Objective: T-cell acute lymphoblastic leukemia (T-ALL) is a highly aggressive hematological malignancy with poor overall prognosis. This study systematically evaluates the anti-leukemic therapeutic potential and underlying redox-driven mechanisms of PZ2, a novel organic arsenic compound, in T-ALL. Methods: The cytotoxicity of PZ2 against T-ALL cell lines and normal cells was assessed to determine its therapeutic window. RNA-seq was utilized to map transcriptomic alterations. Intracellular redox homeostasis, mitochondrial membrane potential (MMP), cell cycle, and apoptosis were evaluated using

BACKGROUND/OBJECTIVES: Neutrophil recovery after bloodstream infection (BSI) in acute leukemia is clinically important, but evaluation is complicated by early death and recurrent infectious episodes. We assessed clinical, treatment-related, and microbiological factors associated with recovery while accounting for death as a competing event.
METHODS: This retrospective single-center cohort included 205 microbiologically documented BSI episodes in 101 adults with acute leukemia treated between 2020 and 2025. Neutrophil recovery was defined as an absolute neutrophil count >500/µL after BSI onset

BACKGROUND: Early relapse and a poor prognosis are standard features of T-acute lymphoblastic leukemia (T-ALL), a hematological malignancy that presents significant treatment challenges. Glucocorticoids (GCs) are the first line of treatment for T-ALL; nevertheless, GC resistance may develop in patients with newly diagnosed and recurrent T-ALL due to abnormalities in apoptotic pathways. Here, we investigated the impact of the targeted medication, selumetinib, on the expression of proteins that regulate apoptosis in malignant T-ALL cells. We examined the Bcl-2, Bax, P53, Beclin1, and NFE2L2 expr

The B-cell lymphoma 2 inhibitor venetoclax has therapeutic activity in several hematological malignancies. In acute myeloid leukemia (AML), venetoclax combined with hypomethylating agents is the standard of care for patients unfit for intensive chemotherapy, but intrinsic and acquired resistance are common. Loss of p53 function is strongly associated with venetoclax resistance, and adding venetoclax to 5-azacitidine provides no overall survival benefit in TP53-mutant AML. Other frequent mechanisms of venetoclax resistance in AML include FLT3 mutations, myeloid cell leukemia 1 (MCL-1) upregulat

Acute myeloid leukemia (AML) is the most common form of acute leukemia. It is characterized by unfavorable prognosis, elevated rates of recurrence, short overall survival and high mortality rates. Genetic and epigenetic aberrations contribute to its initiation and progression, presenting a considerable clinical challenge. Genomic investigations revealed that FMS-like tyrosine kinase 3 (FLT3), a receptor tyrosine kinase regulating the proliferation and differentiation of hematopoietic cells, is overexpressed in AML leukemic blasts. To date, FDA has approved only three FLT3 inhibitors for target

Acute myeloid leukaemia (AML) with FLT3 internal tandem duplication (FLT3-ITD) frequently relapses after FLT3 tyrosine kinase inhibitor (TKI) therapy, highlighting the need to define mechanisms of drug tolerance. Here, we identify the mitochondrial complex I core subunit NDUFS2 as a determinant of FLT3 TKI tolerance in FLT3-ITD AML. A metabolism-focused CRISPR-Cas9 sensitiser screen, integrated with transcriptomic profiling of gilteritinib-resistant cells and primary samples, identified NDUFS2 as upregulated in acquired resistance and preferentially expressed in FLT3-ITD AML. NDUFS2 knockdown

Although survival in pediatric acute myeloid leukemia (AML) has improved over the past decades, intensive chemotherapy may be associated with severe and life-threatening toxicities. This case series describes pediatric AML patients in whom therapy was discontinued due to excessive toxicity. Patients were identified through a national database of two consecutive AML treatment protocols in the Netherlands (2010-2023). Seven patients were included; discontinuation of treatment was because of a severe fungal infection in five out of seven cases. Five out of seven patients had a favorable genetic p

Despite widespread immunophenotyping in acute myeloid leukemia (AML), its immunophenotypic heterogeneity remains incompletely understood. Here, we applied state-of-the-art computational modeling to >50 million single cells from diagnostic bone marrow aspirates of 502 AML patients. Jointly modeling leukemic and non-malignant cells, we identified seven patient clusters with distinct immunophenotypic signatures. These clusters showed strong concordance with recurrent mutations, including NPM1, IDH1/IDH2, CEBPA, and t(8;21)(q22;q22); RUNX1-RUNX1T1 fusions, and enabled accurate prediction of genomi

