Pirtobrutinib for the Treatment of Therapy-Resistant Chronic Lymphatic Leukemia With CNS Localization.

During the last decade, many new treatment options for chronic lymphatic leukemia (CLL) have become available, one of them being covalent-binding Bruton's tyrosine kinase inhibitors (cBTK inhibitors [cBTKis]). There are currently three cBTKis approved for treatment of CLL/SLL: acalabrutinib, ibrutinib, and zanubrutinib. In relapsed and heavily pretreated patients, BTK inhibitors achieve a median PFS of 4 years, and in frontline, around 80% of CLL patients are still in remission and alive after 4 years of treatment with BTK inhibitors. Despite their efficacy, 40% of patients have to discontinue

Prolonged Active SARS-CoV-2 Infection in a Patient With Chronic Lymphocytic Leukemia on Venetoclax.

We report a case of prolonged active SARS-CoV-2 infection in a patient with chronic lymphocytic leukemia (CLL) receiving venetoclax. The patient demonstrated persistent symptoms over a span of 105 days and repeatedly positive SARS-CoV-2 polymerase chain reaction (PCR) testing. The case highlights the diagnostic and therapeutic challenges in immunocompromised hosts, including limited guidance and drug availability. Management required multiple antiviral regimens and consideration of alternative therapies. This case underscores the need for further research that can inform more robust guidelines

A comparison of overall survival and quality of life in patients with MDS treated with azacytidine vs decitabine.

Patients with high-risk myelodysplastic neoplasms or syndromes (MDS) are considered for therapy with hypomethylating agents (HMA) to help restore hematopoiesis, delay progression to acute myeloid leukemia, and improve overall survival (OS). Azacytidine (AZA) and decitabine (DEC) have commonly been used without direct clinical trial comparison between the 2 agents. Prior studies have demonstrated no difference in OS; however, these analyses are limited by the lack of patient-level and disease-risk factors that may influence efficacy and tolerability. This study retrospectively evaluated differe

Granulocyte-derived Resistin Inhibits Monocyte Maturation and Promotes an Immunosuppressive Phenotype in Chronic Myelomonocytic Leukemia Monocytes.

Chronic myelomonocytic leukemia (CMML) is a hematological neoplasm characterized by expansion of malignant monocytes. However, as a stem cell neoplasm, the disease clone comprises most blood lineages, including granulocytes. To investigate their pathogenic contribution, we performed extensive phenotypic, transcriptomic and functional characterization of CMML granulocytes. These cells exhibited reduced granularity and impaired maturation, the latter of which is associated with poor prognosis. Functionally, CMML granulocytes displayed reduced phagocytic capacity and impaired NETosis, together wi

Effectiveness of cardiac resynchronization therapy in cancer therapy-related cardiac dysfunction: a systematic review and meta-analysis.

INTRODUCTION: Advances in cancer treatment have increased the number of survivors, making cancer therapy-related cardiac dysfunction (CTRCD) an important concern. While cardiac resynchronization therapy (CRT) is established for heart failure with reduced ejection fraction and conduction disorders, its role in CTRCD remains unclear. We conducted a meta-analysis to assess the benefit of CRT in patients with CTRCD. METHODS: We performed a literature search of studies showing CRT outcomes in the specific group of CTRCD patients. The outcomes of interest were left ventricular ejection fraction (LV

Graft-versus-host disease prophylaxis shapes T cell biology and immune reconstitution after hematopoietic cell transplant.

Successful hematopoietic cell transplant requires immunosuppression to prevent graft-versus-host disease (GVHD), a lethal, T cell-mediated complication. The phase 3 BMT CTN 1703 trial demonstrated superior GVHD-free/relapse-free survival for posttransplant cyclophosphamide (PT-Cy)-based GVHD prophylaxis versus tacrolimus/methotrexate (Tac/MTX), but the T cell biology underpinning these outcomes remains unknown. To determine this, 324 patients were coenrolled onto BMT CTN 1801 to investigate the distinct T cell immune reconstitution pathways of these regimens, including extensive evaluation of

Prospective phase 2 trial of high- and intermediate-intensity myeloablative conditioning for cord blood transplantation.

