevant. Current drug therapy in CMML is mostly palliative and not disease-modifying. Allogeneic haematopoietic stem cell transplantation is a potentially curative treatment modality in CMML and its timing is determined by formal prognostic models including the recently devel

Comorbidities and mutations including single- and multihit TET2 mutations in relation to outcome in chronic myelomonocytic leukaemia-A population-based study.

To explore the relation between disease characteristics, comorbidities, mutations and overall survival (OS) in chronic myelomonocytic leukaemia (CMML), we collected data from a population-based cohort of 149 consecutive patients. TET2 mutation (TET2MT) was associated with higher haemoglobin, less leucocytosis and longer OS compared to no TET2MT (TET2WT), despite patients being significantly older. Patients with multihit TET2MT had the most favourable outcome (HR 0.55, CI 0.35-0.88, p < 0.05). Multihit TET2MT was associated with lower lactate dehydrogenase and less monocytosis, indicating multi

Alternative 5-Azacitidine 5-Day 100 mg/m2 Dosage Shows Non-Inferiority to Classical Schedule for Myelodysplastic Neoplasm (MDS) and Chronic Myelomonocytic Leukaemia (CMML) Treatment.

INTRODUCTION: 5-Azacitidine (AZA) is a major treatment option for myelodysplastic neoplasms (MDS) and myelodysplastic/myeloproliferative neoplasms (MDS/MPN). Here we evaluate the efficacy and toxicity of an alternative AZA regimen (100 mg/m2/day for 5 days/28 days) in 68 patients (51 MDS, 17 MDS/MPN) treated between 2008 and 2018. RESULTS: Median patient age was 66 years, with most patients (98%) having intermediate or high-risk disease. Overall response rate (ORR) was 62% with 22% complete responses (CR). Median OS and median PFS were 22.5 and 18.2 months, respectively. Inferior response rat

Clonal relationship between juvenile xanthogranuloma and juvenile myelomonocytic leukaemia: insights from a PTPN11 missense variant.

Juvenile xanthogranuloma (JXG) is typically a benign non-Langerhans cell histiocytosis, but it has been rarely associated with juvenile myelomonocytic leukaemia (JMML). Mutations in the RAS-MAPK (mitogen-activated protein kinase) pathway are implicated in both conditions, suggesting a shared genetic basis. However, only one prior case has demonstrated a clonal relationship. We report the case of a 21-month-old boy with a progressive yellow papulonodular rash, dusky plaques and recurrent infections. Skin biopsy confirmed JXG, and exome sequencing revealed a somatic PTPN11 (c.226G>A) variant in

Assessment of peripheral blood DNA methylation signatures as pharmacodynamic and predictive biomarkers during azacitidine therapy in juvenile myelomonocytic leukaemia: Results of the EWOG-MESRAT study.

EWOG-MESRAT (European Working Group-Methylation Signatures and Response to Azacitidine Therapy; DRKS00007185) is an investigator-initiated trial that studied EPIC array-based DNA methylation patterns and next generation sequencing (NGS)-based variant allele frequencies (VAFs) of driver mutations in peripheral blood (PB) and bone marrow (BM) of 11 patients with newly diagnosed juvenile myelomonocytic leukaemia (JMML) during therapy with azacitidine. We demonstrate that the pharmacodynamic activity of azacitidine can efficiently be monitored in PB and BM. DNA methylation subgroup classification

Tandem CD19/CD22 CAR T-cells as potential therapy for children and young adults with high-risk r/r B-ALL.

BACKGROUND: Chimeric antigen receptor (CAR) T-cells targeting CD19 have shown impressive outcomes in refractory/relapsed B-cell acute lymphoblastic leukaemia (r/r B-ALL); however, frequent relapse demands multi-targeted approaches. METHODS: We report Spanish clinical data on the safety and efficacy of tandem anti-CD19/CD22 CAR T-cells administered on a compassionate use basis in a cohort of 10 heavily pretreated paediatric, adolescent, and young adult (AYA) patients with r/r B-ALL. FINDINGS: Most (9/10) of the patients had relapsed B-ALL, 7 having received previous anti-CD19 CAR T-cell thera

Co-operative epigenetic remodelling by Setd2 deficiency and Kras mutation drives juvenile myelomonocytic leukaemia development and MEK inhibitor sensitivity.

Juvenile myelomonocytic leukaemia (JMML) primarily stems from the Rat Sarcoma (RAS)/Extracellular Signal-Regulated Kinase (ERK) pathway mutations, and currently, there are no effective regimens against JMML, yet various studies have highlighted the importance of deciphering secondary genetic and/or epigenetic alterations in pursuit of treatments. Setd2-mediated-H3K36me3 participates in myriad biological functions and primarily serves as a tumour suppressor gene in carcinogenesis. However, little is known about the synergism between Setd2 deficiency and Kras mutation during JMML onset and progr

Clinical and molecular characterization of patients with juvenile myelomonocytic leukaemia.

Juvenile myelomonocytic leukaemia (JMML) is a rare haematological malignancy caused by mutations in the Ras signalling pathway. Next-generation sequencing (NGS) and DNA methylation profiling used for diagnostic and risk stratification purposes are now standard of care in Europe and the United States for patients with JMML. To better understand how implementing these types of technologies would impact the treatment of JMML patients in different settings, molecular profiling was performed on 81 patients treated for JMML in Egypt from 2009 to 2022. NGS increased the number of patients with a mole

CBL-Mutated Juvenile Myelomonocytic Leukaemia With Loss of Heterozygosity on 11q Detected by Microarray: Not Always Such a Favourable Outcome.