BACKGROUND: Intracranial hemorrhage (ICH) is the leading cause of early death in acute promyelocytic leukemia (APL). Current risk assessment relies on static coagulation parameters at admission, but APL-associated coagulopathy evolves rapidly. Whether dynamic changes in coagulation markers predict hematoma expansion remains unknown.
OBJECTIVE: To determine whether the fibrinogen decline rate and D-dimer increase rate predict hematoma expansion in APL-related ICH, and to compare their predictive performance with static admission values.
METHODS: This single-center retrospective cohort study i

Classic Philadelphia chromosome-negative myeloproliferative neoplasms (MPNs) are hematopoietic stem cell cancers that result in aberrant trilineage myeloid cell proliferation, bone marrow fibrosis, and increased risk of acute myeloid leukemia. MPNs are driven by deregulated activity of the JAK2 kinase, induced by mutations in the JAK2, CALR, and MPL genes, but approved JAK2 inhibitors primarily offer palliative effects, not remission. Cell models that demonstrate MPN oncogene driven JAK2 activity requisite for cell proliferation are important research tools for the development of anti-JAK2 and

FLT3-mutated acute myeloid leukemia (AML) is associated with high relapse rates and treatment resistance. Triplet therapy with azacitidine (AZA), venetoclax (VEN), and the FLT3 inhibitor gilteritinib (GILT) yields high response rates but is limited by myelosuppression and infections. We conducted a multicenter retrospective study of 29 FLT3-mutated AML patients treated with GILT-triplets. The primary endpoint was overall survival (OS); secondary endpoints event-free survival (EFS) and time to neutrophil and platelet recovery. Patients received one of three regimens: concomitant triplet therapy

Intrathecal methotrexate (MTX) remains a critical component in multiagent chemotherapy regimens for acute lymphoblastic leukemia (ALL). However, MTX-induced neurotoxicity (MIN) can cause significant morbidity. Aminophylline can be a treatment for MIN, mitigating neurotoxicity, but its use has been restricted to case reports and small patient series. In this report, we present treatment outcomes of seven patients who developed MIN and received aminophylline. The median time-to-improvement after aminophylline was 9 h, and the median time to full resolution of symptoms was 36 h. All patients deve

Dysregulated mRNA translation is a critical driver of acute myeloid leukemia (AML) progression. While translation initiation is extensively studied, mechanisms governing translation elongation in AML remain unclear. Here, we show that the RNA methyltransferase ALKBH8 is overexpressed in patients with AML and correlates with poor prognosis. ALKBH8 is required for the proliferation and leukemogenic capacity of human and murine AML cells in vitro and in vivo. Mechanistically, ALKBH8 promotes translation-coupled mRNA stabilization via its methyltransferase domain. Loss of ALKBH8 induces ribosome p

G protein-coupled receptors (GPCRs) rely on dynamic conformational ensembles to coordinate G protein activation and recruitment of regulatory transducers such as GPCR kinases and β-arrestins. The oxysterol receptor GPR183 has been implicated in hematological malignancies, including acute myeloid leukemia (AML), where a rare A338V substitution occurs within its intrinsically disordered C-terminal tail. Here, we combined bioluminescence/fluorescence resonance energy transfer, bioluminescence- and fluorescence-based assays as well as migration experiments, with molecular dynamics simulations and

We describe a fatal Rickettsia parkeri rickettsiosis case in a 3-year-old boy in Georgia, USA, receiving chemotherapy for B-cell acute lymphoblastic leukemia, hospitalized with febrile neutropenia. Plasma microbial cell-free DNA sequencing confirmed the diagnosis. This report highlights the need to maintain suspicion for tickborne diseases in immunocompromised hosts in some settings.

Acute myeloid leukemia (AML) is an aggressive malignancy with poor prognosis, primarily driven by leukemic stem cells (LSCs). The RNA demethylase ALKBH5 is aberrantly overexpressed in AML and critically involved in leukemogenesis and LSC maintenance. Herein, we designed and synthesized fifty-seven N-phenylbenzenesulfonamide derivatives as potential ALKBH5 inhibitors. Within this series, G4 emerged as a potent ALKBH5 inhibitor that selectively suppressed ALKBH5-high AML cells over ALKBH5-knockout cells and normal human PBMCs. In G4-treated AML cells, cell cycle arrest and apoptosis were accompa

Biomimetic sodium ion transporters may offer a potential basis for the development of novel anticancer strategies. This study evaluated quinoline oxadiazole-based Na+ transporters (M1, M2 and M3) for selective Na+ transport and anti-leukemic effects. M1 and M3 localized to cellular membranes and exhibited exceptional Na+ selectivity, with transport rates of 22.85 ± 1.71 and 17.71 ± 1.24, respectively. They independently induced rapid Na+ influx, leading to a 2- to 3-fold increase in intracellular Na+ concentration within 3-6 hours. In acute myeloid leukemia cell lines (MV4-11 and THP-1), this