Umbilical cord blood transplantation (UCBT) is an important alternative donor strategy for patients with high-risk hematologic malignancies. While myeloablative conditioning (MAC) is associated with superior disease control following UCBT compared to reduced-intensity conditioning (RIC), its benefits can be offset by non-relapse mortality (NRM). Intermediate-intensity MAC may preserve the anti-leukemic effects of historically high-intensity MAC while reducing NRM. We conducted a prospective phase 2 trial evaluating outcomes of patients undergoing UCBT following either high-intensity or interme

Hematopoietic Cell Transplantation Outcomes in the Era of Post-Transplant Cyclophosphamide: Comprehensive Donor Analysis.

Donor types influence outcomes after allogeneic hematopoietic cell transplantation (HCT), but their relative impact in the post-transplant cyclophosphamide (PTCy) era is uncertain. We performed a multicenter retrospective cohort study using Center for International Blood and Marrow Transplant Research data for adults with acute leukemia or myelodysplastic syndromes undergoing first HCT from matched sibling donors (MSD), 8/8 matched unrelated donors (MUD), 7/8 mismatched unrelated donors (MMUD), or haploidentical donors between 2017 and 2023 with PTCy-based graft-versus-host disease (GVHD) prop

Twenty-Five Years of CML Therapy in India: A Model for Precision Oncology in a Resource-Constrained Setting.

Chronic myeloid leukemia (CML) is the classic example in oncology of a single molecular lesion turned into a manageable, and increasingly curable, disease. This review focuses specifically on what is distinctive about CML in India and is organized so that the features that most clearly set Indian practice apart from the West are presented within their essential clinical context. The disease presents in India roughly 2 decades earlier than in Western registries, more often symptomatically and at a higher risk score, yet almost always still in chronic phase, and it is treated, overwhelmingly, wi

ROR2-expressing acute myelogenous leukemia cells can be selectively targeted by anti-ROR2 CAR T cells.

Chimeric antigen receptor (CAR) T cells have revolutionized the treatment of B-cell malignancies but have not yet succeeded in acute myeloid leukemia (AML), largely due to the lack of cell surface antigens that can distinguish leukemia cells from essential normal hematopoietic stem cells (HSCs). Here, we used a newly developed high-affinity monoclonal antibody (mAb) B16, which is specific for the surface ROR2 in both human and mouse. Although ROR2 is a developmentally restricted protein that is primarily expressed during embryogenesis, we find that ROR2 protein is generally expressed by AML ce

Polycythemia Vera Transforming to Pure Erythroid Leukemia: A Diagnostic Mimic of Thrombotic Microangiopathy With High-Risk TP53 Biology.

Leukemic transformation of polycythemia vera (PV) to acute myeloid leukemia (AML) with a predominantly erythroid blast population is rare and carries a poor prognosis, particularly when driven by a TP53 mutation. We report a 55-year-old man with JAK2 V617F-positive PV, previously noted on surveillance biopsy to harbor a TP53 mutation and Grade 2 marrow fibrosis, who presented three years later with pancytopenia, schistocytosis, hyperferritinemia, hypofibrinogenemia, and altered mental status. The initial clinical presentation raised concern for thrombotic thrombocytopenic purpura (TTP), while

Late-onset hypothalamic-pituitary dysfunction in a childhood cancer survivor treated with cranial irradiation.

Hypothalamic-pituitary dysfunction is a well-recognized late complication in childhood cancer survivors (CCSs) treated with cranial irradiation. We report a 42-year-old male with a history of acute lymphoblastic leukemia diagnosed at 5 years of age and treated with chemotherapy and 26 Gray whole-brain irradiation. He presented with right upper extremity paresis due to bilateral chronic subdural hematomas. Following burr-hole drainage for the left chronic subdural hematoma, he developed anorexia, fatigue, and hyponatremia and was referred to the endocrinology department. Endocrine evaluation de

Deleted in lymphocytic leukemia 2 contributes to lung fibroblast activation and pulmonary fibrosis by interacting with and triggering focal adhesion kinase phosphorylation.

The etiology of idiopathic pulmonary fibrosis (IPF) remains unknown, and therapeutic interventions are currently restricted. The activation of lung fibroblasts and the focal adhesion kinase (FAK) signaling pathway are crucial to the pathogenesis of IPF, yet the exact molecular mechanisms remain incompletely understood. Our study shows that the deleted in lymphocytic leukemia 2 (DLEU2) is elevated in the peripheral blood of patients with IPF, as well as in the lung tissues of mice with pulmonary fibrosis. Functional assays revealed that overexpression of DLEU2 facilitated fibrogenesis, whereas

Aberrant splicing caused by a homozygous AK2 c.498 + 1G > A variant in a patient with reticular dysgenesis and concurrent B-cell acute lymphoblastic leukemia.