CBL syndrome is caused by a heterozygous germline mutation in the CBL gene. It is a rare RASopathy that shares many clinical features with mild forms of Noonan syndrome. These patients have a higher risk of developing juvenile myelomonocytic leukaemia (JMML) during early childhood. Here we report a case of an 11-month-old infant with JMML and CBL syndrome. It was caused by a heterozygous de novo germline mutation in the CBL gene and acquired biallelic inactivation of the gene through copy-neutral loss of heterozygosity (CN-LOH) in haematological cells. CBL mutation was not found in parents and

Bexmarilimab plus azacitidine for high-risk myelodysplastic syndrome and relapsed or refractory acute myeloid leukaemia: results from the dose-escalation part of a multicentre, single-arm, phase 1/2 trial.

BACKGROUND: Bexmarilimab blocks Clever-1 on macrophages to enhance antigen presentation and T cell activation. Because Clever-1 is expressed by myeloid leukaemia cells, bexmarilimab may combat leukaemia and influence the tumour microenvironment to augment the effectiveness of standard-of-care therapy in patients with myelodysplastic syndrome and acute myeloid leukaemia. The aim of this study was to determine the safety of bexmarilimab in combination with standard-of-care treatment in myelodysplastic syndrome and acute myeloid leukaemia and to identify the recommended dose for expansion of bexm

A Sudden Expansion: Acute Subdural Hematoma From Extramedullary Acute Myeloid Leukemia.

Chloromas are extramedullary agglomerations of malignant myeloid blast cells, also known as myeloid sarcoma. Myeloid sarcoma of the central nervous system (CNS) is a very rare presentation of extramedullary leukemic mass infiltration. This may be seen as an intracranial mass due to invasion of the brain parenchyma or as leukemic infiltration of the meninges. Such infiltration may present as a subdural hematoma (SDH), particularly in coagulopathic or immunocompromised patients undergoing chemotherapy. We present a 63-year-old male with acute myeloid leukemia (AML), previously treated with azaci

The interplay between Epstein-Barr virus and cytomegalovirus reactivation following allogeneic haematopoietic cell transplantation in the era of primary cytomegalovirus prophylaxis.

OBJECTIVES: Epstein-Barr virus (EBV) reactivation following allogeneic haematopoietic cell transplantation (allo-HCT) is associated with increased mortality and possible posttransplant lymphoproliferative disorder. With the lack of prophylactic agents, identifying modifiable risk factors to prevent EBV-related mortality is desired. Cytomegalovirus (CMV) DNAemia has been previously associated with EBV DNAemia; the impact of letermovir prophylaxis on this association remains unclear. We aimed to identify risk factors for EBV reactivation; assess the correlation between EBV, CMV reactivation, and

Flexible Self-Powered Respiration Sensor Inspired by Fish Lateral Line Systems.

Respiration is a critical physiological signal, and abnormal respiratory patterns are widely recognized as key indicators of various cardiopulmonary and neurological disorders, making continuous monitoring crucial. However, existing clinical systems are constrained by motion limitations, complexity, and cost, hindering daily use. In this work, we present a bio-inspired, low-cost, flexible, and self-powered respiration sensor (BLFS-RS) inspired by the fish lateral line system, operating via contact electrification and electrostatic induction. It features a semicylindrical structure made of sili

Structural characterization and origin of surface vesicles in monocytes: another membranous pathway from cytoplasm to cell surface.

The monocytes in acute monocytic leukemia (AML-M5b) were analyzed by scanning and transmission electron microscopy (SEM and TEM) to understand more fully their structure and origin. By SEM, monocytes exhibited localized expansions of the surface, some of which appeared to bud off as surface vesicles (SVs). Filopodial processes and pseudopodia were also present. TEM demonstrated that the SVs were composed of a double-membrane at the pole away from the cell body, and a single membrane nearer to the cell body. In the peripheral cytoplasm, intracellular vesicles (IVs) had the appearance of vacuole

Mast-cell expressed membrane protein-1 is expressed in classical monocytes and alveolar macrophages in idiopathic pulmonary fibrosis and regulates cell chemotaxis, adhesion, and migration in a TGFβ-dependent manner.

Mast-cell expressed membrane protein-1 (MCEMP1) is higher in patients with idiopathic pulmonary fibrosis (IPF) with an increased risk of death. Here we aimed to establish the mechanistic role of MCEMP1 in pulmonary fibrosis. We identified increased MCEMP1 expression in classical monocytes and alveolar macrophages in IPF compared with controls. MCEMP1 is upregulated by transforming growth factor beta (TGFβ) at the mRNA and protein levels in monocytic leukemia THP-1 cells. TGFβ-mediated MCEMP1 upregulation results from the cooperation of SMAD3 and SP1 via concomitant binding to SMAD3/SP1 cis-reg

Protocols for Co-Culture Phenotypic Assays with Breast Cancer Cells and THP-1-Derived Macrophages.

Tumor mass comprises not only cancer cells but also heterogeneous populations of immune and stromal cells, along with the components of the extracellular matrix, collectively called the tumor microenvironment (TME). This diverse population of cells can communicate with each other, which can positively or negatively affect tumor growth and progression to malignancy. The most common type of immune cells in the TME are macrophages. Macrophages continuously differentiate into a broad landscape of tumor-associated macrophages (TAMs) in response to numerous signals from the TME, which makes studies
