INTRODUCTION: Anti-CD19 CAR T-cell therapy has improved outcomes in B-cell non-Hodgkin lymphoma (B-NHL), but only about half of patients achieve durable benefit. As CD20 is expressed in > 90% of B-NHL, this phase I trial evaluated the CD20-directed CAR T-cell product MB-CART20.1 in adults with relapsed/refractory CD20-positive B-NHL without curative treatment options.
METHODS: In this phase I dose-escalation trial, patients received 1.0 × 10⁵ cells/kg body weight (BW) MB-CART20.1 at the safety dose level (SDL) or 1.0 × 10⁶ cells/kg BW at dose level 1 (DL1).
RESULTS: Recruitment was terminate

OBJECTIVE: This study aims to develop a predictive model for cytarabine syndrome (CS)-related fever in pediatric patients with acute lymphoblastic leukemia (ALL). By integrating clinical features and laboratory indicators, we seek to establish an evidence-based diagnostic tool to assist clinicians in decision-making and optimize the use of antimicrobial agents.
METHODS: Study Design: A retrospective case-control study was conducted involving 72 ALL children (32 in the CS group vs. 40 in the infection group) who developed fever (≥37.5 °C) following cytarabine treatment. Model Development: Mult

Genomic instability and immune escape are linked features of hematologic malignancies, but the mechanisms that allow malignant cells to tolerate persistent nuclear and genotoxic stress while avoiding damaging DNA-sensing responses remain incompletely defined. Barrier-to-autointegration factor 1 (BANF1) is a small DNA-binding protein that bridges chromatin, couples it to nuclear-envelope proteins, supports postmitotic nuclear assembly and rupture repair, modulates several DNA-damage-response pathways, and restricts productive engagement of cyclic GMP-AMP synthase (cGAS) with self-DNA. Human BAN

Graphitic carbon nitride (g-C3N4) is a versatile material for biomedical, energy, and environmental applications. Bulk g-C3N4 is commonly used as a precursor for nanoscale objects, yet systematic links between synthesis conditions, nanostructure morphology, surface chemistry, and biological response remain limited. Here, we fabricated a palette of carbon nitride nanostructures via hydrothermal alkaline etching. By varying the hydroxide (LiOH, NaOH, or KOH) and reaction conditions, etching yields carbon nitride quantum dots (CNQDs), carbon nitride nanowires (CNNWs) and their networks, as well a

Bone marrow vascular niche regulates both hematopoiesis and hematopoietic disorders, including acute and chronic leukemia. However, it is still not fully understood at the structural and functional levels owing to its inaccessibility in situ. Here, a biomimetic bovine decellularized bone marrow extracellular matrix (dbECM) mixed hydrogel was developed. Proteomics validation confirmed its retention of bone marrow-specific ECM components. The mechanical properties and microstructure of the mixed hydrogel provided a biomimetic platform that integrates biochemical and physical cues for niche funct

Chimeric antigen receptor (CAR)-T cell therapy has achieved remarkable remission rates exceeding 80% in relapsed/refractory pediatric B-cell acute lymphoblastic leukemia (B-ALL), yet 40-50% of patients eventually relapse within the first year. This therapeutic ceiling is not a single failure mode but the convergence of at least four interrelated layers: loss of CAR-T persistence, antigen escape driven by CD19 splicing variants and lineage switch, CAR-T cell-intrinsic exhaustion from chronic tonic signaling, and an immunosuppressive bone marrow microenvironment that actively extinguishes effect

Gut microbiota dysbiosis has emerged as a key feature in pediatric acute lymphoblastic leukemia (ALL), driven by chemotherapy, antibiotic exposure, and treatment-related nutritional impairment. Increasing evidence indicates that disruption of microbial diversity and depletion of short-chain fatty acid-producing bacteria are closely associated with infectious complications, gastrointestinal toxicity, and potentially reduced treatment efficacy. This review synthesizes current evidence on the interplay between gut microbiota and pediatric ALL across four dimensions: clinical outcomes, treatment-r

BACKGROUND: Chimeric antigen receptor (CAR) T-cell therapy has emerged as a promising strategy for management of relapsed/refractory acute myeloid leukemia (R/R AML). Safety and preliminary activity of CD38-targeted CAR-T cells in R/R AML were demonstrated in our prior study; however, relapse with CD38-positive blasts persisted, which was partly associated with insufficient effector function and persistence of CAR-T cells. Given that CAR-T cell hypofunction is promoted by inadequate antigen stimulation, a lipid nanoparticle-delivered CD38 mRNA vaccine was developed to enhance CD38 CAR-T cell e