Reticular dysgenesis (RD) is a rare autosomal recessive severe combined immunodeficiency characterized by profound neutropenia, T-cell lymphopenia and sensorineural hearing loss, and is caused by biallelic loss-of-function variants in AK2. We identified a homozygous AK2 splice-site variant (c.498 + 1G > A) in a boy with RD and concurrent B-cell acute lymphoblastic leukemia (B-ALL), a clinical combination not previously reported in association with an AK2 variant. An in vitro minigene assay showed that c.498 + 1G > A disrupts the canonical 5' splice donor site and generates an aberrant transcri

Efficacy of continuous ibrutinib monotherapy in patients with relapsed/refractory chronic lymphocytic leukemia with high-burden and low-burden TP53 mutations: Multicenter retrospective analysis of the Polish Adult Leukemia Group (PALG).

BACKGROUND: TP53 gene aberrations, including deletions (del(17p)) and mutations, are the strongest predictors of shorter response duration and survival with most therapies for chronic lymphocytic leukemia (CLL). However, the impact of isolated TP53 mutations (without concomitant deletion of the 2nd allele) on outcomes of treatment with Bruton tyrosine kinase inhibitors remains less clear, especially in the context of low-burden mutations. OBJECTIVES: This retrospective analysis aimed to assess the efficacy of ibrutinib in patients with relapsed/refractory CLL and TP53 mutations. MATERIAL AND

Pediatric-inspired USC-ALL regimen vs hyper-CVAD in adults with Ph-negative and Ph-like acute lymphoblastic leukemia.

Adult acute lymphoblastic leukemia (ALL) outcomes remain inferior to pediatric results, motivating adoption of pediatric-inspired regimens (PIR) in adults. The optimal frontline regimen for high-risk subgroups, such as Philadelphia chromosome-like (Ph-like) ALL, has not been established. In this retrospective analysis, 160 adults with Philadelphia chromosome-negative B-cell or T-cell ALL treated between 2013 and 2024 received either the USC-ALL regimen, a pediatric-inspired protocol incorporating synchronized pegaspargase and high-dose methotrexate during two induction and six consolidation cy

Clinical relevance of the newly reclassified HLA-C*04:09L low-expression allele in allogeneic hematopoietic cell transplantation: a case report.

In the context of allogeneic hematopoietic cell transplantation (allo-HCT), misclassifying null- and low-expression alleles could result in an HLA mismatch with life-threatening clinical consequences. HLA-C*04:09N was considered one of the most frequent null alleles, but it was recently reclassified as a low-expression allele, HLA-C*04:09L, based on a recent work which described that could be detected at the cell surface and capable of inducing peptide-specific T-cell activation in vitro. The authors suggested that these findings could imply clinical consequences in the field of transplantatio

Combining the Telomerase Inhibitor Imetelstat with the BCL-2 Inhibitor Venetoclax Enhances Apoptosis and Improves Survival in Acute Myeloid Leukemia Cellular and Animal Models.

Background/Objectives: Acute myeloid leukemia (AML) is a heterogeneous disease with limited treatment options. New therapies with novel mechanisms of action are needed. Telomerase and BCL-2 are overexpressed in AML and correlate with disease severity and poor prognosis, suggesting dual targeting may be beneficial. Methods: We evaluated the effect of imetelstat, a telomerase inhibitor, in combination with venetoclax, a BCL-2 inhibitor, in AML cell lines, AML patient-derived samples, and in a MOLM-13 AML xenograft model. Results: In AML cell lines and patient samples, imetelstat plus venetoclax

Efficacy of Sorafenib Combined with Venetoclax and Azacitidine in Newly Diagnosed FLT3-ITD-Mutant Acute Myeloid Leukemia: Remission Efficacy and Prognostic Influence of Post-Remission Consolidation Therapy.

Background: Mutations of the FLT3 gene, particularly internal tandem duplication (FLT3-ITD), define a biologically aggressive AML subtype with historically poor outcomes. Here we retrospectively assessed the clinical performance and tolerability of sorafenib with venetoclax plus azacitidine (VA) as front-line therapy in patients with newly diagnosed FLT3-ITD-mutated AML. Methods: Forty-five consecutive patients who received sorafenib plus VA induction were retrospectively analyzed.