BACKGROUND: Chimeric Antigen Receptor T-cell (CAR T-cell) therapy is a promising treatment option for patients with refractory or relapsed B-cell malignancies. The success of this therapy depends on the efficient collection of lymphocytes (CD3+ T cells) through apheresis. This study analyzed the factors influencing CD3+ cell counts in apheresis products for CAR T-cell therapy.
METHODS: This exploratory analysis evaluated patients enrolled in the registration phase I/II trials of talicabtagene autoleucel at a tertiary care oncology centre in Western India. Patients were enrolled after obtainin

INTRODUCTION: Myeloid neoplasms harboring t(8;22)(p11;q11)/BCR::FGFR1 rearrangement are exceptionally rare and are typically associated with aggressive clinical behavior. The coexistence of trisomy 8 with concurrent RUNX1 and STAG2 mutations in this cytogenetic context has not been systematically described.
CASE PRESENTATION: We report a 71-year-old female who presented with fever, dysuria, cough, shortness of breath, and melaena. Initial laboratory evaluation revealed anemia, leukocytosis, and severe thrombocytopenia, with approximately 60% peripheral circulating blasts. Bone marrow examinat

Allogeneic hematopoietic cell transplantation (allo-HCT) is an effective therapy for selected hematologic diseases. However, graft-versus-host disease (GVHD) limits broader application of allo-HCT. The catecholaminergic sympathetic nervous system modulates immune cells. Beta-2 adrenergic Receptor (β2AR) acts as an immune checkpoint, yet its role in CD8+ T cells remains poorly understood. We used an MHC-mismatched model of acute GVHD to study how β2AR activity on donor CD8+ T cells affects GVHD severity. Host mice receiving β2AR+/+ (WT) CD8+ T cells demonstrated less severe GVHD, lower inflamma

We report a case of prolonged active SARS-CoV-2 infection in a patient with chronic lymphocytic leukemia (CLL) receiving venetoclax. The patient demonstrated persistent symptoms over a span of 105 days and repeatedly positive SARS-CoV-2 polymerase chain reaction (PCR) testing. The case highlights the diagnostic and therapeutic challenges in immunocompromised hosts, including limited guidance and drug availability. Management required multiple antiviral regimens and consideration of alternative therapies. This case underscores the need for further research that can inform more robust guidelines

Patients with high-risk myelodysplastic neoplasms or syndromes (MDS) are considered for therapy with hypomethylating agents (HMA) to help restore hematopoiesis, delay progression to acute myeloid leukemia, and improve overall survival (OS). Azacytidine (AZA) and decitabine (DEC) have commonly been used without direct clinical trial comparison between the 2 agents. Prior studies have demonstrated no difference in OS; however, these analyses are limited by the lack of patient-level and disease-risk factors that may influence efficacy and tolerability. This study retrospectively evaluated differe

Umbilical cord blood transplantation (UCBT) is an important alternative donor strategy for patients with high-risk hematologic malignancies. While myeloablative conditioning (MAC) is associated with superior disease control following UCBT compared to reduced-intensity conditioning (RIC), its benefits can be offset by non-relapse mortality (NRM). Intermediate-intensity MAC may preserve the anti-leukemic effects of historically high-intensity MAC while reducing NRM. We conducted a prospective phase 2 trial evaluating outcomes of patients undergoing UCBT following either high-intensity or interme

Donor types influence outcomes after allogeneic hematopoietic cell transplantation (HCT), but their relative impact in the post-transplant cyclophosphamide (PTCy) era is uncertain. We performed a multicenter retrospective cohort study using Center for International Blood and Marrow Transplant Research data for adults with acute leukemia or myelodysplastic syndromes undergoing first HCT from matched sibling donors (MSD), 8/8 matched unrelated donors (MUD), 7/8 mismatched unrelated donors (MMUD), or haploidentical donors between 2017 and 2023 with PTCy-based graft-versus-host disease (GVHD) prop

Chimeric antigen receptor (CAR) T cells have revolutionized the treatment of B-cell malignancies but have not yet succeeded in acute myeloid leukemia (AML), largely due to the lack of cell surface antigens that can distinguish leukemia cells from essential normal hematopoietic stem cells (HSCs). Here, we used a newly developed high-affinity monoclonal antibody (mAb) B16, which is specific for the surface ROR2 in both human and mouse. Although ROR2 is a developmentally restricted protein that is primarily expressed during embryogenesis, we find that ROR2 protein is generally expressed by AML ce

Leukemic transformation of polycythemia vera (PV) to acute myeloid leukemia (AML) with a predominantly erythroid blast population is rare and carries a poor prognosis, particularly when driven by a TP53 mutation. We report a 55-year-old man with JAK2 V617F-positive PV, previously noted on surveillance biopsy to harbor a TP53 mutation and Grade 2 marrow fibrosis, who presented three years later with pancytopenia, schistocytosis, hyperferritinemia, hypofibrinogenemia, and altered mental status. The initial clinical presentation raised concern for thrombotic thrombocytopenic purpura (TTP), while
